Treatment of the wide open wound in the Ehlers-Danlos syndrome

  • Ehlers-Danlos syndrome (EDS) is an inherited disorder of collagen biosynthesis and structure, characterized by skin hyperextensibility, joint hypermobility, aberrant scars, and tissue friability. Besides the skin, skeleton (joint) and vessels, other organs such as the eyes and the intestine can be affected in this syndrome. Accordingly, interdisciplinary cooperation is necessary for a successful treatment. Three basic surgical problems are arising due to an EDS: decreased the strength of the tissue causes making the wound dehiscence, increased bleeding tendency due to the blood vessel fragility, and delayed wound healing period. Surgery patients with an EDS require an experienced surgeon in treating EDS patients; the treatment process requires careful tissue handling and a long postoperative care. A surgeon should also recognize whether the patient shows a resistance to local anesthetics and a high risk of hematoma formation. This report describes a patient with a wide open wound on the foot dorsum and delayed wound healing after the primary approximation of the wound margins.

The Optimal Time of Fiberoptic Bronchoscopy to Locate the Bleeding Site in Patients with Hemoptysis (객혈의 병소를 확인하기 위한 굴곡성 기관지경 검사의 시행시기)

  • Background: Hemoptysis is a common clinical symptom, responsible for 11% of admission to the hospital chest service. Correct diagnosis, accurate localization of the bleeding source and proper management are imperative to reduce the risk of massive hemoptysis. We performed the study to define the optimal time of fiberoptic bronchoscopy in 63 patients with hemoptysis admitted to Kyung Hee University Hospital between Aug 1989 and Aug 1992. Methods: Retrospective analysis of medical records concerning the cause, amount, duration of hemoptysis and the timing of fiberoptic bronchoscopy in 63(M:F=36:27) patients. Results: 1) The main causes of hemoptysis were pulmonary tuberculosis(52.4%) bronchiectasis(27.0%) and lung cancer(11.1%). 2) The bleeding sites were localized in 26 patients(41.3%). 3) The rates of localization of bleeding site were not related to the amount and duration of hemoptysis. 4) The rates of localization of bleeding site were 61.8%(21/34) during hemoptysis, 18.2%(22) within 24hr after resolution of hemoptysis, 14.3%(1/7) thereafter. Conclusion : Early bronchoscopy, especially during hemoptysis may show higher rates of successful localization than delayed bronchoscopy.

Delayed Traumatic Intracerebral Hemorrhage in Patient with Hemoperitoneum Operation (혈복강 수술 환자에게서 발생한 지연성 외상성 뇌실질내 출혈)

  • Delayed traumatic intracerebral hemorrhage (DT-ICH) is a rare event in head trauma patients. However, it develops unexpectedly and results in very severe brain damage. Thus, close monitoring of the neurologic status is needed for every trauma patient. Sometimes, however, neurologic monitoring cannot be done because of sedation, especially in cases of abdominal surgery. In this case report, we describe the case of a 37-yr-old, male patients who had hemoperitoneum because of spleen and renal injury. At the initial operation, massive bleeding was found, so gauze-packing surgery was done first. After the first operation, we sedated the patient for about two days, after which the packed gauze was removed, and the abdominal wound was closed. Immediately after the second operation, we found pupil dilation. Emergent CT was performed. The CT revealed DT-ICH with severe brain edema and midline shifting. However, the patient condition deteriorated progressively despite emergency operation, he expired 2 days after hematoma evacuation.

Use of sugammadex in Rett syndrome: A case report

  • Rett syndrome (RS) is a neurodevelopmental disorder characterized by loss of cognitive, motor, and social skills, epilepsy, autistic behavior, abnormal airway patterns, gastroesophageal reflux, nutritional problems, and severe scoliosis. Although girls with RS show normal or near-normal growth until 6-8 months, they lose their skills after that. The anesthetic management of these patients requires care because of all these clinical features. Especially in the postoperative period, prolonged apnea is common and extubation is delayed. In this case report, the effect of using sugammadex was presented in a 16-year-old girl with RS. The patient's all bimaxillary teeth and 4 wisdom teeth were extracted under general anesthesia in one session with minimal surgical trauma and moderate bleeding. Sugammadex can be a rapid and reliable agent for the reversal of the neuromuscular block in neurodegenerative patients.

  • This study aims to examine the relative importance of various tasks of EMT and to examine knowledge level and problems associated with AED. It is based on the survey of paramedics and 119 rescures. The results for this study were as follows ; Among the tasks of EMT, 'foreignbody removal from mouth' were rated as the most important, followed by '$O_2$ supply', 'CPR', 'Intubation for CPR', '$O_2$ supply' were rates as the most necessary, followed by 'foreignbody removal from mouth', 'external bleeding control'. In terms of the knowledge level about AED, survey respondents are most knowledgeable about 'role of other rescures during the preparation for AED' and 'treatment after 3 AEDS. The most important reason that the uses of AED is delayed is 'unclear boundary of treatment directed by medical control'. The results of the study suggest that we need guideline for EMT's tasks, efficient communication system between 119 and emergency health are center for prompt AED, and training and education program for AED.

  • Hemophilia is a hereditary condition in which one of the normal blood clotting factors is absent. This sex-linked, recessive disease, transmitted by the female and expressed in some of her male offspring, is characterized by significantly delayed coagulation times and by the tendency to hemorrhage. On the dental treatment especially, surgical procedure, appropriate methods of hemorrhage control are required. These methods are replacement therapy, antifibrinolytic agents, and local hemostatic methods. In this case, we successfully extract residual roots of hemophiliac patient without eccessive post operative bleeding using replacement therapy and surgical splint.

  • Diffuse cavernous hemangioma of the rectosigmoid is a rare lesion usually presented in children and young adult, often with life threatening hemorrhage. The clinical diagnosis is difficult and often delayed because of lack of awareness of the clinical features. We report a case of diffuse cavernous hemangioma of the rectosigmoid in a 12 year old boy who had undergone suture ligation under the impression of hemorrhoid at his age 3. Sphincter saving coloanal pull through procedure were applied as the surgical treatment. Sclerotherapy was needed with recurrent rectal bleeding 6 months after the pull through operation. The patient is well at 12 months follow-up.

Clinical Evaluation of Hemoptysis (객혈환자의 임상적 고찰)

  • Between January 1990 and June 1993, the retrospective analysis was done in 48 consecutive patients with hemoptysis. According to clinical condition of patients, managements were divided into 3 subgroups; group 1[percutaneous bronchial artery embolization, group 2[operation after percutaneous bronchial artery embolization],group 3[delayed and emergency operation]. It was characterized that recurrence of hemoptysis was very frequent and most frequent underlying cause was pulmonary tuberculosis. In 40 patients[83%] urgent examination with flexible broncoscope was done and localization of the bleeding source was possible only in 24[60%] patients. The amount of hemoptysis was variable but there are no difference between groups and 22 patients[45%] had a prior episode of hemoptysis usually within 3 months of their admission. The recurrence was limited only in group 1[3/22] and the mortality rate was 6%[3/48]. We suggest that percutaneus bronchial artery embolization may be effective in recurrent massive hemoptysis but definitive management was operation.

  • Warfarin sodium is a widely used oral anticoagulant and it sometimes causes hemorrhage in various sites such as traumatic or operative wounds, urologic organs, gastrointesitnal tract and subcutaneous tissues. Warfarin induced hemorrhage in upper respiratory tract causing upper airway obstruction is extremely rare. Despite the life-threatening nature of this complication, the symptoms are obscure like sore throat or hoarseness, suggesting infection and early diagnosis can be delayed. Careful medical history is emphasized When bleeding is observed, the patient should be admitted for close observation and prompt treatment. Recently we have exrperienced a case of laryngeal and upper tracheal submucosal hemorrhage induced by long term warfarin in 64 years old mm. We described the case with a review of the literature.

  • Multiple myeloma is a lymphohematopoietic disorder leading to abnormal hemostasis and significant pathologic changes of skeletal system. It induces multiple circular or oval-shaped radiolucent lesions which are characterized by 'punched-out appearance'. The surrounding trabecular bone normally shows no significant sclerotic reaction. Multiple myeloma patients may visit dental clinics, without perception of the disease themselves, due to discomfort from edema of orofacial region, oral ulcers, tooth mobility, pain or gingival bleeding. Multiple myeloma is susceptible to various complications, including delayed hemostasis and infection, which could occur during routine dental treatment such as periodontal and surgical operation. For radiographic diagnosis of multiple myeloma, common radiologic features of this tumor could be visualized by panoramic radiographs in the dental clinics, and further medical examinations and treatment can be recommended as a result.
