Nursing Delivery System Improvement Plan in A Hospital (간호전달 체계 개선 방안 - 일 병동을 중심으로 -)

  • Background : In many Nursing Delivery System, Nursing Department at D Hospital had used to traditional nursing practice model what is called functional activities based system. It has a lot of merit that carried out specialized and rapid works but tend to ignore indivisual professional responsibility and task-based work assignments. In addition this system showed high turnover rates due to heavy workload, timesum of handing over duties, lack of support from peers and interstaff communication. So we performed conversion of Nursing Delivery System to My Patients Nursing Care System for providing comprehensive nursing to patient and reducing turnover rates and increasing job satisfaction to nurse. Method : 1. 1st step(96.4.9): Detected the problem of Nursing delivery System and estabilished improving planning 2. 2nd step(96.4.26): Visited other hospital on job training 3. 3th step(96.4.29): Discussed to premonitoring problem after conversion Nursing Delivery System and prepared structure 4. 4th step(96.5.6): My Patients Nursing Care System practical application 5. 5th step(96.7.20): Held complementary meeting 6. 6th step(96. 7): The other ward application 7. 7th step(96. 10): Extended application to whole wards Results: 1. Workload: (1) reduction(55.6%) (2) addition(44.4%) 2. Strong points after conversion: (1) decreased timesum of handing overduties (35.2%) (2) increased responsibility(33%) (3) broaden nurse's outlook to duties(14.8%) 3. Shortcoming after conversion: (1) understanding difficulties except my patient(57.8%) (2) weak teamwork(23.3%) (3) intensive stress to low grade nurse(12.2%) 4. Effective complemental way: (1) manpower(76.7%) (2) conversion of though (8.9%) (3) education(14.4%) 5. Patient's satisfaction: (1) satisfaction(64%) (2) no effect(36%) 6. Physician and peer's satisfaction: (1) satisfaction(12.5%) (2) dissatisfaction(21.6%) (3) no interest(44.3%) 7. Nurse's satisfaction: (1) satisfaction(74.7%) (2) dissatisfaction(5.5%) (3) unknown(20.5%) 8. Want to continued: (1) want(76.4%) (2) try to any other system(18%) Conclusion : Even though Nursing Delivery System conversion still has many problem, we gained more merits than traditional nursing delivery system. So we suggest that My Patients Nursing Care System should be encouraged for comprehensive nursing care and satisfaction to nurses.

Empirical Equation for Pollutant Loads Delivery Ratio in Nakdong River TMDL Unit Watersheds (낙동강 오염총량관리 단위유역 유달율 경험공식)

  • In this study daily flow rates and delivered pollutant loads of Nakdong river basin are simulated with modified TANK model and minimum variance unbiased estimator. Based on the simulation results, flow duration curves, load duration curves, and delivery ratio duration curves have been established. Then GIS analysis is performed to obtain several hydrological geomorphic characteristics such as watershed area, stream length, watershed slope and runoff curve number. Finally, multiple regression analysis is carried out to estimate empirical equations for pollutants delivery ratio. The results show that there is positive relation between the flow rates and delivery ratios, and the proposed empirical formulas for delivery ratio can predict well river pollutant loads.

  • Gene therapy has become a promising strategy for the treatment of genetically based diseases, such as cancer, which are currently considered incurable. A major obstacle in the field of cancer gene therapy is the development of a safe and efficient delivery system for therapeutic gene transfer. Non-viral vectors have attracted great interest, as they are simple to prepare, stable, easy to modify and relatively safe compared to viral vectors. In this review, an insight into the strategies developed for polyethylenimine (PEI)-based non-viral vectors has been provide, including improvement of the polyplex properties by incorporating hydrophilic spacer, poly(ethylene glycol) (PEG). Moreover, this review will summarize the strategies for the tumor targeting. Specifically, a targeted polymeric gene delivery system, PEI-g-PEG-RGD, will be introduced as an efficient gene delivery vector for tumor therapy, including its functional analysis both in vitro and in vivo.

  • The multilayered monolithic type transdermal delivery device has been designed and analyzed by a numerical analysis. The device consists of three layered polymer matrices which posess the different diffusion parameters, respectively. The purpose of this study was to design an ideal transdermal drug delivery device which is capable of initial burst and zero order release later on. Numerical modelings were simulated for a dispersed and a dissolved multilayered monolithic system. The results showed that the dispersed multilayered monolithic system could meet the requirements for an ideal transdermal delivery device.

  • Although insulin has been available for the treatment of diabetes mellitus for more than half a centry, the deficiency of conventional insulin therapy for diabetic patients have, to this date, not been satisfactorily overcome by any method. The development of potential delivery systems for insulin is highly important to prevent excessive fluctuation of plasma glucose levels, which results in long term complications in the diabetic. There are three major approaches toward development of glucose responding insulin delivery systems: A bioengineering approach is to devise mechanical components capable of releasing insulin in amounts appropriate to varying blood-glucose requirements. A biological approach relies upon cultured, living pancreatic beta cells encapsulated to constitute an insulin delivery unit. A biochemical approach is to synthesize a stable and biologically active glycosylated insulin that is complementary to the binding sites of lectin. This paper will cover several specific areas, including pancreatic transplantation(total or isolated islet cells), artificial pancreases(bioengineering or biological approach), controlled delivery system, glucose sensitive membrane systems, and a self-regulating insulin delivery system.

  • To solve the internal security problem such as human error and bad faith, the automation of computer system management is significant. For this purpose, we focus attention in the automation of Mail Delivery Service. Today, requirement for reliable mail delivery system becomes larger and larger. However, existing systems are too strict about their configuration. Hence, we propose the method based on Reinforcement Learning (RL) to achieve proper MX record ordering. A measure on validity of the design of system, such as network topology number of servers and so on, is also obtained as side benefit. In order to verify the usability of the presented method, we did on a small model of mail delivery system. As a result, we show that RL is available for determination of the proper MX record ordering. Additionally, we suggest that it is also available for comparing validity of setting of MTA and the network design.

  • Early morning delivery possesses distinct characteristics that differentiate it from standard delivery services. This service typically involves delivering products to customers during the early morning hours, primarily before 7 AM. While online early morning delivery offers various advantages from a customer perspective, it also presents challenges that sellers and online shopping companies need to overcome. The early morning delivery market is experiencing significant growth in the online food retail sector, incorporating both PC-based online shopping and mobile shopping. The objective of this research is to identify the factors influencing customer satisfaction and the intention to reuse in the context of early morning delivery for online shopping. To model the online shopping environment with early morning delivery, independent factors were categorized into three types: System Properties, Product Characteristics, and Delivery Characteristics. This study examined the relationships among these three independent factors, the mediating factor of customer satisfaction, and the dependent variable of the intention to reuse. To conduct this research, empirical validation of the research hypotheses was carried out using the final dataset for analysis. Within this study, the previously explored System Properties, Product Characteristics, and Delivery Characteristics were established. Summarizing the findings of the analysis, it was discovered that System Properties and Product Characteristics played a significant role in determining the quality of early morning delivery services for online shopping. While product diversity and convenience had a positive impact, it is noteworthy that Delivery Characteristics did not influence customer satisfaction. Consequently, it can be concluded that there is no effect on the intention to reuse.

Development of the Portable Drug Delivery Systems with a Piezoelectric Micropump (압전 마이크로펌프 방식의 휴대용 약물전달장치 개발)

  • The therapy of injecting a fixed amount of a prescribed drug for a predetermined time is an effective treatment in relieving pain during anticancer treatments. Due to recent medical technology development, cancer is currently classified as a disease that can be managed in the patient's lifetime. If patients were able to use a drug delivery system that was portable, sustainable and had an accurate flow control, they would be able to inject medication whenever they need. In this study we developed a piezoelectric micropump for a drug delivery system by designing a pump chamber, check valve and diaphragm. We also developed a driving circuit that consumes low power and to which we applied a variety of signals. We fabricated a portable drug delivery system with this piezoelectric micropump and driving circuit. In addition, through a performance test, we confirmed that the system can precisely control the drug flow rate.

Wind Tunnel Test for the Inflation Characteristics of the Korean Low Cost Low Altitude Aerial Delivery System (한국형 저비용 저고도 공중보급 체계용 낙하산 전개 특성 풍동시험)

  • A wind tunnel test for the scaled parachute models was performed to verify aerodynamic characteristics for practical usage of Korean low cost low altitude aerial delivery system. The cruciform shaped cargo parachute models for heavy and light weight were ejected into wind tunnel test section; and the drag forces acting on the models in steady condition were measured in accordance with velocity. Also, the maximum opening forces during inflation were obtained and captured by a high speed camera to analyze the inflation characteristics and evaluate the design of the low cost aerial delivery system. The results showed a reliable stability and met the design requirement of delivery operation system for R.O.K Air Force.

  • The market of programmable implantable pumps has bound to a monopolistic situation, inducing high device costs, thus making them inaccessible to most patients. Micro-mechanical and medical innovations allow improved performances by reducing the dimensions. This affects the consumption and weight, and, by reducing the number of parts, the cost is also affected. This paper presents the procedure followed to design an innovative implantable drug delivery system. This drug delivery system consists of a low flow pump which shall be implanted in the human body to relieve pain. In comparison to classical known solutions, this pump presents many advantages of high interest in both medical and mechanical terms. The first section of the article describes the specifications which would characterize a perfect delivery system from every points of view. This concerns shape, medication, flow, autonomy, biocompatibility, security and sterilization ability. Afterwards, an overview of existing systems is proposed in a decisional tree. Positive displacement motorized pumps are classified into three main groups: the continuous movement group, the fractioned translation group and the alternative movement group. These systems are described and the different problems which are specific to these mechanisms are presented. Since none of them fully satisfy the specifications, an innovation is justified.. The decisional tree is therefore extended by adding new principles: fractioned refilling and fractioned injection within the fractioned translation movement group, spider guiding system within the alternative translation movement group, rotational bearing guided device and notch hinge guided device in the alternative rotation movement group.
