승모판 치환술의 임상적 고찰 (Clinical Evaluation of Mitral Valve Replacement)

  • From August, 1986 to December, 1989, mitral valve replacement was performed in 93 patients. Of the valve implanted, 42 were Duromedics, 35 St. Jude Medical, 15 Carpenter-Edwards and 1 Ionescu-Shiley. The hospital mortality rate was 3.2%[3 patients] and the late mortality rate was 4.3% [4 patients]. The causes of hospital death were LV rupture in 1, renal failure in 1 and hypoxic brain damage in l. The causes of late death were congestive heart failure in 1 and sudden death in 3. Follow-up was done on 78 surviving patients; mean follow-up period was 29.22$\pm$9.09 months. The actual survival rate was 91.8% at 4 years. We concluded, therefore, that good clinical results could be achieved with mitral valve replacement in short-term follow-up, and long-term follow-up is also necessary.

  • From August, 1986 to December, 1993, mitral valve replacement was performed in 178 patients. Of the valve implanted, 114 were St.Jude Medical, 47 Duromedics, 16 Carpenter-Edward and 1 Ionesc-Shiley. The hospital mortality rate was 2.8%[5 patients and the late mortality rate was 7.5%[13 patients . The causes of hospital death were LV rupture in 1, renal failure in 1, cardiac tamponade in 1, valve malfunction in 1 and hypoxic brain damage in 1. The causes of late death were sudden death in 6, congestive heart failure in 4, brain ischemic injury in 3. Follow-up was done on 155 surviving patients : mean follow-up period was 50.94$\pm$8.04 months. The actual survival rate was 88.2% at 8 years. We concluded, therefore, that good clinical results could be achieved with mitral valve replacement in mid-term follow-up, and long-term follow-up is also necessary.

Survival Analysis of Patients with Gastric Cancer Undergoing Surgery at the Iran Cancer Institute: A Method Based on Multi-State Models

  • Background: Gastric cancer is one of the most common causes of cancer deaths all over the world and the most important reason for its high rate of death is its belated diagnosis at advanced stages of the disease. Events occur in patients which are regarded not only as themselves factors affecting patients' survival but also which can be affected by other factors. This study was designed and implemented aiming to identify these events and to investigate factors affecting their occurrence. Materials and Methods: Data from 330 patients with gastric cancer undergoing surgery at the Iran Cancer Institute from 1995-1999 were analyzed. The survival time of these patients was determined after surgery and the effects of various factors including demographic, diagnostic and clinical as well as medical, and post-surgical varuiables on the occurrence of death hazard without relapse, hazard of relapse, and death hazard with a relapse were assessed. Results: The median survival time for these patients was 16.3 months and the 5-year survival rate was 21.6%. Based on the results of multi-state model, age and distant metastases affected relapse whereas disease stage, type and extent of surgery, lymph nodes metastases, and number of renewed treatments affected death hazard without relapse. Moreover, age, type and extent of surgery, number of renewed treatments, and liver metastases were identified as factors affecting death hazard in patients with relapse. Conclusions: Most cancer studies pay heed to factors which have effect on death occurrence, but some events occur which should be taken into consideration to better describe the natural process of the disease and provide researchers with more accurate data.

Comparative Analysis on the Cremation Rate of the Capital Area and the Increase Rate of Cremated Bodies Using the Online Cremation Reservation Service in Funeral Information System

  • In the study, we show the annual average increasing rate of death bodies in the capital area was 2.1% while the annual average increasing rate of cremated bodies was 4.5%. As the annual average increasing rate of cremated bodies was about 2.1 times higher than the annual average increasing rate of death bodies, the increase rate of cremated bodies contrast to the increase rate of death bodies was relatively high. Also, the cremation rate of the capital area was increased by 9.5% from 80.3% in 2011 to 89.8% in 2017 while the increase rate of cremated bodies was 27.2%(N=21,694 people) from 79,866 people in 2011 to 101,560 people in 2017. Thus, the increase rate of cremated bodies contrast to the increase of cremation rate of the capital area was relatively very high. On the basis of 2017, the utilization rate of public cremation facilities in by capital area was 94.5% in Incheon Metropolitan City, 91.1% in Seoul Metropolitan City, and 20.9% in Gyeonggi-do, which showed huge differences in the utilization rate of public cremation facilities in the jurisdiction area of by capital area. Focusing on the actual state shown in the results of this study above, it would be necessary for Seoul Metropolitan City to carry forward a supply & demand policy of cremation facilities through the extension of cremation furnaces, for Gyeonggi-do to carry forward a policy to build the public cremation facilities focusing on the basic local government in the jurisdiction area, and for Incheon Metropolitan City to enforce the measures for the joint use of cremation facilities with the basic local government in the adjacent Gyeonggi-do jurisdiction area in order to improve the efficiency of the operation of cremation facilities.

부산지역의 암 사망에 관한 역학적 연구 (An Epidemiologic Study on Death Caused by Cancer in Pusan)

  • This study surveyed and measured the level and structure of cancer deaths and their trends over time for offering the fundamental data of e cancer prevention and control in Pusan city in the future. Authors conducted the study of descriptive epidemiology using materials derived from the computerized data of total 3,722 certified cancer deaths in Pusan city from January 1 to December 31, 1993 registered on the National Statistical Once, the Republic of Korea. The obtained results were as follows: 1. According to the total registered cases of deaths(16,331 cases) in Pusan city during 1993, cancer(3,722 cases) and cerebrovascular disease(2,118 cases) were the first and second cause of deaths as 23.1% and 16.9%, respectively. These pattern showed the change between cancer (14.7%) and cerebrovascular disease(18.5%) in order of frequency in comparison to 1982. Also, the total number of cancer deaths was increased in comparison to 1982. The rate of death certification by physicians was 87.1% of all registered deaths, which was increased to 6.8% in comparison to 1982(80.3%). 2. Crude death rate and cancer specific death rate was 4.06 per 1,000 populations and 93.8 per 100,000 populations(male:117.8, female:70.0), respectively. The former was similar to that of 1982, but the latter was increased to 1.6 times as that of 1982. 3. Age-adjusted cancer specific death rate by standardization with whole country population was 111.9(male:141.5, female:106.7) per 100,000 populations, higher than not age-adjusted cancer specific death rate(93.8), and the sex difference was statistically significant with male predominance (p<0.05). 4. Cancer specific death rate by age was generally increased with age and most of cancer deaths(male:91.8%, female:88.5%) occurred since 40 years old. 5. The major cancer(cancer specific death rate per 100,000 populations) in male was liver(30.6) followed by stomach(25.6), lung(21.9), and GB and EHBD(5.7), in female stomach(15.7), liver(9.9), lung(7.3), and uterus(6.9). The relative frequency of the leading three cancer among total cancer deaths marked 66.3% in male and 47.l% in female, and decreased in comparison to 1982(male:72.2%, female:54.5%). 6. The total ratio of male to female cancer specific death rate showed 1.68 to 1 with male predominance. And the ratio was above 2.0 in larynx, oral cavity & pharynx, esophagus, liver, lung, bladder cancer and the ratio was $1.0\sim1.9$ in stomach, pancreas, gall bladder and EHBD, brain, rectum and anus cancer, leukemia, but the ratio was reversed in thyroid and colon cancer. In conclusion, cancer was the first cause of deaths. The proportion of lung cancer was increased, that of stomach & uterine cancer was decreased relatively, and liver cancer was constantly higher proportion. In the future, it is necessary to conduct the further investigations on the cancer risk factors considering areal specificity.

통계청 사망자료를 이용한 우리나라 의사들의 사망률에 관한 연구 1992-2002 (Mortality among Medical Doctors Based on the Registered Cause of Death in Korea 1992-2002)

  • Objective : To compare the mortality rate of Korean medical doctors to that of the general Korean population for the period 1992-2002. Methods : The membership records of the Korean Medical Association were linked to the 1992-2002 death certificate data of Korea s National Statistical Office using 13-digit unique personal identification numbers. The study population consisted of 61,164 medical doctors with a follow-up period of 473,932 person-years. Standardized mortality ratios(SMRs) were calculated to compare cause-specific mortality rates of medical doctors to those of the general population. Results : We confirmed 1,150 deaths at ages from 30 to 75 years from 1 January 1992 to 31 December 2002. The SMR for all-cause of death was 0.47(95% CI : $0.44{\sim}0.50$). The SMRs for smoking-related diseases such as cerebrovascular accidents and chronic obstructive pulmonary disease were smaller than the SMR of all-cause of death. However, the SMRs for colorectal and pancreatic cancers were not significantly lower than those of the general population. Transport accidents and suicides accounted for 72% (94 of 131) of external causes of death. The SMR for suicide was 0.51 (95% CI : $0.38{\sim}0.68$). Conclusions : The mortality rate of South Korean medical doctors was less than 50% that of the general population of South Korea. Cause-specific analysis showed that mortality rates in leading causes of death were lower among medical doctors although differences in mortality rates between medical doctors and the general population varied with the causes of death. These health benefits found among medical doctors may be attributable to the lower level of health damaging behaviors (e.g., lower smoking rates) and better working conditions.

대규모 젖소 목장의 10년간 송아지 폐사 원인 분석 (The dairy calf mortality : the causes of calf death during ten years at a large dairy farm in Korea)

  • The objective of this study was to investigate the calf death and analyse the causes of the mortality by based on medical records and autopsy findings during 10 years in a large dairy farm. Total of 1,361 calf born and 146 calf dead during the invested period. Mortality rate was 10.7% and showed the big difference by year-specific mortality from 2.8% (4 calves) to 19.2% (28 calves). The highest rate of mortality was 1 week age (18.5%, 27 calves) and followed by 2 week age (11.6%, 17 calves) and mortality of more old calf tended to be reduced. The death less than 4 weeks and 8 weeks of age of the entire mortality accounted for 41.1% (60/146 calves) and 70.0% (102/146 calves), respectively. Causes of calf death were digestive diseases (53.4%), respiratory diseases (17.1%), musculoskeletal disease (8.2%), and systemic disease (8.2%) in order. Specific causes of calf death was highest in enteritis (43.2%), followed by pneumonia (14.4%), sepsis (8.2%) and fractures (3.4%). Seasonally, most of calf death happened in winter (48.6%) and then fall (21.2%). This results showed that enteritis and pneumonia are the main reason of calf death but other reasons were involved in calf death on the based on autopsy finding. On going research relating factors of calf mortality is needed.

  • We've reviewed 5,067 cases of claims by death that had occured for about 5 years from January, 1979 to October, 1983. As a result, We came to following conclusions; First, the greater part of deaths above mentioned were due to the Disease of Adult such as the Malignant neoplasm(18.4%), a Heart disease(10.4%), Liver cirrhosis(9.1%) and the Cerebrovascular disease(7.6%) as well as Accidental Death(26.5%), which were occupied by 72% of the whole. Second, classifying them by medical examination or non-medical, deaths in case of non-medical examination showed 80.8% of the. whole. And for age, sections ranging from 40 to 49 and from 30 to 39 took the overwhelming portions by 36.9% and 29.8% respectively, both of which showed 66.7% of the whole. Third, for the period elapsed, deaths within 1 year from the entrance showed 21.3% and that from 1 years to under 2 years, 19.9%. Thus the rate of early death under 2 years stood for 41.2% of the whole. Fourth, for occupation of the insureds, commerce occupied 20.4%, company employees 14.4%, agriculture 13.3%. These three categories marked 48.8% of the whole. From this, it appeared that the accidental death rate of the insured with the risky occupation was much greater than orthers.

환자안전사고에 의한 손상환자의 병원내 사망 관련 요인 : 2013-2017 퇴원손상심층조사자료 활용 (Factors Related to In-Hospital Death of Injured Patients by Patient Safety Accident : Using 2013-2017 Korean National Hospital Discharge In-depth Injury Survey)

  • This study aimed to analysis factors related to in-hospital death of injured patients by patient safety accident. A total of 1,529 inpatients were selected from Korea Centers for Disease Control and Prevention database(2013-2017). Frequency, Fisher's exact test, t-test, ANOVA, logistic regression analyses by using STATA 12.0 were performed. Analysis results show that the mortality rate was lower for female than male but the mortality rate was higher for the older age, the higher the CCI, head (or neck), multiple, systemic damage sites, internal and others, metropolitan cities based on Seoul and 300-499 based on the bed size of 100-299. Based on these findings, the possibility of using the in-depth investigation of discharge damage from the Korea Centers for Disease Control and Prevention as a data source for the patient safety survey conducted to understand the actual status of patient safety accident types, frequency, and trends should be reviewed. Also, it is necessary to prevent injury and minimize death by identifying factors that affect death after injury by patient safety accident.

우리나라 전체근로자와 외국인근로자의 산업재해율과 사망만인율 비교 연구 (The Comparative Study on the Occupational Injury Rate and Mortality Rate of the Total Workers and Foreign Workers)

  • The objective of this study is to compare on the occupational injury rate and mortality rate of all workers and foreign workers. By doing so, this study seeks to find out the improvements necessary to secure the basic safety net for foreign workers, as well as to find out the legal and institutional measures in place to make timely political intervention and protect the health of foreign workers. The status of foreign workers in Korea, their employment trends by industries, and occupational accident types and scales of Korean and foreign workers were analyzed from 2005 to 2009. Each year, foreign workers' occupational injury rate was much lower than Korea's overall occupational injury rate. But when analyzed by industry, occupational injury rate (death rate per 10,000 workers) of manufacturing industry was about 2 to 5 times higher than the overall occupational injury rate in Korea. Also, construction industry showed 3 to 16 times higher rate than the rate of overall industries. Although the death rate per 10,000 workers showed a declining yearly trend, foreign workers' occupational injury rate has the tendency to increase gradually. In particular, occupational injury rates and death rates per 10,000 workers were considerably high in the manufacturing and construction industries.
