한의 의료기관 비급여 진료 실태조사 (A Survey on Uncovered Services in National Health Insurance of Traditional Korean Medicine Institution)

  • Objectives : This study was conducted to survey on uncovered services in National Health Insurance(NHI) of Traditional Korean Medicine(TKM) Institution. Methods : For TKM doctors working in hospital, it was surveyed to professors working in university-affiliated hospital. A total of 40 professors were participated online survey and all of them was included. For TKM doctors working in clinic, an e-mail survey was conducted for members of the association of Korean medicine. A total of 436 TKM doctors, 279 TKM doctors were included study and 157 were excluded because of duplication clinic or not working in clinic. It was conducted general status and uncovered services in NHI status. Results : The proportion of uncovered services in NHI was 54.7% for hospitals, 39.0% for clinics and there was a significant difference between hospital and clinic. Decoction and herbal(bee venom) acupuncture were most commonly used in both institutions. For decoction treated patients, It was commonly treated Sibjeondaebo-tang, Bojungykki-tang, Gwibi-Tang and patients chief complaints was thoraco-lumbar pain, functional dyspepsia, fatigue. Conclusions : It is necessary to expansion of benefits range of the NHI on TKM services.

녹용대보탕(鹿茸大補湯)이 AAPH로 유발된 산화병태(酸化病態)모델 흰 쥐의 항산화(抗酸化) 작용(作用)에 미치는 영향 (The Anti-oxidative Effect of Oral Administration of NYD (Nocyongdaebo-tang) in Oxidized Rats induced by AAPH)

  • 1. Objectives This study aimed to investigate the anti-oxidative effect of Nocyongdaebo-tang (NYD) decoction in oxidized rats induced by AAPH. 2. Methods AAPH (2.2-azobis-2 aminodinopropane-hydrochloride) was injected intraperitoneally to induce oxidization and NYD was orally administered to the rats. To observe the anti-oxidative effect of NYD, we performed blood chemistry analysis, histological analysis, and evaluated the levels of SOD, catalase, glutathione, NO and MDA in liver. 3. Results & Conclusions 1) Serum Albumin level was increased significantly in the NYD group as compared with the control group and the saline group. 2) Serum LDL-cholesterol level was reduced significantly in the NYD group as compared with the control group. 3) GPT level in liver was reduced significantly in the NYD group as compared with the control group. 4) SOD, catalase activity and glutathione levels were increased significantly in the NYD group as compared with the control group and the saline group. 5) The levels of NO and MDA were reduced significantly in the NYD group as compared with the control group. 6) The degenerative change of liver tissue of NYD group was decreased in comparison with the control group. In conclusion, NYD decoction is considered to have an anti-oxidative effect in rats.

폴리페놀 및 DPPH 라디칼 소거활성 관점에서의 복합갈근탕 및 단미혼합갈근탕의 동등성 (Equivalence of Mixed and Individual Preparation of Galgeun-tang in terms of Polyphenol Contents and DPPH Radical Scavenging Activity)

  • Traditional medicines usually use combinational formula that are prepared by mixing different varieties of medicinal herbs and boiling them in water to yield a decoction. In recent years a modified method has been proposed and practiced wherein the individual herbs are boiled with water separately and later these extracts are mixed together for use. We attempted to evaluate their equivalence in terms of its polyphenol contents and DPPH radical scavenging activity. The polyphenol contents as well as the DPPH radical scavenging activities were very similar to each other.

  • The decocting method of herbal medicines is various with a prescription or herbal medicine's characteristics. But it has common principles by which effective elements can be easily extracted with synergistic actions of herbs and which the therapeutic effect of a medicine is amplified. When decocting a herbal medicine, the volume of water is an important factor. Fire for decocting drugs and decocting time are also important factors. The excessive water and unproper decocting time and temperature can reduce the effect of the decoction. Besides a better decoction can be obtained by squeeze. According to herbal medicine's characteristics and prescription, there are herbs that should be treated after a specific method. For example, decoct first and decoct later. It also affects the therapeutic if a right taking method is not carried out. Methods of taking drugs include both the time and the method. We can confirm these decocting and taking methods in the ancient medical literatures of herbal medicine. This article deals with these things in detail.

  • Traditional medicines usually use combinational formula that is prepared by mixing different varieties of medicinal herbs and boiling them in water to yield a decoction. In recent years a modified method has been proposed and practiced wherein the individual herbs are boiled with water separately and later these extracts are mixed together for use. This practice has lead to a constant controversy concerning the chemical and biological equivalence between the preparations arriving out of these two methodologies. In this research, we attempted to study the effects of traditional and individual preparation of Ssanghwa-tang (ST) to evaluate their equivalence in terms of its antioxidant activity and polyphenol content. The study results indicate that the polyphenol contents as well as the extraction yields of these two preparations were very similar to each other.

전탕 용기에 따른 보중익기탕과 육미지황탕 전탕액 비교 연구 (Compositional Differences of Bojungikgi-tang and Yukmijihwang-tang Decoctions Produced by the Extractors Made of Various Materials)

  • This study compared Bojungikgi-tang (BIT) and Yukmijihwang-tang (YJT) decoctions produced using the extractors made of various materials, such as glass, earthenware, and stainless steel, for 60, 90, and 120 min. The extraction yield was measured and the amounts of marker compounds in decoctions were determined by quantitative analysis using high performance liquid chromatography. The extraction yields of BIT decoctions produced by three extractors were not significantly different, whereas the extraction yield of YJT produced using glass extractor was significantly higher than that of YJT produced using earthenware extractor after 90 min. The amounts of most marker compounds were highest in BIT decoctions produced using glass extractor. However, variations of the amounts of marker compounds were observed in YJT decoctions and the volatile compound was not easily vaporized in YJT decoction produced using earthenware extractor. This study suggests that the extractor made of various materials may affect the extraction efficiency and the contents of marker compounds, especially in the decoction produced using earthenware extractor, although the results were dependent on the characteristics of herbal formula.

교통사고 이후 신장외상 의심소견에 의한 무증상성 미세혈뇨의 저령탕 가감 투여 경과 : 증례보고 (Modified Zhu Ling Tang Administration for Asymptomatic Microhematuria Due to Suspicion of Kidney Trauma Following a Motor Vehicle Accident: a Case Report)

  • Objectives: This study aimed to introduce a clinical case of asymptomatic microhematuria due to suspicion of kidney trauma following a motor vehicle accident. Methods: A 19-year-old male was hospitalized with the chief complaint of back pain after a motor vehicle accident. He showed an elevated urine red blood cell (RBC) level in the laboratory test, although no specific traumatic lesions were present on the body. He was assessed to have asymptomatic microhematuria, and Zhu Ling Tang (polyporus decoction) was given to him to be taken three times daily. Results: After the treatment, the level of urine red blood cells returned to normal range. Adverse events were not observed. Conclusions: Zhu Ling Tang caused a short-term improvement in the urinalysis levels of a patient with asymptomatic hematuria. However, further studies are needed, as this study is only a single-case report.

대장암에 대한 본초 및 처방의 국내 최신 연구 경향 분석 (Trend Review of Research about Herbal Medicine Treatment on Colorectal Cancer. - Based on Korean Medicine)

  • Objectives : To analyze the latest study trend on effect of herbal medicine treatment for Colorectal Cancer in South Korea. Methods : The theses about colorectal cancer(from 1th January 2000 to 6th August 2016) were searched from National Assembly Library, KIOM OASIS, and National Digital Science Library. Results : 54 theses were analyzed from 87 studies. They were classified by the type of research; experimental theses and case report. The number of experimental theses is 48 and case report is 6. And, they were classified by the topic of research; Single herb, Herbal decoction, oriental foods and Pharmacopuncture, the number of theses about single herb is 29, herbal decoction is 14, oriental foods is 8 and Pharmacopuncture is 3. Rhus Verniciflua(乾漆), Beakduong-tang(白頭翁湯), Wild Ginseng Pharmacopuncture is the most commonly used herb in our research. Conclusion : There were many attempts to develop the way to cure colorectal cancer using korean herbs, and some meaningful results were reported. However, more clinical researches based on human body and more scientifically designed clinical studies are required to prove the effect on colorectal cancer.

  • Objectives: This case is significant as a rare observational record at the Korean medical practice field. This case reports progression of co-administration of Korean herbal medicines and conventional medicines for cough in patients with myocardial infarction relapse suspected of developing acute respiratory infection. Methods: First, the chest radiography, CBC count and urinalysis were performed to estimate patient's systematic condition. After the estimated diagnosis, the patient was treated with modified Dingchuan decoction, antibiotics, and complex syrup for cough. We used the Cough-Specific Quality-of-Life Questionnaire (CQLQ) to assess patient's respiratory symptoms. Results: Co-administration of Korean herbal medicines and conventional medicines resulted improving of cough and sputum symptoms. Laboratory analysis items and total score of CQLQ also showed significantly improved results. Adverse effects were not observed. Conclusions: In this case, we concluded that co-administration of Korean herbal medicine and conventional medicine may be an effective therapy for the treatment of cough in patients with myocardial relapse of developing acute respiratory infection.

  • Objective : The purpose of this study is to report the clinical effectiveness of herbal decoction(Samul-tang hap Soyo-san.) on whole body nummular eczema. Method : A patient suffering from nummular eczema received herbal decoction(Samul-tang hap Soyo-san), acupuncture, and electronic-moxibustion treatment. The effectiveness of treatment was evaluated by EASI, DLQI, photography and subjective evaluation of the patient's improvement of symptoms. Results : After treatment, eczema lesions were remarkably improved. Compared to its peak, EASI improved by 69% and DLQI by 53%. According to photo and symptom changes, purulent exudates were stopped and pruritus was decreased. Conclusion : This study suggests that Samul-tang hap Soyo-san could show effectiveness on nummular eczema.
