A Study of the Case Record on Dyspnea and Wheezing Asthma Recorded in Xu Ming Yi Lei An ((${\ll}$속명의류안(續名醫類案)${\gg}$에 기재(記載)된 천(喘) 및 효천(哮喘)에 관(關)한 의안(醫案) 연구(硏究))

  • Objectives : Select and analyze the case record of dyspnea and wheezing asthma recorded in Xu Ming Yi Lei An that is the most abundant and wide in contents in existing case records that are systematic, comprehending relatively modern Traditional Chinese Medicine to secure more deep and objective basis of Traditional Chinese Medicine approach for dyspnea and wheezing asthma to analyze and review possibility for clinical application in this study. Methods: The study was conducted with the case records of dyspnea and wheezing asthma in whole Xu Ming Yi Lei An. Pattern identify and classify selected case records and again classified with deficiency syndrome and excess syndrome. Also analyzed prescriptions and herbs used in the case records. Nature of herbs and properties and flavors that were used in the case records were classified and frequency of each nature of herbs were analyzed. Applicable case records were interpreted and suggested prescriptions, pulse feelings, pattern classification were analyzed and described. Results : Among the 5254 case records stated on the complete collection, it is researched that there are 63 case records for the symptom complex of dyspnea as 1.2% of the whole case records, and the case records on the symptom complex of wheezing asthma are 14 as the 0.27% of the total examples. 63 case record examples related with symptom complex of dyspnea were pattern identified and classified. As a result, deficiency syndrome of the Kidney(33 %), deficiency syndrome of the Spleen(26.0%), Wind-Cold(12.3%), phlegm turbidity(12.3%), Heat in the Lung(8.2%), asthenia of the Lung(8.2%) were investigated as above order. 14 case record examples related with wheezing asthma were pattern identified and classified. As a result, phlegm-Heat(26.3%), upper excess and lower deficiency(26.3%), external affections Wind-Cold(15.8%), Dampness-phlegm(10.5%), Lung asthenia(10.5%), Cold phlegm(5.3%), mutual deficiency and detriment of Heart and Kidneys(5.3%) were investigated as above order. Symptom complex of dyspnea has 67.1% of deficiency syndrome, 32.9% of excess syndrome resulting more deficiency syndrome than excess syndrome. Symptom complex of wheezing asthma has 42.1 % of deficiency syndrome and 57.9% of excess syndrome resulting more excess syndrome than deficiency syndrome. In case of symptom complex of dyspnea prescription used in the case record, the order of frequency is as following. Palmijihwang-tang, Bojung-ikgitang, Yungmijihwang-tang, Ijintang, Sojaganggitang, Igongsan. In case of symptom complex of wheezing asthma prescription in the case record, Yungmijihwang-tang, Ohotang, Dodamtang were mostly used. Herbs used in case records of symptom complex of dyspnea are Ginseng Radix, Poria, Glycyrrhizae Radix, Aconiti Iateralis Preparata Radix, Atractylodis Macrocephalae Rhizoma, Dioscoreae Rhizoma, Angelicae Gigantis Radix, Rehmanniae Radix Preparat, Pinelliae Rhizoma, Zingiberis Rhizoma Recens are mostly used. Nature of herb properties used for symptom complex of dyspnea and symptom complex of wheezing asthma are herbs that are warm properties. When the symptom complex of dyspnea and the symptom complex of wheezing asthma were treated. if the patient felt tenderness at Pyesu, doctors conducted pricking blood around the opposite Pyesu or Sipseon acupoint. when the patient didn't have tenderness at Pyesu by soft press, pricking blood was performed both sidees, right and left Pyesu. In case of the treatment of symptom complex of dyspnea and symptom complex of wheezing asthma, when they got treatment, when the symptom complex of disease is severe, a doctor cauterized the opposite Pyesu while the other Pyesu felt tenderness, and decided how the above treatment is performed whether the degree of the symptom compolex of disease is severe or not. In case of the treatment of symptom complex of dyspnea and symptom complex of wheezing asthma, if the person felt tenderness at Pyesu and is caught by the Wind-Cold pathogen, slight acupuncture is treated at relevant Pyesu with Fire needling. When patient with symptom complex of dyspnea and symptom complex of wheezing asthma cannot hawk sputum up from the oral and laryngopharynx, suction method is treated. Conclusion : With this study, actual traditional and clinical pattern identification form and characteristics of symptom complex of dyspnea and symptom complex of wheezing asthma were recognized. Modern case report utilizing in clinical application need to be secured and an incurable disease asthma need to be diagnosed and improvement for treatments have to be searched through other case records.

The Effect of Fasting and Vitamin B6 Repletion on Vitamin$B_6$ Metabolism in Rats (금식 또는 Vitamin$B_6$ 보충급식이 흰쥐의 Vitamin B6 대사에 미치는 영향)

  • The purpose of this study were to investigate the effect of fasting and vitamin B6 repletion on tissue concentration of pyridoxal 5-phosphate and urinary excreteion of 4-pyridoxic acid in vitamin B6 deficient rats. Sixty six rats(6 per group) were fed either a vitamin B6 deficient diet (-B6) or a control diet (+B6) for 6 weeks and then rats were repleted with +B6 diet for 2 weeks. Rats were fasted for 1 and 3 days and for 3 days after repletion. Pyridoxal 5-phosphate (PLP) concentration in plasma, liver, skeletal muscle, and heart muscle and urinary 4-pyridoxic acid (4-PA) excretion were compared. Fasting resulted in a significant increase in PLP concentration in the plasma, liver and heart muscle of rats fed the -B6 diet. Skeletal muscle PLP concentration was significantly decreased in +B6 rats but not in -B6 rats. Following vitamin B6 repletion, PLP concentration in the plasma, liver and heart muscle in previously -B6 rats was similar to the respective concentration in +B6 rats while PLP concentration in the skeletal muscle of previously -B6 rats increased, but it was not reached to that of +B6 rats. At day 1 and 2 of the fast, urinary 4-PT excretion increased in both +B6 and -B6 rats although there was no supply of vitamin B6 due to fasting. These results suggest that vitami B6 is redistributed as PLP when there is a caloric deficit and PLP is supplied by an endogenous source, possibly PLP bound to skeletal muscle glycogen phosphorylase.

  • Objectives : The aim of this study was to help treatment of Anxiety disorder patients through the study on Keongke(驚悸) and Cheongchung(怔忡) in "Donguibogam" Methods : Author searched the contents of Keongke(驚悸) and Cheongchung(怔忡) in "Donguibogam" and classfied them by concept, pathogenesis and treatment. Results : 1. Concepts of Keongke(驚悸) and Cheongchung(怔忡) are to leap up, be nervous and fear something. Keongke(驚悸) and Cheongchung(怔忡) are same kinds of disease but they are only distinguished as their seriousness. 2. There are four main causes of Keongke(驚悸) and Cheongchung(怔忡) such as phlegm and fluid retention(痰飮), insufficiency of the heart(心虛), being blocked of qi(氣鬱) and Hwa(火). Additionally the diseases are caused by astonishment(驚) excessive thought(思慮過度), insufficiency of the liver(肝虛), excessive perspiration and dirarrhea (汗,下過多) insufficiency of qi(氣鬱) 3. the number of herbal medicines which treat Keongke(驚悸) and Cheongchung(怔忡) are 68. In result of analyzing them, the number of herbal medicines to treat phlegm(痰) are 22 and the number of herbal medicines to treat deficiency of the heart blood(血心虛) are 18. 4. The number of herbs which treat Keongke(驚悸) and Cheongchung(怔忡) are 25. They stabilize Hon-Baek(魂魄), spirit(精神) and mind(心神), supplement the heart blood, and treat Damhwa(淡火) and Hwa(火). Poria(茯神) take a rule of leading to the causes of Keongke(驚悸) and Cheongchung(怔忡).

  • Tongue diagnosis is a profound and special part of the whole Oriental Medicine. We examined the method, the principle and the meaning of tongue diagnosis according to a literature cited, considered a meaning of tongue diagnosis. As a result, we come to a conclusion like that. 1. Tongue is related with internal organs by meridian system, especially has a direct connection with heart and spleen. 2. The heart, a master of internal organs, has its specific opening in the tongue. The spleen, source of nutrients for growth and development, has its specific body opening in the mouth. So tongue reflects states of internal organs, Qi, blood, the constructive energy and the defensive energy. 3. When doing tongue diagnosis, we must pay attention to the position and the order of inspection of the tongue. We must diagnose by referring to a ray of light, diet, season, age, physical constitution, habit and taste, can make a accurate diagnosis. 4. We can classify constitutions, distinguish syndromes, suppose prognosis, make a prescription by using tongue diagnosis. 5. Reddened tip of the tongue represents flaring-up of the heart fire, and it means psychologic stress. Dental identations on the tongue edges represents deficiency of Qi of the spleen, and it means physiologic fatigue. 6. Through observing humidity of fur of the tongue, we can guess psychologic stress and physiologic fatigue. Through observing thickness of fur of the tongue, we can guess function of spleen and stomach.

Clinical Features of Electrogastrogram in Dyspeptic Patients with Stomach Qi Deficiency (위기허증으로 진단된 소화불량 환자의 임상적 특징; 위전도 검사를 중심으로)

  • Objectives: This study investigated the cutaneous electrogastrogram (EGG) and other clinical characteristics of dyspeptic patients who have been diagnosed with Stomach Qi Deficiency (SQD) using the Scale for Stomach Qi Deficiency (SSQD). Methods: This study reviewed the clinical records of 38 patients with dyspepsia who were evaluated with SSQD and EGG at the Department of Digestive Diseases of Kyung Hee University Korean Medicine Hospital in Seoul, Korea from November 1, 2019 to February 29, 2020. We evaluated the EGG and other clinical characteristics of the SQD patients to determine if there was an association between the SSQD scores and the EGG. Results: In terms of the EGG, the SQD patients showed no significant increase in the percentage of normal slow wave after a meal and a slightly decreased power ratio at Channel 1 and Channel 2. We also found an association between the SSQD scores and the EGG parameters at Channel 1 and Channel 3. The average Ryodoraku score of the patients was 33.00±14.90 (μA). In the Heart Rate Variability (HRV) test, the average Total Power (TP) and Low Frequency/High Frequency (LF/HF)) ratio was 1356.60±13 6.41(ms2) and 1.68±2.25, respectively. Conclusions: The results of this study suggest that clinicians can use Electrogastrography to enhance accuracy when diagnosing the SQD pattern.

  • Dreams are experiences of life that is recognized to have taken place in .the mind while asleep. The phenomenon of dreams is occured by external stimuli such as somatic factor and emotion-arousing pre-sleep stimuli. The contents of dreams reflect the dreamer's unique psychic situation, we can view them from category of Yin and Yang and the Five Elements(陰陽五行). Also dreams are characterized by an objectivity that provides whatever in necessary for psychic balance, regardless of the ego's wishes. Hence dreams are objective sources of psychotherapy. The doctor must not treat patient by the doctor's subject mind. The Orient has taken the idea that human beings have the divine center in the heart, so dreams in the psychotherapy of the oriental medicine are more valuable as representation of inner world. The images of dream are classified as the deficiency and excessiveness of Yin-Ki and Yang-Ki of the internal organs, an property and source of stimulus. Dream applies to diagnose the disease of internal organs and to decide to the method of medical treatment.

  • Since ancient times, records pertaining to chi-reinforecenent of ginseng have been documented in books of Chinese nedicine. We, therefore, assessed the therapeutic effects of ginseng on patients with chi-deficiency syndrome. Eigheen cases of chi-deficiency, screened at the outpatient service in the internal department of Chinse medicine at the China Medical College Hospital. were prescribed two grams of Korean red ginseng powder three times daily for a two week period. For evaluation, a questionnaire and diagnosis by sphygmography were conducted prior to and after administration of the prescription. For control purposes, fourteen volunteers fasted for more than five days (seven had access only to ginseng. and the other seven were given only mineral water) and were subsequently evaluated by the same criteria. Three results were obtained for the ginseng treated group: (1) improvements in symptoms of chi-deficiency such as fatigue and dizziness; (2) changes in pulse waves with time domains showing alternate P-waves, strengthened dicrotic waves, and some minute waves in the end-diastolic period; (3) frequency domains with enhanced amplitudes around 12Hz. These three findings, which are consistent with each other, suggest the reinforcement of the effect of ginseng on 'heart-spleen chi' in Chinese medicine.

  • Objectives : We suggest the method of oriental neuropsychiatry treatment about amnesia through herb therapy. Methods : We investigate cause of disease, component of herbs about amnesia with classic current oriental medicine books. Results : Amnesia is due to simsinbulgyo(心腎不交), biwieyangher(脾胃陽虛), dammisimgyoo(痰迷心竅), emotional damage(七情所傷), extravasated blood(瘀血), deficiency of kidney (腎虛). There is 138 kind of herbs are used in our study that we find out that most frequently used herb is ginseng(人蔘). Heart meridian is the highest use in the all meridians. Sungon(性溫) is the highest use in the all kimi(氣味) Conclusions : We could confirm that herbs of amnesia treatment was related to the three vital organs(臟) named of spleen(脾), lung(肺), kidney(腎).

Herba Asari, Radix Angelicae Dahuricae, Flos Magnaliae, Fructus Xanthii, Rhizama Acori Graminei, Herba Menthae, Zanthoxyli Fructus and Spina Gleditsiae are used in nasal stuffness.

  • Historically, the approach to any reconstructive challenge, whether intentionally or intuitively, can be seen to follow distinct guidelines that could aptly be called "reconstructive metaphors." These have been intended to inform us as to the "what, "when" and "where" this attempt can best be achieved. Yet the "how" or means to accomplish this goal, usually also intuitively well understood, in a similar vein can now be expressed to be within our "reconstructive toolbox." The latter will distinctly mirror our individuality and contain not only the various hardware that we deem essential, but also the means to access whatever technology we may be comfortable with. No toolbox, even if overflowing will ever be full, as potential options and the diversity they represent surely approaches infinity. But the truly excellent reconstructive surgeon will know when their toolbox is in any way lacking, and fears not remedying that deficiency even if the talents of another colleague must be sought, so as always to ensure that the patient will obtain the best appropriate treatment!
