뇌졸중의 효용가중치에 대한 메타분석 (A Meta-Analysis of Utility Weights for Stroke)

  • The aims of this study are to conduct meta-analysis for obtaining pooled estimates of the utility weight for stroke and to explore the study design characteristics that determine the utility weight for stroke. Medline (Pubmed), CEA Registry (Tufts Medical Center), and KERIS (Korea Education & Research Information Service) were searched to find out the literature that reported quality of life with stroke. Patient characteristics, utility weights and standard errors were extracted and stratified by severity, study method, respondent and country. All estimates were measured on a 0 to 1 scale with 0 representing the death and 1 representing the perfect health. The pooled estimates of the utility weight were 0.627 for total stroke, 0.756 for mild stroke, 0.631 for moderate stroke, and 0.389 for severe stroke (p=0.0001). Non-patients showed significantly lower utility weights than patients for severe (p=0.0122) stroke.

Esophageal Cancer in Korea: Epidemiology and Treatment Patterns

  • According to statistics from 2017, esophageal cancer is the fifteenth most common cancer and the eleventh most common cause of cancer-related death in Korea. The most common pathology is esophageal squamous cell carcinoma. Moreover, the incidence of esophageal cancer has been gradually decreasing in Korea, and the percentage of early-stage cases has gradually increased to the point that it is higher than that of other countries. The 5-year relative survival rate has improved over time. Approximately 800 esophagectomy procedures are performed annually. Using a cut-off number of 21 cases per 2 years to define high-volume centers, it was found that 70% of esophagectomies were performed by a few high-volume centers. Unfortunately, there is no nationwide registry or database on esophageal cancer and esophagectomy in Korea. Efforts to establish a nationwide database on esophageal cancer and esophagectomy should be made.

Trends of Breast Cancer and its Management in the Last Twenty Years in Aden and Adjacent Governorates, Yemen

  • Background: Breast cancer is the most common cancer of women and the principal cause of death in middle aged women. The objective of this study was to describe the trend of breast cancer and its management in Aden and adjacent south-eastern governorates of Yemen during the last 20 years. Patients and Methods: This is a retrospective analysis of previous studies on patients with breast cancer in Aden and adjacent south-eastern governorates, Yemen (January 1989 through December 2007). The studied variables were: sex, age, time and type of presentation, disease stage, pathological types and the performed surgical treatment. The sources of information were the treatment registry of Aden health office, archives of Al-Gamhouria teaching hospital; major referral and other public and private hospitals in Aden and Aden Cancer Registry. Results: The total number of patients was 476, 99% being females. The age range was 19-88 years. The most affected age was 30-50 years (60.5%), 95% presenting after one month of having breast symptoms. Forty-five percent presented with signs of advanced local disease, while 59.2% had palpable axillary lymph nodes on presentation. Early breast cancer (stages I-II) occurred in 47%, and late breast cancer (stages III-IV) in 51.5%. Invasive ductal carcinoma was the commonest pathology (89.3%). The main surgical treatment was mastectomy (modified radical mastectomy (50%). Conclusion: Breast cancer is predominantly a disease of young with late presentation and advanced disease. Improving health awareness and earlier diagnosis of the disease by health education, encouraging breast self-examination, and providing the mammography equipment and mammary clinics in hospitals are recommended. Establishment of oncology and radiotherapy centers in Aden is a necessity.

Capture-recapture 방법을 이용한 광주광역시 지역암등록 자료의 완전성 추정 (Completeness Estimation of the Population-based Cancer Registration with Capture-Recapture Methods)

  • Objectives : This study aimed to estimate the completeness of cancer registration with Capture-recapture method. Methods : The study was conducted in the population based cancer registry of Kwangju, Korea, for which there are three main sources of notification: reports by Korean Central Cancer Registry, reports by pathology data, and the others reports by radiology data, death certificates, etc. The defined cases in three sources were matched by 13 digits Resident Register Number. To derive an estimates, log-linear models were applicated. Results : Overall completeness was estimated to be around 93%. There was some variation with age(consistently high levels below age group 60-74 years, a minimum of 88.6% above 75 years). Among the most common cancer sites, estimates of completeness were highest for thyroid cancer(97.1%), while lower estimates of completeness were derived for stomach cancer(92.3%), liver cancer(92.6%). Conclusions : Careful application of Capture-recapture method may provide an alternative to traditional approaches for estimating the completeness of cancer registration in Kwangju city.

한국 8개 지역암등록본부 자료를 활용한 2000-2002년 한국인 국가 암통계 추정 (An Estimation of the National Cancer Incidence in Korea for 2000-2002 Using the Databases of 8 Population-based Regional Cancer Registries)

  • Objectives: Valid data on the national cancer incidence (NCI) is the data should be needed to plan, monitor and evaluate the national cancer control programs. The purpose of this study was to estimate the NCI for 2000-2002 from 8 population-based cancer registries database in Korea (KRCR DB). Methods: We defined the expected number of cancer cases in each registry as the number of observed cases and then adding to the weighted observed cases, according to sex, age groups, and the proportion of the population covered by each registry for the population of the eight regions and the population of all areas with excluding the 8 regions. From the expected number of total cancer incidents, he estimated NCI was calculated by dividing the expected number of cancer cases by he umber of the total population. The standard error (SE) of the estimated incidence was also taken from the expected number of total cancer incidents. Results: The overall estimated crude rates in 2000-2002 ere 267.1 and 219.0 per 100,000 for men and women, respectively. The overall age-standardized rates (ASR) were 290.1 and 180.7 per 100,000, respectively. Compared with the ASRs obtained from Korea National Cancer Incidence database (KNCI DB), the estimated ASRs from the KRCR DB did not show statistically significant differences except for some cancers in women. For the aspect of the SE, index of DCO(death certificate only) and of MV(microscopically verified), the estimated ASRs from the KRCR DB are more accurate and they have higher quality rather than the calculated ASRs from the KNCI DB. Conclusions: We found that this developed method using the KRCR DB is valid and it could be another strategy for estimating the NCI in Korea.

Cohort Study Protocol: A Cohort of Korean Atomic Bomb Survivors and Their Offspring

  • In 1945, atomic bombs were dropped on Hiroshima and Nagasaki. Approximately 70 000 Koreans are estimated to have been exposed to radiation from atomic bombs at that time. After Korea's Liberation Day, approximately 23 000 of these people returned to Korea. To investigate the long-term health and hereditary effects of atomic bomb exposure on the offspring, cohort studies have been conducted on atomic bomb survivors in Japan. This study is an ongoing cohort study to determine the health status of Korean atomic bomb survivors and investigate whether any health effects were inherited by their offspring. Atomic bomb survivors are defined by the Special Act On the Support for Korean Atomic Bomb Victims, and their offspring are identified by participating atomic bomb survivors. As of 2024, we plan to recruit 1500 atomic bomb survivors and their offspring, including 200 trios with more than 300 people. Questionnaires regarding socio-demographic factors, health behaviors, past medical history, laboratory tests, and pedigree information comprise the data collected to minimize survival bias. For the 200 trios, whole-genome analysis is planned to identify de novo mutations in atomic bomb survivors and to compare the prevalence of de novo mutations with trios in the general population. Active follow-up based on telephone surveys and passive follow-up with linkage to the Korean Red Cross, National Health Insurance Service, death registry, and Korea Central Cancer Registry data are ongoing. By combining pedigree information with the findings of trio-based whole-genome analysis, the results will elucidate the hereditary health effects of atomic bomb exposure.

일개 군 사망신고자료에 기재된 사인의 정확성과 관련요인 (Accuracy of the Registered Cause of Death in a County and its Related Factors)

  • 전라남도의 일개 군에서 1998년 1월 1일부터 1998년 12월 31일까지 사망으로 신고된 건수는 총 504건이었으며, 이중 설문조사와 의무기록조사로 확인사인의 추정이 가능했던 388건을 연구대상으로 하였다. 확인사인을 신고사인과 비교하고 사망자와 사망관련 변수, 신고자의 특성 그리고 사망신고담당 공무원의 특성에 따른 양자간의 일치율을 통해 신고사인의 정확성과 관련된 인자를 파악하였다. 신고사인과 확인사인의 전체적 일치율은 19대 분류상 62.6%을 보였다. 19대 분류에 의한 사인별 일치율을 보면 손상, 중독 및 사망의 외인이 가장 높았고, 그 다음으로는 내분비, 영양 및 대사질환, 신생물, 순환기계질환 순이었다. 반면에 낮은 일치율을 보인 질환은 피부 및 피하조직의 질환, 근골격계 질환, 정신 및 행동장애 이었다. 사망자 및 사망관련변수에 따른 확인사인과 신고사인의 일치율은 사망자가 남자일 때, 사망 연령이 50대와 60대일 때, 그리고 사망원인 진단자가 의사일 때 높았다. 신고자의 특성별로는 신고자의 연령이 증가함에 따라 일치율이 감소하였으며, 신고자의 교육 정도와 직업과도 유의한 관련성이 있었다. 또한 사망신고 담당 공무원의 특성에 따라서는 공무원의 직급이 6급인 경우 7급 이상인 경우에 비해 일치율이 높았다. 확인사인과 신고사인의 일치여부를 종속변수로 하여 단변량분석을 실시한 후 통계적으로 의미 있는 변수에 대한 로지스틱 회귀분석의 결과를 보면, 사망원인 진단자가 의사인 경우 비차비 2.67(95% 신뢰구간: 1.21-5.89)로 높았으며, 사망신고담당 공무원의 직급이 7급 이상인 경우 비차비 0.30(95%신뢰구간: 0.12-0.78)로 낮았다. 사망신고자료에 기재된 신고사인의 정확성과 관련된 요인에 대한 결과를 종합하여 볼 때 무엇보다 중요한 일은 사망을 의사로 하여금 진단하게 하는 일이다. 또한 사망신고담당 공무원의 업무를 명확히 해야 하며 이들을 대상으로 하는 정기적이며 강화된 교육도 강조되어야 할 것이다.

The Economic Burden of Cancer in Korea in 2009

  • Background: Cancer imposes a significant economic burden on individuals, families and society. The purpose of this study was to estimate the economic burden of cancer using the healthcare claims and cancer registry data in Korea in 2009. Materials and Methods: The economic burden of cancer was estimated using the prevalence data where patients were identified in the Korean Central Cancer Registry. We estimated the medical, non-medical, morbidity and mortality cost due to lost productivity. Medical costs were calculated using the healthcare claims data obtained from the Korean National Health Insurance (KNHI) Corporation. Non-medical costs included the cost of transportation to visit health providers, costs associated with caregiving for cancer patients, and costs for complementary and alternative medicine (CAM). Data acquired from the Korean National Statistics Office and Ministry of Labor were used to calculate the life expectancy at the time of death, age- and gender-specific wages on average, adjusted for unemployment and labor force participation rate. Sensitivity analysis was performed to derive the current value of foregone future earnings due to premature death, discounted at 3% and 5%. Results: In 2009, estimated total economic cost of cancer amounted to $17.3 billion at a 3% discount rate. Medical care accounted for 28.3% of total costs, followed by non-medical (17.2%), morbidity (24.2%) and mortality (30.3%) costs. Conclusions: Given that the direct medical cost sharply increased over the last decade, we must strive to construct a sustainable health care system that provides better care while lowering the cost. In addition, a comprehensive cancer survivorship policy aimed at lower caregiving cost and higher rate of return to work has become more important than previously considered.

Colorectal Cancer Incidence and Mortality in China, 2010

  • Background: The National Central Cancer Registry of China (NCCR) affiliated to the Bureau of Disease Control, National Health and Family Planning Commission of China is responsible for cancer surveillance in the entire country. Cancer registration data from each local registry located in each province are collected by NCCR annually to be analyzed and published to provide useful information for policy makers and cancer researchers. Materials and Methods: Until 1st June, 2013, 219 population-based cancer registries submitted data of 2010 to the National Central Cancer Registry of China covering about 207,229,403 population, and 145 cancer registries were selected after quality evaluation for this study. Colorectal cancer cases were selected from the database according to ICD-10 coded as "C18-C20". We calculated the crude incidence and mortality rates by sex, age groups and location (urban/rural). The China population in 2000 and Segi's population were used as standardized populations for the calculation of age-standardized rates. The 6th National Population Census data of China was used to combined with the cancer registries' data to estimate the colorectal cancer burden in China in 2010. Results: Colorectal cancer was the sixth most common cancer in China. It was estimated that there were 274,841 new cases diagnosed in 2010 (157,355 in males and 117,486 in females), with the crude incidence rate of 20.1/100,000, highest in males in urban areas. Age-standardized rates by China standard population of 2000 (ASRcn) and World standard population (Segi's population, ASRwld) for incidence were 16.1/100,000 and 15.9/100,000 respectively. There were 132,110 cases estimated to have died from colorectal cancer in China in 2010 (76,646 men and 55,464 women) with the crude mortality rate of 10.1/100,000. The ASRcn and ASRwld for mortality were 7.55/100,000 and 7.44/100,000 respectively, higher in males and urban areas than in females and rural areas. The incidence and mortality rates increased with age, reaching peaksin the 80-84 year old, and oldest age groups, respectively. Conclusions: Colorectal cancer is one of the most common incident cancers and cause of cancer death in China. Primary and secondary prevention, with attention to a health lifestyle, physical activity and screening should be enhanced in the general population.

Breast Cancer Characteristics and Survival Differences between Maori, Pacific and other New Zealand Women Included in the Quality Audit Program of Breast Surgeons of Australia and New Zealand

  • Background: The Quality Audit (BQA) program of the Breast Surgeons of Australia and New Zealand (NZ) collects data on early female breast cancer and its treatment. BQA data covered approximately half all early breast cancers diagnosed in NZ during roll-out of the BQA program in 1998-2010. Coverage increased progressively to about 80% by 2008. This is the biggest NZ breast cancer database outside the NZ Cancer Registry and it includes cancer and clinical management data not collected by the Registry. We used these BQA data to compare socio-demographic and cancer characteristics and survivals by ethnicity. Materials and Methods: BQA data for 1998-2010 diagnoses were linked to NZ death records using the National Health Index (NHI) for linking. Live cases were followed up to December $31^{st}$ 2010. Socio-demographic and invasive cancer characteristics and disease-specific survivals were compared by ethnicity. Results: Five-year survivals were 87% for Maori, 84% for Pacific, 91% for other NZ cases and 90% overall. This compared with the 86% survival reported for all female breast cases covered by the NZ Cancer Registry which also included more advanced stages. Patterns of survival by clinical risk factors accorded with patterns expected from the scientific literature. Compared with Other cases, Maori and Pacific women were younger, came from more deprived areas, and had larger cancers with more ductal and fewer lobular histology types. Their cancers were also less likely to have a triple negative phenotype. More of the Pacific women had vascular invasion. Maori women were more likely to reside in areas more remote from regional cancer centres, whereas Pacific women generally lived closer to these centres than Other NZ cases. Conclusions: NZ BQA data indicate previously unreported differences in breast cancer biology by ethnicity. Maori and Pacific women had reduced breast cancer survival compared with Other NZ women, after adjusting for socio-demographic and cancer characteristics. The potential contributions to survival differences of variations in service access, timeliness and quality of care, need to be examined, along with effects of comorbidity and biological factors.
