The "Somun Jogyongron(素問 調經論)" describes excess and deficiency syndromes. The study suggests that excess syndrome(實證) is caused by vigorous pathogenic fire(火邪)(the spirit(神)), pathogenic dryness(燥邪)(Gi(氣)), pathogenic wind(風邪)(blood(血)), pathogenic dampness(濕邪)(physique(形)) or pathogenic coldness(寒邪)(will(志)). When pathogenic fire is dominant within the body, Gi and blood becomes excessive and come out of the body, but the body cannot take them back, leading to the symptom in which the patient cannot stop laughing. When pathogenic dryness prevails, the lung(肺) cannot function properly. This means that the convergence(收斂) function of the clearing the lung and descending Gi(肅降) is deteriorated, and the patient shows symptoms of dyspnea and cough. Strong pathogenic wind increases the ascencling Gi in the liver(肝氣) and fuel angry emotion when the patient becomes upset. When pathogenic dampness is dominant, spleen(脾) function drops due to lumping effects, and the patient will experience abdominal distention(腹脹), which will disturb urination and defecation. When pathogenic coldness prevails, abdominal distention occurs due to condensating effects, and Yang Gj(陽氣) in the kidney(腎) is disturbed, leading to digestion disorders and eventually water-grain dysentery. Deficiency syndrome is caused by the lack of essential Gi(精氣) in the five viscera(五藏). Deficiency of sprit means the lack of Gi in the heart(心氣), so the patient becomes vulnerable to sadness. Deficiency of Gi means the lack of Gi in the lung(肺氣), so the patient may have breathing disorders. Deficiency of blood means the lack of Gi in the Liver(肝氣), so the patient can be easily scared. Deficiency of physique means the lack of Gi in the spleen(脾氣), making it difficult to use arms and legs. Deficiency of will means the lack of Gi in the kidney(腎氣), so Gowl syndrome(厥證) can ensue.
Objectives to study definition and treatment of dizziness in the books of Cold damage which are classified as symptoms of all time. Methods 1. Quote provisions related to dizziness in "Sanghanlon(傷寒論)" 2. Among the books of Cold damage which are classified as symptoms, select 19 literatures on dizziness with table of contents and collect data and categorize in two perspectives on theories and disease pattern. 3. Compare and draw a chart all data collected in above methods. Results & Conclusions 1. In the books on Cold damage, dizziness is expressed in 2 ways such as head dizziness(頭眩) and fainting(鬱冒). fainting is much more several symptoms than head dizziness and it is the difference that it has mental confusion. 2. The cause of head dizziness after promoting sweating, vomiting and purgation is that source qi(元氣) of upper energizer(上焦) is deficiency, and cause of head dizziness before using method of treatment is wind(風), heat(熱), phlegm(痰), blood deficiency(血虛) and etc. 3. Main cause of fainting is that deficiency is getting severe so that cold invades, and symptoms are blood deficiency, after giving birth and excess pattern of fire and heat. 4. Objectives : By analyzing symptoms of tinnitus, we tried to find out the relationship between deficiency and excess(虛實) and visceral pattern identification(臟腑辨證). By testing Tinnitus Handicap Inventory(THI), we evaluated the quality of life of people suffering from various aspects of tinnitus, as well as comparing treatment efficiency. Methods : 52 patients were recruited in this study who was ill with tinnitus. They wrote out the questionnaire about tinnitus and the THI. Results : The vast majority of high-grade tiny noise patients are deficiency type(虛症). The vast majority of low-grade roar patients are excess type(實症). Tinnitus persistence and feeling of ear occlusion were prominent in low-grade roar patients. low-grade roar patients suggest that overall quality of life is lower than high-grade tiny noise patients. In terms of treatment efficiency, spleen-stomach weakness(脾胃虛弱)-type, stomach heat(胃熱)-type and phlegm-fire(痰火)-type was higher than kidney essence depletion(腎精虧損)-type and liver qi depression(肝氣鬱結)-type regardless of the aspect of tinnitus. Conclusions : The present study suggests that tinnitus pattern may be helpful in differentiating patients with tinnitus, and the effectiveness of treatment can be predicted through differentiation.
Objectives The purpose of this study is to analyze the types of childhood simple obesity and suggest clinical symptoms, treatment principles, and traditional Chinese medicine (TCM) treatments for each type. Methods All kinds of literature published by the China National Knowledge Infrastructure (CNKI) up to August 20, 2022 were analyzed. We extracted information about types of childhood simple obesity, relevant clinical symptoms, treatment principles and TCM treatments. Results 25 studies were included. Spleen deficiency with dampness obstruction, gastrointestinal dampness-heat, internal excess of phlegm-dampness were the most reported. Spleen deficiency with dampness obstruction has symptoms of powerless, heavy limbs, pale tongue, teeth-marked tongue, sunken and slippery pulse. As a treatment, herbal medicine (HM) like modified Banggihwanggitang and acupoint like Joksamri were mainly reported. Gastrointestinal dampness-heat has symptoms of thirst, constipation, edacity, rapid hungering, heavy limbs, red tongue, slippery and rapid pulse. HM like Modified Xiehuangsan to clear heat was mainly reported. Internal excess of phlegm-dampness has symptoms of heavy limbs, lack of strength, tongue with white slimy fur, slippery pulse. Modified Ijintang to dry dampness to resolve phlegm was mainly reported. Conclusions This study analyzed types of pattern, clinical symptoms, treatment principles, and TCM treatments of childhood simple obesity. Based on this study, it is necessary to derive a standardized dialectical information that reflects the domestic situation.
Objectives : The aim of this survey study is to analysis clinical symptoms and patterns of dementia patients. The results of this study will be used to support development of the oriental herbal medicine for dementia. Methods : Clinical data were collected from 41 dementia patients and 20 aged persons who have no cognitive impairment in 3 sites including 2 hospitals and 1 sanatorium. 'The instrument of oriental medical evaluation for dementia' is used to evaluate clinical symptoms and patterns of all subjects. Results : 1. Subjects of deficiency syndrome patterns were nearly three times more than subjects of excess syndrome patterns. 2. In dementia patient group, the average rate for 4 clinical symptoms, related with treatment principle of clear heat, was 26.9%. 3. The average rate for 4 clinical symptoms, related with condition of urine and feces, was 15.6%. 4. The average rate for 5 clinical symptoms, related with anger, irritation, anxiety and restlessness, was 40.0%. Conclusions : 1. It is needed to develop new herbal medicine for dementia focuing on clear heat, anger, irritation, anxiety, restlessness, and condition of urine and feces. 2. 'The instrument of oriental medical evaluation for dementia' has strong tendency that excessively reflects general geriatric symptoms, related to deficiency syndrome patterns. so, more clinical symptoms of excess syndrome should be added in this tool.
Background & Object : The aim of this study was to investigate the usefulness of electrogastrography on differential diagnosis of deficiency or excess Condition in patients with functional dyspepsia Methods : Selected symptoms for diagnosis of deficiency and excess in functional dyspepsia have been reviewed in some literatures, notably within Oriental medicine, Donguibogam and Gogeumdoseojipseong-uibujeolrok. This is what was investigated in this study. 93 patients (male 32, female 61) were divided into three groups; Non-pain group, Pre-treatment pain group (chief complain was abdominal pain at the first medical examination), and Mid-treatment pain group(they had no pain at the time of first medical examination, but showed abdominal pain within two weeks). 10 healthy people participated as normal controls. Gastric motility were recorded and analyzed using electrogastrography during fasting and postprandial period. In assessment, effectiveness was divided into validity, sensitivity and specificity. Results : Epigastric pain reflected the tendency for excess condition in comprehensive diagnosis, which was linked with postprandial arrhythmia in electrogastrography(p=0.001). Postprandial arrhythmia detected abdominal pain with a validity of 78.6%, a sensitivity of 79.2%, and a specificity of 78%(p<0.001). Conclusions : Results 1Tom Postprandial arrhythmia in electrogastrography support that the index of excess condition in the comprehensive diagnosis of symptoms is useful for patients with functional dyspepsia.

Objectives : To satisfy the demand of good treatment of constipation Methods : we investigated the literatures of Oriental Medicine about Constipation. Results: 1. There are three categories of etimological factors of constipation, that is, endogenous, exogenous and non-exo-endogenous factor. The endogenous factor is caused by seven emotions, called depression of Ki and stagnation of Ki. The exogenous factor is six excessive atmospheric influences, for example, wind, cold, dampness, heat and dryness.
