Objectives: Metabolic syndrome is considered a coronary heart disease risk factor and its prevalence rate is increasing in Korea. Because obesity is relevant to metabolic syndrome, we investigated the relationship between metabolic syndrome and the Obesity Pattern Identification Questionnaire in middle-aged health check-up examinees. Methods: This was a cross-sectional study with 125 patients who visited a health promotion center of university hospital from October 2012 to January 2013. We analyzed the association of Obesity Pattern Identification Questionnaire and the diagnostic criteria of metabolic syndrome. Results: Pi deficiency (脾虛), phlegm (痰飮), liver stasis (肝鬱) and food accumulation (食積) pattern showed significantly highs score in the group with hypertriglyceridemia. Also, females demonstrated significantly high scores of liver stasis (肝鬱) and food accumulation (食積) in the group with hypertriglyceridemia. The questions of Pattern Identification that showed especially significant high score in the group of hypertriglyceridemia are as follows: 'Easily get annoyed', 'Usually worried', 'Frequently overeating or bingeing', and 'Having more after getting full'. There are positive correlations between triglyceride and the score of Pi deficiency (脾虛), phlegm (痰飮) and food accumulation (食積) pattern. Conclusions: Obesity Pattern Identification Questionnaire can be used for the management of hypertriglyceridemia in an effort to prevent metabolic syndrome.

The purpose of this study is to investigate clinical frequency and priority of five pattern identification settled by Korean Standard Differentiation of the Symptoms and Sign for Stroke. The present study was done over 177 hospitalized patient with stroke in the Daejeon University Oriental Medical Hospital in the period of November 2006 to July 2007. Stroke patients had been interviewed by residents and specialists who studied standard operation procedures in Fundamental study for Standardization and Objectification of Differentiation and Pattern Identification of Syndrome of Oriental Medicine for Stroke. In Heat-transformation pattern group, men showed significantly high frequency and in Deficiency of Gi pattern group, women showed significantly high frequency. In Heat-transformation pattern group, the indicators such as 'aversion to heat during fever', 'flushed face', 'full and rapid pulse', 'dryness of the eyes, 'yellow coating of the tongue', 'feverishness of the limbs', 'dryness of the eyes' showed significantly high frequency. In Dampphlegm syndrome group, the indicators such as 'white coating of the tongue', 'yellowish face', 'thick coating of the tongue', 'wheezing in the throat with sputum', 'swollen tongue', 'slippery pulse' showed significantly high frequency. In Deficiency of Gi pattern group, the indicators such as 'pale tongue', 'lassitude', 'pale face', 'weakness pulse' showed significantly high frequency. In Deficiency of Eum group, the indicators such as 'short and rapid pulse', 'mirror-like tongue' showed significantly high frequency. For more sensitive Korean Standard Differentiation of the Symptoms and Signs for Stroke, Large scale study is to be done, giving weight on the important indicators.

The purpose of this study is to select the major pulse indicators and evaluate their significance in discriminating the subtypes of Pattern Identifications (PI) from stroke patients. Decision tree analysis was carried out using clinical data collected from 835 stroke patients with the same subtypes diagnosed identically by two experts with more than 3 year clinical experiences. Among the 10 pulse indicators, 6 major pulse indicators (slow, rapid, strong, weak, slippery, and fine pulse) were selected by decision tree analysis. The accumulated distributions of six pulse indicators in each PI showed that strong was major pulse indicator in Fire-Heat pattern, slippery in Dampness Phlegm pattern, weak in Qi Deficiency pattern. But there were two major combinations in Yin deficiency pattern, weak or fine with rapid pulse and weak or fine without rapid pulse. Therefore, it is suggested that 6 pulse indicators can be used for discrimination of PI in stroke patients, though the combination studies between these pulse indicators and the other PI indicators are left for further study.

Objectives : The aim of this study was to develop a standard tool of pattern identification for chronic cough, which will be applied to clinical research. Methods : The items and structures of the instrument were based on reviews of textbooks and published literature. The advisor committee on this study consisted of 11 Korean respiratory internal medicine professors, one clinical fellow, and five clinicians with 10 years of experience. The questionnaire, which includes the signs and symptoms of chronic cough, was studied by the delphi method. Delphi examination was carried out via email, through evaluating the importance of symptoms included in each pattern. Results : We divided the pattern identification of chronic cough into five patterns: Wind-Cold, Phlegm-Turbidity, Liver-Fire, Lung Deficiency, and Kidney Yang Deficiency. By the Delphi method and a score evaluation, 38 items were chosen for pattern identification of chronic cough. Conclusions : Through this study, we created a Korean instrument for the pattern identification tool for chronic cough. We expect to apply this tool to subsequent research as its validity and reliability are further confirmed.

Purpose : This study was conducted to evaluate the red cell indices and frequency of iron deficiency anemia based on the feeding patterns of nine-month-old infants. Methods : Blood tests were performed on 253 nine-month-old infants, who visited Il Sin Christian Hospital for health check-ups from January to December 2007. Their parents answered telephonic questions regarding their feeding patterns and weaning foods. Results : Three infants groups were created according to feeding patterns before they started weaning foods. One group was exclusively breast-fed (48.6%), another had mixed feeding (27.3%), and the third had artificial milk feeding (24.1%). Red cell indices (hemoglobin, hematocrit, MCV, MCH) of the breast-fed group were comparatively lower than those of the other two groups (P<0.05). Twenty-five infants (9.9%) were diagnosed with iron-deficiency anemia. According to feeding patterns, the frequency of anemia was highest in the breast-fed group. Six infants who started weaning foods before six months of age (113 infants) were diagnosed with iron-deficiency anemia (5.3%), and nineteen who started after six months of age (140 infants) were diagnosed with iron-deficiency anemia (13.6%). Conclusion : When nine-month old infants visit hospitals for health check-ups, pediatricians must consider their feeding pattern and weaning foods histories, and then recommend screening blood tests for iron-deficiency anemia.

Objective : Yukmijihwang-tang is one of the important medicines for blood- deficiency and yin-deficiency. Atopic dermatitis usually shows dampness-heat pattern in its acute stage and blood-deficiency or yin-deficiency pattern in its chronic stage. Therefore, I hypothesized that Yukmijihwang-tang is effective on atopic dermatitis and investigated the effects of Yukmijihwang-tang on NC/Nga mice's atopic dermatitis induced by DNCB. Methods : The NC/Nga mice with atopic dermatitis were divided into four groups: three experimental groups and one control group. The experimental groups were respectively put on 2.5g(YM-2.5), 5g(YM-5) and 10g(YM-10) of Yukmijihwang-tang extract per their weight once a day for 10 days while the control group was fed normal saline. After 10 days, I measured TEWL(transepidermal water loss), observed scratching behaviors, conducted a skin biopsy and checked levels of Total IgE, IL-4 and $IFN-\gamma$ on NC/Nga mice. Results : 1. Yukmijihwang-tang significantly suppressed skin dryness. In particular, YM-5 and YM-10 had better skin hydration than YM-2.5. 2. Yukmijihwang-tang significantly suppressed pruritus while there was no significant difference among the experimental groups. 3. Yukmijihwang-tang retained skin structure(epidermis, dermis and subcutaneous fat). 4. Yukmijihwang-tang reduced Total IgE level while there was no significant difference between the control group and the experimental groups. 5. Yukmijihwang-tang did not reduce IL-4(Th2 cytokine) level. 6. Yukmijihwang-tang significantly reduced $IFN-\gamma$(Th1 cytokine) level. In particular, YM-2.5 and YM-5 had lower $IFN-\gamma$ level than YM-10. Conclusion : The results suggest that Yukmijihwang-tang suppresses skin dryness and pruritus, retains skin structure and is effective on chronic atopic dermatitis which is associated with Th1 cytokines in the immune response.
Objectives: This study aimed to assess a Qi Blood Yin Yang evaluation method systematically and objectively and to identify the correlation between the Qi Blood Yin Yang deficiency pattern (QBYYDP) and facial color. Methods: Thirty-seven participants (17 males, 20 females) were enrolled in this study. Twenty-four (10 males, 14 females) had ages from 40 to over 60, and 13 (7 males and 6 females) were in their twenties. After sufficient rest, facial images were taken with a camera. Based on the results from a questionnaire survey, we divided the participants into five groups: the normal and the Qi-, Blood-, Yin-, and Yang-deficient groups, after which the relationships between the L, 'a', and 'b' values in the Lab color system and the characteristics of the participants in each of the deficient groups were elucidated using a facial color analysis program. Results: The color analysis for Qi-deficient (QD) participants revealed that the L value was fairly decreased in comparison with the normal participants, but the 'a' and 'b' values were almost the same. A comparison between the normal and the Yang-deficient (YaD) groups revealed that the L values were somewhat lower compared to the normal group, but the 'a' and 'b' values were not statistically different. For the Yin-deficient (YiD) group, the L value was slightly lower compared to the normal group, but the 'a' and 'b' values were almost the same and the R values were slightly increased. For the Blood-deficient (BD) group, the L values were slightly increased compared to the normal group, but the 'a' and 'b' values were decreased slightly. Conclusion: This study obtained objective, reliable data for judging the QBYYDP by using facial images and a color analysis program. However, further study with at least 10 or more subjects in each of the deficient groups is necessary to confirm our findings.

Here, overabundant qi arises due to the works of multiplication and insultation. When this overabundant qi is in force, the retaliating qi appears without fail to create a parallel. This is the Autonomous Equilibrium Mechanism found in the natural world. Studying this mechanism is deeply significant in understanding the mechanisms of diseases. Methods : The paper is written by reviewing the texts found in Huangdineijing's Chapters of Yunqi, which are $Q{\grave{i}}jiaobiandalun$, Wuchangzhengdalun, $Liuyuanzhengj{\grave{i}}dalun$, Zhizhenyaodalun, and Suwenliuqixuanzhumiyu. Results & Conclusions : The overabundance and retaliation in Five Circuits take the form of the restrained child of the Five Circuits takes revenge on the overabundant qi on behalf of his mother. The overabundance and retaliation in Six Qi take the form of rapid healing of Benqi which was in stagnation. Traditionally, overabundant qi is the only one in existence when the Five Circuits are in excess and the year of Hai yin wu wei you xu's regular transformation. During this time, retaliating qi does not exist. When Five Circuits are in deficiency and in the year of Si shen zi chou mao chen's pattern transformation, both the overabundant and deficiency qis exist. However, regardless of regular transformation or pattern transformation, overabundant qi and retaliating qi cannot exist at the same time. This seems to be the rational conclusion. There are some regulations that overabundance and retaliation follow. First, the strong and weak, and number of days coincide. Second, overabundance qi appear during the first half of the period when the qi of controling heaven is in place. During the later half of the period when the qi of terrestrial effect is in force, retaliation qi is the one that appears. Third, overabundance and retaliation does not end with one time. Rather, they will continue to repeat appearance and disappearance without any set pattern. Fourth, the overabundance and retaliation of the guest qi and dominant qi only has overabundance and no retaliation.

In this paper, when a physician make a diagnosis of the pattern identification (PI) in Korean stroke patients, the development methods of the PI classification function is considered by diagnostic questionnaire of the PI for stroke patients. Clinical data collected from 1,502 stroke patients who was identically diagnosed for the PI subtypes diagnosed by two physicians with more than 3 years experiences in 13 oriental medical hospitals. In order to develop the classification function into PI using Korean Stroke Syndrome Differentiation Standard was consist of the 44 items (Fire heat(19), Qi deficiency(11), Yin deficiency(7), Dampness-phlegm(7)). Using the 44 items, we took diagnostic and prediction accuracy rate through of discriminant model. The overall diagnostic and prediction accuracy rate of the PI subtypes for discriminant model was 74.37%, 70.88% respectively.
Objectives : This article is a study on to which categories of modern diseases blood stasis patterns are assigned and the meaning of blood stasis interpreted with perspectives of Korean Medicine and modern medicine. Methods : We reviewed "Neijing", "Shanghanlun", "Yilingaicuo", "Xuezhenglun" and other books and modern clinical papers related with blood stasis. Results : 1. Blood stasis patterns are related with disorders of hemorrheology, hemodynamics, platelet function, microcirculation, microelements and endothelial damage. 2. From the types of syndrome differentiation, diverse diseases classified in type of qi deficiency with blood stasis and type of blood stasis due to qi stagnation are reported, which reflects qi and blood are closely connected. And many diseases are classified in type of kidney deficiency with blood stasis, which has something in common with chronic diseases can achieve effect from treatment considering blood stasis. 3. Diseases related with kidney involve menopausal disorder, mazoplasia, prostatitis, erectile dysfunction, chronic nephritis, renal calculus, osteoporosis and bursitis. Diseases related with heart involve coronary artery disease, arrhythmia and cerebral thrombosis. Diseases related with spleen involve gastritis, colonitis and digestive organ ulcer. Diseases related with liver involve hepatitis, hyperthyroidism and stroke. Diseases related with lung involve neurodermatitis, bronchitis and paranasal sinusitis. Conclusions : Blood stasis pattern which is one of the areas to draw medicine's attention has broad clinical application.
