Purpose. The aim of the study was to compare the lingualized implant placement creating a buccal cantilever with prosthetic-driven implant placement exhibiting excessive crown-to-implant ratio. Materials and Methods. Based on patient's CT scan data, two finite element models were created. Both models were composed of the severely resorbed posterior mandible with first premolar and second molar and missing second premolar and first molar, a two-unit prosthesis supported by two implants. The differences were in implants position and crown-to-implant ratio; lingualized implants creating lingually overcontoured prosthesis (Model CP2) and prosthetic-driven implants creating an excessive crown-to-implant ratio (Model PD2). A screw preload of 466.4 N and a buccal occlusal load of 262 N were applied. The contacts between the implant components were set to a frictional contact with a friction coefficient of 0.3. The maximum von Mises stress and strain and maximum equivalent plastic strain were analyzed and compared, as well as volumes of the materials under specified stress and strain ranges. Results. The results revealed that the highest maximum von Mises stress in each model was 1091 MPa for CP2 and 1085 MPa for PD2. In the cortical bone, CP2 showed a lower peak stress and a similar peak strain. Besides, volume calculation confirmed that CP2 presented lower volumes undergoing stress and strain. The stresses in implant components were slightly lower in value in PD2. However, CP2 exhibited a noticeably higher plastic strain. CONCLUSION. Prosthetic-driven implant placement might biomechanically be more advantageous than bone quantity-based implant placement that creates a buccal cantilever.
Materials with differing surfaces have been developed for clinical implant therapy in dentistry and orthopedics. This study was designed to evaluate bone response to titanium alloy containing Ti-32Nb-5Zr with nanostructure, anodic oxidation, heat treatment, and ibandronate coating. Rats were randomly assigned to two groups for implantation of titanium alloy (untreated) as the control group and titanium alloy group coated with ibandronate as the experimental group. Then, the implants were inserted in both tibiae of the rats for four weeks. After implantation, bone implant interface, trabecular microstructure, mechanical fixation was evaluated by histology, micro-computed tomography (${\mu}CT$) and the push-out test, respectively. We found that the anodized, heat-treated and ibandronate-coated titanium alloy triggered pronounced bone implant integration and early bone formation. Ibandronate-coated implants showed elevated values for removal torque and a higher level of BV/TV, trabecular thickness and separation upon analysis with ${\mu}CT$ and mechanical testing. Similarly, higher bone contact and a larger percentage bone area were observed via histology compared to untreated alloy. Furthermore, well coating of ibandronate with alloy was observed by vitro releasing experiment. Our study provided evidences that the coating of bisphosphonate onto the anodized and heat-treated nanostructure of titanium alloy had a positive effect on implant fixation.
Radiographic planning is needed for implant placement in order to determine implant length, jaw bone volume, anatomical stucture and so on. Radiographic examination includes conventional radiography, conventional tomography and CT scan. The most accurate mesurement can be obtained from CT scan. For the cross-sectional view of mandible, CT scan reconstruction is generally needed. But the cross-sectional view of mandible can be reformed by personal computer. This study was performed to examine the clinical usefulness of reformed image using personal computer in comparison with CT scan reconstructed image. CT axial slices of 4 mandibles of 4 volunteers were used. Digital imaging system was composed of Macintosh Ⅱ ci computer, high resolution Sony XC-77 CCD camera, Quick Capture frame grabber board and 'NIH Image' program. Seven reconstructed cross-sectional images within CT machine(CT group) were obtained. And seven reformed cross-sectional images(PC group) after digitization of CT axial slices into the personal computer were obtained. PC group was compared with CT group in the objective and subjective aspects. The results were as follow: 1. Measurement of mandibular height & width in both group showed insignificant difference(P>0.05). 2. Subjective assessment of the mandibular canal in both group showed insignificant difference(P>0.05). 3. Image reformation using personal computer could provide panoramic view, which could not be obtained in CT scan reconstruction.
Purpose: This study investigated the accuracy of free-hand implant surgery performed by an experienced operator compared to static guided implant surgery performed by an inexperienced operator on an anterior maxillary dental model arch. Methods: A maxillary dental model with missing teeth (No. 11, 22, and 23) was used for this in vitro study. An intraoral scan was performed on the model, with the resulting digital impression exported as a stereolithography file. Next, a cone-beam computed tomography (CBCT) scan was performed, with the resulting image exported as a Digital Imaging and Communications in Medicine file. Both files were imported into the RealGUIDE 5.0 dental implant planning software. Active Bio implants were selected to place into the model. A single stereolithographic 3-dimensional surgical guide was printed for all cases. Ten clinicians, divided into 2 groups, placed a total of 60 implants in 20 acrylic resin maxillary models. Due to the small sample size, the Mann-Whitney test was used to analyze mean values in the 2 groups. Statistical analyses were performed using SAS version 9.4. Results: The accuracy of implant placement using a surgical guide was significantly higher than that of free-hand implantation. The mean difference between the planned and actual implant positions at the apex was 0.68 mm for the experienced group using the free-hand technique and 0.14 mm for the non-experienced group using the surgical guide technique (P=0.019). At the top of the implant, the mean difference was 1.04 mm for the experienced group using the free-hand technique and 0.52 mm for the non-experienced group using the surgical guide technique (P=0.044). Conclusions: The data from this study will provide valuable insights for future studies, since in vitro studies should be conducted extensively in advance of retrospective or prospective studies to avoid burdening patients unnecessarily.

Purpose: Bone-to-implant contact (BIC) is difficult to measure on micro-computed tomography (CT) because of artifacts that hinder accurate differentiation of the bone and implant. This study presents an advanced algorithm for measuring BIC in micro-CT acquisitions using a spiral scanning technique, with improved differentiation of bone and implant materials. Methods: Five sandblasted, large-grit, acid-etched implants were used. Three implants were subjected to surface analysis, and 2 were inserted into a New Zealand white rabbit, with each tibia receiving 1 implant. The rabbit was sacrificed after 28 days. The en bloc specimens were subjected to spiral (SkyScan 1275, Bruker) and round (SkyScan 1172, SkyScan 1275) micro-CT scanning to evaluate differences in the images resulting from the different scanning techniques. The partial volume effect (PVE) was optimized as much as possible. BIC was measured with both round and spiral scanning on the SkyScan 1275, and the results were compared. Results: Compared with the round micro-CT scanning, the spiral scanning showed much clearer images. In addition, the PVE was optimized, which allowed accurate BIC measurements to be made. Round scanning on the SkyScan 1275 resulted in higher BIC measurements than spiral scanning on the same machine; however, the higher measurements on round scanning were confirmed to be false, and were found to be the result of artifacts in the void, rather than bone. Conclusions: The results of this study indicate that spiral scanning can reduce metal artifacts, thereby allowing clear differentiation of bone and implant. Moreover, the PVE, which is a factor that inevitably hinders accurate BIC measurements, was optimized through an advanced algorithm.
PURPOSE. Template-guided implant therapy has developed hand-in-hand with computed tomography (CT) to improve the accuracy of implant surgery and future prosthodontic treatment. In our present study, the accuracy and causative factors for computer-assisted implant surgery were assessed to further validate the stable clinical application of this technique. MATERIALS AND METHODS. A total of 102 implants in 48 patients were included in this study. Implant surgery was performed with a stereolithographic template. Pre- and post-operative CTs were used to compare the planned and placed implants. Accuracy and related factors were statistically analyzed with the Spearman correlation method and the linear mixed model. Differences were considered to be statistically significant at $P{\leq}.05$. RESULTS. The mean errors of computer-assisted implant surgery were 1.09 mm at the coronal center, 1.56 mm at the apical center, and the axis deviation was $3.80^{\circ}$. The coronal and apical errors of the implants were found to be strongly correlated. The errors developed at the coronal center were magnified at the apical center by the fixture length. The case of anterior edentulous area and longer fixtures affected the accuracy of the implant template. CONCLUSION. The control of errors at the coronal center and stabilization of the anterior part of the template are needed for safe implant surgery and future prosthodontic treatment.
Computer-guided system은 술 전에 임플란트 위치를 계획하고 이와 일치하도록 구강 내에 임플란트를 식립할 수 있게 하는 방법이다. 하지만 이렇게 임플란트를 식립한다 할지라도 실제 매식된 임플란트의 위치는 원래 계획하였던 위치와 차이가 있을 수 있다. 이 연구의 목적은 실제 임상에서 computer guided system을 이용하여 임플란트를 식립한 환자들의 경우 계획한 위치와 실제 식립된 임플란트 사이에 발생하는 변위량의 범위를 알아보고 그 임상적 적합성을 평가하는 것이다. 'NobelGuide' system (Nobel Biocare AB, G$\ddot{o}$teborg, Sweden)을 이용하여 Br${\aa}$nemark MK III Groovy RP (Nobel Biocare AB, G$\ddot{o}$teborg, Sweden)임플란트 식립을 시행 받은 다섯 명의 환자를 선정하였다. 수술용 형판에 지대주 유사체를 연결한 후 술 전 측정 모형을 제작하였고 최종 보철물 제작 시 최종 인상을 채득하여 술 후 측정 모형을 제작하였다. 두 측정 모형의 CT 방사선 사진을 촬영 후 3차원적으로 재현하였고 재현된 모델 상에서 식립된 임플란트 위치를 지정하였다. 각 임플란트는 임플란트 경부와 첨단의 중심점을 연결하여 임플란트 축을 설정하였으며 두 축 간의 각도가 측정되었다. 임플란트 간 거리는 각 임플란트의 경부에서, 설정된 임플란트 축이 지나가는 중심점 간의 거리를 측정하였다. 총 5명 환자의 58개 부위의 임플란트 간 술 전과 술 후 임플란트 거리와 각도 변위량이 기록되었으며 평균 및 최대 변위값을 산출하였다. 술 전과 술 후 임플란트 위치 간 거리의 변위량은 평균 0.41 mm였고 최대 1.7 mm의 범위 하에 있었다. 술 전과 술 후 임플란트 간 위치의 각도의 변위량은 평균 $1.99^{\circ}$를 나타냈으며 최대 각도 변위량은 $6.7^{\circ}$를 나타내었다. 술 전 계획된 임플란트와 술 후 식립된 임플란트 간의 길이와 각도에 따른 평균 변위량은 computer-guided implant system을 실제 임상에 적용하는 데 있어 큰 문제가 존재하지 않고 '수동적 적합(passive fit)'을 얻기에 무리가 없을 허용 가능할 만한 값을 나타냈다.
When performing the inferior alveolar nerve block anesthesia, surgeon often faced a difficulty of the surgical operation due to the incomplete anesthesia. One of the reason is the variety of mandibular canal anatomy. Up to now, there are some reports of index cases about bifid mandibular canal among mandibular canal anatomic variation, and some classification is applied according to anatomical location and configuration. When surgical operation is performed involving mandible such as dantal implant treatment, extraction of an impacted third molar, sagittal split ramus osteotomy, etc, the position of mandibular canal should be considered. Bifid mandibular canal clinically causes troublesome cases of anesthesia when inferior alvelor nerve block, especially is performed extraction of an impacted third molar. Therefore, It is important for clinicians to recognize the presence of bifid canals on radiographys. Nowadays, the position of mandibular canal can be measured precisely by using Dental CT. It is not found by panorama image but is found by Dental CT sometimes. Among the patients, which take panorama and Dental CT simultaneously, for tooth extraction of lower impacted third molar in our department, we report the case that did not identifying in panorama but identifying it in Dental CT.
Purpose: To compare the visibility of the mandibular canal at the different radiographic methods such as conventional panoramic radiographs, Vimplant multi planar reformatting (MPR)-CT panoramic images, Vimplant MPR-CT paraxial images and film-based DentaScan MPR-CT images. Materials and Methods: Data of 11 mandibular dental implant patients, who had been planned treatment utilizing both panoramic and MPR-CT examination with DentaScan software (GE Medical systems, Milwaukee, USA), were used in this study. The archived axial CT data stored on CD-R discs were transferred to a personal computer with 17' LCD monitor. Paraxial and panoramic images were reconstructed using Vimplant software (CyberMed Inc., Seoul, Korea). Conventional panoramic radiographs, monitor-based Vimplant MPR-CT panoramic images, monitor-based Vimplant MPR-CT paraxial images, and film-based DentaScan MPR-CT images were evaluated for visibility of the mandibular canal at the mental foramen, 1 cm, 2 cm, and 3 cm posterior to mental foramen using the 4-point grading score. Results: Vimplant MPR-CT panoramic, paraxial, and DentaScan MPR-CT images revealed significantly clearer images than conventional panoramic radiographs. Particularly at the region 1 em posterior to mental foramen, conventional panoramic radiographs showed a markedly lower percentage of 'excellent' mandibular canal images than images produced by other modalites. Vimplant MPR-CT and DentaScan MPR-CT images did not show significant difference in visibility of the mandibular canal. Conclusion: The study results show that Vimplant and DentaScan MPR-CT imaging systems offer significantly better images of the mandibular canal than conventional panoramic radiograph.
Several FE models were developed based on micro-CT images of a mandibular specimen. A new dental implant model was suggested from parameter study for the relation between shape and interfacial stress of dental implants. It is found that the proposed model is highly beneficial.
