Microbial Contamination of Reusable Suction Container and Cost Analysis of Reusable Suction Container and Disposable Suction Container (재사용 흡인 용기의 미생물 오염도 및 재사용 흡인 용기와 일회용 흡인 용기의 비용 분석)

  • Purpose: The purpose of this study was to check the degree of residual microbial contamination after disinfection of reusable suction containers, used in an intensive care unit (ICU) and present basic data for efficient use through cost analysis in comparison to disposable suction containers. Methods: This study was conducted on 32 reusable suction containers used in an ICU on a selected specific day. After disinfection and washing, specimens were collected from the used containers and cultured to check for microbial contamination. Additionally, a comparative narrative study analyzes the cost of using reusable suction containers and disposable suction containers. Data were analyzed with the SPSS WIN 20.0 program using real numbers and percentage ${\chi}^2$-test. Results: As a result of the study, microorganisms were found in all samples where in 30 were gram-positive (62.5%) while 13 were gram-negative (27.1%). Based on level of contamination, microorganisms were less than 10CFU/ml in 18 samples (56.3%); 11-99CFU/ml in six samples (18.8%); and more than 100CFU/ml in eight samples (25%). Cost per day for a reusable suction container was $10,655+{\alpha}$ while cost per day for a disposable suction container was 10,666 won. Conclusion: This study found that reusable suction containers, even after disinfection, accounted for factors of potential infection as well as microbial contamination. So, disposable suction containers are superior in cost-effectiveness and highly efficient for use with infected patients.

  • To reduce the economic impact and control Low pathogenic avian influenza (LPAI), vaccination with inactivated vaccine has been considered in this country. We tried to develop inactivated vaccine with reassorted H9N3 AI virus which has different type of neuraminidase compare to those of field AI virus. Before reassorted vaccine was produced, we confirm the virus as master seed by limiting dilution, RT-PCR and sequencing method. Also, we evaluate the biological characteristics of the virus to find out the possibility of prevention against field infection of AI virus. Finally, we evaluate the safety and efficacy of the vaccine made of reassorted AI virus in the specific pathogen free (SPF) chickens. After limiting dilution, we choose RV7CE4 as a vaccine candidate and compare the gene sequence of this vaccine strain to those of AI05GA which is parents strain. Compared to amino acid sequences of specific gene of AI05GA and RV7CE4, exhibited a high degree of amino acid sequence homology. In the safety and efficacy test, there were no specific clinical signs or mortality. Reassorted H9N3 viruses were reisolated in cloaca swab on 5 days post inoculation. In the vaccine study, once or twice vaccination was performed and challenged with H9N2 field virus (01310). Vaccine has no adverse effect on birds and formed good immune capability which reduce viral shedding in the birds infected with 01310. Based on the above result, we developed reassorted H9N3 vaccine which will efficiently prevent the low pathogenic AIV (H9N2) infection in the poultry farms.

  • The detection of Bordetella bronchiseptica which is supposed to be an agent of the infectious atrophic rhinitis of swine, is likely to receive more attention in the future as the pork industry comes to realize that eradication of this infection from breeding herds is a practical possibility. Experiments described here were carried out to establish the rapid plate agglutination test for the detection of the infectious atrophic rhinitis of swine in the field using the criteria of antigen preparation, effects on the antigenecity after storing of the antigen and reaction appearing time. Also, the agglutinabilities between the plate and tube method were compared and the degree of pathological lesions were recorded in relation to tube agglutination titers. Obtained results were as follows: 1. No differences were noted in the agglutinabilities on the plate agglutination test between the treatments in antigen preparation-formolized, merthiolate-killed and living organism. 2. The agglutinability of the antigens did not show any significant changes until 10 weeks of storage at 4 C; however, after 10 weeks of storage, non-specific reaction was observed with the HPCD control sera. 3. The results of the plate and tube agglutination tests were not comparable but the effective use of the plate method in Bordetella bronchiseptica eradication programs in pigs especially in the sow is stressed as a screening test.

The Impact of COVID-19 on Health Prevention Behaviors in College Students: Focusing on the Health Belief Model (일부 대학생의 코로나19에 대한 건강예방행위에 미치는 영향: 건강신념모델을 중심으로)

  • Purpose: The purpose of this descriptive research is to identify how stress from Covid-19, health beliefs, and social support of college students affect health prevention. Methods: The subjects of the study were 128 university students, excluding health major students, at one university in D City. The survey was conducted from August 1 to 31, 2020. The survey questionnaire consists of 8 items on stress from COVID-19, 12 items adapted from a health belief measurement tool, 12 items from a social support measurement tool, and 11 items adapted from a tool that measures health preventive behaviors. The collected data were analyzed using the hierarchical multiple regression analysis method with SPSS 26.0. Results: In model 1, stress from COVID-19 was statistically significant (β=-.403, p=.003). Model 2 added four health belief factors into Model 1. Stress (β=-.419, p<.001), perceived severity (β=-.193, p=.030), and perceived barriers (β=-.182, p=.009) were statistically significant. In model 3, stress (β=-.413, p<.001), perceived barriers (β=-.147, p=.034), and social support (β=.194, p=.011) were statistically significant. The regression equation was significant (F=15.395, p=<.001) and the model's explanatory power was 53.1%. Conclusion: The results show that when college students had a high degree of health beliefs about COVID-19, the degree of health preventive behaviors was proportionally high. To make them practice preventive health behaviors, it is necessary to develop infection control education programs to improve health beliefs.

Comparison of the Effects of Internet Diabetic Education on Obesity in Type 2 Diabetic Patients (제 2형 당뇨환자 비만도별 인터넷 당뇨 교육 효과 비교)

  • Purpose: The purpose of this study was to evaluate the effects on obesity of nurse education using the short message service (SMS) of cellular phones and wire Internet. Method: Forty patients in an experimental group, which was assessed pre- and post intervention, completed this study. The patients were divided into two groups according to degree of obesity. The goal of the intervention was to keep blood glucose concentrations close to the normal range. The intervention was applied weekly for 3 months. Participants were requested to input the blood glucose level everyday at http://www.biodang.com by cellular phone or wire internet. The researcher sent optimal recommendations to each patient using SMS of cellular phones and wire Internet. The plasma glucose levels and serum lipids were measured before and after the intervention. Results: After 3 months of education, Glycosylated hemoglobin (HbAlc) decreased by 1.4% in non-obese patients and 0.7% in obese patients. Fasting plasma glucose (FPG) decreased 22.6mg/dl in non-obese patients and 22.3mg/dl in obese patients. Two-hour plasma glucose (2HPG) decreased 97.0mg/dl in non-obese patients and 67.8mg/dl in obese patients. Conclusion: These results indicate that a nurse SMS intervention would improve HbAlc, FPG, and 2HPG in both non-obese and obese patients.

Effects of long double-stranded RNAs on the resistance of rock bream Oplegnathus fasciatus fingerling against rock bream iridovirus (RBIV) challenge

  • To determine whether rock bream Oplegnathus fasciatus can be protected from rock bream iridovirus (RBIV) infection by intramuscular injection of long double-stranded RNAs (dsRNAs), we compared protective effect of virus-specific dsRNAs corresponding to major capsid protein (MCP), ORF 084, ORF 086 genes, and virus non-specific green fluorescent protein (GFP) gene. Furthermore, to determine whether the non-specific type I interferon (IFN) response was associated with protective effect, we estimated the activation of type I IFN response in fish using expression level of IFN inducible Mx gene as a marker. As a result, mortality of fish injected with dsRNAs and challenged with RBIV was delayed for a few days when comparing with PBS injected control group. However, virus-specific dsRNA injected groups exhibited no significant differences in survival period when compared to the GFP dsRNA injected group. Semi-quantitative analysis indicated that the degree of antiviral response via type I IFN response is supposedly equal among dsRNA injected fish. These results suggest that type I IFN response rather than sequence-specific RNA interference might involve in the lengthened survival period of fish injected with virus-specific dsRNAs.

  • Purpose: The purpose of this study was to investigate the degree of LUTS (Lower Urinary Tract Symptoms) and factors affecting LUTS in patients with prostate cancer. Methods: Fifty-four patients with prostate cancer participated and they completed a structured questionnaire at a hospital in Seoul. The LUTS level was measured by using the International prostate symptom score (1992) and the data were analyzed by Wilcoxon rank sum test and Kurskal-Wallis test. Results: Most subjects obtained a higher than moderate LUTS score. The participants who had a lower perceived health status and did not drink alcohol were more likely to had a lower LUTS score than other patients who had higher perceived health status and drink alcohol. In addition, eye diseases were significantly associated with the LUTS score. In LUTS, all the patients received higher IPSS-QOL (International prostate symptom score quality of life) and frequency, nocturia. Conclusion: For proper nursing care related to LUTS, nurses should focus on the factors identified when performing nursing assessments and should develop nursing intervention programs for symptoms management.

Use of Quantitative Models to Describe the Efficacy of Inundative Biological Control of Fusarium Wilt of Cucumber

  • Fusarium wilt of cucumber caused by Fusarium oxy-sporum f. sp. cucumerinum is a serious vascular disease worldwide. Biological control of Fusarium wilt in several crops has been accomplished by introducing non-pathogenic Fusarium sup. and other biocontrol agents in soil or in infection courts. In this study, quantitative models were used to determine the biocontrol efficacy of inundatively applied antagonist formulations and the length of their effectiveness in controlling Fusarium wilt of cucumber. Quantitative model of the form [Y=L (1${-exp}^{-kx}$)] best described the relationship between disease incidence (Y, %) and inoculum density (X) of isolates F51 and F55. Isolate F51 was selected as a more virulent isolate based on the extent of its effectiveness in causing the wilt disease. The degree of disease control (Xi/X) obtained with the density of the biocontrol agent (Z), was described by the model [Xi/X=A (1${-exp}^{-cz}$)]. The zeolite-based antagonist formulation amended with chitosan (ZAC) was better at lower rates of application and peaked at around 5 g/ kg of the potting medium, whereas the peat-based antagonist formulation (PA) peaked at around 10 g/kg of the potting medium. ZAC formulation provided significantly better suppression of Fusarium wilt as described by the curvilinear relationship of the type Y= a+bX+c$X^2$, where Y represents percent disease incidence and X represents sustaining effect of the biocontrol agent.

Adhesion Prevention with Guardix® (Sodium Hyaluronate) After Flexor Tendon Repair in Rabbits (토끼 모델에서 굴근 인대 봉합 후 가딕스®의 유착 방지 효과)

  • Purpose: Adhesion is the most common and troublesome complication after repair of flexor tendon injury. Recently, use of sodium hyaluronate derivatives for adhesion prevention is increasing. A commercial product, Guardix$^{(R)}$, sodium hyaluronate(NaHe) combined with carboxymethylcellulose(CMC) has been newly developed as a preventive material for adhesion. We have investigated its effect in rabbits. Methods: Twenty seven male New Zealand white rabbits were operated under ketamine anesthesia. After tendon repair in zone II of the hind paw, Guardix$^{(R)}$(experimental group) or normal saline(control group) was administered. Biomechanical tests were performed to estimate adhesion formation at 2, 4, 8, and 12 weeks after the operation. Maximum tensile load to flex the distal interphalangeal joint 50 degree from its resting state(MTL50) was measured, depicting the amount of adhesion formed. Subsequently, breaking strength was assessed. Results: There were no postoperative complications such as infection, wound dehiscence, or hematoma. MTL50 was significantly lower in the experimental group than in the control group at 4, 8, 12 weeks (p<0.05). Mean value of MTL50 was 6.64N in the experimental group and 28.53N in the control group at 12 weeks after surgery. There were no significant differences in breaking strength. Conclusion: Our results indicate that Guardix$^{(R)}$ is helpful in reducing adhesion formation and does not interfere with normal healing processes of the tendon.

Path Analysis of Performance of Multidrug-Resistant Organisms Management Guidelines among Intensive Care Unit Nurses : With Focus on the Theory of Planned Behavior and Patient Safety Culture (중환자실간호사의 다제내성균 감염관리지침 수행에 영향을 미치는 요인에 관한 경로 분석 ; 계획된 행위이론과 환자안전문화를 중심으로)

  • Purpose : The purpose of this study was to analyze factors influencing the adherence to guidelines for intensive care unit (ICU) nurses to control infections due to multidrug-resistant organisms (MDRO). Method : Participants were 194 ICU nurses at 3 university hospitals. Questions for the survey inquired about attitude, subjective norms, perception of patient safety culture to carry out MDRO management guidelines, perceived behavior control (PBC), and intention, based on the theory of planned behavior. Path analysis were utilized. Results : The path analysis presented that PBC, perception of patient safety culture, and intent had a direct effect on MDRO management guidelines. Attitude towards following the manual did not have any correlation. The hypothetical model based on the theory of planned behavior was revealed as applicable; the degree of the variance in explaining adherence to the manual was 23 %, and the variance in explaining intention to fulfill the manual was 33 %. Conclusion : The results of this study suggest that we should develop a program to improve PBC to increase adherence to MDRO management guidelines. ICU nurses' perceptions of patient safety culture should also be surveyed.
