Objectives The quality of life in cancer patients is important. The aim of this study is to report a case which showed symptoms improvement after treatment with modified Galgeunhaegi-tang. Methods We retrospectively reviewed the medical records. The patient's body weight and the circumference of lower extremity were measured and the subjective symptoms such as chest pain were evaluated by the NRS(Numeric Rating Scale). And the frequency of defecation was checked daily. Results The patient had received chemotherapy from March to November 2014 for 8th times. During the chemotherapy, the patient developed symptoms such as weight gain, edema of lower extremity, chest pain, headache, low back pain, and constipation. But additional evaluation and management with western medicine couldn't solve the problem. So he started to take Korean medicine. And he was transferred to our cancer center to keep continuing the treatment. After the treatment, the body weight, the lower extremity circumference and the subjective severity of chest pain, headache, low back pain were all decreased. During hospitalization at our center, the patient didn't suffer from constipation. Conclusions An elderly non-small cell lung cancer patient showed the improvement of symptoms that were continued during chemotherapy. As we can see in this case, Korean medicine can be a solution of the symptoms induced by chemotherapy, if there is no symptom improvement with western medicine.
Purpose: We evaluated the efficacy and safety of combined oral and enema therapy using polyethylene glycol (PEG) 3350 with electrolyte solution for disimpaction in hospitalized children. Methods: We retrospectively studied 28 children having functional constipation who received inpatient treatment between 2008 and 2016. The amount of oral PEG 3350 electrolyte solution administered was 50-70 mL/kg/d (PEG 3350, 3-4.1 g/kg/d), and an enema solution was administered 1-2 times a day as a single dose of 15-25 mL/kg (PEG 3350, 0.975-1.625 g/kg/d). A colon transit time (CTT) test based on the Metcalf protocol was performed in some patients. Results: Administration of oral and enema doses of PEG 3350 electrolyte solution showed $2.1{\pm}0.3$ times and $2.9{\pm}0.4$ times, respectively. After disimpaction, the frequency of defecation increased from $2.2{\pm}0.3$ per week to once a day ($1.1{\pm}0.1$ per day). The number of patients who complained of abdominal pain was reduced from 15 (53.6%) to 4 (14.3%). Before hospitalization, nine patients underwent a CTT test, and 5 of 9 patients (55.6%) were classified as belonging to a group showing abnormalities. And in some patients, mild adverse effects were noted. We examined electrolytes and osmolality before and after disimpaction in 16 of 28 patients, and no abnormalities were noted. Conclusion: In terms of therapeutic efficacy and safety, combined oral and enema therapy using high-dose PEG 3350 with electrolytes is considered superior to conventional oral monotherapy or combined oral and enema therapy on an outpatient basis.
Objective: This study reports the effect of Hyungbangjihwang-tang-gami as a treatment for Soyang-in patients with chronic diarrhea. Methods: We treated three male patients with chronic diarrhea who did not improve after taking herbal medicine, as determined by zang-fu (organ) pattern diagnosis. After changing our approach to a Sasang constitution, we prescribed Hyungbangjihwang-tang-gami and assessed the effect by the frequency or form of defecation. Results: The patients' complaints worsened when they were administered a herbal medicine unsuited to their Soyang constitutions, which caused an exuberance of Yang. Changing the herbal medicine to Hyungbangjihwang-tang-gami improved their symptoms. Conclusions: This study suggested that Soyang-in patients may have side effects when administered unsuitable herbs, even if they were diagnosed using zang-fu (organ) pattern diagnosis. When we treat patients, we have to consider their Sasang constitution, as well as the zang-fu (organ) pattern diagnosis.

Objective: Jowiseunggi-tang (JWSGT) is a traditional herbal medicine commonly used for purgative activity in constipation. This study evaluates JWSGT for the treatment of opioid-induced constipation (OIC), the most common and debilitating gastrointestinal effect of opioid use. Methods: A 64-year-old man with floor of mouth cancer was hospitalized for OIC, and JWSGT was administered orally twice a day for 10 days, along with acupuncture, moxibustion, and cupping. The primary outcome measures were defecation type according to the Bristol Stool Form Scale (BSFS) and the frequency of bowel movements. The Brief Fatigue Inventory (BFI) and the Functional Assessment of Cancer Therapy-General (FACT-G) were used as secondary measures. Results: After three days of JWSGT administration, spontaneous bowel movements were observed two to three times per week with improved BSFS from type 2 to 4. A reduction in BFI score (8.7 to 2.0) and an increase in FACT-G score (44.3 to 59.0) suggested an improvement in fatigue level and quality of life. Conclusion: This is the first report to assess the efficacy of JWSGT for the management of OIC in patients with cancer, and JWSGT may be an effective option to improve symptoms and quality of life in this group.

Compared to infant formula, breast milk is the best source of nutrition for infants; it not only improves the neonatal intestinal function, but also regulates the immune system and gut microbiota composition. However, probiotic-fortified infant formula may further enhance the infant gut environment by overcoming the limitations of traditional infant formula. We investigated the probiotic formula administration for one month by comparing 118 Korean infants into the following three groups: infants in each group fed with breast milk (50), probiotic formula (35), or placebo formula-fed group (33). Probiotic formula improved stool consistency and defecation frequency compared to placebo formula-fed group. The probiotic formula helped maintaining the level of secretory immunoglobulin A (sIgA), which had remarkably decreased over time in placebo formula-fed infants (compared to weeks 0 and 4). Moreover, probiotic formula decreased the acidity of stool and considerably increased the butyrate concentration. Furthermore, the fecal microbiota of each group was evaluated at weeks 0 and 4. The microbial composition was distinct between each groups, and the abundance of health-promoting bacteria increased in the probiotic formula compared to the placebo formula-fed group. In summary, supplementation of probiotic infant formula can help optimize the infant gut environment, microbial composition, and metabolic activity of the microbiota, mimicking those of breast milk.

Objectives: This study reports the case of a patient with right hemiparesis after a left thalamic infarction, in which the patient experienced improved clinical symptoms (hemiparesis, constipation) after treatment with a complex Korean medicine treatment. Methods: A 81-year-old male patient diagnosed with acute left thalamic infarction was treated with daily acupuncture and herbal medicine (Boyanghwanoh-tang-gagam) three times a day. Jowiseunggi-tang extract granules were administered to treat constipation by adjusting the dosage according to the reported symptoms. The patient's clinical course was assessed using changes in Modified Barthel Index (MBI) score, Manual Muscle Testing (MMT) grade, and defecation frequency . Results: The patient's MBI score improved from 35 to 69 and was discharged with the ability to walk under supervision. The MMT grade also improved from 3/4 to 5/5 in the upper/lower extremities. Other clinical symptoms, including constipation, also improved, leading to the discontinuation of magnesium medication. Conclusion: These findings suggest that complex Korean medicine treatments, including herbal medicine and acupuncture, may be an effective treatment for post-stroke hemiplegic patients.
This case report highlights the efficacy of traditional Korean medicine in treating chronic levator ani syndrome. A 47-year-old male suffered from chronic anorectal pain for 14 years. Over the last 5 months, the severity and frequency of the pain increased, and he was unresponsive to Western medicine and acupuncture outside Korea. The patient reported moderate anorectal pain from early morning until midday, affecting daily efficiency and concentration. The pain was triggered by defecation and alleviated by lying down. He was diagnosed with levator ani syndrome related to "cold symptoms of the liver and kidney - Yin", for which modified Nangan-jeon was prescribed. Indirect moxibustion (CV4, Ki1), Aconitum ciliare Decaisne pharmacopuncture (GV1, BL33), and acupuncture (HT7, ST36, SP6, LR3, LI4) were also administered weekly. After 8 weeks, the anorectal pain decreased by 2 points on a numeric rating scale, leading to patient satisfaction and return to his home country.
Purpose : This study was performed evaluate optimal dose for disimpaction, efficacy and safety of PEG 4000 in children with chronic functional constipation. Methods : Eighty six children with chronic functional constipation were enrolled in this prospective study at Konkuk university hospital March, 2003 through August, 2006. Success in disimpaction with PEG 4000 was defined as meeting at least two out of three criteria; resolution of chief complaint, getting easiness of defecation with respect to frequency of bowel movement and hardness of stool based on defecation diary, and decrease in fecal impaction on simple abdominal X-ray test. Adverse effects of PEG 4000 were monitored clinically and biochemically. Results : Eighty three out of 86 children completed the study, and success rate of disimpaction was 99% (82/83). The mean dose of PEG 4000 for disimpaction was $0.93{\pm}0.28g/kg/day$ (0.4-2.0 g/kg/day, Max.: 30 g/day). Frequency of bowel movement increased ($5.02{\pm}2.71/wk$ vs. $11.25{\pm}5.43/wk$) in most children (79/83). Fecal impaction on simple abdominal X-ray test improved with statistical significance in 25 children (P=0.0007). Because of adverse effect of PEG 4000, 3 children did not complete the study; urticaria, severe diarrhea, diarrhea and abdominal pain. One 6-year-old girl who completed the study complained tingling sensation in the hand and foot without laboratory abnormality (4/86, 4.7%). Laboratory test revealed hyperosmolality without clinical symptom in 1 child, and eosinophilia in 6 children. Conclusion : The average safe and effective dose of PEG 4000 for disimpaction was $0.93{\pm}0.28g/kg/day$ (0.4-2.0 g/kg/day, Max : 30 g/day) in children with chronic functional constipation.

Objectives: It is a statistical analysis study to examine the results of CaPSPI (Diagnostic System for Climacteric and Postmenopausal Syndrome Pattern Identification), developed for objective defecation of climacteric and postmenopausal syndrome. Methods: Total 341 people's questionnaire responses were statistically analyzed. 275 people involved in developing CaPSPI 2018 (E) and 146 people involved in 2019-2020 study of research1,3). Results: The frequency of diagnosis for examination was the highest at liver depression, 93.8% for 320 times, the lowest at heartheat, 62.8% for 214 times. The frequency of treatment for examination was the highest at liver depression, 54.3% for 185 times, and the lowest at dual deficiency of heart-spleen, 16.7% for 57 times. The diagnosis ratio was the lowest at dual deficiency of heart-spleen, 19.72%, and the highest at liver depression, 57.81%. As a result of comparing these diagnoses with the Kupperman's index, all showed significant differences. As a result of comparing these disease elements, all showed significant differences. The correlation between diagnosis and dialectic elements was found to have similar results with the korean medical pathology, and in 7 dialectics except for heartheat, the treatment version was more severe or progressing to perjury than for examination. Conclusions: The CaPSPI shows the characteristics of korean medicine well, and it is needed to utilize the high correlative disease elements to upgrade the system.
Changes in elimination behavior, including urination and defecation, are common clinical signs of numerous disorders in cats. Therefore, this study attempted to automatically measure the elimination behavior of cats using the litter box and develop an early warning system for the guardian in case of abnormalities. To construct an early warning system for abnormal changes through cat elimination behavior, it consisted of a litter box, an automatic sensor for data collection and data wifi transmission, a server for data analysis, and a mobile phone app for result transmission and early warning. To establish the reference interval (RI), the elimination behavior was monitored for more than 2 weeks using a motion sensor within a litter box in 37 healthy cats and 19 diseased cats. The data were expressed as daily total visits, daily total stay duration, average stay duration per elimination, weekly total visits, and weekly total stay duration. Healthy cats showed median daily total visits of 3 times/day (RI 1.0-7.0) and daily total stay duration of 192 s/day (RI 8.0-452.0). For weekly data, the median total visits were 20 times/week (RI 3.0-35.25) and the median total stay duration was 1,147 s/week (RI 80.0-2,249.5). The average stay duration per elimination was 59 s/elimination (RI 4.67-132.0). Diseased cats showed more frequent elimination behavior than healthy cats (p < 0.001). Otherwise, for each elimination, diseased cats had shorter stay durations than healthy cats (p < 0.001). This study established the RIs of elimination behavior parameters (frequency and duration) in healthy cats. The present study might help guardians and veterinarians detect changes in elimination behaviors in diseased cats at an early stage.
