Rasch 모형을 이용한 한국어판 중환자실경험 측정도구의 문항 분석 (Item analysis of the Korean version of the Intensive Care Experience Questionnaire: Using the Rasch Model based on Item Response Theory)

  • Purpose : This study aimed to examine the item characteristics of the Korean version of the intensive care experience questionnaire (K-ICEQ) using the Rasch analysis model of the item response theory. Methods : In this methodological study, the validity of the scale was examined, and a secondary analysis was conducted using cohort data of patients who were discharged from the intensive care units (ICU). Data from 891 patients who responded to the K-ICEQ upon ICU discharge were analyzed. The WINSTEP program was used to analyze item characteristics, including item difficulty, fit indices, appropriateness scale, and separation reliability. Results : The difficulty level of all 26 items of the K-ICEQ was appropriate, and the fit indices of the 25 items, except for item 18, were good. The 5-point scale of the K-ICEQ was not appropriate in the three subscales. The item separation reliability was good in all subscales, but did not meet the criteria in terms of respondents. Conclusion : The results of examining the item characteristics of the K-ICEQ revealed a good degree of difficulty, fitness, and item separation reliability. To increase the validity of the K-ICEQ, we suggest the rearrangement of the overall item order, modification of the item description of the "recall of experience" subscale, and reduction of the scale response level.

The study on development of easily chewable and swallowable foods for elderly

  • BACKGROUND/OBJECTS: When the functions involved in the ingestion of food occurs failure, not only loss of enjoyment of eating, it will be faced with protein-energy malnutrition. Dysmasesis and difficulty of swallowing occurs in various diseases, but it may be a major cause of aging, and elderly people with authoring and dysmasesis and difficulty of swallowing in the aging society is expected to increase rapidly. SUBJECTS/METHODS: In this study, we carried out a survey targeting nutritionists who work in elderly care facilities, and examined characteristics of offering of foods for elderly and the degree of demand of development of easily chewable and swallowable foods for the elderly who can crush foods and take that by their own tongues, and sometimes have difficulty in drinking water and tea. RESULTS: In elderly care facilities, it was found to provide a finely chopped food or ground food that was ground with water in a blender for elderly with dysmasesis. Elderly satisfaction of provided foods is appeared overall low. Results of investigating the applicability of foods for elderly and the reflection will of menus, were showed the highest response rate in a gelification method in molecular gastronomic science technics, and results of investigating the frequent food of the elderly; representative menu of beef, pork, white fish, anchovies and spinach, were showed Korean barbecue beef, hot pepper paste stir fried pork, pan fried white fish, stir fried anchovy, seasoned spinach were the highest offer frequency. CONCLUSIONS: This study will provide the fundamentals of the development of easily chewable and swallowable foods, gelification, for the elderly. The study will also illustrate that, in the elderly, food undergone gelification will reduce the risk of swallowing down to the wrong pipe and improve overall food preference.

하악전돌증 환자에서 기관내 삽관을 위한 기도평가에 관한 연구 (Airway Evaluation for Endotracheal Intubation of Mandibular Prognathic Patient)

  • Background: The fundamental responsibility of an anesthesiologist is to maintain adequate gas exchange. Failure to maintain a patent airway can result in brain damage or death. Generally, in patients with mandibular prognathism, who have the protruded mandible, the mask ventilation was thought to be not easy. The purpose of this study was to observe the degree of the difficulty of airway management in mandibular prognathism using some anatomic criteria for defining and grading difficulty of airway and difficulty of endotracheal intubation with direct laryngoscope. Methods: The observations and measurements are done to the 54 patients with mandibular prognathism, who were scheduled for corrective esthetic surgery. The case study is done to the 30 patients with normal mandible for control group. In all patients, mouth opening distance (MOD), mouse opening angle (MOA), mandibular length (ML), mandibular depth (MD), thyromental distance (TMD), thyromental area (TMA), Mallampati grades, and Cormack and Lehane grades are measured. T-test and Chi-square test are done (P < 0.05). Results: In the mandibular prognathism cases, the measurements of MD, TMD and TMA are more greater than those of controls (P < 0.05). Mallampati grades with tongue thrust are higher in the female mandibular prognathism cases than those of female controls. Most of the grades of the mandibular prognathism cases with Cormack and Lehane grading system are I or II being easy intubation cases (P < 0.05) Conclusions: In the patients of mandibular prognathism, the intubation with laryngoscope will be easer than that of normal mandible in general. It is for that their laryngeal aperture can be easily visible when the laryngoscope are used.

임상시험 시험대상자설명서의 가독성 평가 (Readability of Patient Information Leaflets in Clinical Trials)

  • Background: Elements of informed consent including capacity, disclosure, understanding, voluntariness, and permission of the participant, are all crucial for clinical trials to be legally and ethically valid. During the informed consent process, the patient information leaflet is an important information source which prospective research subjects can utilize in their decision-making. In the adequate provision of information, KGCP guideline necessitate 20 specific items, as well as the use language that individuals can understand. This study measures the vocabulary level of patient information leaflets in an effort to provide an objective evaluation on the readability of such material. Methods: The word difficulty of 13 leaflets was quantitatively evaluated using Kim kwang Hae's vocabulary grading framework, which was compared to the difficulty level of words found in the $6^{th}$ grade Korean textbook. The quantitative outcomes were statistically analyzed using chi-squared tests and linear by linear association for ordinal data. Results: There was a statistically significant difference between the vocabulary level and frequency of words in leaflets and the 6th Korean textbook. The leaflets were on average 260 sentences and about roughly 15 pages long, including lay language (easier or equal to language used in primary school) of around 12% less; technical language of around 4.5% more. As the vocabulary grades increase, there was a distinct difference in vocabulary level between Korean textbook and each information leaflet (p < 0.001). Conclusion: Patient information leaflets may fail to provide appropriate information for self-determination by clinical trial subject through the difficulty level of its wording. Improvements in the degree of patients' understanding and appropriate use of information leaflets are collaboratively equipped to strengthen patient's autonomy and therefore guaranteeing participant's rights.

DACUM 기법을 이용한 재활간호사의 직무분석 (Task Analysis of the Job Description of Rehabilitation Nurse based on DACUM)

  • Purpose: The aim of this study was to develop and to analyze the duty and the task of rehabilitation nurse in Korea. Method: The definition of rehabilitation nurse and job description was developed based on developing curriculum(DACUM) by panels who have experienced in DACUM analysis and rehabilitation nursing. 228 nurses who were working at rehabilitation centers and rehabilitation unit in general hospitals were participated. The questionnaire included frequency, importance, and difficulty of duties and tasks. The data were collected in March and April 2009, analyzed by descriptive statistics. Results: The job description of rehabilitation nurse in Korea revealed 11 duties, and 61 tasks. On the all 11 duties, the highest duty in frequency and importance was direct nursing care ($2.47{\pm}.31$, $2.77{\pm}2.8$) and the highest duty in difficulty was self-development ($2.39{\pm}.48$). Among the tasks of duties 'direct nursing care' in 'manage medication' ($2.87{\pm}.35$) in frequency, the task 'prevent and manage bedsore' ($2.91{\pm}.31$) of duty 'direct nursing care' in importance, and 'cope emergency situations' ($2.72{\pm}.49$) of duty direct nursing care in difficulty showed the highest degree. Conclusion: The political efforts for the legislation of role and task of rehabilitation nurse were needed.

초등학생의 수직선 이해와 사용의 어려움 (Difficulty of understanding and using the number line by Elementary school students)

  • 본 연구는 초등학생들이 수 개념과 관련하여 수직선을 어떻게 이해하고 사용하는지, 또 그 학습의 어려움은 무엇인지 파악하고자 하였다. 이를 위하여 수직선 은유가 수 개념과 어떻게 관련되는지 살펴보았고, 프로이덴탈의 수 개념지도론에서 수직선의 역할에 대하여 고찰하였다. 실제 초등학생들의 수직선에 대한 이해와 사용의 어려움을 파악하기 위해 실시한 검사는 수직선에 주어진 위치에서 적절한 수를 대응시키는 문항과 학년별로 수직선이 활용되는 관련 단원 내용을 묻는 문항으로 이루어졌다. 같은 내용과 구조의 문항이지만 수직선으로 표현된 것은 해결하지 못하면서 수 트랙이나 다른 그림으로 표현된 것은 해결하는 학생들이 다수 관찰되었고, 본 연구에서는 이러한 현상의 의미를 해석하고자 하였다. 또한 다양한 교수-학습 자료(수 트랙, 그림, 빈 수직선, 이중 수직선등)를 활용하여 수직선 이해의 어려움을 보완하고 관련 수 개념 학습을 돕는 방안을 제안하였다.

의료과오소송 입증책임론의 전개와 발전 (The Development on Medical Malpractice Lawsuit and its Burden of Proof)

  • The medical practice does not always get a satisfatory result since the disease progress of patients are depended on patients' physical constitution and the doctors cannot control the outcomes about patients' physiological and biological reaction after the treatment. Moreover, the medical practice may bring wrong result fatalistically because of the unpredictablility of life. To demand for compensation of the damage to the doctors about these wrong result, the patient side holds the burden of proof that is between medical practice and demage, and there is damage from doctor's malpractice according to the accepted theory about the fundamental principle of distribution of the burden of proof. This falls not only under the liability of Tort Law, but also liability of Contract Law. However, the patient may be in difficult situation to prove the malpractice of doctors since he or she cannot recognize the facts because he or she was in unconscious while the medical practice was conducted, or they cannot judge precisely even though they recognize the facts. Nevertheless, the lawsuits against medical malpractice are the field that never achieves the equality of arms since the most of the evidence belong to the doctor's side. Hence, to maintain the principle of the equality of arms under the constitution, the theory leads to alleviate the burden of proof that patients hold. However, the doctors cannot be asked for the burden of proof that they conduct medical practice without errors. Because the doctors may experience difficulty to prove their innocence as the patients because of the unique characteristic that medical practices have. Therefore, the methods of the alleviation of the patient's burden of proof should have the equality of arms and the equal opportunity between the patients and the doctors with the evaluation of the justifiable interest from both the patients and the doctors. As the methods of the alleviation of the burden of proof, the alleviation of the demands and the degree of the burden of proof or resolutely the conversion of the burden may be considered. However, Recognizing the exception from general principle with converting the burden of proof is not proper in principle because the doctors may experience difficulty of the proof as the patients may have. If the difficulty of proof can be resolved by alleviating of the demands and the degree of the burden of proof, it is more desirable resolution rather than converting the burden of proof.

치위생과 학생들의 성취목표지향성과 자기효능감이 전공적응 및 만족에 미치는 영향 (The effects of dental-hygiene students' goal achievement orientation on their self-efficacy and major adaptation and satisfaction)

  • Objectives : The purpose of this research is to examine the effects of the goal achievement orientation of the students in dental-hygiene practice classes on their self-efficacy and on their adaptation to and satisfaction with their major. Methods : A survey was conducted among the dental-hygiene students in schools located in the Daegu region on October 24-31, 2011. A total of 212 accomplished questionnaires were gathered as sources of final analytical data. Statistical analysis was conducted on the collected data, using the SPSS 19.0 program. Results : 1. The students who had good personal relationships(p=.000) and who chose their major and school based on their aptitude(p=.029) were found to have a higher degree of satisfaction than those who did not have good personal relationships and who did not choose their major and school based on their aptitude. 2. Self-confidence(p=.004), self-control efficacy(p=.000), mastery approach(p=.000), and performance approach(p=.003) were found to have positive correlations with major adaptation while task difficulty preference(p=.000) was found to have a negative correlation with the same. 3. Self-confidence(p=.004), self-control efficacy(p=.000), mastery approach(p=.000), and performance approach(p=.045) were also found to have a positive correlation with major satisfaction while task difficulty preference(p=.000) was found to have a negative correlation with the same. 4. Mastery approach(p=.000) in goal achievement orientation was found to have a positive effect on self-efficacy while mastery avoidance(p=.001) was found to have a negative effect on the same. Mastery avoidance showed the greatest effect on self-confidence(p=.000) while mastery approach showed the greatest effect on self-control efficacy(p=.000) and task difficulty preference(p=.000). 5. In goal achievement orientation, mastery approach(p=.000) was found to have a positive effect on major adaptation while mastery avoidance was found to have a negative effect on the same. Meanwhile, with regard to self-efficacy, self-control efficacy(p=.037) and task difficulty preference(p=.008) were found to have positive effects on major adaptation. 6. In the case of major satisfaction, mastery approach(p=.000) in goal achievement orientation was found to have a positive effect on it, and in self-efficacy, self-control efficacy(p=.038) and task difficulty preference(p=.002) were found to have positive effects on it. Conclusions : Accordingly, to raise the level of the dental-hygiene students' adaptation to and satisfaction with their major, the students should be given appropriate tasks so that they can raise their mastery goal, and they should be encouraged to raise their approach goal rather than their avoidance goal. In addition, the students should be provided assistance in setting self-expectations and in developing confidence in their capabilities.

선박 접이안 조종 시뮬레이터를 이용한 조종위험도 평가 기법에 관한 연구 (A Study on Evaluation Technique of Manoeuvring Difficulty by Using the Ship Manoeuvre Simulator for Berthing/Deberthing)

  • 선박이라는 거대한 물체를 항만 또는 조선소내의 안벽에 접안하거나 이로부터 이안시키는 작업은 도선사나 도크마스터의 고유한 업무로서 상당한 집중력과 고도의 위험성을 내포하고 있다. 이와 같은 접이안 작업 시에는 예선의 사용량이 많아지고, 본선이 저속으로 움직이므로 외력의 영향을 크게 받는다. 본 연구에서는 외력의 영향 특히 강풍 하에서 주로 예선을 이용하여 접이안 조종 작업을 수행할 수 있는 2차원 가상시스템은 구축하였다. 또한 조종위험도를 평가할 수 있는 객관적인 기준을 제안하고 이를 위해 시뮬레이션 실험은 수행하였다. 이 가상시스템을 이용하여 접이안 조종 작업을 수행한 결과 운항자가 주관적으로 체감한 위험의 정도를 수치화하여 표현한 주관적 평가와 조종운동과 관련된 주요 파라미터에 관련된 객관적인 지수와의 상관관계를 분석하여 조종위험도의 평가 기법에 대해 논의한다.

학교수학의 각 영영에 대한 선호도 연구 (Studies on the Degree of Likings for Each Units of School Mathematics)

  • School mathematics are composed of five major units of numbers& algebraic expressions, equations & inequalities, functions, figures, and statistics & probabilities. But if we look into the general activities of mathematics teachers in their class, they rather do not take into account students\` cognitive and affective traits or degree of difficulty which each of the unit has. For successful teaching of mathematics, teachers should take into consideration many affective items which influence the students\` scholastic achievement. Among them student\`s liking for the mathematics is commonly accepted as the most important factor for successful learning. In this study, with the five units of school mathematics, we investigated the students\` degree of likings for each unit. To fined out whether there are any differences in students\` likings for the mathematics, between regions and kind of schools, we classified the population according to the locations and kinds of schools. To do this, we divided the whole region into four parts such as Seoul, large city, medium city and town. Moreover, we partitioned the whole secondary school students into four groups of middle school students, vocational high school students, pro-science students of academic high schools, and pro-liberal arts students of academic high schools. From each partition, we sampled similar size of experimental groups which came up to total 1260 students. Analysing the answer sheets which the students responded about the questionnaire, we investigated the following questions using the ANOVA test. 1. Is there any differences in the trend of likings for each unit between the regional classifications? 2. Is there any differences in the trend of likings for each unit between the classifications of secondary schools? 3. What trends of changes are there in the degree of likings for each unit according to the rising of students\` grade?
