The Role of Air-Vacuum Cushion Device in Patients with Rectal Cancer in Radiation Therapy (직장암 환자에서 방사선치료시 Air-vacuum Cushion의 유용성)

  • Prupose : We analyzed setup errors induced by using air-vacuum cushion as immobilization device in patients with rectal cancer. Materials and methods : We had treated the twenty patients with rectal cancer by 6 MV, 10 MV X-ray from Aug. 1998 to Aug. 1999 at Chungnam National University Hospital. All patients were treated at prone position. They were separated to two groups, control group, 10 patients using styrofoam, and test group, 10 patients using styrofoam and air-vacuum cushion. We measured errors of posterior field for x, y axis and lateral field for z, y axis with simulation film and EPID image using a matching technique. Results : In control group, the mean displacement values of pelvic bone landmark for x axis and y axis were 0.02 mm. 0.78 mm, respectively and the standard deviations of systematic error were 2.13 mm, 2.40 mm, respectively and the standard deviation of random error were 1.46 mm. 1.51 mm, respectively. In test group, the mean displacement values of x axis and y axis were -0.33 mm. 0.81 mm, respectively and the standard deviations of systematic error were 1.71 mm, 3.08 mm, respectively and the standard deviations of random errors were 1.40 mm. 1.88 mm, respectively. The mean displacement values of z axis and y axis were 2.98 mm. 0.74 mm, respectively and the standard deviations of systematic error were 4.75 mm, 2.65 mm, respectively and standard deviations of random error were 2.69 mm. 1.86 mm, respectively. The statistical difference of field size by using air vacuum cushion between two groups in posterior direction and lateral direction was not shown. Conclusion : We think that use of air-vacuum cushion may not be an advantage for improving setup accuracy in rectal cancer patients.

Experimental study on the horizontal bearing characteristics of long-short-pile composite foundation

  • Long-short pile composite foundations bear both vertical and horizontal loads in many engineering applications. This study used indoor model tests to determine the horizontal bearing mechanism of a composite foundation with long and short piles under horizontal loads. A custom experimental device was developed to prevent excessive eccentricity of the vertical loading device caused by the horizontal displacement. ABAQUS software was used to analyze the influence of the load size and cushion thickness on the horizontal bearing mechanism. The results reveal that a large vertical load leads to soil densification and increases the horizontal bearing capacity of the composite foundation. The magnitude of the horizontal displacement of the pile and the horizontal load borne by the pile are related to the piles' positions. Due to different pile lengths, the long piles exhibit long pile effects and experience bending deformation, whereas the short piles rotate around a point (0.2 L from the pile bottom) as the horizontal load increases. Selecting a larger cushion thickness significantly improves the horizontal load sharing capacity of the soil and reduces the horizontal displacement of the pile top.

  • This study is to develop a device of bath-seat with massage which is attached to bath-seat and is able to sustain massage on perineal region and anus effectively. And also develop new programmed driving-seat that is able to protect drowsiness. This seat is operated easily by user controlling time period (eg. 30 minutes or 60 minutes) of vibration. During the vibration, user can make a choice of making n sound which is proved as a better effects for concentration and comfort.

  • The airbag is a widely accepted device for occupant protection in the automotive industry. As the injuries induced by airbag deployment have become a critical issue, revisions to Federal Motor Vehicle Safety Standard (FMVSS) 208 were required to create advanced airbags that can protect occupants of varying statures. In this paper, we developed a new low-risk deployment passenger airbag by adding the Passenger Protection Wrap (PPW). The PPW reduces the cushion impact force to the occupant in order to ensure pressure dispersion. A series of tests were conducted by using FMVSS 208 test procedures to demonstrate the proposed system. It was found that the system not only satisfied the injury criteria of FMVSS 208 but was also effective for protecting passengers of all sizes (male, small female, 3-year-old, 6-year-old).

  • A bushing is a device used in automotive suspension systems to cushion the force transmitted from the wheel to the frame of the vehicle. A bushing is essentially a hollow cylinder which is bonded to a solid metal shaft at its inner surface and a metal sleeve at its outer surface. The shaft is connected to the suspension and the sleeve is connected to the frame. The cylinder provides the cushion when it deforms due to relative motion between the shaft and sleeve. The relation between the force applied to the shaft or sleeve and its deformation is nonlinear and exhibits features of viscoelasticity. An explicit force-displacement relation has been introduced for multi-body dynamics simulations. The relation is expressed in terms of a force relaxation function and a method of determination by experiments on bushings has been developed. Solutions allow for comparison between the force-displacement behavior by experiments and that predicted by the proposed method. It is shown that the predictions by the proposed force-displacement relation are in very good agreement with the experimental results.

  • Seismic excitations may impart a significant amount of energy into structures. Modern structural design attitudes tend to absorb some part of this energy through special dissipaters instead of heavy plastic deformations on the structural members. Different types of dissipater have been generated and utilized in various types of structures in last few decades. The expected earthquake damage is mainly concentrated on these devices and they may be replaced after earthquakes. In this study, a low-cost device called energy dissipative steel cushion (EDSC) made of flat mild steel was developed and tested in the Structural and Earthquake Engineering Laboratory (STEELab) of Istanbul Technical University (ITU). The monotonic and cyclic tests of EDSC were performed in transversal and longitudinal directions discretely. Very large deformation capability and stable hysteretic behavior are some response properties observed from the tests. Load vs. displacement relations, hysteretic energy dissipation properties as well as the closed form equations to predict the behavior parameters are presented in this paper.

Development of Knee Weight-bearing Radiography Assist Device (무릎 체중부하 촬영 보조장치 개발)

  • The knee arthritis patients are generally performed standing position radiography. However, patients with pain caused by degenerative diseases or injuries in the knee have difficulty maintaining a standing position. Therefore, this study aims to develop a knee joint standing weight bearing projection assist device to solve these problems when patients undergo knee radiography due to various diseases. The design of the knee joint weight-bearing radiography assist device is carried out with 3D design and drawing production to secure basic data, electric support and frame manufacture. A fixed device for maintaining the patient's knee standing weight-bearing projection, an electric digital height device, a digital protractor, and a safety device were designed. The arm support is made of PVC with a cushion to relieve the impact and make it easier to remove debris. The digital electric device can be moved up and down according to the patient's height, and a remote control is attached. The safety device is made to be adjustable in size by attaching metal hooks on both sides of the frame and to shield the scrotum. A digital protractor was attached to the side frame to set the desired knee angle. When a self-made assist device was used to perform a knee joint standing weight bearing projection, it helped maintain arm support and lower extremities position. In addition, the height could be adjusted using an assist device during standing projections, which helped the patient maintain the position.

  • The chairbound, handicapped person often requires a cushion to distribute the supportive forces over the largest area possible in order to reduce the risk of the development of a pressure sore. Pressure sores are areas of damaged skin caused by staying in one position for too long and can cause serious infections, some of which are life-threatening. When sitting upright, the greatest proportion of body weight is centerd over the ischial tuberosities. So, it is important that comfortable seating and largest distribution of pressure to prevent pressure sores. Therefore, the objective of this study was to develop the physical pressure measurement device and orthodontic chair to prevent pressure sores.

Reliability and Validity of the Measurement of Pelvic Movement in Low Back Pain Patients using Cushion Sensor in Sitting Position (앉은 자세에서 방석센서를 이용한 요통환자 골반가동성 측정의 신뢰도와 타당도)

  • PURPOSE: Postural and structural asymmetry due to muscle imbalances around the lower back and pelvis are the causes of back pain. Muscle imbalances in patients with chronic low back pain affect the pelvic tilt and movement, and it is necessary to assess the pelvic movement ability using the appropriate tools to determine the mediating effects of lower back pain. This paper reports the reliability and validity of the Sensbalance Therapy Cushion (STC) for pelvic movement and proprioception. METHODS: In this study, the Wii balance board (WBB) was used as a golden standard for pelvic movement measurements. FABQ, KODI, Myovision, and Pelvic movement were measured in 50 patients with chronic low back pain. The correlation between the lower-back muscle activity and pelvic movement was checked. The pelvic movement parameter was measured twice to determine the intra-rater reliability. RESULTS: The STC showed high test-retest reliability in the pelvic tilt measurements (ICC = .672 - .809). The test-retest reliability of proprioception measurements (ICC = .588 - .859) and reaction time measurements (ICC = .542 - .836) were also high. The relationship between the WBB and STC showed a significant positive correlation with the pelvic tilt test (p < .01). The posterior pelvic tilt and lower-back muscle activity showed a significant negative correlation (p < .01). The pelvic left tilt and lower-back muscle activity showed a significant negative correlation (p < .05). CONCLUSION: The results revealed the high reliability and validity of the STC. Therefore, the STC can be used as an objective measuring device for evaluating pelvic tilt, proprioception, and reaction time in low back pain patients.

Reliability and validity of pelvic mobility measurement using a cushion sensor in healthy adults

  • Objective: To prevent low back pain, an objective evaluation tool to evaluate pelvic mobility and exercise to improve the flexibility of the lumbar region is needed. The purpose of this study was to compare the results of pelvic mobility measurements using the Wii Balance Board (WBB) and Sensbalance Therapy Cushion (STC), evaluate the usefulness of the STC as a tool for measuring pelvic mobility. Design: Cross-sectional study. Methods: Fifty healthy subjects participated in this study. The subjects performed pelvic mobility range, proprioception, reaction time and reach of the arm using the STC. The pelvic movement parameter was measured two times to determine the intra-rater reliability. To measure the correlation between lumbar muscle tension and pelvic mobility, Myovision was used to measure tension of L4, L5 level erector spinae muscle. Correlations between measured variables were checked to determine the validity of the pelvic mobility assessment tool. Results: STC showed high test-retest reliability in pelvic tilt measurement and reaching task [intraclass correlation coefficients (3,1)=0.804-0.915]. The relationship between WBB and STC showed a significant positive correlation with the pelvic tilt and reaching task (p<0.05). Posterior tilt and erector spinae activation (Lt. L5) showed a significant negative correlation (p<0.05). Left, right tilt and erector spinae activation (L5) showed a significant negative correlation (p<0.05). Conclusions: This study confirmed the advantages of the STC and found efficiency as an objective measuring device of pelvic mobility.
