Cancer Perceptions Among Smokeless Tobacco Users: A Qualitative Study of US Firefighters

  • Background: Prevalence rates of smokeless tobacco (SLT) use among firefighters are remarkably high and substantially higher than similar occupational groups and the general U.S. population. The purpose of this study was to explore the perspectives of fire service personnel regarding cancer and its associations with tobacco and SLT use. Methods: This descriptive study used a qualitative approach. Key informant interviews were conducted in 39 career firefighters and fire service administration from across the U.S. Discussion were recorded, transcribed verbatim and transferred to NVivo software for narrative analysis. Topics explored included cancer perceptions, attitudes and beliefs, and cultural factors related to SLT use behaviors. Results: Major themes that emerged among fire service personnel included concerns about cancer and its risk factors including firefighting tasks, such as fire overhaul operations, and from their lifestyle behaviors, such as alcohol and tobacco use. Firefighters also suggested a number of reasons for their increased SLT use, such as fire department tobacco-free policy and fire service culture. Conclusion: The current study provides a rich foundation for future research, prevention, and intervention efforts for the fire service and research communities regarding tobacco and SLT use and cancer risk. Additional research on firefighters' cancer beliefs deserves future research in order to improve messaging about the risks of cancer due to firefighting.

Rearranging Everyday Lives among People with Type 2 Diabetes in Korea

  • Purpose: The purpose of the study was to understand what are the experiences and management of type 2 diabetes in everyday lives among Korean people. Methods: A grounded theory method was utilized to explore how people with type 2 diabetes to experience and manage their disease under the Korean socio-cultural context. The data were collected via narrative in-depth interviews with 21 people with type 2 diabetes during 2010-2011 and all interviews were transcribed for verbatim analysis. Results: The core category was 'Rearranging everyday lives by accepting diabetes as lifelong annoying companion.' Four stages were identified: ignoring; struggling compromising and conciliating. Each stage illustrates major problems and/or strategies that the participants face in dealing with diabetes. The process illustrates the transference from their ordinary life, in which diabetes or health was ignored, to the health-oriented life, within which diabetes is integrated into their lives. The most difficult barriers they faced in everyday lives include social stigma of diabetes and collectivistic culture in Korea. Within the culture, the group goals are concerned over individual ones, making it harder for the participants to take care of their own health. Conclusion: The findings of the study imply that health care professionals may consider the influence of social stigma in caring diabetic patients. Also, the intervention study is warranted to educate Korean people with diabetes to get aware of the sociocultural context and stigma as well as personal difficulties in self-caring diabetes.

북한 이탈 어머니들의 자녀양육행동과 양육효능감, 적응스트레스 및 외상 후 스트레스 증상 (Relationships between Parenting Behavior, Parenting Efficacy, Adaptation Stress and Post Traumatic Stress Disorder among Mothers who Defected from North Korean)

  • Purpose: The purpose of this study was to investigate the relationships between parenting behavior, parenting efficacy, adaptation stress and PTSD (Post Traumatic Stress Disorder) among mothers who have defected from North Korean. Methods: A descriptive study was conducted to illustrate the parenting behavior, parenting efficacy, adaptation stress and PTSD and report the relationships among them. Data were collected between August and November of 2009 in Hanawon and 105 mothers who met eligibility criteria participated in the study. Results: Ignorant/controlling parenting behavior of parenting showed the highest score in our study. The level of parenting efficacy was $2.72{\pm}0.36$ and adaptation stress level was $3.35{\pm}0.47$ showing perceived hostility the highest and homesickness the lowest. Participants with moderate to severe PTSD accounted for 79.5% of the mothers, and 75% of them reported symptoms for 3 months or more. Authoritative parenting behavior showed negative relationships with parenting efficacy, cultural crisis and fear, adaptation stress and PTSD. Conclusion: Mothers who have defected from showed negative parenting behaviors and high adaptation stress level and PTSD while parenting efficacy was high. These findings indicate that parenting intervention programs which can guide these mothers towards positive parenting behaviors need to be developed.

Prevalence and Determining Factors Related to Depression Among Adult Women in Korea

  • Purpose. Korean women are likely to experience symptoms of depression, possibly due to socially fixed limitations on the roles that Korean women are expected to perform. Also if a Korean woman experinces negative relationship problem or stress in her family, she would feel responsible, which will worsen her depression. Nonetheless, much of the research on depression among Korean women has focused on menopausal women. This study aims to understand the depression of Korean women to provide fundamental data to develop nursing intervention method for promoting women's health. Methods. The present investigation assessed the prevalence and correlates of depression in a large sample of Korean women, aged 18 or older, from the general population. With a probability sample of 3312 women drawn from two areas in Korea, a survey, which contains the Center for Epidemiological Studies Depression Scale (CES-D) and background, was completed. Results. According to CES-D classification criteria, $36.5\%$ of the women in the sample displayed either no depression or mild depression, $55.6\%$ exhibited moderate depression, and $7.8\%$ manifested severe depression. Significant bivariate relationships were observed between depression and each measured background variable except alcohol use. Logistic regression analysis indicated that the strongest combination of predictors of depression included income, menopausal, and marital status. Conclusion. The data support the premise that Korean women disproportionately experience elevated levels of depression. Consistent with the theory, depression may be related to social pressures to conform to the traditional roles. The study suggests the need for further research, primary prevention activities, and increased access to treatment.

출토복식 보수방법에 관한 고찰 - 경주노씨 수함(1516~1573)공(公) 일가(一家) 출토복식 사례를 중심으로 - (A Study on the Methodology of Excavated-Costumes - With a Focus on the Cases of Excavated Costumes from the Grave of Nor Su-ham(1516~1573)'s Family -)

  • This study aims to suggest the methodology for conservation of excavated costumes especially repair and restoration process. Early excavated costumes had not attracted attention from the field of academia and the public. So not many scholars, organization and society participated in the excavations but as its value has gotten more recognition, there has been a reversal in trend. And so many organizations have taken part in it. Excavating includes conservation processes such as washing, repair and restoring, and then it is published by reports or books after the process is complete. However the method of conservation has varied depending on the institution. In particular, repair and restore methods do not include anything specific details, and often times, only has descriptions of the before and after state, and so a more unified method needs to be suggested and shared. This study defined 'Conservation', 'Repair' and 'Restoration' and then applied it to the short history of Korean excavated-costumes. Then it suggested ways to repair and restore excavated-costumes in terms of construction, textile, and damage by the unpublished cases of excavated-costumes of Noh Su-ham(1516~1573)'s Family. It was also referred to repair and restoration process such as shaping correction, dyeing, supporting, sewing and after treatment by texts and pictures. Other contents such as a list of relics and its details, will be published through a report or a book. The whole process of conservation was in progress based on the ethical guidelines of conservation, 'Minimum intervention' and 'Reversibility'.

여성결혼이민자의 우울에 영향을 미치는 스트레스 요인 (Effects of Multiple Stress Factors on Depression among Female Marriage Immigrants in Korea)

  • Purpose: The purpose of this study was to describe levels of multiple stress factors and depression, and to examine the effects of the stress factors on depression among female marriage immigrants in Korea. Methods: Participants were 322 female marriage immigrants currently residing in Korea, who migrated from China, Vietnam, the Philippines, and other Asian countries. Stress of female marriage immigrants was measured on the SMFMI (Stress Measure of Female Marriage Immigrants in Korea), consisting of 21 items in four factors (cultural, household economic, emotional, and parenting and discrimination stress). CES-D was used to assess depression among marriage immigrants. Descriptive statistics, t-test, ANOVA with Scheffe's post hoc tests, and multiple regressions were performed for data analyses. Results: The average score for stress was 1.34 (SD=.98, theoretical range: 0-4) and the average score for depression was 17.07 (SD=10.09) in these female marriage immigrants. Adjusting for household income, employment status, duration since immigration, and Korean language proficiency, household economic stress (p<.001) was identified as the strongest predictor in explaining depression of female marriage immigrants (Adjusted $R^2=.331$). Conclusion: Health care professionals should prioritize intervention strategies to alleviate household economic stress for mental health promotion in female marriage immigrants in Korea.

문화예술교육이 아동의 행동과 사회기술에 미치는 영향 : 예비연구 (Effects of Art & Culture Education on Behavior and Social Skill in Children : A Preliminary Study)

  • Objectives : Art and culture education might play a good role in children's emotions ; however, few studies to verify this have been conducted. The objective of this study was to examine the effect of an art and culture education program on behavior and social skills in children aged 7-10 years. Methods : Fifteen children (mean age $7.31{\pm}0.95$ years) were recruited from a community center and participated in an art and culture education program. The participants received a once a week program for 17 sessions, and we compared scores on the Child Behavior Checklist (CBCL) and Social Skills Improvement System-Rating Scales (SSIS-RS) before and after intervention. Results : The participants' subscale scores of CBCL-aggressive behavior, social immaturity, internalizing behavior, externalizing behavior, total behavior problems and social skill scores on the SSIS-RS showed improvement (p<.05) after 15-weeks of the art and culture education program. Conclusion : These results suggest a positive effect of an art and culture education program on the reduction of externalizing and internalizing behaviors and improvement of social skills in children. Further prospective, controlled studies in large samples are needed in order to confirm our findings.

여대생의 BMI에 따른 식이섭취와 혈중지질 농도 및 항산화능 (Body Mass Index, Dietary Intake, Serum Lipids and Antioxidant Status of Young Females)

  • Many epidemiological and observational studies show that distorted body image of normal body weight is widespread among Korean young females. This study was performed to evaluate the prevalence of overweight and underweight of young females and, to compare nutrient intakes, body composition, serum indices (lipids, MDA: Malondialdehyde, TAS : Total Antioxidant Status) with different BMI groups. The subjects were 75 university students in Seoul. We divided the subjects into 3 groups according to their BMI by IOTF guideline (UW: underweight group, BMI < 18.5, NW: normal body weight group; 18.5 $\leq$ BMI < 23.0, OW: overweight group; BMI $\geq$ 23.0). Data on dietary intakes, body compositions and serum indices were obtained in 3 groups. Differences on all of the above variables were assessed by body weight groups. Using IOTF guidelines, the prevalence of overweight and underweight in young females were 23%, 61%, 16% respectively. $\beta$-carotene and vitamin A intake of UW were significantly higher than that of NW (p < 0.05). Fiber intakes of NW was significantly higher than that of W (p < 0.05). But intakes of energy and the other nutrients were not significantly different among BMI groups. Calcium and folate intakes were 75%, 61% of KDRIs. Serum TGs were significantly higher in OW than that of NW, UW (p < 0.05), but level of MDA and TAS were not significantly different. The association of overweight and low intake of $\beta$-carotene and vitamin A may be one of many factors predisposing obese females to a high risk of oxidative stress later in life. This requires urgent nutritional intervention programs involving enough intake of fruit and vegetables, with modification of inappropriate dietary habits.

기혼여성의 유방암과 사회연결망 특성에 따른 유방촬영술 수검행위 (Mammography Screening according to Breast Cancer Disease and Social Network Characteristics of Married Korean Women)

  • Purpose: This study was done to examine differences in mammography screening according to breast cancer and social network characteristic. Methods: Data were collected from 187 married women 35 years and older who were using public health centers, health promotion centers, cultural centers, obstetrics and gynecology hospitals or other relevant community sites. Data were collected between October 24 and December 4, 2008. Data were analyzed using the SPSS/WIN 15.0 program. Results: The participation rate for mammography screening was 35.3%. The following general and breast cancer characteristics showed statistically significant differences: religion, family incomes, regular medical-care, general health examinations during past 2 years, and history of breast disease. The following social network characteristics showed statistically significant differences: social norms and subjective norms. Using logistic regression analysis, regular medical-care, breast cancer risk appraisal, social norm, and subjective norms were highly predictive of subsequent mammography. Conclusion: The results of this study indicate that it is important to develop and provide tailored intervention programs through integrated socially mediated programs. By consciously including social network and support systems, breast cancer detection efforts would not end as a one-time event, but naturally build on network structure of adults women, thus facilitating regular mammography screening.

I Do Not Even Say "It" - a Mixed Methods Study on Breast Cancer Awareness of Omani Women

  • Background: The incidence of breast cancer is rising in Oman, and the disease is diagnosed at late stages, when treatment success is limited. Omani women might benefit from better awareness, so that breast cancer can be detected early and treated. This study was conducted to assess Omani women's levels of breast cancer awareness and early detection practice, and explore factors which might influence these levels. Materials and Methods: A mixed methods study was conducted in 2014, including a quantitative survey of 1,372 and a qualitative assessment of 19 Omani women, aged ${\geq}20years$ from five Omani governorates using convenient sampling. Demographic information and scores for awareness levels were used in a multivariate regression model to investigate factors associated with awareness. Thematic analysis and interpretive description were used to analyse the qualitative data. Results: The overall means for early detection and general awareness scores were 0.58 (SD 0.24) and 0.46 (SD 0.21), respectively. General awareness was significantly associated with age, education, income and familiarity with cancer patients (p<0.05), while early detection was significantly associated with age, marital status and education. A majority of women (59.5%) agreed with a belief in 'evil eye' or envy as a risk factor for breast cancer. Women discussed various factors which may empower or inhibit awareness, including the cultural-religion-fatalistic system, personal-familial-environmental system, and healthcare-political-social system. Conclusions: The overall low scores for awareness and early detection, and the survey of local beliefs highlight a severe necessity for a contextually-tailored breast cancer awareness intervention programme in Oman.
