치매 노인의 구강건강관리실태 및 문제점에 관한 질적 연구 (A qualitative study on the present conditions and problems of oral health care in senile dementia patients)

  • Objectives: The purpose of this study was to investigate the present conditions and problems of oral health care in senior citizens with dementia using a qualitative research method, through focused group interviews. Methods: Data was collected for approximately one month from May 2019. The subjects were divided into two groups: care workers and family caregivers. Fifteen participants were included in the study. Results: In-depth interviews with the care workers revealed the following three categories: characteristics of senile dementia patients, oral health care in senile dementia patients, and oral health care education. In-depth interviews with the family caregivers revealed the following four categories: characteristics of senile dementia patients, oral health care in senile dementia patients, oral health care education, and burden of care. The central themes common to both the care workers and family caregivers were the challenges owing to the characteristics of senile dementia patients, poor health condition of the senile dementia patients, difficulty in oral health care of the senile dementia patients, the desire to receive oral health care education and related information, and to access the information more easily. Additional central themes specific to the care workers were, the applicability of the intervention programs, variability between the facilities, and the problems of oral health care education. An additional central theme specific to the family caregivers was the burden of care. Conclusions: It is necessary to provide oral health care education and information to care workers and family caregivers of senile dementia patients, and to manage and support the dental health professionals ready to care for senile dementia patients. In addition, support to the family caregivers should not be limited only to the financial aspects, but also consider the psychological and emotional difficulties.

요양보호사와 간병인의 치매환자에 대한 구강관리실태 및 노인 구강보건지식 (Oral care status and elderly oral health knowledge of care workers and caregivers with regard to patients with dementia)

  • Objectives: The purpose of this study was to contribute to the development of an oral hygiene care program for patients with dementia by understanding the oral care status and oral health knowledge of care workers and caregivers with regard to patients with dementia. Methods: For about two months from May to June 2018, a survey of care workers and caregivers working in long-term nursing homes and elderly-specialized nursing hospitals was performed, and 442 people were selected for the final analysis. The frequency and percentage were calculated to understand the oral care status and oral health knowledge regarding patients with dementia, and an independent t-test was conducted to determine the difference between the elderly oral health knowledge of care workers and caregivers. Multiple regression analysis was also carried out to examine factors affecting elderly oral health knowledge. Results: With respect to the factor of oral health education in elderly oral health knowledge, oral health knowledge was shown to be highest when participants responded that education was unnecessary (p<0.001), and regarding the will to participate in oral health education, oral health knowledge was highest when participants responded as having no desire to participate (p<0.05). Conclusions: These results suggest that a standard manual should be developed for the accurate and qualitative management of oral hygiene care tasks performed by care workers and caregivers who are in charge of oral care for patients with dementia in order to provide continuous and systematic oral care.

치매안심센터 종사자의 직무만족, 조직몰입 및 이직의도 분석 (Analysis of Job Satisfaction, Organizational Commitment, and Turnover Intention of Dementia Care Center Employees)

  • Purpose : This study aimed to identify the job satisfaction, organizational commitment, and turnover intention of workers at dementia care centers, and the correlations between these factors. Methods : A survey of 131 dementia care center employees was conducted. This comprised 48 questions covering respondent characteristics, organizational satisfaction, organizational commitment, and turnover intention. Results : Employees with no intention to change jobs had significantly higher job satisfaction and organizational commitment than those with job change intentions. Turnover intention was significantly correlated with both job satisfaction and organizational commitment. The higher the organizational commitment and the lower the turnover intention, the higher the job satisfaction. The higher the job satisfaction and the lower the turnover intention, the higher the organizational commitment. Analysis of the sub-components of the main variables found The lower the work environment, compensation, and sense of belonging to the organization, the higher the turnover intention. Conclusion : Regular educational programs are needed to improve the expertise of workers at the dementia care center, and opportunities for employees to communicate their needs should be maximized. In addition, higher salaries will attract more able and willing employees, reduce turnover, and provide and a sense of of belonging to the organization. Attention to the proper distribution of work and recruitment of appropriate human resources will further improve the working environment. If the job satisfaction and organizational commitment of workers at the dementia care center are increased and the number of employees intending to change jobs is reduced, the length of employee service will increase and contribute to improved care and service quality for dementia patients.

치매에 대한 태도 결정요인에 관한 한일 비교: 노인입소시설 케어워커를 대상으로 (A Comparative Study on the Factors that Determine the Attitude Toward Dementia in Korea and Japan: Focusing on the Care Workers Who Work in Nursing Homes)

  • 본 연구는 한국과 일본의 노인입소시설 케어워커를 대상으로 노인에 대한 태도 및 치매지식이 치매에 대한 태도에 어떠한 영향을 미치는지 실증적으로 검증하였으며 나아가 치매에 대한 태도를 결정하는 요인에 있어 한일 간에 차이가 있는지 비교 분석 하였다. 조사는 한국의 케어워커 320명, 일본의 케어워커 350명을 대상으로 하였으며 설문지를 활용한 우편조사방법으로 이루어졌다. 자료 분석에는 t-test, 다중회귀분석을 활용하였으며 이에 따른 연구결과는 다음과 같다. 먼저, 한일 간에 노인에 대한 태도 및 치매지식, 치매에 대한 태도를 비교한 결과 치매지식에서는 유의미한 차이가 없었으나 치매에 대한 태도와 노인에 대한 태도는 한국에 비해 일본의 케어워커가 긍정적인 것으로 나타났다. 다음으로 치매에 대한 태도에 상대적으로 영향력이 큰 변인을 살펴보면 한국은 치매지식(β=.305 p<0.001), 일본은 노인에 대한 태도(β=.348 p<0.001)라는 것을 알 수 있었다. 둘째, 노인에 대한 태도와 치매지식은 한일 양국에서 공통적으로 치매에 대한 태도에 영향을 미치는 변인으로 나타났다.

  • Purpose: The study evaluated the determinants of caregiving self-efficacy among dementia caregivers. Methods: Data were collected through a structured questionnaire survey from dementia sufferers and caregivers by nurses or social workers caring for dementia sufferers at health centers during July to September 2007. Multiple stepwise regression analysis using SAS Version 9.1 was performed to examine the determinants of caregiving selfefficacy. Results: Factors affecting caregiving self-efficacy were behavioral and psychological symptoms of dementia (BPSD), activities of daily living (ADL), and age of dementia sufferer. Conclusion: To increase dementia caregivers' self-efficacy, there is a need to reduce difficulties of dementia caregivers in caring BPSD and increasing the ADL level of dementia sufferers by providing guidelines of care and intervention programs for BPSD and ADL management.

요양원 종사자의 감정노동, 소진이 직무스트레스 및 직무만족에 미치는 영향 : 치매환자 돌봄 경험을 중심으로 (The Effects of Emotional Labor, Burnout on Job Stress and Job Satisfaction of Nursing Home Workers : Focusing on Care Experience of Dementia Patients)

  • 치매환자는 꾸준히 증가하는 추세로 사회적 문제로 대두되고 있으며, 환자들에게 서비스를 제공하는 자들에 대한 처우의 관심도 증가하고 있다. 따라서 본 연구는 치매환자를 돌보는 요양원 종사자의 돌봄 경험을 살펴보고, 그들의 감정노동, 소진이 직무스트레스와 만족에 미치는 영향과 돌봄 경험의 조절효과를 분석하였다. 연구결과는 다음과 같다. 첫째, 요양병원 종사자의 돌봄 경험은 초기 간호사 마음가짐, 초기 간호수행(돌봄)시 치매환자에 대한 인식, 업무수행(돌봄)과정의 애로사항, 돌봄 과정에서 생기는 신체적 정신적 한계, 업무수행(돌봄)과정의 만족, 치매환자 간호수행(돌봄)을 통한 경험, 개인적 삶에 대한 마음가짐의 변화로 나타났다. 둘째, 감정노동과 소진은 직무스트레스에 정(+)의 영향을 미치는 것으로 나타났으며, 이 과정에서 돌봄 경험은 감정노동과 소진이 직무 스트레스에 미치는 영향을 유의하게 조절(완화)하는 역할을 하는 것으로 나타났다. 마지막으로 직무 스트레스와 직무 만족 간의 관계는 지지되었다.

Strategies for Deriving Strategic Products to Enter the Dementia Industry on SMEs-Focused on AHP Analysis

  • Many SMEs are interested in entering the dementia industry as the national dementia responsibility system begins in earnest. However, it is ambiguous about which technology and field to enter, and accordingly, it is impossible to make a quick judgment and misses the time to enter the market. Therefore, the purpose of this study is to provide a simple but clear information to many SMEs in this situation, which products and related technologies will be able to help product development and market entry in the dementia prevention and diagnosis technology market more easily. In this regard, I would like to suggest the direction through hierarchical analysis (AHP) through conducting with a group of experts who can make professional judgments about the development of dementia medical technology, including the four-year university senior welfare department, nurses, directors and directors of long-term care institutions, medical device workers and experts' opinions on what sectors SMEs can most effectively apply to product development to enter the dementia market.

치매안심센터 이용 보호자의 미충족 치매정보와 종사자의 프로그램 효과성 인식에 관한 연구 (A Study on Unmet Dementia Information of Dementia Caregiver Using Dementia Relief Center and Program Effect Recognition of Workers)

  • 본 연구는 치매안심센터의 치매환자 보호자에게 미충족 치매정보를, 종사자에게는 프로그램에 대한 효과성을 파악하기 위한 서술적 조사연구이다. 대상자는 S시 25개 자치구의 치매안심센터에서 보호자 114명, 종사자 217명을 대상으로 하였으며, 기간은 2019년 11월 1일부터 2020월 1월 30일까지이다. 연구도구 중 미충족 치매정보는 7문항의 Likert 5점 척도, 위기관리수준은 6항목, 10문항의 Likert 5점 척도로 구성된 설문지를 사용하였다. 연구결과 우울한 보호자가 그렇지 않은 보호자에 비해 미충족 치매정보를 경험할 비율이 12.3배 높은 것으로 나타났다(p<.001). 위기관리수준이 미흡하다고 인식한 종사자일수록 프로그램 효과성이 8.98배 높게인식하였으며(p=.004), 사회복지사가 간호사에 비해 2.81배나 높게인식하였다(p=.091). 결론적으로, 치매안심센터 서비스 수준을 단기간에 최고로 만들기 위한 일방적 정책보다 지역사회 주민들과 다양한 의견교환을 통해 그들의 필요를 충족해 나가는 쌍방향 운영이 필요하다.

치매노인을 위한 노인요양시설의 배회 공간 특성 분석 (Analysis on Wandering Space Design of Elderly Care Facilities for the Elderly with Dementia)

  • The purpose of this study was to examine wandering behaviors of the elderly with dementia at elderly care facilities, thus finding out the current status of supportability and safety of wandering space for facility residents and looking into the convenience of management for the control and Support of the elderly with dementia in wandering space, from the aspect of managers. Study methods consisted of literature examination, on-site assessment and investigation, observation and questionnaire survey. And via a questionnaire survey of facility workers at study subject facilities, the current status and facility workers' perception of wandering space and their job-satisfaction were examined. Collected data of 133 respondents were used for analysis. Data were processed with the SPSS 18.0 program, and such analytic techniques as frequency, percentage, mean, multiple response analysis, chi-square test and correlation analysis were carried out. The problems of physical environment of wandering space for the elderly with dementia: First, it is urgent to establish wandering space. Second, thoughtlessly installed chairs in the corridor impeded wandering behaviors. Third, the grips installed in the corridor space were irregular, thus causing inconvenience for wandering behaviors. Thus making residents' use of it difficult. In order to improve these problems, it is necessary to newly or additionally establish wandering space and modify the location of chairs or establish alcoves, thus reducing impedimentary elements for wandering behaviors. Providing a proper wandering space is important for facility residents. And it is also important for facility workers to manage facility residents.

Factors Influencing Care Behavior of Certified Care Assistants for Older Adults with Dementia

  • Purpose: Korea is one of the most rapidly aging countries, accompanied by an increasing number of older adults with dementia. Certified-care-assistants are the primary care workers for these older adults. This cross-sectional descriptive study compared the factors influencing care behaviors among Korean and non-Korean certified-care-assistants in Korea. Methods: Participants, 113 Korean certified-care-assistants and 71 non-Korean certified-care-assistants, completed a survey questionnaire. The questionnaire was designed to assess dementia knowledge, attitude and care behaviors toward older adults with dementia. To determine the differences in these two groups, t-test was used. A stepwise multiple regression was performed to identify factors related to care behavior. Results: Significant differences in dementia knowledge, attitude and care behavior toward older adults with dementia emerged between Korean and non-Korean certified-care-assistants. Factors influencing care behavior varied by group. Dementia knowledge and education level were the significant factors of care behaviors in Korean certified-care-assistants, whereas attitude toward older adults with dementia was the significant factor in non-Korean certified-care-assistants. Conclusion: Findings indicate that different factors influence the care behavior of two groups. These findings can be utilized to develop certified-care-assistants' training curriculum that needs to be tailored by nationality to improve the quality of care for older adults with dementia.
