하악 정중부와 양측 체부에 걸친 botryoid odontogenic cyst: 증례보고 및 문헌고찰 (Botryoid Odontogenic Cyst on Mandibular Anterior and Both Body Area: a Case Report)

  • Botryoid odontogenic cyst (BOC) is considered a rare multilocular variant of the lateral periodontal cyst. In this report, a 67-year-old male visited with chief complaint of severe mobility on mandibular incisors. Multilocular radiolucent lesion was seen from the right premolar to the left premolar area, involving almost the whole mandible. Histologically, the botryoid odontogenic cyst showed focal nodular thickening of the lining epithelium. These thickening often showed swirling appearance of the cells. Cyst enucleation and bone graft on mandible anterior and both body area were performed under general anesthesia, and postoperative healing was favorable without recurrence.

Simultaneous placement of an interpositional free gingival graft with nonsubmerged implant placement

  • Purpose: Various surgical techniques target achieving adequate keratinized tissue around dental implants; however, these techniques are usually performed before implant placement or upon the exposure of submerged implants. The aim of this case report is to describe a simultaneous placement of an interpositional free gingival graft (iFGG) with that of nonsubmerged implants in a patient lacking keratinized tissue and to assess the longterm outcome of this grafted gingiva. Methods: A wedge-shaped free gingnival graft (FGG), including an epithelium-connective tissue (E-C) portion and a connective-tissue-only (CT) portion, was harvested from the palate. The CT portion was inserted under the buccal flap, and the E-C portion was secured tightly around the implants and to the lingual flap. Results: At the 8-year follow-up, the gingival graft remained firmly attached and was well maintained, with no conspicuous shrinkage or reported discomfort during oral hygiene procedures. The use of an iFGG at a nonsubmerged implant placement minimizes the required number of surgical steps and patient discomfort while providing adequate buccal keratinized tissue. Conclusions: Therefore, the technique could be considered an alternative method in increasing the keratinized tissue for cases that have a minimal amount of keratinized tissue.

  • 상처회복은 염증, 재상피화 그리고 기질의 재형성등이 관여하는 복잡한 과정이다. 이중 재상피화에 관여하는 각질화세포의 이동경로를 분석하기 위하여 무당개구리 피부 상처유도 후 투과전자현미경과 cytokeratin에 대한 조직면역화학법을 이용하였다. 정상조직의 cytokeratin발현은 기저층의 세포들과 선상피에서 확인되었다. 상처 유도후 3시간 조직에서, 기저세포층에서 강한 반응이 관찰되었고, 1일과 2일 사이에서는 재생되는 각질화세포에서 강한 면역반응이 확인되었다. 상처반응 중기인 7일부터 10일 사이에서도 재생된 세포의 기저층세포에서 강한 반응이 일어났다. 19일경과의 조직에서는 기저층과 유극층의 세포들에서 cytokeratin의 발현이 증가하였다. 따라서, 재상피화에 관여하는 각질화세포는 기저층의 세포로부터 시작하여 상처부위로 이동하여 과립층과 유극층으로 분화됨으로서 재상피가 진행되었다.

Transactivators for the Odontoblast-specific Gene Targeting

  • Dentin, a major component of teeth, is formed by odontoblasts which produce the dentin matrix beneath the dental epithelium and induce the mineralization of dentin. To date, the biochemical properties of dentin matrix proteins have been well characterized, but upstream regulators of these proteins are not yet well known. Recently in this regard, several transcription factors have been identified as potential regulators of matrix proteins. Most transcription factors are generally involved in diverse biological processes and it is essential to identify those that are odontoblast-specific transactivators to further understand the process of dentin formation. We thus analyzed the expression pattern of dentin matrix proteins and the activities of established transactivators containing a Cre-locus. Expression analyses using in situ hybridization showed that dentin matrix proteins are sequentially expressed in differentiating odontoblasts, including type-I collagen, Dmp-1 and Dspp. The activities of the transactivators were evaluated using ${\beta}$-galactosidase following the generation of double transgenic mice with each transactivator and the ROSA26R reporter line. The ${\beta}$-galactosidase activity of each transactivator paralled the expression of the matrix proteins. These results thus showed that these transactivators could be utilized for odontoblastspecific conditional gene targeting. In addition, time- and tissue-specific conditional gene targeting might also be achieved using a combination of these transactivators. Odontoblast-specific conditional gene targeting with these transactivators will likely also provide new insights into the molecular mechanisms underlying dentin formation.

  • Enamel knot (EK)-a signaling center-refers to a transient morphological structure comprising epithelial tissue. EK is believed to regulate tooth development in early organogenesis without its own cellular alterations, including proliferation and differentiation. EKs show a very simple but conserved structure and share functions with teeth of recently evolved vertebrates, suggesting conserved signaling in certain organs, such as functional teeth, through the course of evolution. In this study, we examined the expression patterns of key EK-specific genes including Dusp26, Fat4, Meis2, Sln, and Zpld1 during mice embryogenesis. Expression patterns of these genes may reveal putative differentiation mechanisms underlying tooth morphogenesis.

소구치에 발생한 Dens Evaginatus의 임상적 연구 (A Clinical Study of Dens Evaginatus in the Premolars)

  • The dens evaginatus was a developmental variation which has arisen as a result of an evagination of inner enamel epithelium into the enamel organ. It has been given various. names by authors and was thought to be confined to Mongolian race. This study was performed to observe the incidence of dens evaginatus, and its ill-effects on the teeth and surrounding structures in 6356 Korean students and 10227 Korean adults. In plaster model, analysis was performed in accordance with forms and location of dens evaginatus on the. occlusal surface in the premolars. The pathologic changes caused by dens evaginatus were observed in paralleling periapical radiograms The results were as follows: 1. The prevalence of dens evaginatus in the student's group was 2.6%, and showed no sex predilection in the occurrence of evaginated teeth. 2. The sequence of dens evaginatus was in order of mandibular 2nd premolar, mandibular 1st premolar, maxillary 2nd premolar, and maxillary 1st premolar, respectively. 3. Of the cases with dens evaginatus, 73.5 % occured bilaterally. 4. The nipple form was the most frequent in respect of elevation of tubercle on the, occlusal surface. 5. In the base form of the tubercle, the occurrence of grooved form was the highest. 6. In the maxilla, those cases which the tubercle arose from the lingual ridge of the buccal cusp were most predominant. And in the mandible, those cases which the tubercle arose from the center of the occlusal surface were the most frequent. 7. The pulpal and periapical complications were shown in 24.4% of evaginated teeth in. periapical radiogram.

Influence of Helicobacter pylori Infection on Endoscopic Findings of Gastric Adenocarcinoma of the Fundic Gland Type

  • Purpose: Gastric adenocarcinoma of the fundic gland type (chief cell predominant type) (GA-FG-CCP) was first reported as a rare adenocarcinoma found in the normal fundic mucosa. Recent studies have proposed the possibility that GA-FG-CCPs were also generated in the atrophic mucosa after Helicobacter pylori (HP) eradication therapy. However, little is known on the endoscopic findings of GA-FG-CCP generated in the atrophic mucosa due to its extreme rarity. Materials and Methods: A total of 8 patients who underwent endoscopic submucosal resection and were diagnosed with GA-FG-CCP generated in the HP-uninfected mucosa (4 cases, HP-uninfected group) or HP-eradicated atrophic mucosa (4 cases, HP-eradicated group) were retrospectively analyzed, and their endoscopic findings, including magnifying endoscopy with narrow band imaging (M-NBI), and pathological features were compared. Results: While GA-FG-CCPs in the 2 groups displayed similar macroscopic appearance, M-NBI demonstrated that characteristic microvessels (tapered microvessels like withered branches) were specifically identified in the HP-eradicated group. Pathological investigation revealed that a decreasing number of fundic glands and thinned foveolar epithelium covering tumor ducts were thought to lower the thickness of the covering layer over tumor ducts in the HP-eradicated group. Moreover, dilation of vessels just under the surface of the lesions contributed to the visualization of microvessels by M-NBI. Conclusions: The change in background mucosa due to HP infection influenced the thickness of the covering layer over the tumor ducts and M-NBI finding of GA-FG-CCP.

Pathogenesis and prognosis of primary oral squamous cell carcinoma based on microRNAs target genes: a systems biology approach

  • Oral squamous cell carcinoma (OSCC) is the most prevalent head and neck malignancy, with frequent cervical lymph-node metastasis, leading to a poor prognosis in OSCC patients. The present study aimed to identify potential markers, including microRNAs (miRNAs) and genes, significantly involved in the etiology of early-stage OSCC. Additionally, the main OSCC's dysregulated Gene Ontology annotations and significant signaling pathways were identified. The dataset GSE45238 underwent multivariate statistical analysis in order to distinguish primary OSCC tissues from healthy oral epithelium. Differentially expressed miRNAs (DEMs) with the criteria of p-value < 0.001 and |Log2 fold change| > 1.585 were identified in the two groups, and subsequently, validated targets of DEMs were identified. A protein interaction map was constructed, hub genes were identified, significant modules within the network were illustrated, and significant pathways and biological processes associated with the clusters were demonstrated. Using the GEPI2 database, the hub genes' predictive function was assessed. Compared to the healthy controls, main OSCC had a total of 23 DEMs. In patients with head and neck squamous cell carcinoma (HNSCC), upregulation of CALM1, CYCS, THBS1, MYC, GATA6, and SPRED3 was strongly associated with a poor prognosis. In HNSCC patients, overexpression of PIK3R3, GIGYF1, and BCL2L11 was substantially correlated with a good prognosis. Besides, "proteoglycans in cancer" was the most significant pathway enriched in the primary OSCC. The present study results revealed more possible mechanisms mediating primary OSCC and may be useful in the prognosis of the patients with early-stage OSCC.

Histological analysis on tissues around orthodontically intruded maxillary molars using temporary anchorage devices: A case report

  • Before progress was recently made in the application of temporary anchorage devices (TADs) in bio-mechanical design, orthodontists were rarely able to intrude molars to reduce upper posterior dental height (UPDH). However, TADs are now widely used to intrude molars to flatten the occlusal plane or induce counterclockwise rotation of the mandible. Previous studies involving clinical or animal histological evaluation on changes in periodontal conditions after molar intrusion have been reported, however, studies involving human histology are scarce. This case was a Class I malocclusion with a high mandibular plane angle. Upper molar intrusion with TADs was performed to reduce UPDH, which led to counterclockwise rotation of the mandible. After 5 months of upper molar intrusion, shortened clinical crowns were noticed, which caused difficulties in oral hygiene and hindered orthodontic tooth movement. The mid-treatment cone-beam computed tomography revealed redundant bone physically interfering with buccal attachment and osseous resective surgeries were followed. During the surgeries, bilateral mini screws were removed and bulging alveolar bone and gingiva were harvested for biopsy. Histological examination revealed bacterial colonies at the bottom of the sulcus. Infiltration of chronic inflammatory cells underneath the non-keratinized sulcular epithelium was noted, with abundant capillaries being filled with red blood cells. Proximal alveolar bone facing the bottom of the gingival sulcus exhibited active bone remodeling and woven bone formation with plump osteocytes in the lacunae. On the other hand, buccal alveolar bone exhibited lamination, indicating slow bone turnover in the lateral region.

성견에서 수직적 골결손부에 골이식후 교정력 적용시기에 따른 치주조직의 반응 (Periodontal Response According to the Timing of Orthodontic Force Application alter Bone Graft into Angular Bony Defect in the Dog)

  • 본 연구는 수직적 골결손부에 골이식을 시행한 후 교정력 적용시기에 따른 치주조직의 반응을 알아보고자 시행하였다. 체중 15 kg내외의 성견 9마리에서 상악 좌우측 제 3절치의 원심 치근면에 수직적 골결손을 형성하고 치주질환을 유발시킨 2주 후 치주판막술을 시행하고 골결손 기저부 수준의 치근면에 기준 홈을 형성하였다. 골이식 후 치유기간에 따라 2주군, 4주군, 6주군으로 각각 3마리씩 분류하고 우측 제 3절치는 치주판막술만 하는 비이식측으로, 좌측 제 3절치는 치주판막술과 골이식을 병용한 이식측으로 설정하여 양측 모두 술후 2주, 4주, 또는 6주부터 120 gm의 교정력을 적용하여 8주 동안 치체이동 시킨 후 2주간 보정하였다. 이후 동물을 희생시키고 조직표본을 만든 후 치주조직의 반응을 비교 관찰하여 다음과 같은 결과를 얻었다. 1. 모든 실험동물에서 비이식측에 비해 이식측의 골재생양이 많이 나타났다. 2. 6주군에서는 홈에서 백악-법랑 경계부까지 거리의 1/2이상 치조골과 백악질이 재생되었고 상피의 하방증식없이 결합조직대가 치근면과 부착을 이루었다. 3. 4주군에서는 5주군과 유사한 치주조직 재생소견을 보였으나 그 양은 더 적게 나타났다. 4. 2주군의 경우 골재생이 양측 모두 홈에 한정되어 나타났으며 상피의 하방증식이 홈의 기저부까지 연장되고 백악질과 결합조직의 재생은 관찰되지 않았다. 이상의 결과는 치주질환에 의한 수직적 골결손 환자에서 치주판막술과 골이식을 시행하고 교정적 치아이동을 할 경우 교정력 적용시기에 따른 치주조직의 반응이 차이가 있음을 시사하였다.
