Impact of Adjuvant Chemotherapy in Elderly Breast Patients in Taiwan, A Hospital-Based Study

  • Purpose: Decisions as to whether to provide adjuvant treatment in older breast cancer patients remains challenging. Side effects of chemotherapy have to be weighed against life expectancy, comorbidities, functional status, and frailty. To aid decision-making, we retrospectively analyzed 110 women with breast cancer treated with a curative intention from 2006 to 2012. Survival data with clinical and pathological parameters were evaluated to address the role of adjuvant chemotherapy in this study population. Method: A total of 110 elderly (>70 years) patients that received mastectomy at two hospitals in Taiwan were observed retrospectively for a medium of 51 months. After mastectomy, patients received conservative treatment or adjuvant chemotherapy, or hormone therapy following clinical guidelines or physician's preference. Data were collected from the cancer registry system. Results: Median age at diagnosis was 75.7 years. Thirty-five percent of patients received adjuvant chemotherapy, these having a significantly younger age ($mean=74.0{\pm}5.3$ vs $77.5{\pm}5.3$, p<0.001) and higher tumor staging (p=0.003) compared with their non-chemotherapy counterparts.Five-year overall survival was non-significantly higher in patients who received adjuvant chemotherapy (with chemotherapy 64.2% vs without chemotherapy 62.6%, p=0.635), while five-year recurrence free survival was non-significantly lower (with chemotherapy 64.1% vs without chemotherapy 90.5%, p=0.80). Conclusions: In this analysis, adjuvant chemotherapy tended to be given to patients with a younger age and higher tumor staging at our institute. It was not associated with any statistically significant improvement in survival and recurrence rate. Until age specific recommendations are available, physicians must use their clinical judgment and assess the tumor biology with the patient's comorbidities to make the best choice. Clinical trials focusing on this critical issue are warranted.

  • Information systems nowadays are heterogeneous and distributed which integrate the enterprise information by processes. They are also very complex, because they are linked together by processes. It aims to integrate the systems so that these systems work as one system. A process is a framework which contains all of the business activities in an enterprise, and has a lot of information which is needed for measuring performance. A process consists of activities, and an activity contains events which can be considered information sources. In most cases, it is very valuable to determine if a process is meaningful, but it is difficult because of the complexity in measuring performance, and also because finding relationships between business factors and events is not a simple problem. So it would reduce operation cost and allow efficient process execution if I could evaluate the process before it ends. In this paper we propose an event based process measurement model. First, we propose the concept of process performance measurement, and a model for selecting process and activity indexes from the events which are collected from information systems. Second, we propose at methodologies and data schema that can store and manage the selected process indexes, the mapping methods between indexes and events. Finally, we propose a process Performance measurement model using the collected events which gives users a valuable managerial information.

  • To develop a crisis management for aviation safety, this study has defined crisis management includes risk management which is eliminates or lowers risks prior to accidents and emergency response after the accidents. This study takes a look at different kinds of crisis managements, risk managements and statistical methods of other nations and fields in order to develop a risk management model. Through surveys which have 102 risk factors that include air traffic control, maintenance and airport sectors, the weight of each risk factor was calculated and the probability was divided to develop a model for risk management. The risk management model of this study is conducted using four steps (risk management plan, risk factor identification, weight and probability analysis, decision making) and 4 standards of weight along with 5 standards of probability This study takes a look at Predictions through a quantitative method using a risk index for the risk management model An effective risk management model should have a wide and continuous collection of data and adopt various methods using this model. The crisis management could not be very effective only using a pre-active risk management. So it should also be conducted by using a pro-active response system to protect additional damage and to prevent accidents of the same nature. From the results, the most important points were the establishment of command and control accountabilities, and cooperation of related organizations.

  • In 2004, in order to create core competences and major growth drivers for endogenous regional developments, the Korean government adopted the policies for establishing regional innovation systems, The main organization for the policies is the regional innovation council which is composed of diverse groups of scholars, professionals, and government officials. The regional innovation council of Kang-won Province consists of a couple of sub-divisions such as Operation Committee, Administration Office, Planning and Moderation Department, Strategic Industry Department, Culture and Tourism Industry Department, Regional Industry Department, and Regional Human Resource Development Department. The primary roles and jobs are planning, reviewing, evaluating and finalizing regional innovation policies. However, because of conflicts with local government, insufficient budget problems, organization structure problems, and decision making process problems, the regional innovation councils could not perform their original roles and functions. They could not adopt radical or non-traditional strategies. Although it's failure, a lot of people recognize the importance of regional innovation councils' roles and functions, I believe there would be another regional innovation councils coming continuously which would be take more important roles and have more power over budgeting problems. The purpose of this article is to identify the primary sources of strategies and organization structure problems of the previous regional innovation councils and to provide a couple of answers. In this paper, the author primarily analyze the problems of the Kang-won innovation council. A couple of answers to the desirable organization structures and the desirable roles of regional innovation councils will be presented for discussion.

Study on a Methodology for Developing Shanghanlun Ontology (상한론(傷寒論)온톨로지 구축 방법론 연구)

  • Knowledge which is represented by formal logic are widely used in many domains such like artificial intelligence, information retrieval, e-commerce and so on. And for medical field, medical documentary records retrieval, information systems in hospitals, medical data sharing, remote treatment and expert systems need knowledge representation technology. To retrieve information intellectually and provide advanced information services, systematically controlled mechanism is needed to represent and share knowledge. Importantly, medical expert's knowledge should be represented in a form that is understandable to computers and also to humans to be applied to the medical information system supporting decision making. And it should have a suitable and efficient structure for its own purposes including reasoning, extendability of knowledge, management of data, accuracy of expressions, diversity, and so on. we call it ontology which can be processed with machines. We can use the ontology to represent traditional medicine knowledge in structured and systematic way with visualization, then also it can also be used education materials. Hence, the authors developed an Shanghanlun ontology by way of showing an example, so that we suggested a methodology for ontology development and also a model to structure the traditional medical knowledge. And this result can be used for student to learn Shanghanlun by graphical representation of it's knowledge. We analyzed the text of Shanghanlun to construct relational database including it's original text, symptoms and herb formulars. And then we classified the terms following some criterion, confirmed the structure of the ontology to describe semantic relations between the terms, especially we developed the ontology considering visual representation. The ontology developed in this study provides database showing fomulas, herbs, symptoms, the name of diseases and the text written in Shanghanlun. It's easy to retrieve contents by their semantic relations so that it is convenient to search knowledge of Shanghanlun and to learn it. It can display the related concepts by searching terms and provides expanded information with a simple click. It has some limitations such as standardization problems, short coverage of pattern(證), and error in chinese characters input. But we believe this research can be used for basic foundation to make traditional medicine more structural and systematic, to develop application softwares, and also to applied it in Shanghanlun educations.

Cor Triatriatum with Infracardiac Total Anomalous Pulmonary Venous Drainage

  • We report one case of an 18-day-old female patient, weighing 3.4 kg, with severe cyanosis. The diagnosis was made with only transthoracic echocardiography, which revealed cor triatriatum with an atretic small opening of fibromuscular membrane, obstructive infracardiac total anomalous pulmonary venous drainage(TAPVD), severely restrictive interatrial communication, and scanty mitral inflow and aortic forward flow. The preoperative decision-making for biventricular repair was not easy due to collapsed left heart system caused by remarkably reduced blood flow An emergent operation was performed due to severe cyanosis. All left heart structures were somewhat hypoplastic but thought to be adequate for systemic circulation. Biventricular repair was done without specific intraoperative problems. The postoperative course was uneventful. The patient has been doing well with no evidence of pulmonary vein stenosis or mitral regurgitation for 4 months after operation.

  • The Park Act (1967~1980) was the first law to define urban parks in Korea. The urban parks of that time were similar to a reservation area used for other purposes after giving regulation. Because 'the urban park as a reservation area' in the past is a repeated park issue in the present, it is necessary to consider the issues of the original law system that created the cause. From this perspective, this study analyzed the legislation to reserve an urban park by collecting bills and information about the factual relationship between 1960 70s park issues and the Park Act. Analysis showed that the reason for the adoption of different kinds of urban parks in the law of a nature park is that a negative list separated from the Urban Planning Act is required to curb private usage. Inherent in the Park Act, however, was the problem of allowing the encroachment of urban parks by governmental power. (1) The Park Act sets out a wide range of cases to abolish urban park. (2) Unclear setting of governmental power could abuse the urban park. (3) Insufficient standards were able to erode the urban park with large for-profit facilities. (4) The inactivity of the Urban Public Park Committee had reduced democratic decision-making and professional judgement on park issues. Therefore, the Park Act was characterized as infringing on the environment and right to urban parks and took a passive attitude in creating parks and in citizen usage thereof. The Park Act had limitations as a progenitor for establishing the characteristics and concepts of urban parks.

An Empirical Study on the Improvment of VTS in Korea (우리나라 선박교통제제도의 개선방안에 관한 실증연구 - 포항항을 중심으로 -)

  • The purpose of this study is to find out the impacts of VTS on the marine safety and users' opinion on the VTS which have been being operated in the port of Pohang for the last 3 years and is to suggest a guideline to the successful operation of VTS in the future. This study is based upon the questionnaire survey and the respondents include 236 masters/mates of merchant vessels who have visited the port of Pohang and 6 pilots who have been working in the port. From the questionnaire results, this study notes the following conclusions related to the VTS operations. (1) A few of the respondents (mainly foreigners) do not understand the entering procedure of the port and the fundamental concept of VTS. Accordingly, the more active VTS services have to be provided for the mariners. (2) It was found that themost dangerous factors in tehvicinity of the port were the floating materials, fishing nets, andillegal fishing activities inthe fairway. Therefore, the proper surveillance, stricter enforcement of Acts and the instructive education for the fishermen are required to avoid the risks. (3) A majority of the respondents agreed the VTS has contributed to the safety of vessel traffic, and they pointed out ' the assiatances in reduced visibililty conditions' is the most important task of VTS. The amount of 75.6% of the respondents answered that they have experienced the assistance from VTS more than 1 time since the system was established in the port of Pohang . Also 44.2% of the respondents considered they were able to avoid marine casualties such as collision, ramming or agrounding with the VTS assistances. (4) 49.2% of the respondents preferred the passive information services , while, 38.8% of them preferred the positive control advices in the case of encountering any potential risks. VTS iperators have to consider seriously when they provide the positive control advices of ship's course and speed. (5) A majority of the respondents confirmed that the prot and its approaches is suitable for the VTS coverge . To extend the service areas of the VTS and to improve radar detecting ability, the use of radar transponders are seen as the ideal method. (6) A minority of the respondents pointed out 'the improper orders or recommendatinos caused by the poor decision making' firstly, ' the language problem(sea-speaking in English)' secondly, as the deficiency of personal qualification. It seems, therefore, theat the personal efforts of the operators and systematic training programmes for them are necessary to solve the problems.

  • Reduced crew performance is frequently cited as a major causal factor in maritime accident causation. Although considerable research has been conducted on the hours of work undertaken by seafarers through interviews and the analysis of records, experimental studies to observe the effects of factors such as high workload, shift patterns, stress, sleep deprivation and disturbance on the cognitive performance of mariners have been limited. Other safety-critical transport industries, such as aviation and rail, have developed fatigue management tools to help manage the work patterns of their operators. Such a tool for mariners would assist shipboard crew, marine pilots and shore management in planning and improving work schedules. The overall aim of this paper is to determine a fatigue factor, which can be applied to human performance data, as part of a software program that calculates total cognitive performance. This program enables us to establish the levels of cognitive performance of a group of marine pilots to test a decision-making task based on radar information. This paper addresses one of the factors that may contribute to the determination of various fatigue factors: the effect of different work patterns on the cognitive performance of a marine pilot.

Development of Work Stress Measurement Tool for Academic Librarians (대학도서관 사서의 직무스트레스 측정 도구 개발)

  • The purpose of this study is to develop a job-stress scale for librarians who work in university libraries. The study was first conducted by analyzing existing representative job-stress scales that are used domestically and internationally. To understand the characteristic of particular job stress that academic librarians have, the in-depth interview among qualitative research methods was selected, and 15 librarians who work at a 4-year system university libraries participated in this study. Based on the results of the questionnaire survey, the reliability and validity were verified. To analyze the validity, exploratory factor analysis was carried out. To extract factors, principal component analysis was used. To extract factors, principal component analysis was used. For the rotation method, a varimax rotation was applied. A tertiary measurement tool with a total of 46 questions for 11 factors was developed after removing measurement questions that were rejected as a result of the analysis. As a result of factor analysis on the tertiary measurement tool, 11 factors were extracted. Those 11 factors include 'peer relation conflict factor(factor 1)', 'superior-subordinate relation conflict factor (factor 2)', 'work compensation evaluation factor(factor 3)', 'emotional labor factor(factor 4)', 'physical environmental factor(factor 5)', 'employment stability(factor 6)', 'job demand factor(psychological) (factor 7)', 'decision-making and responsibility factor(factor 8)', 'work complexity factor(factor 9)', 'work boundary conflict factor(factor 10)', and 'job demand factor(physical)(factor 11)'.
