A Patient with Lower Limbs' Delayed Motor Development was Demonstrated Hang-ji : A Case Report (행지(行遲)로 진단된 하지부 운동발달지연 환아의 치험 1례 보고)

  • Objectives This is the clinical report about 1 case of patient with delayed motor development in lower limbs. Methods The herb medication, acupuncture therapies were applied for treating delayed motor development from June 9 to November 20, 2007. Results After 5 months treatment, the patient's delayed motor development were improved by oriental medical therapy. Conclusions We report the good results of oriental medical therapy on a pediatric patient with delayed lower limbs motor development. For more accurate studies, further studies would be needed with more cases.

Estimating Failure Rate Using Warranty Claim Data with Delayed Report of Customers (고객의 지연보고를 고려한 보증수리내역자료에서의 고장률 추정)

  • Warranty claim data analysis is a useful tool for the manufacturer because it contains many useful informations regarding reliability of the product in the real-world environments. Because of the nature of uncertainty and the incompleteness of data, some bias patterns are observed on warranty claim rate known as 'spikes'. Two types of spikes are considered. One is due to manufacturing-related failures. The other is caused by customer's behavior. This paper proposes a model by considering two types of spikes. Warranty claim data is analyzed with the proposed model. To represent spikes observed on the early warranty period, we classify failures into manufacturing-related failures and usage-related failures. Uniform distribution is assumed for the time delayed to diagnose and report by customers. By reducing maximum value of the delayed time by customers, the proposed model characterizes customer's rush in the vicinity of the warranty expiration limit. Experimental results by using the real warranty claim data show that the proposed model is better than the existing one in respect to MSE(Mean Squared Error). Moreover it is expected to estimate the failure rate more realistically with proposed model because it considers the delayed time to diagnose and report by customers.

A Case Report of Delayed Healing in Femoral Shaft Fractured Child (대퇴골 간부 골절 후 골절유합이 지연된 환아의 한의치료 증례보고)

  • Objectives: The purpose of this study is to report the clinical effects of oriental medical treatment on delayed healing in femoral shaft fractured child. Methods: We treated the patient with herb medicine named Kamiboatang and Pyritum. Child took Kamiboatang and Pyritum for 2 months, and Kamiboatang for 1 month. After 3 months treatments, we examined the case with radiological findings. Results: The patient's delayed femoral shaft fracture was improved after 3 months oriental medical treatment. Conclusions: This case showed that oriental medical treatment on delayed healing in femoral shaft fractured child was effective. To prove the effectiveness of Kamiboatang and Pyritum, the more clinical study of Oriental medical treatment for this case might be also needed.

  • Carbon monoxide poisoning leads to hypoxia, which eventually leads to tissue ischemia. The delayed neuropsychiatric syndrome can arise in the period from 4 days to 5 weeks following carbon monoxide poisoning. This case report describes a male who suffered from delayed sequelae after self-inflicted carbon monoxide poisoning. At the time of admission, he had symptoms of quadriplegia, both upper limb ankylosis, gait disturbance, and dysuria. He was treated with acupuncture, electro-acupuncture, physical therapy, and Uwhangchungsim-won. Paraplegia improved considerably after 7 days in hospital, and paralysis of the both upper extremities improved after 14 days in hospital. He was able to walk holding on to his wheelchair after 28 days in hospital. He was hospitalized for 55 days and was discharged from the hospital in a fine condition for everyday life. This case report suggests the possibility that korean medicine for delayed neuropsychiatric syndrome.

  • Pediatric carpal fractures are rare and often difficult to detect. This paper reviews the current literature on pediatric non-scaphoid carpal fractures, with a case report of a lunate fracture associated with a distal radius and ulnar styloid fracture, managed nonoperatively in a 12-year-old boy. There is lack of consensus regarding the management of these fractures due to the low number of reported cases. A frequent lack of long-term follow-up limits our understanding of the outcomes, but good outcomes have been reported for both nonoperative and operative management. This case report brings attention to the current time period for the definition of delayed union in pediatric carpal fractures, and emphasizes the need for prolonged follow-up for the detection of delayed complications leading to functional impairment.

  • Bullet injuries to the spine may cause injury to the anatomical structures with or without neurologic deterioration. Most bullet injuries are acute, resulting from direct injury. However, in rare cases, delayed injury may occur, resulting in claudication. We report a case of intradural bullet at the L3-4 level with radiculopathy in a 30-year-old male. After surgical removal, radicular and claudicating pain were improved significantly, and motor power of the right leg also improved. We report the case of intradural bullet, which resulted in delayed radiculopathy.

  • Paresthesia is an altered sensation of the skin, manifesting as numbness, partial loss of local sensitivity, burning, or tingling. The inferior alveolar nerve (IAN) is the third branch of the trigeminal nerve and is very important in dental treatment. IAN paresthesia may occur after various dental procedures such as simple anesthetic injections, surgical procedures, and endodontic treatment, and is reported to range from 0.35% to 8.4%. The altered sensation usually follows immediately after the procedure, and reports of late onset of nerve involvement are rare. This report presents a rare case of delayed paresthesia after dental surgery and discusses the pathophysiology of IAN delayed paresthesia.

  • Although a majority of patients with splenic rupture present acutely, up to 15% present with a delayed rupture days to weeks following a substantial abdominal injury. The mortality for patients presenting with acute splenic rupture is approximately 1% whereas that associated with delayed rupture approaches 15%. Although many cases of delayed splenic rupture have been reported, the majority of those reports present delayed splenic rupture associated with an underlying systemic disorder such as liver or kidney disease, or another hematologic disorder. We found a delayed splenic rupture case that documented the normal spleens of young healthy soldiers after trivial abdominal trauma, and we have had successful treatment experience with delayed rupture of a normal spleen after trivial trauma. Therefore, we want to review the literature and discuss the phenomenon of delayed rupture of the spleen following trivial trauma.

A Case Report of Delayed Neuropsychiatric Sequelae in a Patient Due to Carbon Monoxide Intoxication (일산화탄소 중독에 의한 Delayed Neuropsychiatric Sequalae 환자 치험 1례)

  • Carbon monoxide intoxication leads to hypoxia, which eventually leads to tissue ischemia. Delayed neuropsychiatric syndrome (DNS) can be a consequence and appears in the form of cognitive impairment and movement disorders after a lucid interval. A 58-year-old Korean male was admitted to our hospital with delayed neuropsychiatric sequelae (DNS) after an 11-day lucid interval following recovery from acute carbon monoxide intoxication. We treated him with herbal medication, acupuncture, electroacupuncture, and moxibustion. The effects were assessed by the activity index and Mini Mental State Examination-Korea (MMSE-K) scores, and by changes in gait disturbance. In this case, the clinical symptoms, including gait disturbance, and the activity index and MMSE-K scores, improved after traditional Korean treatment. The findings of this case report suggest that traditional Korean medicine treatment can be effective for treatment of symptoms of delayed neuropsychiatric sequelae due to CO intoxication.

  • Occipito-atlanto-axial subluxation is a type of high cervical injury. Misdiagnosis of this instability might cause several complications, including neurologic deficits. A radiologic evaluation must be performed and specific parameters that imply cervical instability must be confirmed. Full understanding of these parameters may prevent not only delayed diagnosis but also unexpected complication. We report a case of delayed detection of a cervical instability and emphasize the role of proper diagnosis.
