본 증례의 편측성 하악 과두 골절 환자는 외상 후 1개월간 다수의 상악 전치부 치아 외상이 간과되어 교합이 붕괴된 상태로 내원하였다. 변위된 상악 전치부 치아들의 외과적 정복 후 지속적인 기능 운동을 진행하여, 10개월 후 정상 교합 및 하악 과두 골절부의 골유합이 확인되는 양호한 결과를 얻을 수 있었다.
본 연구는 경희대학교 치과대학병원 소아치과에 치아외상을 주소로 내원한 411명의 어린이를 대상으로 치아외상의 발생 연령과 남녀간의 성차, 발생의 양상, 부위, 원인, 장소 및 외상 후 내원 시기와 계절적인 발생빈도 등을 조사하여 외상의 예방, 적절한 진단과 치료 그리고 보호자의 계도 및 외상에 관한 홍보에 도움이 되고자 시행하였다. 1998년부터 2000년 사이에 치아외상을 주소로 내원한 환자 중 생후 5개월에서 6세 사이 남아 261명, 여아 150명의 외상치아 745개를 대상으로 다음과 같은 결과를 얻었다. 1.치아 외상은 남아가 여아보다 1.74배 많은 것으로 나타났으며, 남녀 모두 $1{\sim}2$세(34.1%) 및 $2{\sim}3$세(24.3%) 사이에 많이 발생하였다. 2. 치아 외상 양태는 진탕 32.2% 함입 11.9%, 측방변위 10.1%의 순으로 나타났다. 3. 상악 유중절치가 외상 치아의 75.3%로 가장 많이 발생하였다. 4. 치아 외상의 원인은 '넘어짐' 46.0%, '부딪힘' 40.3%, '떨어짐' 6.0% 순으로, 장소는 집안 48.0%, 집밖 37.7%, 유치원 9.2% 순으로 발생하였다. 5. 치아 외상 후 24시간 내에 내원하는 경우가 46.0%, 1주일내 29.5%, 1시간내가 7.5%를 보였다. 6. 월별 치아 외상은 10월(12.7%), 6월(12.2%), 5월(11.7%) 순으로 발생하였다.
치아외상은 현대사회에서 점차 증가하고 있으며 특히 어린이와 청소년에서 빈발한다. 그러나 치아외상의 위험성이 부각되고 있는 것에 반해, 이에 대한 국내 연구는 미미한 실정이다. 본 연구는 청소년의 치아 외상 유병률을 조사하고 위험요인을 평가하며 외상에 대한 주관적 인지정도를 파악할 목적으로 시행되었다. 양산시 소재 중학교에 재학중인 청소년 1,371명을 대상으로 임상검사 및 설문조사를 실시하여 다음과 같은 결과를 얻었다. 임상검사 상 치아 외상 유병률은 16.8%로 남자(19.2%)가 여자(13.7%)에 비해 높게 나타났고(p < 0.05), 외상 병력이 있는 사람의 일인 당 평균 외상 치아 수는 1.34개였다. 치종 별로는 상악 중절치에서, 외상의 유형에 따른 분류에서는 법랑질 파절에서 가장 높은 비중을 보였다. Class II division 1과 8.0 mm 이상의 수평피개가 외상의 위험요인으로 판단되었다(p < 0.05). 설문조사로 드러난 본인의 외상 인지도는 임상 검사의 결과와 일치하지 않았고, 치아 외상으로 인한 치료 결과에 대하여 남자가 더 높은 만족도를 나타냈다(p < 0.05).
치아 및 치조골은 복합적인 구조이므로 치아의 함입이나 측방 탈구와 같은 치조와의 변위가 있는 다수 치아를 포함한 외상에서 치조골 골절이 동반될 수 있다. 치조골이 골절되면 치주인대 및 치수로의 혈행이 단절되어 치아의 합병증을 유발할 수 있고, 특히 유치열기 외상은 후속 영구치배에 손상을 줄 가능성이 있으므로 장기간의 관찰이 필요하다. 치아 및 치조골 골절의 치료 시에는 환자의 연령과 외상의 위치 및 범위, 유치의 변위 정도 및 방향을 고려해야하며, 골절편을 재위치 시키고 고정하기 위하여 아치바(arch bar), 레진-강선 고정, 교정용 밴드, 아크릴 또는 금속 캡 스플린트(acrylic or metal cap splint), 화이버 스플린트(fiber-splint) 등의 다양한 방법이 고안되었다. 본 증례는 외상으로 인하여 치조골이 골절되어 내원한 1세 11개월 된 환아로서 모형상에서 레진-강선 오픈 캡 스플린트(resin-wire open cap splint)를 제작하여 고정한 후 양호한 치료 결과를 얻었다. 이 방법은 부가적인 기공 과정이 필요하지만, 유치열기에서 사용할 수 있고, 시술시간이 짧아 비협조적인 환아에게 진정요법 없이 적용할 수 있으며, 일반적인 아크릴 캡 스플린트에 비해 부피가 작아 불편감이 적고, 교합을 방해하지 않으며, 접착시 시멘트가 빠져나갈 공간이 있어서 스플린트의 정확한 안착이 가능하다. 또한 비교적 통증이 적으며 침습적이지 않아 출혈이 없으므로 의과적문제가 있는 경우에도 사용할 수 있다.
Endothelial cells are a vital constituent of most mammalian organs and are required to maintain the integrity of these tissues. These cells also play a major role in angiogenesis, inflammatory reactions, and in the regulation of thrombosis. Angiogenesis facilitates pulp formation and produces the vessels which are essential for the maintenance of tooth homeostasis. These vessels can also be used in bone and tissue regeneration, and in surgical procedures to place implants or to remove cancerous tissue. Furthermore, endothelial cell regeneration is the most critical component of the tooth generation process. The aim of the present study was to stimulate endothelial regeneration at a site of acute cyclophosphamide (CP)-induced endothelial injury by treatment with human umbilical cord-derived endothelial/mesenchymal stem cells (hEPCs). We randomly assigned 16 to 20-week-old female NOD/SCID mice into three separate groups, a hEPC ($1{\times}10^5$ cells) transplanted, 300mg/kg CP treated and saline (control) group. The mice were sacrificed on days 5 and 10 and blood was collected via the abdominal aorta for analysis. The alanine transaminase (ALT), aspartate aminotransferase (AST), serum alkaline phosphatase (s-ALP), and albumin (ALB) levels were then evaluated. Tissue sections from the livers and kidneys were stained with hematoxylin and eosin (HE) for microscopic analysis and were subjected to immunohistochemistry to evaluate any changes in the endothelial layer. CP treatment caused a weight reduction after one day. The kidney/body weight ratio increased in the hEPC treated animals compared with the CP only group at 10 days. Moreover, hEPC treatment resulted in reduced s-ALP, AST, ALT levels compared with the CP only group at 10 days. The CP only animals further showed endothelial injuries at five days which were recovered by hEPC treatment at 10 days. The number of CD31-positive cells was increased by hEPC treatment at both 5 and 10 days. In conclusion, the CP-induced disruption of endothelial cells is recovered by hEPC treatment, indicating that hEPC transplantation has potential benefits in the treatment of endothelial damage.

Objectives: Oral and maxillofacial surgeons must gain mastery of various approaches to the midface due to the increasing incidence, complexity, and severity of presenting midfacial fractures. Unlike in the case of other body parts, the need to preserve facial aesthetics makes it more difficult for the surgeon to select an approach for managing the facial injuries. The midfacial degloving (MFD) approach is a combination of intraoral and intranasal incisions made to access the midface without any external incision. The aim of the present study was to evaluate the efficacy of MFD in maxillofacial surgery and to assess its advantages and complications. Materials and Methods: The MFD approach was used in five cases, with three cases treated with open reduction and internal fixation and two cases operated on for posttraumatic deformity. Nasal dorsum augmentation was completed in three cases and nasal osteotomy was performed in one case. The bicoronal flap technique was combined with MFD for frontal bone augmentation in one case. The intraoperative time required for flap completion and the ease of performing the planned procedures were noted. Postoperative evaluation was done for reduction, aesthetics, function, and complications. Results: Access was excellent for performing all planned procedures. Average time spent for flap elevation and exposure of the midface was 63 minutes. Complications like postoperative swelling, infraorbital nerve paresthesia, and intranasal crusting were all transient. No long-term complications like stenosis of the nose, sneer deformity, or weakness of the facial muscles were noticed. Additionally, no complications were noted when MFD was combined with bicoronal flap. Conclusion: Though the MFD approach is technically demanding and takes more time than other facial approaches, it should be learned and applied by maxillofacial surgeons in selective cases, as it provides complete exposure of the midface without facial scarring.
Traumatic injuries of the face often involve root fractures especially in anterior teeth. The prognosis and the treatment of the root fracture depend on the extent of the fracture line, general health and patient compliance. This case report outlines a new conservative trial treatment modality to stabilize the maxillary central incisors with horizontal root fracture on the cervical to middle third by fabricating canine guidance to remove loading on the traumatized maxillary central incisors during eccentric movements and thus inducing spontaneous healing of the fractured line between the fragments. Radiographs after thirty months showed adequate healing with no signs of pathological changes including root resorption, ankylosis or displacement. Long term follow-up revealed that vitality, stability and aesthetics were maintained and the patient was satisfied with the outcome.
Background: Head or scalp injury is a life-threatening and typically accidental human injury. Most medical departments require immediate medical treatment and proper treatment with specialized medical personnel and facilities. However, in low-resource environments, such as the rural region of West Africa, the authors have treated emergency trauma patients and provided immediate treatment despite lack of resources. Case presentation: We reviewed three cases of scalp injury patients, with representative clinical information, and used these cases to outline feedback on scalp trauma treatment based on the specialty knowledge of general and emergency surgeon. Conclusions: Oral and maxillofacial surgeons are medical specialists that can immediately diagnose and treat these scalp injuries based on their medical knowledge and experience with the maxillofacial region.

The present study was performed to evaluate the function of the mandible according to the pattern of fracture and treatment methods of condylar fractures of the mandible and help operators in making a treatment plan. Sixty patients (average follow-up period was $7.8{\pm}9.4$ months) who were treated for condylar fracture from June, 2002 to May, 2006 at the Department of Oral and Maxillofacial surgery, Chonnam National University Hospital were reviewed. The common causes of the condylar fracture were traffic accident and fall-down (35.0%). In concomitant injuries, laceration was 46.7% and the fracture of the mandibular symphysis was highest incidence (60.0%). The common site of the fracture was the condylar head (47.8%), followed by subcondyle (36.2%) and condylar neck (15.9%). Under 15 years old patients, the closed reduction was performed in 87.5% out of the patients. All of the condylar fragments were fixed to the mandible with titanium miniplates in cases of open reduction. The mean period of intermaxillary fixation (IMF) was $14.2{\pm}6.5$ days in closed reduction and $10.0{\pm}4.2$ days in open reduction. The old patient with bilateral condylar head fractures, who were treated by closed reduction with IMF for 3 weeks, showed the limitation of mandibular movements. But, there was no significant different results between open reduction and closed reduction with the respect of the Helkimo's mandibular mobility index and clinical dysfunction index (DI). Complications, such as fibrous ankylosis and resorption of the mandibular condyle, were not observed in all patients. These results suggest that the good results can be obtained by closed reduction with proper IMF periods and functional exercise in most condylar fractures of the mandible except severely displaced extracapsular fractures.
