Clinical Characteristics and Diagnosis of Laryngeal Tuberculosis (후두결핵의 임상양상과 진단)

  • Background and Objectives : Clinical suspicion and appropriate diagnostic procedures are essential for the timely management of extrapulmonary type of mycobacterial disease. In the hope of suggesting a suitable guideline for the early diagnosis of laryngeal tuberculosis, the authors reviewed their clinical pathways and the characteristics of patients with laryngeal tuberculosis who were managed in the recent 10 years at a single tertiary referral hospital, Samsung Medical Center. Subjects and Method : Retrospective chart review was performed for the 25 adult patients with laryngeal tuberculosis. Among 25 cases, 12 were pathologically confirmed by laryngeal biopsy and the other 13 were clinically diagnosed by cumulative clinical information; definite laryngitis on laryngoscopy, positive AFB (acid fast bacillus) smear/culture or active pulmonary tuberculosis on chest X-ray, and substantial response to anti-tuberculosis medication. Results : Chest X-ray revealed active pulmonary tuberculosis in 72% of patients (N=18/25). Sputum AFB smear/culture was positive in 95% of all tested patients (N=21/22) and in 100% of the tested patients who have stable or no evidence of pulmonary tuberculosis (N=5/5). All patients except one who had coexisting laryngeal malignancy showed considerable improvement in their subjective symptoms and laryngeal findings within the first 2 months of anti-tuberculosis medications and they achieved complete response on subsequent sputum studies, chest X-ray and laryngeal findings after $7.0{\pm}2.3$ months of the medications. Conclusion : We suggest that chest X-ray and sputum AFB smear/culture to be the first step of work-up for the patients having laryngeal tuberculosis in suspicion since laryngeal tuberculosis is largely associated with active pulmonary tuberculosis and/or sputum AFB study offers high yield even in case of primary laryngeal tuberculosis. However laryngeal biopsy must be considered in case showing unsatisfactory response to the anti-tuberculosis medication for more than 2 months.

  • Background: The polymerase chain reaction (PCR) test is important for the confirmatory diagnosis of tuberculosis (TB) caused by Mycobacterium tuberculosis. The aim of this study was to analyze the yield of repeated PCR testing in patients with confirmed pulmonary TB. Methods: The medical records of 130 patients, who had more than two consecutive PCR tests and a M. tuberculosis-positive sputum culture from August, 2006 to December, 2007, were retrospectively reviewed for the purposes of this study. A positive TB-PCR test was defined as at least one positive test result. Results: The cumulative positive PCR test rate was 80% (104/130), with gradually increasing rates of positive findings upon the first, second and third TB-PCR tests with 52.3%, 68.5% and 75.4%, respectively. However, further testing did not increase the positive rate further. Conclusion: Repeated PCR testing at least three times for M. tuberculosis is helpful for diagnosis of pulmonary TB.

Pretreatment of normal responders in fresh in vitro fertilization cycles: A comparison of transdermal estradiol and oral contraceptive pills

  • Objective: The aim of this study was to investigate the impact of pretreatment with transdermal estradiol ($E_2$) compared to oral contraceptive pills (OCPs) on controlled ovarian stimulation (COS) response in normal responders undergoing fresh in vitro fertilization (IVF)-embryo transfer (ET) cycles. Methods: A retrospective cohort study was performed of normal responders undergoing fresh IVF-ET cycles who received pretreatment with transdermal $E_2$ versus OCPs prior to fresh IVF-ET. The total days of ovarian stimulation, total dosage of gonadotropins, total number of oocytes, and mature oocytes retrieved were noted. Pregnancy outcomes after ET were also recorded. Results: A total of 2,092 patients met the inclusion criteria: 1,057 and 1,035 patients in the transdermal $E_2$ and OCP groups, respectively. Patients in the OCP group had a longer duration of COS ($10.7{\pm}1.63days$, p< 0.01) than the $E_2$ group ($9.92{\pm}1.94days$). Patients in the OCP group also required higher cumulative doses of gonadotropins ($2,657.3{\pm}1,187.9IU$) than those in the $E_2$ group ($2,550.1{\pm}1,270.2IU$, p= 0.002). No statistically significant differences were found in the total and mature oocytes retrieved or in the rates of biochemical pregnancy, clinical pregnancy, spontaneous miscarriage, and live birth between the groups. Conclusion: Our findings suggest that compared to OCPs, pretreatment with transdermal $E_2$ is associated with a shorter duration of ovarian stimulation and lower gonadotropin utilization, without compromising the oocyte yield or pregnancy outcomes in normal-responder patients undergoing fresh IVF.

  • This study has been conducted to evaluate the effects of a single stage anaerobic co-biogasification of food waste and primary sewage sludge mixture (1 : 5 V/V%) according to mixing ratio (0% : CAP0, 0.5% : CAP0.5, 1%: CAP1.0) in red pepper powder, which was reported as the material anti-bacterial and anti-fungi, under mesophilic condition ($35^{\circ}C$). during 31 days. It showed that red pepper powder effected anaerobic bacteria, Especially, Hydrolytic bacteria and methanogenic bacteria was inhibited much more than Acetogenic bacteria with red pepper powder. at CAP0, Experimental cumulative methane yield (ECMY) and Experimental bio-energy production (EBEP) were 0.17 L $CH_4/g$ $VS_{fed}$ and 1,465 cal/g $VS_{fed}$ individually as the highest value during 31 days.

Determination of Phenolics, Sugars, Organic Acids and Antioxidants in the Grape Variety Kalecik Karası under Different Bud Loads and Irrigation Amounts

  • Irrigation applications (IA) and increased bud load (BL) are fundamental practices that are used to achieve optimum yields in grape production, while maintaining fruit quality parameters. Two different irrigation amounts (IA-I and IA-II) based on growth stages, in addition to a non-irrigated (rain-fed) control, along with two different BL applications [normal bud load based on traditional pruning practices (1BL) and double bud load (2BL)] were evaluated over a two-year experiment for their effects on the biochemical composition of the berries. Berries from the rain-fed vines had higher sugar levels, whereas no significant change was detected in organic acid levels. The increased bud load (2BL) treatment yielded less sugar in the berries compared to the 1BL control in both years. The total non-colored phenolic compounds (NPC) were greater in the irrigated (especially IA-I) and 2BL treatments than in those of the rain-fed and 1BL control. However, total anthocyanin was greater in the non-irrigated and 1BL control than in the irrigated and 2BL treatments. The antioxidant contents of the berries also varied according to the treatments and years. Our results implied that implementing a higher bud load along with the IA-I irrigation application, in which irrigation applications were 50 and 75% of the cumulative evaporation from the Class A pan during berry set to veraison and veraison to harvest growth stages, respectively, can help in obtaining greater yields in high-plateau viticulture. Thus, if more buds are left on the vines, along with sufficient irrigation and rainfall, yield may increase while maintaining or increasing the biochemical composition of the berries.

Seismic behavior of full-scale square concrete filled steel tubular columns under high and varied axial compressions

  • A building structural system of moment resisting frame (MRF) with concrete filled steel tubular (CFST) columns and wide flange H beams, is one of the most conveniently constructed structural systems. However, there were few studies on evaluating seismic performance of full-scale CFST columns under high axial compression. In addition, some existing famous design codes propose various limits of width-to-thickness ratio (B/t) for steel tubes of the ductile CFST composite members. This study was intended to investigate the seismic behavior of CFST columns under high axial load compression. Four full-scale square CFST column specimens with a B/t of 42 were carried out that were subjected to horizontal cyclic-reversal loads combined with constantly light, medium and high axial loads and with a linearly varied axial load, respectively. Test results revealed that shear strength and deformation capacity of the columns significantly decreased when the axial compression exceeded 0.35 times the nominal compression strength of a CFST column, P0. It was obvious that the higher the axial compression, the lower both the shear strength and deformation capacities were, and the earlier and faster the shear strength degradation occurred. It was found as well that higher axial compressions resulted in larger initial lateral stiffness and faster degradation of post-yield lateral stiffness. Meanwhile, the lower axial compressions led to better energy dissipation capacities with larger cumulative energy. Moreover, the study implied that under axial compressions greater than 0.35P0, the CFST column specimens with B/t limits recommended by AISC 360 (2016), ACI 318 (2014), AIJ (2008) and EC4 (2004) codes do not provide ultimate interstory drift ratio of more than 3% radian, and only the limit in ACI 318 (2014) code satisfies this requirement when axial compression does not exceed 0.35P0.

Upcycling the Spent Mushroom Substrate of the Grey Oyster Mushroom Pleurotus pulmonarius as a Source of Lignocellulolytic Enzymes for Palm Oil Mill Effluent Hydrolysis

  • Mushroom cultivation along with the palm oil industry in Malaysia have contributed to large volumes of accumulated lignocellulosic residues that cause serious environmental pollution when these agroresidues are burned. In this study, we illustrated the utilization of lignocellulolytic enzymes from the spent mushroom substrate of Pleurotus pulmonarius for the hydrolysis of palm oil mill effluent (POME). The hydrolysate was used for the production of biohydrogen gas and enzyme assays were carried out to determine the productivities/activities of lignin peroxidase, laccase, xylanase, endoglucanase and β-glucosidase in spent mushroom substrate. Further, the enzyme cocktails were concentrated for the hydrolysis of POME. Central composite design of response surface methodology was performed to examine the effects of enzyme loading, incubation time and pH on the reducing sugar yield. Productivities of the enzymes for xylanase, laccase, endoglucanase, lignin peroxidase and β-glucosidase were 2.3, 4.1, 14.6, 214.1, and 915.4 U g-1, respectively. A maximum of 3.75 g/lof reducing sugar was obtained under optimized conditions of 15 h incubation time with 10% enzyme loading (v/v) at a pH of 4.8, which was consistent with the predicted reducing sugar concentration (3.76 g/l). The biohydrogen cumulative volume (302.78 ml H2.L-1 POME) and 83.52% biohydrogen gas were recorded using batch fermentation which indicated that the enzymes of spent mushroom substrate can be utilized for hydrolysis of POME.

  • The characteristics of product materials obtained from thermal degradation of low-qualify pyrolytic oil were investigated in this study. The reactants were produced by pyrolysis of mixed plastic waste with film type in a commercial rotary kiln reaction system. The properties of reactants were measured by elemental analysis, calorimetry analysis and SIMDIST analyst. The result of degradation experiments with different reaction temperature programs was discussed through product yields, cumulative yields and production rates of oil products. The multi-step reaction temperature program resulted in higher yields of product oils and lower yields of residues than one-step reaction temperature program. The product characteristics such as production yield and the rate of oil products etc. were influenced by reaction temperature program in the continuous thermal degradation.

Growth and Yield of Direct Seeded Rice in Different Seeding Dates (벼 건답직파재배에서 파종기 이동에 따른 생육 및 수량)

  • In order to determine the critical seeding date in dry paddy field at southern plain area of Gyeongbug province, dry seeds of three different rice cultivars was sown April 25 to June 20 at 15 or 10-day intervals. The number of days from seeding to emergence decreased in late seeding date : from 32 days at April 25 to 7 days at June 10 sowing. The number of seedlings showed 121-154 plants per square meter were not much differed among the varieties and seeding dates. Culm length, panicle length and ripened grain ratio decreased in late seeding date, but number of panicles per square meter, 1,000 grain weight and spikelets number per panicle were similar among the seeding dates. The cumulative effective temperature(mean air temperature-15$^{\circ}C$) from emergence to heading was very constant having 655$\pm$9$^{\circ}C$ in Keomhobyeo, 771$\pm$9$^{\circ}C$ in Donghaebyeo and 801$\pm$3$^{\circ}C$ in Milyang 95, respectively. Grain yield of Dong Haebyeo seeded in June 20 and Milyang 95 seeded in June 10 were much lower compared with other seeding dates and increased green kernel in late seeding date of Donghaebyeo and Milyang 95.

Enhanced Production of Phenolic Compounds from Pumpkin Leaves by Subcritical Water Hydrolysis

  • Enhanced production of individual phenolic compounds by subcritical water hydrolysis (SWH) of pumpkin leaves was investigated at various temperatures ranging from 100 to $220^{\circ}C$ at 20 min and at various reaction times ranging from 10 to 50 min at $160^{\circ}C$. Caffeic acid, p-coumaric acid, ferulic acid, and gentisic acid were the major phenolic compounds in the hydrolysate of pumpkin leaves. All phenolic compounds except gentisic acid showed the highest yield at $160^{\circ}C$, but gentisic acid showed the highest yield at $180^{\circ}C$. The cumulative amount of individual phenolic compounds gradually increased by 48.1, 52.2, and $78.4{\mu}g/g$ dry matter at $100^{\circ}C$, $120^{\circ}C$, and $140^{\circ}C$, respectively, and then greatly increased by $1,477.1{\mu}g/g$ dry matter at $160^{\circ}C$. The yields of caffeic acid and ferulic acid showed peaks at 20 min, while those of cinnamic acid, p-coumaric acid, p-hydroxybenzoic acid, and procatechuic acid showed peaks at 30 min. Antioxidant activities such as 2,2-diphenyl-1-picrylhydrazyl and ferric reducing antioxidant power values gradually increased with hydrolysis temperature and ranged from 6.77 to 12.42 mg ascorbic acid equivalents/g dry matter and from 4.25 to 8.92 mmol $Fe^{2+}$/100 g dry matter, respectively. Color $L^*$ and $b^*$ values gradually decreased as hydrolysis temperature increased from $100^{\circ}C$ to $140^{\circ}C$. At high temperatures ($160^{\circ}C$ to $220^{\circ}C$), L* and b* values decreased suddenly. The $a^*$ value peaked at $160^{\circ}C$ and then decreased as temperature increased from $160^{\circ}C$ to $220^{\circ}C$. These results suggest that SWH of pumpkin leaves was strongly influenced by hydrolysis temperature and may enhanced the production of phenolic compounds and antioxidant activities.
