A Strategy Toward Reconstructing the Healthcare System of a Unified Korea

  • This road map aims to establish a stable and integrated healthcare system for the Korean Peninsula by improving health conditions and building a foundation for healthcare in North Korea through a series of effective healthcare programs. With a basic time frame extending from the present in stages towards unification, the roadmap is composed of four successive phases. The first and second phases, each expected to last five years, respectively, focus on disease treatment and nutritional treatment. These phases would thereby safeguard the health of the most vulnerable populations in North Korea, while fulfilling the basic health needs of other groups by modernizing existing medical facilities. Based on the gains of the first two phases, the third phase, for ten years, would prepare for unification of the Koreas by promoting the health of all the North Korean people and improving basic infrastructural elements such as health workforce capacity and medical institutions. The fourth phase, assuming that unification will take place, provides fundamental principles and directions for establishing an integrated healthcare system across the Korean Peninsula. We are hoping to increase the consistency of the program and overcome several existing concerns of the current program with this roadmap.

A Study of Predictive Factors Affecting Health: Promoting Behaviors of North Korean Adolescent Refugees

  • Objectives: The present study aimed to analyze the factors that could affect the health-promoting behaviors of North Korean adolescent refugees residing in South Korea. Methods: Questions about their sociodemographic variables, subjective health status, healthy living habits, and health-promoting behaviors were asked. Results: Statistically significant differences were found in religion (t=2.30, p<0.05), having family members in South Korea (t=2.02, p<0.05), and subjective health status (t=4.96, p<0.01). Scores on health-responsible behaviors were higher with higher age (t=2.90, p<0.01) and for subjects without family or friends (t=2.43, p<0.05). Higher physical-activity behaviors were observed in males (t=3.32, p<0.01), in those with better subjective health status (t=3.46, p<0.05) and lower body mas index (t=3.48, p<0.05), and in smokers (t=3.17, p<0.01). Nutritional behaviors were higher in those who followed a religion (t=2.17, p<0.05). Spiritual growth behaviors were higher in those who followed a religion (t=4.21, p<0.001), had no family in South Korea (t=2.04, p<0.05), and had higher subjective health status (t=5.74, p<0.01). Scores on interpersonal relationships and stress-management behaviors were higher for those with higher subjective health status. A multiple regression analysis showed greater effects on health-promoting behaviors when subjective health status was better. Older people and non-smokers exhibited more health-responsible behaviors, while more physical-activity behaviors and spiritual growth activities were observed when subjective health status was better. Interpersonal relationship behaviors had positive effects on those with good subjective heath status and on non-smokers. Conclusions: Based on the results of the current study, an alternative was suggested for promoting health in North Korean adolescent refugees.

장애 권리 기반한 국제협력: 북한 관련하여 (Disability-Rights Based International Cooperation: With Some References to North Korea)

  • 본 논문은 2006년 유엔 장애인권리협약의 맥락에서 사회복지 관점으로부터 인권 및 장애인 권리의 현주소를 탐구하려고 시도하였다. 전반적인 논의는 일반적인 인권과 특히 장애인권의 과제들에 대한 과감한 해결이 어려운 북한의 상황에 초점이 맞춰져 있다. 유엔 장애인 권리협약은 모든 비준 당사국이 협약을 이행하기 위해 법제도의 개혁과 조화, 정책 및 프로그램 개발을 시작하도록 촉구한다. 남북한도 모두 예외가 아니다. 북한 인권에 관해서는 유엔 조사위원회에 의하지 않더라도 그 비참한 상황은 잘 드러나 있다. 그러나 이 논문에서는 한국의 인권이 북한보다 우월하다고 보는 것은 아니다. 이 논문은 그 문제에 대해 남북한과 그밖에 다른 나라들에게 공통된 추가 조치를 위한 영역들을 제시하였다. 장애인 권리에 관한 일반적인 논의와 별개로 본 논문의 특별한 기여는 북한에 관한 최신 정보와 자료를 도출하려고 노력한 사실에 있다. 그것은 유엔과 북한 자체로부터 나온 여러 출처에 의존하였다. 북한 장애인 당국은 장애인의 인권 개선을 위해 외부로부터의 도움을 청하려는 열의로 매우 노력하고 있음을 알 수 있다. 또한 그것은 재정적 지원 및 물질적 지원에 대한 국제적 협력의 많은 필요성을 보여준다. 본 논문은 유엔장애권리협약 제32조가 규정하는 국제협력의 기치 아래, 남북한의 장애인단체간 협력에 긍정적 신호로 평화와 안정을 위한 단계적 조치를 취하는 남북한 간의 최근 정치발전에 주목하고 있다. 보다 비판적으로, 본 논문에서는 균형 잡힌 법제도 개혁, 정책 개발을 보장하며 국제 협력 분야를 선명하게 하기 위해 전반적인 데이터 기반을 개선해야 할 필요성을 지적하였다.

북한 보건의료인력개발을 위한 제언 (A Proposal for the Development of Personnel in the DPRK for Public Health and Medicine)

  • The government of South Korea and its medical personnel must make a way by which health professionals who have escaped from the Democratic People's Republic of Korea (DPRK) can play a positive and practical role in unification and south-north medical unification while south-north authority talks on DPRK public health and medicine manpower development are not going smoothly. Medical personnel escaped from the DPRK have to be recruited for the interviewer of the national examination, to improve the accuracy of national examination interviews. For those medical professionals who have escaped from the DPRK with 6 years' medical college education, but failed the interview on the national examination, we propose here a course of 3.6 months for them to have a right to apply the Korean Medical Licensing Examinations (KMLE). We also propose that medical professionals who have escaped from the DPRK who have graduated from a 6-year medical college in the DPRK and who are medical doctors over the fifth grade or with more than 6 years of experience can be qualified as unification medical doctors and be exempted from the KMLE, getting the right to go directly into an internship and residency. They should be permitted to work in manpower development projects for the health professions. They should also be given opportunities such as to become psychiatrists who treat the mental illness of persons escaped from the DPRK and people from North Korea after unification. Medical students in South Korea should earn college credits on the topic of medical unification and not only students, but all South Korean medical personnel, should prepare for north-south medical unification with an open mind. A way for each medical college to participate in DPRK manpower development for the health professions through a memorandum of understanding between the medical colleges of the south and north.

전환기부터 산업화 시기까지 한국인의 가치 탐색 (Exploring the Values of Korean in the Period of Transitional and Industrial Era)

  • 본 연구는 해방 이후부터 70년대 말 산업화 시기까지 한국인이 중요시 여겼던 가치가 어떻게 변화하고 있는지 탐색하는데 목적이 있다. 두 시기에 대한 구분은 본 연구의 관점에서 볼 때, 한국사회, 혹은 한국인들의 가치체계 변화에 지대한 영향을 미친 역사적 사건을 중심으로 '전환기', '산업화 시기'로 구분하였으며 정치, 경제 및 사회적 요인들을 간략히 살펴봄으로써 시기별 가치를 살펴보고자 하였다. 연구 목적에 따라 시기별로 나타나는 주요가치와 인재상을 도출하고자 하였을 뿐만 아니라 하나의 가치가 어떻게 변모하고 있는지 탐색하였다. 시기별로 나타나는 주요가치와 인재상은 다음과 같다. 전환기의 주요가 치로는 예의와 규범을 제시하였고, 추구하는 인재상은 무실역행(務實力行)하는 자세가 강조되는 실천적 지식이었다. 산업화 시기의 주요가치로는 성실, 근면, 자조, 협동을 제시하였고, 추구하는 인재상은 경제발전에 필수적이고 강력한 직업능력을 갖춘 기능인이었다. 시기별로 지속적으로 탐색한 가치는 애국심이었다. 전환기의 애국심은 초기에는 전통적 지배구조의 붕괴와 지도자의 부재로 인하여 국가와 민족이 충의 대상이었다면 지도자의 선출과 국가의 건립 이후에는 새로운 지배구조가 형성되어 국가보다는 지도자나 상급자에 대한 충성으로 변모하는 경향을 보였다. 산업화 시기 애국심은 국가경제정책을 충실히 이행하고 반공정신으로 무장하여 북한을 압도하는 통일의지를 가지는 것 등으로 비교적 다른 시기에 비하여 목적성이 매우 뚜렷한 경향을 보였다.

통일한국 북측해역 기선설정에 관한 쟁점연구 - 국가승계론을 중심으로 (A Study on the Maritime Baseline of the Unified Korea's Northern Part : Focused on the State-Succession)

  • This study is intended for the Maritime Boundaries(Baseline) of the Unified Korea. According to the international law, North-Korea is recognized as a nation. Then with unification, the state succession will be a very important issue. Thus we should seek for the expected problems and counter-measures, by analyzing of the State-succession. There is a conflict of the positive or negative reaction about the state succession. However, in general, 'the principle of continuity' has been applied for at the national border treaty, regardless of the type of State succession. This can be found on Article 11 & 12 of the 「1978 Vienna Convention on Succession of States in Respect of Treaties」, Article 62, paragraph 2 of the 「1969 Vienna Convention on the Law of Treaties」, and a series of international case law. Currently it is being understood as customary international law. In summary, although South and North Korea, China, Russia and Japan are not the parties of 「1978 Vienna Convention」, the unified Korea will necessarily have a duty to succeed national boundaries(also, maritime baseline) of North Korea. Specifically, we have an objective and rigorous review of the treaty of maritime baseline that is signed between North Korea and neighboring countries, such as 「National Border Treaty Regime between North Korea and China」 and 「Treaty between the Union of Soviet Socialist Republics and the Democratic people's Republic of Korea on the Demarcation of the Soviet-Korea National Border」. Also, we analyzed 'Historical Bays' and 'Straight Baseline' system. By this, we are able to occupy a favorable position when renegotiating with neighboring countries, at the point of unification.

북한가정, 다문화가정과 한국가정 청소년의 건강행태 (Health behavior of North Korean, multicultural and Korean family adolescents in Korea: the Korea Youth Risk Behavior Web-based Survey, 2011-2013)

  • Background: This study investigated the health behaviors of North Korean, multicultural, and Korean family adolescents in Korea. Methods: The study subjects were from the Korea youth risk behavior web-based survey (2011-2013) dataset. We identified 1,954 multicultural family adolescents and twice as many Korean family adolescents. Frequency analysis was used to assess nativity and nationality of the parents, and the chi-square test was used to compare the general characteristics and health behavior of the multicultural and Korean families. Conditional multiple logistic regression was used to compare health behavior between multicultural and Korean families. Results: Of the 5,862 multicultural and Korean family adolescents, current smoking in North Korean family adolescents was higher than Korean family adolescents (adjusted odds ratio [aOR], 2.32; 95% confidence interval [CI], 1.60-3.35) after adjusting for sex, school year, perceived school record, family structure, father's education, mother's education, perceived economic status, place of residence, and survey year. A high drinking rate was significantly greater in North Korean family adolescents compared to Korean family adolescents (aOR, 2.94; 95% CI, 1.89-4.57). Odds ratios for sufficient physical activity of vigorous intensity (aOR, 1.82; 95% CI, 1.31-2.54) were significantly higher in North Korean family adolescents than in Korean family adolescents. Fast food intake (aOR, 1.96; 95% CI, 1.34-2.86) was significantly higher in North Korean family adolescents compared to Korean family adolescents. Conclusion: There were differences in health behavior between North Korean, multicultural, and Korean family adolescents. Further studies are needed to investigate the factors associated with these differences.

자녀를 양육하는 북한이탈 기혼여성의 직장생활 체험에 관한 해석학적 현상학적 연구 (Hermeneutic Phenomenological Study on the Experiences of Employment of Married North Korean Women Defectors Rearing Children)

  • Purpose: This study aimed to understand the experiences of married North Korean women's child-rearing, working lives, and their home and work environment in depth. Methods: This study adopted van Manen's hermeneutic phenomenological method to qualitatively analyze data. The participants were 8 married North Korean women defectors. Data were collected through in-depth interviews and observations from July 4 to August 20, 2018. Results: Nine essential themes emerged: more personal challenges after overcoming a life-threatening crisis; hopes of firmly settling in this land; the wound from the north, which chased them here; a body that becomes stronger through hardship; being stuck in a past full of anxiety and pain; the present is full of hope; hope for the future; sense of alienation from coworkers that cannot be overcome; and sense of power to endure an exhausting work life. Conclusion: This study provided a broader understanding of the life and experiences of married women from North Korea. It highlights the need for nurses to recognize their importance in nursing care. The study also suggests that academic and practical approaches for nursing, and basic data for a nursing intervention for married women from North Korea be provided. The study findings can be used as a basis for preparing a national policy that will help North Korean defectors to find employment and gain stability.

북한이탈주민 출신 간호대학생의 대학생활 경험분석 (The Analysis of College Life Experience of North Korean Defectors Nursing Students)

  • 본 연구의 목적은 북한 이탈주민 출신 간호대학생의 대학 생활경험을 질적 연구방법을 통하여 심층적으로 이해하는 것이다. 2017년 9월부터 10월까지 자신의 대학 생활경험을 풍부하게 나타낼 수 있는 북한 이탈 간호대학생 8명을 표집하여 심층 면접하였다. Colaizzi의 현상학 질적 연구방법으로 분석하여 4가지 주제 묶음과 5가지의 하위범주가 도출되었다. 연구결과 대상자는 대학입학 준비의 어려움을 겪었으며, 남한에 정착하면서 말투의 차이와 남한용어의 이해 부족으로 의사소통의 어려움과 남북한 교육방법과 평가의 차이와 문화적 차이로 어려움에 경험하는 것으로 나타났다. 본 연구를 통하여 간호대학에 입학하는 북한 이탈주민 출신 간호대학생이 대학 생활에 정착하여 학업을 중도에 포기하지 않도록 대학 생활적응을 위한 프로그램의 개발과 적용이 필요할 것으로 판단된다.

What Is It to Be Mentally Healthy from the North Korean Refugees' Perspective?: Qualitative Research on the Changes in Mental Health Awareness among the North Korean Refugees

  • Objective We investigated how mental health awareness among North Korean refugees transformed depending on temporal-spatial context changes. Methods In 2013, we conducted interviews with 10 refugees (eight women) who had been in South Korea for over a year and performed a qualitative analysis of the change in mental health awareness in the differences between living in North Korea, escape (a related period of forced sojourn in a third country), and settlement in South Korea. Results We classified 39 concepts into five main categories. The first two categories (while living in North Korea) were "a mindset for the system, but not for individual mental health" and "being confined in a social environment that was indifferent to mental health." A third category appeared during escape: "focusing on survival amid continuity of intense suffering." The final two categories appeared when settling in South Korea: "recognition of mental health amid cultural shock" and "introspection and sorting oneself out." Conclusion This qualitative study enabled a better multi-dimensional understanding of the social and cultural aspects involved in improving mental health awareness among North Korean refugees in South Korea. It is desirable to integrate mental health as a part of daily life and to expand training for North Korean settlers.
