독실라민 중독시 발생할 수 있는 발작의 특성과 위험인자 (The Clinical Features and Risk Factors of Seizure After Doxylamine Intoxication)

  • Purpose: Doxylamine is antihistamine drug that is used as a hypnotic. It is also used for suicidal attempts because it can be easily purchased at the pharmacy without a prescription. There were many articles about the complications after doxylamine intoxication such as a rhabdomyolysis, but only a few articles have reported on seizure. We reviewed the cases of doxylamine intoxication with seizure that were treated in the emergency department. Methods: We reviewed the medical records of the patients who were over 15 years old and who were intoxicated by doxylamine at 3 emergency medical centers from January 2006 to June 2010. We reviewed the patients' age, gender, the dose of doxylamine ingested, if gastrointestinal decontamination was done, the time from intoxication to hospital arrival, the seizure history, treatment of seizure, the electroencephalography (EEG) results, the brain computed tomography (CT) results and the blood test results. Results: There were 168 patients who were intoxicated by doxylamine during the study period. Twelve patients had a seizure episode. The differences between the patients who developed seizure and the patients who did not were the dose and the serum levels of sodium and creatinine. The only clinically meaningful difference was the amount of doxylamine. The amount of doxylamine ingested (>29 mg/kg) predicted the development of seizure with a sensitivity of 75% and a specificity of 92% on the ROC curve. One patient among the seizure patients expired in the emergency department. Conclusion: In case of doxylamine intoxicated patients, there is close relationship between seizure and ingested amount, so close observation needs to be done for the patients who ingest too much because doxylamine can cause death. Further prospective studies are needed for doxylamine intoxicated patients with a seizure episode.

Preoperative Risk Factors for Pathologic N2 Metastasis in Positron Emission Tomography-Computed Tomography-Diagnosed N0-1 Non-Small Cell Lung Cancer

  • Background: Accurate mediastinal lymph node staging is vital for the optimal therapy and prognostication of patients with lung cancer. This study aimed to determine the preoperative risk factors for pN2 disease, as well as its incidence and long-term outcomes, in patients with clinical N0-1 non-small cell lung cancer. Methods: We retrospectively analyzed patients who were treated surgically for primary non-small cell lung cancer from November 2005 to December 2014. Patients staged as clinical N0-1 via chest computed tomography (CT) and positron emission tomography (PET)-CT were divided into two groups (pN0-1 and pN2) and compared. Results: In a univariate analysis, the significant preoperative risk factors for pN2 included a large tumor size (p=0.083), high maximum standard uptake value on PET (p<0.001), and central location of the tumor (p<0.001). In a multivariate analysis, central location of the tumor (p<0.001) remained a significant preoperative risk factor for pN2 status. The 5-year overall survival rates were 75% and 22.9% in the pN0-1 and pN2 groups, respectively, and 50% and 78.2% in the patients with centrally located and peripherally located tumors, respectively. In a Cox proportional hazard model, central location of the tumor increased the risk of death by 3.4-fold (p<0.001). Conclusion: More invasive procedures should be considered when preoperative risk factors are identified in order to improve the efficacy of diagnostic and therapeutic plans and, consequently, the patient's prognosis.

Upregulation of Carbonyl Reductase 1 by Nrf2 as a Potential Therapeutic Intervention for Ischemia/Reperfusion Injury during Liver Transplantation

  • Currently, liver transplantation is the only available remedy for patients with end-stage liver disease. Conservation of transplanted liver graft is the most important issue as it directly related to patient survival. Carbonyl reductase 1 (CBR1) protects cells against oxidative stress and cell death by inactivating cellular membrane-derived lipid aldehydes. Ischemia-reperfusion (I/R) injury during living-donor liver transplantation is known to form reactive oxygen species. Thus, the objective of this study was to investigate whether CBR1 transcription might be increased during liver I/R injury and whether such increase might protect liver against I/R injury. Our results revealed that transcription factor Nrf2 could induce CBR1 transcription in liver of mice during I/R. Pre-treatment with sulforaphane, an activator of Nrf2, increased CBR1 expression, decreased liver enzymes such as aspartate aminotransferase and alanine transaminase, and reduced I/R-related pathological changes. Using oxygen-glucose deprivation and recovery model of human normal liver cell line, it was found that oxidative stress markers and lipid peroxidation products were significantly lowered in cells overexpressing CBR1. Conversely, CBR1 knockdown cells expressed elevated levels of oxidative stress proteins compared to the parental cell line. We also observed that Nrf2 and CBR1 were overexpressed during liver transplantation in clinical samples. These results suggest that CBR1 expression during liver I/R injury is regulated by transcription factor Nrf2. In addition, CBR1 can reduce free radicals and prevent lipid peroxidation. Taken together, CBR1 induction might be a therapeutic strategy for relieving liver I/R injury during liver transplantation.

호스피스 완화의료에서의 초기평가와 돌봄 계획의 수립: 평가도구를 중심으로 (Initial Assessment and Care Planning in Palliative Hospice Care: Focus on Assessment Tools)

  • 포괄적이고 총체적 돌봄을 제공하는 호스피스 완화의료의 경우 평가도구를 사용하여 문제목록의 객관화하고 돌봄 계획의 구체화하는 것이 필요하다. 초기평가는 입원 당일 혹은 1일 이내에 환자의 증상과 사회적, 영적 문제, 완화의료에 대한 요구가 무엇인지 파악하며 개괄적인 돌봄의 방향을 설정하는 과정이다. 현재 병에 대한 인식과 예후를 파악하는 것부터 연명의료계획서 작성까지 함께 검토한다. 초기평가도구로는 간단하지만 포괄적으로 신체적, 정신적, 사회적, 영적 영역을 아우르는 13개의 간단한 질문으로 구성된 NEST를 추천한다. 초기평가에서 파악된 문제에 대하여 구체적인 평가도구들을 활용하여 돌봄 계획을 수립하며 입원 후 3일이내 시행하는 것이 좋다. 신체적 영역의 경우 기능상태를 포함하여 통증과 비 통증 증상, 삶의 질을 함께 파악할 수 있는 다증상 평가도구가 도움이 될 수 있다. 환자의 증상은 단독으로 의미를 가지는 것이 아니라 복합적인 상호작용으로 발생하며, 그에 따라 포괄적으로 접근해야 하기 때문이다. 정신심리적 영역은 심리적 고통과 불안, 우울을 평가한다. 사회적 영역의 평가는 의사결정, 사회경제적 환경 파악, 가족 평가 및 임종 준비의 단계로 이루어진다. 영적 평가 역시 중요한데 FACIT-Sp나 SHI를 사용할 수 있다. 평가도구를 활용하는 것은 환자의 고통을 경감시키고 삶의 질을 향상시킬 뿐 아니라 의료진이 체계적으로 훈련되는 방안이 될 수 있으며 그 과정 자체가 더 나은 돌봄 제공을 위한 발판이 될 수 있다.

건강한 소아에서 살모넬라 장염의 합병증으로 발생한 독성거대결장 (Toxic Megacolon as a Complication of Infectious Colitis Caused by Salmonella enteritidis Group D in a Previously Healthy Child)

  • 독성거대결장은 염증성 장질환, 감염성 대장염 등에 의해 발생하는 치명적인 합병증이다. 중증도에 따라 장절제부터 사망까지 예후가 나빠 조기 진단과 적절한 치료가 매우 중요한 질환이다. 하지만, 소아에서 그 유병률은 매우 낮으며 염증성 장질환과 같은 기저질환이 없이 발생할 경우 조기진단이 늦어질 수 있다. 기저질환이 없는 12세 남자가 하복부 통증과 발열, 그리고 혈변을 주소로 병원을 방문했다. 항생제 치료에도 불구하고 환자의 증상은 악화되었다. 3병일째 복부 컴퓨터단층촬영에서 횡행결장의 심각한 팽창이 관찰되었고 이를 통해 독성거대결장으로 진단할 수 있었다. 광범위항생제와 함께 정맥 스테로이드 치료를 시행하였으며, 환자의 증상은 호전되었다. 이후 대변 배양 검사에서 Salmonella enteritidis group D가 배양되었으며, 직장 내시경 검사를 통해 염증성 장질환을 배제할 수 있었다. 저자는 독성거대결장을 적절한 영상 검사를 통해 조기 진단하였고, 내과적으로 성공적인 치료를 하여 이에 대한 경험을 공유하고자 한다.

코로나 19 지표에 따른 교통수요 현황 및 상관관계 분석 (Analysis of the Current Status and Correlation of Traffic Demand according to the COVID-19 Indicator)

  • 코로나 19가 국내에서 2020년 1월 첫 확진자가 발생하며 현재까지 pandemic 상황이 지속하고 있다. 전례 없는 상황에 코로나 19는 교통 분야에도 영향을 끼쳤으며 시민들의 생활패턴변화로 교통량의 변화 및 대중교통 이용변화에 적절한 대응 방안이 존재하지 않았다. 현재 각 지자체에서는 pandemic 상황에 대한 질병 대책 방안은 별도로 수립하고 있지 않다. 향후 교통분야 질병 대책 방안 수립을 위해 교통량 및 대중교통 이용 건수를 분석하였으며 예측모형 개발을 위해 코로나 19 현황과 상관도 분석을 수행하였다. 분석 결과 교통량은 감소하였지만 개인 교통수단의 증가로 교통량 감소량은 대중교통 이용 건수에 비해 감소율이 낮다. 또한 대중교통 이용 현황은 초기에는 확진자 수에 영향을 받았지만 시간이 지남에 따라 확진자 수 보다는 사망자 수와 치명률에 더욱 민감하게 반응하는 것으로 분석되었다.

25-Hydroxyvitamin D level is associated with mortality in patients with critical COVID-19: a prospective observational study in Mexico City

  • BACKGROUND/OBJECTIVES: Considering the high number of deaths from coronavirus disease 2019 (COVID-19) in Latin American countries, together with multiple factors that increase the prevalence of vitamin D deficiency, we aimed to determine 25-hydroxyvitamin D (25[OH]D) levels and its association with mortality in patients with critical COVID-19. SUBJECTS/METHODS: This was a prospective observational study including adult patients with critical COVID-19. Data, including clinical characteristics and 25(OH)D levels measured at the time of intensive care unit admission, were collected. All patients were followed until hospital discharge or in-hospital death. The patients were divided into those surviving and deceased patient groups, and univariate and multivariate logistic regression analyses were performed to determine independent predictors of in hospital mortality. RESULTS: The entire cohort comprised 94 patients with critical COVID-19 (males, 59.6%; median age, 61.5 years). The median 25(OH)D level was 12.7 ng/mL, and 15 (16%) and 79 (84%) patients had vitamin D insufficiency and vitamin D deficiency, respectively. The median serum 25(OH)D level was significantly lower in deceased patients compared with surviving (12.1 vs. 18.7 ng/mL, P < 0.001). Vitamin D deficiency was present in 100% of the deceased patients. Multivariate logistic regression analysis revealed that age, body mass index, other risk factors, and 25(OH)D level were independent predictors of mortality. CONCLUSIONS: Vitamin D deficiency was present in 84% of critical COVID-19 patients. Serum 25(OH)D was independently associated with mortality in critical patients with COVID-19.

중·고령 초기 유방암 생존자를 돌보는 가족의 경험 (Family Members' Caregiving Experience for Middle-aged and Elderly Breast Cancer Survivors at Early Stages)

  • 본 연구는 중·고령 초기 유방암 생존자를 위한 가족들의 돌봄 경험에 대해 이해하고자 한 질적 연구이다. 총 4명의 유방암 생존자 가족들을 대상으로 초점집단면접(FGI)을 실시하였고 추가로 1명의 가족에 대해 개별심층면접을 실시하였다. 초점집단면접과 개별심층면접 내용을 주제 분석한 결과, 다음 4가지 주제가 나타났다: 첫째, 살얼음 위를 걷는 듯한 일상의 감정들 (치료시기마다 서로 뒤얽히는 감정들, 생존자의 고통을 지켜볼 수밖에 없는 안타까움, 암 재발과 죽음에 대한 두려움, 관계 변화에 대한 걱정, 딸로서 겪는 다양한 감정), 둘째, 생활의 중심이 된 돌봄 (낯설고 서툴렀던 돌봄, 내 생활을 잃어감), 셋째, 회복의 시작, 생존자와 가족이 느끼는 온도 차이 (생존자가 느끼는 심연의 고통을 헤아리지 못함, 생존자가 아직 환자라는 사실을 잊고 소홀히 대함), 넷째, 앞으로 나아가기 (나아질 것이라는 희망, 지치지 않기 위한 나를 위한 돌봄). 본 연구 결과를 토대로 유방암 생존자 가족의 심리사회적 욕구를 반영하고 돌봄 과정에서 가족 지지를 지원하기 위한 사회복지서비스 및 정책 방안에 대해 제언하였다.

  • 프로피온산혈증은 PCCA 및 PCCB 유전자의 돌연변이로 발생하며 대사산물의 축적으로 신생아기부터 근긴장 저하, 구토, 케톤산증, 고암모니아혈증, 경련 등이 나타나 사망에 이를 수 있고, 비교적 후기에 신경학적 증상과 함께 발현하는 경과를 보이기도 한다. 저자들은 특별한 과거력 없이 지내던 중 확장성 심근병증이 확인된 16세 남아에서 유전자 검사로 프로피온산혈증이 진단된 1례를 보고하는 바이다.

Late Gadolinium Enhancement of Left Ventricular Papillary Muscles in Patients with Mitral Regurgitation

  • Objective: Arrhythmogenic mitral valve prolapse (MVP) is an important cause of sudden cardiac death characterized by fibrosis of the papillary muscles or left ventricle (LV) wall, and an association between late gadolinium enhancement (LGE) of the LV papillary muscles and ventricular arrhythmia in MVP has been reported. However, LGE of the papillary muscles may be observed in other causes of mitral regurgitation, and it is not limited to patients with MVP. This study was to evaluate the association of LGE of the LV papillary muscles or ventricular wall on cardiac magnetic resonance imaging (CMR) and ventricular arrhythmia in patients with mitral regurgitation. Materials and Methods: This study included 88 patients (mean age ± standard deviation, 58.3 ± 12.0 years; male, 42%) with mitral regurgitation who underwent CMR. They were allocated to the MVP (n = 43) and non-MVP (n = 45) groups, and their LGE images on CMR, clinical characteristics, echocardiographic findings, and presence of arrhythmia were compared. Results: LV myocardial wall enhancement was more frequent in the MVP group than in the non-MVP group (28% vs. 11%, p = 0.046). Papillary muscle enhancement was observed in 7 (7.9%) patients. Of the 43 patients with MVP, 15 (34.8%) showed LGE in the papillary muscles or LV myocardium, including 12 (27.9%) with LV myocardial wall enhancement and 4 (9.3%) with papillary muscle enhancement. One patient with bilateral diffuse papillary muscle enhancement experienced sudden cardiac arrest due to ventricular fibrillation. Univariable logistic regression analysis showed that high systolic blood pressure (BP; odds ratio [OR], 1.05; 95% confidence interval [CI], 1.01-1.09; p = 0.027) and ventricular arrhythmia (OR, 6.84; 95% CI, 1.29-36.19; p = 0.024) were significantly associated with LGE of the papillary muscles. Conclusion: LGE of the papillary muscles was present not only in patients with MVP, but also in patients with other etiologies of mitral regurgitation, and it was associated with high systolic BP and ventricular arrhythmia. Papillary muscle enhancement on CMR should not be overlooked.
