Evaluation of Validity of Deficiency and Excess Pattern Identification Questionnaire (검진용 허실 변증 진단 설문지 타당도 평가)

  • The purpose of this study was to evaluate the validity of Deficiency and Excess Pattern Identification Questionnaire. The number of subjects enrolled in this study was 431. Pearson Correlation Coefficient analysis were conducted to reveal the correlation among Deficiency and Excess Pattern Identification Questionnaire, Grip Strength and Quality of life. ROC-curve analysis were used to suggest optimal cut off value. Cohen Kappa also used to analyze for diagnostic validity. The significant p-value was < .05. The Deficiency questionnaire had positive correlation with the score of the expert and negative correlation with Grip Strength and Quality of life (p<.001). The Excess questionnaire had positive correlation with the score of the expert and negative correlation with Quality of life (p<.001). The optimal cut off value was 56.5 to separate deficiency and non deficiency and 47.5 to separate excess and non-excess. Furthermore, AUC was .900 and .851 accordingly. Cohen Kappa value between deficiency questionnaire and the expert was .640. Cohen Kappa value between excess questionnaire and the expert was .513. This study reveals that Deficiency and Excess Pattern Identification Questionnaire is valid. However, further study considering of gender, age, and health condition and so on, is needed.

A Study on the Improvement of Evaluation Methods for Roadside Slope Revegetation - Focus on the Cut-soil Slope - (도로비탈면 녹화공사 평가방법의 개선 방안 연구 - 절토부 토사를 중심으로 -)

  • Despite the eco-friendly route plan, road slopes were collapsed due to road construction, resulting in human and property damage. To solve this problem, the Ministry of Land, Transport and Maritime Affairs established guidelines in 2009 to conduct a test-bed for slope recording considering the conditions of the site and the ecological environment, and divided them into recording quality and economic evaluation. The following results were obtained by analyzing 183 construction methods of cut-soil sections at 60 sites from 2012 to 2018 for road slope afforestation research. Straw net+seedspray, vegetation media spray method 1T, 3T were used the most, and vegetation media spray method 3T(patent) was excellent in quality, and straw net+seedspray was excellent in economics. As a result of analyzing the market unit price and the construction unit price, vegetation media spray method submitted the construction unit price at up to 60% lower than the market unit price. As a result of the analysis of the key factors of the greening method evaluation, the economic assessment had the greatest influence on all evaluation items. Problems in the evaluation method of revegetation were first identified as problems in the allocation of points and secondly as problems in the evaluation criteria. As for the improvement of the economic assessment criteria, the method was proposed to evaluate the same method based on market unit price when the same method was constructed, and not to conduct an economic assessment if there was a difference in market unit price between methods, or to add weight to the scores. Based on the monitoring data of 60 road slopes, this study drew up problems and improvement measures. However, with regard to scoring, research on appropriate scoring is needed by examining the current status.

Validation of the Risk Prediction Tool for Wound Infection in Abdominal Surgery Patients (복부 수술환자의 수술부위 감염 위험 예측 도구의 타당도 검증)

  • Purpose : This retrospective investigation study aimed to determine the predictive validity of superficial surgical site infection assessment tools by measuring the risk score at the surgical site. Methods : This study included patients hospitalized to the general surgery department of a Hospital from January 2021 to December 31, 2021. The inclusion criteria were age ≥19 years, general abdominal surgery under general anesthesia, and hospital stay longer than 2 days. Patients who had undergone transplantation were excluded. Results : Tool validity results showed that tools including surgical time and operative procedure were more accurate than previously developed tools, with a sensitivity of 71.1%, specificity of 71.4%, positive prediction of 12.3%, negative prediction of 97.8%, and area under the curve of 0.743 (95% confidence interval, 0.678~0.745). The tool's cut-off score was 15, and the risks of infection was increased by 6.14 times at or above this cut-off point. Preoperative hair removal period, surgical wound classification, surgery time, body temperature on the second day after surgery, drainage tube type, and suture type affected the risk of infection at the surgical site. Conclusion : The incidence of healthcare-associated infections has been declining in the past decade; however, surgical site infections still account for a considerable proportion. Therefore, early identification of high-risk groups for surgical site infection is crucial for reducing the incidence of surgical site infection using appropriate management.

Knowledge and Performance of Universal Precautions by Nursing Students (일부 간호대학생의 혈액매개질환 예방 지침에 대한 지식과 실천 정도)

  • The purpose of this study was to identify knowledge of universal precautions and its performance in practice. The research was conducted from November 2 to 30, 1998. A total 515 student nurses ; 249 from a baccalaureate nursing college and 266 from second and third year of a 3-year community nursing college were surveyed. The results are as following : 1. The average score for universal precautions knowledge was 270.41$\pm$19.43/300(range 150-300). The results showed that 99.2% of students avoid injury from used needles, 98.6% answered that they always wash their hands if they had contact with the patient's blood and they always dispose of used needles in special collectors (97.7%) for needles. But, 39.2% responsed that they dispose of used needles after recapping them. 2. The average score for universal precautions knowledge of the senior students in the 4-year college was the highest (277.65$\pm$13.99). 3. The average score for the performance of universal precautions knowledge was 53.18 $\pm$5.91(range 14-70). The items : ‘I cautiously avoid injury from the used needles’(4.92$\pm$0.33), ‘I always wash my hands if there has been contact with the patient's blood’(4.91$\pm$0.34), and ‘I always disposed of used needles in the appropriate collector’(4.89$\pm$0.42) showed the highest performance. However ‘I always dispose of used needles after recapping them’(2.19$\pm$1.39) and ‘I always use protection goggles when in danger of contamination’(2.19$\pm$1.20) showed low performance level. 4. The highest average score for universal precautions performance was shown among the second year students in 3-year nursing college (54.19$\pm$6.92) between the groups. It showed that the level of the universal precautions performance was higher for those who had education on university precautions prior to performance of the universal precautions than for those without any prior education. 5. The percentage of students who reported the experience of direct contact with patients' blood and/or body fluids was 42.30%. The experience of direct contact with blood and/or body fluids of the educational group was significantly higher than those were not educated. 6. The most frequent cause of the direct contact was ‘needle pricking and/or skin cut’(63.04%). The most frequent substance with which the students contact was ‘blood’(59.85%). The majority of the sample had answered that the mode of contamination was ‘unknown’(63.54%). The majority of the sample answered that strategies used after contamination included ‘washing with soap’(33.61%). Reviewing the chart of patients or asking other health professionals(28.85%). 7. The number of students who had the experience of a needle stick and/or skin cut was 145(28.16%). The clinical practice places where the incidents occurred were mainly in the internal medicine unit (45.07%) and the surgical unit (31.92%) followed by the intensive care unit and the emergency unit in order. The experience of a needle stick and/or skin cut happened during on intra-muscular injection 47.34% and intravenous injection 21.81%. The causes of the needle stick and/or skin cut were ‘putting the needle cap back on 77(35.81%)’. The number of students who took an appropriate post management blood test and/or vaccination was 27(18.62%). 8. The Pearson Correlation Coefficient between the knowledge of universal precautions and performance of universal precautions in practice showed a positive correlation.

Effects of Edible Coatings on the Quality of Fresh-cut Pears (가식성 코팅처리가 신선절단 배의 품질에 미치는 영향)

  • To examine the effects of edible coatings on the quality of fresh-cut fruits, Shingo pear was coated with several concentration of albumin, WSF(whole soy flour), dextrin and SPE (sucrose polyester) solution and quality characteristics, sensory evaluation and respiration rate were determined during storage at $4^{\circ}C$ for 10 days. Weight loss rate was reduced in coated fresh-cut pears. 2% dextrin and 1% WSF for pears were more effective in reducing weight loss rate during storage. In preventing the browning of fresh cut fruits, browning was inhibited by coating with 0.5% WSF, 1% albumin, 3% dextrin for pears. Compared with non-coated pears, 0.5% WSF, 1% albumin and 3% dextrin coating were effective in inhibiting the color changes. Even if hardness had a tendency to decrease in coated and no71-coated fruits during storage dextrin coating induced texture softening in fresh-cut pears. The changes in decrease of titratable acidity and vitamin C content and increase of pH and soluble solid were recessed by coatings but there was no differences in accordance with coating materials. Sensory evaluation of coated pears was conducted in according to coating materials and concentrations. Pears coated with 0.5% albumin, 4% dextrin, 1.0% SPE and 0.5% WSF recorded high sensory score. Among the tested coating materials and concentrations, 4% dextrin was the best in all organoleptic quality of coated pears. Respiration rate was repressed by coating with WSF and dextrin in pears.

  • Background: The decision to administer oral anticoagulation therapy depends on accurate assessment of stroke risk in patients with atrial fibrillation (AF). Various stroke risk stratification schemes have been developed to help inform clinical decision making. The CHADS2 and CHA2DS2-VASc scores have been used in estimating the risk of stroke in patients with AF. Recently R2CHA2DS2-VASc score was developed. The objective of the current study is to validate the usefulness of the R2CHA2DS2-VASc score and to compare the accuracy of the CHADS2, CHA2DS2-VASc, and R2CHA2DS2-VASc scores in predicting a patient's risk of stroke. Methods: Based on medical records, we conducted a retrospective study of patients hospitalized with AF from March 2011 to July 2013. A total of 448 AF patients were included in this study. The receiver operating characteristic (ROC) curve analysis in MedCalc was used for comparison with respective diagnostic values. Results: The patient characteristics showed male predominance (60.9%). Among the 448 AF patients, 131 (29.2%) patients had strokes during the study. A R2CHA2DS2-VASc score of more than 5 is the optimal cut-off value for prediction of stroke. A risk score of three, the area under the ROC curve (AUC) of R2CHA2DS2-VASc score (AUC 0.631; 95% confidence interval, 0.585-0.679) was the highest. A significant difference was observed between AUC for R2CHA2DS2-VASc, CHADS2, and CHA2DS2-VASc scores, but no meaningful difference between CHADS2 and CHA2DS2-VASc scores. Conclusion: We determined the usefulness of the R2CHA2DS2-VASc score, which showed better association with stroke than the CHADS2 and CHA2DS2-VASc scores.

Predictive Value of IHC4 Score for Pathological Response to Neoadjuvant Chemotherapy in Hormone Receptor-Positive Breast Cancer

  • Purpose: This study aimed to explore the value of IHC4 in predicting pathological response after neoadjuvant chemotherapy in patients with hormonal receptor (HR)-positive breast cancer (BC). Materials and Methods: In this retrospective exploratory study, data for 68 HR-positive BC patients who received neoadjuvant chemotherapy were recorded. IHC4 scores were calculated based on estrogen receptors/progesterone receptors, Ki-67 and HER2 status. Logistic and ordinal regression analyses in addition to likelihood ratio test were used to explore associations of IHC4 scores and other clinico-pathological parameters with pathological complete response (pCR) and pathological stage. Results: Taking the 25th percentile as the cut-off, a lower IHC4 score was associated with an increased probability of pCR (low; 52.9% vs. High; 21.6%, OR=4.1, 95% CI=1.28-13.16, p=0.018) and a lower pathological stage (OR=3.9, 95% CI=1.34-11.33, p=0.012). When the IHC4 score was treated as a continuous variable, a lower score was again associated with an increased probability of pCR (OR=1.010, 95% CI=1.001-1.018, p=0.025) and lower pathological stage (OR=1.009, 95% CI=1.002-1.017, P=0.008). Lower clinical stage was associated with a better pCR rate that was of borderline significance (P=0.056). When clinical stage and IHC4 score were incorporated together in a logistic model, the likelihood ratio test gave a P-value of 0.004 after removal of the IHC4 score and 0.011 after removal of the stage, indicating a more significant predictive value of the IHC4 score for pCR. Conclusions: This study suggests that the IHC4 score can predict pathological response to neoadjuvant chemotherapy in HR-positive BC patients. This finding now needs to be validated in a larger cohort of patients.

  • The facility diagnosis technique of past subjective judged the technical expert and interpreted stably mainly. The facility the periodical civil official becomes indispensability consequently with the law and the system and objectivity inspection which relates with a this and development of diagnosis technique were necessary. Currently the evaluation elements which relate with an inspection and diagnosis technique depended to the subjectivity of each facility quality, data statistics, diagnosis know-how, various circumstance and the technical expert and the evaluation item came to decide. But the evaluation score distribution of marks of each item depended plentifully in data analysis result and experience. The past method is weak in logically because the objection theoretical background and basis are insufficiency. Consequently in order to apply a data analysis result and an experience logically, it used AHP(Analytic Hierarchy Process) method from this research. It is visible with the fact that the application result reconfirmation and complement process will be necessary the technique which is suitable in the multi facility management. Also it is expected with the fact that the application characteristic from the next relation technique will be excellent.

  • The SK Chemical R&D center is an eco-friendly building designed as top score in the GBCC(GREEN BUILDING CERTIFICATE CRITERIA). This building has applied various eco-friendly technologies such as energy /water resource cutdown, improvement of indoor quality and improvement of user convenience, etc through eco-friendly concept from the design phase. In this thesis, an economic efficiency evaluation has been performed on building energy cwater resource cutdown technologies among them and the results are as follows. The building energy has cut down about 40% compared to ordinary buildings and the investment recovery period was shown as about 5 years. The water resource has cut down about 63% compared to ordinary buildings and the investment recovery period was shown as about 10 years.

A Validation Study of the Korean Child Behavior Checklist 1.5-5 in the Diagnosis of Autism Spectrum Disorder and Non-Autism Spectrum Disorder

  • Objectives: The purpose of this study was to analyze the discriminant validity and the clinical cut off scores of the Child Behavior Checklist 1.5-5 (CBCL 1.5-5) in the diagnosis of autism spectrum disorder (ASD) and non-ASD. Methods: In total, 104 ASD and 441 non-ASD infants were included in the study. T-test, discriminant analysis, receiver operating characteristic (ROC) curve analysis, and odds ratio analysis were performed on the data. Results: The discriminant validity was confirmed by mean differences and discriminant analysis on the subscales of Emotionally reactive, Somatic complaints, Withdrawn, Sleep problems, Attention problems, Aggressive behavior, Internalizing problems, Externalizing problems, and Total problems, along with the Diagnostic and Statistical Manual of Mental Disorders (DSM)-oriented scales between the two groups. ROC analysis showed that the following subscales significantly separated ASD from normal infants: Emotionally reactive, Somatic complaints, Withdrawn, Sleep problems, Attention problems, Aggressive behavior, Internalizing problems, Externalizing problems, Total problems, and DSM pervasive developmental problems. Moreover, the clinical cut off score criteria adopted in the Korean-CBCL 1.5-5 were shown to be valid for the subscales Withdrawn, Internalizing problems, Externalizing problems, Total problems, and DSM pervasive developmental problems. Conclusion: The subscales of Withdrawn, Internalizing problems, Externalizing problems, Total problems, and DSM pervasive developmental problems significantly discriminated infants with ASD.
