Many dentists have been taken an interest in restoration of severly damaged teeth after endodontic treatment and it is a true that there are lots of studies about it. In these days, although we have used Para-Post, pins, threaded steel post, cast gold post and core, and so on, as a method of restoration frequently, it has been in controversy with the effects of them on the teeth and surrounding periodontal tissue. In this study, we assume that the crown of the upper 1st premolar was severly damaged, and after the root canal therapy, two most common types of restoration were carried out ; 1) coronal-radicular amalgam restoration, 2) after setting up the Para-Post, restore with amalgam core and gold crown. After restoration, in order to present the concentration of stress at internal portion of the tooth and the surrounding periodontal tissue, we doveloped a 2-dimensional finite element model of labiopalatal section, then loaded forces from 2 long perpendicular to the lingual incline of buccal ridge an the middle point, parallel to the long direction axis of tooth at the fossa-were applied. The analyzed results were as follows : 1. Stress of the normal first premolar was concentrated on the most weakest anatomical structure, that is, cervical area, and no stress on the bifurcated area of the canal. 2. Crown restoration after root canal therapy causes large stress concentration on the bifurcated area of the canal. This stress concentration has larger value in case of lateral movement of mandible, and there are decrease in the stress concentration compared with natural tooth. 3. Coronal-radicular amalgam restoration method transports more stress to the tooth structure than restoration using Para-Post. 4. There are more stress concentration around Para-Post in the case of lateral movement, and we have more favo rable result when restored with Para-Post. 5. Generally, stress in the lateral movement is larger than stress in the perpendicular load.
Purpose: The emergence profile concept of an implant restoration is one of the most important factors for the esthetics and health of peri-implant soft tissue. This paper reports on two cases of gingival recontouring by the fabrication of a provisional implant restoration to produce an optimal emergence profile of a definitive implant restoration. Methods: After the second surgery, a preliminary impression was taken to make a soft tissue working cast. A provisional crown was fabricated on the model. The soft tissue around the implant fixture on the model was trimmed with a laboratory scalpel to produce the scalloped gingival form. Light curing composite resin was added to fill the space between the provisional crown base and trimmed gingiva. After 4 to 6 weeks, the final impression was taken to make a definitive implant restoration, where the soft tissue and tooth form were in harmony with the adjacent tooth. Results: At the first insertion of the provisional restoration, gum bleaching revealed gingival pressure. Four to six weeks after placing the provisional restoration, the gum reformed with harmony between the peri-implant gingiva and adjacent dentition. Conclusions: Gingival recontouring with a provisional implant restoration is a non-surgical and non-procedure-sensitive method. The implant restoration with the optimal emergence profile is expected to provide superior esthetic and functional results.
In Class II amalgam restoration in deciduous molar, failure rate and incidence of recurrent caries are high as children become older. In order to preserve deciduous molars till the physiologic exfoliation time, stainless steel crown is a choice of the treatment. As a result of a careless treatment, such as overhanging margin, poor marginal adaptation, poor proximal contour and inadequate mesiodistal width give rise to interfering eruption of the adjacent teeth, recurrent caries and chronic gingival irritation and insufficient arch length respectively. In this study, 252 s.s. crowned teeth extracted due to physiologic exfoliation or periapical lesion. The purpose of this study is to analyze the marginal adaptation of stainless steel crown to the deciduous molar in order to obtain better clinical result. The results were as follows : 1. Between the length of s.s. crown and the marginal gap of crown, positive correlations were shown. 2. Largest amount of marginal gap was shown at buccal side in upper deciduous molars and lower first deciduous molar, lingual side in lower second deciduous molar. But no significant diffrence were found statistically compared to second most largest one. 3. Incidence of exposed restoration and recurrent caries were higher in proximal surface than buccal/lingual surface. And extension of restoration below the margin of s.s. crown gives rise to higher rate of recurrent caries. 4. Defect of contour was found in 34%, frequently found in lower 1st deciduous molar and upper 1st deciduous molar. 5. Marginal polishing defects were found in 23%. 6. Ledge was formed in 10% especially in lower 1st deciduous molar and lower 2nd deciduous molar. 7. 16% of the teeth had wear facet due to traumatic occlusion, 7% of them had occlusal perforation.
The objective of this case report is to introduce a simple technique for simultaneously taking a closed-mouth impression and functionally generated path (FGP) for a full coverage crown restoration. A monolithic zirconia crown was the restoration of choice. An alginate impression of the abutment tooth was taken to fabricate a custom-made closed-mouth impression tray covering the abutment tooth and the adjacent teeth. The tray had an FGP table and an abutment tray in cameo and intaglio surfaces, respectively. The impression was taken with silicone impression material after adjusting the abutment tray and inscribing the FGP using self-curing acrylic resins. Plaster casts were made from the impression, and a zirconia crown was fabricated. The crown was cemented to the abutment tooth with minimal adjustments. This simple technique resulted in a well-fitting crown that accounted for mandibular movements. Using the custom closed-mouth impression tray incorporating an FGP table simultaneously aids in fabricating an accurately fitting restoration that incorporates harmonious mandibular movements using a single impression capture.
Intra-alveolar transverse crown & root fx. provokes many problems in treatment. Conventionally, extraction of the injured tooth and its prosthodontic restoration has been the treatment of choice. Though orthodontic extrusion could be an alternative treatment, there would be a situation it's inadequate to apply. Loss of natural tooth would be a psychological damage to the patient, of course. This report describes a replantation method of tooth in case of intra-alveolar transverse crown & root fracture. The fractured tooth was extracted, rotated, then replanted. Fixation and esthetic restoration was done. And then endodontic treatment was followed. Continuing follow-up of its function and endodontic status is required.
This case report presents two maxillary anterior cases for clinical crown lengthening by forced eruption. In the first case, clinical crown of maxillary right lateral incisor was almost lost by fracture. Forced eruption using intracoronal splint and elastic thread accomplished vertical root movement successfully. Then, post & core was inserted and final restoration was harmonious with adjacent teeth. In the second case, the crown portion of maxillary right central incisor was almost mutilated by secondary caries. Forced eruption using removable Hawley appliance and elastic accomplished vertical root movement successfully. Then, post & core was inserted and final restoration was placed. In conclusion, clinical crown lengthening by vertical root movement can be accomplished by a simple appliance without any sacrifice of periodontal support in selected patients. A clinical crown so created can be restored to adequate function and arch integrity without compromising adjacent teeth. Therefore, forced eruption is preferred in the anterior region of the dentition where esthetics is of major concern.
상악 무치악 환자의 구강 회복을 계획할 때 치료 방법으로 임플란트를 이용한 고정성 보철 수복과 총의치 수복 또는 임플란트를 이용한 오버덴쳐 등을 고려할 수 있다. 총의치 수복의 경우 추가적인 수술이 필요 없고 상대적으로 경제적이며 임플란트 지지 고정성 보철 수복의 경우 일반적으로 총의치 수복에 비해 기능적인 부분이 우수하지만 임상적으로 임플란트 식립이 어려운 경우가 존재한다. 최근에는 환자의 요구 및 잔존 치조골의 상태 등을 고려하여 부분적으로 임플란트를 식립한 후 임플란트 지지 서베이드 크라운을 이용한 가철성 국소의치 수복도 시도되고 있다. 본 증례는 상악 전방부의 임플란트 지지 서베이드 크라운을 이용한 가철성 국소의치 수복을 시행한 증례로 심미 및 기능적으로 만족할 만한 결과를 보여 보고하는 바이다.
Restoration of severly damaged teeth after endodontic treatment had been an interest to many dentists, and it is a fact that there have been lots of studies about it. In this study, we assume that the crown of the upper incisor have severly damaged, so, after the root canal therapy, 4 types of restoration had been carried out; 1) coronal-radicular amalgam restoration, 2) after setting up the Para-Post, restore with composite resin core only, 3) after setting up the Para-Post; restore with amalgam core, then cover with the PPM crown 4) after setting up the Para-Post, restore with composite core, then cover with the PPM crown. After restoration, in order to observe the concentration of stress at internal portion of the teeth and the sourrounding periodontal tissue, developing a 2-dimensional finite element model of labiopalatal section, then loading forces from 3 direction - direction of 45 degrees from lingual side near the incisal edge, horizontal direction from labial height of contour, vertical direction at the incisal edge-were applied. The analyzed results were as follows: 1. Stress of the normal central incisor was concentrated on the dentin aroundpulp chamber, labiocervical portion of a tooth and root apex, but with the alveolar bone, in the case of load from the direction of 45 degrees from lingual side near the incisal edge showed remarkable concentration of stress: 2. Coronal-radicular amalgam technique -showed less concentration of stress on the root and surrounding periodontal tissue than the restoration with the Para-Post. 3. The von Mises equivalent stress on the Para-Post showed maximum value at root-core junction rather than both ends and model with PPM restoration with amalgam core showed the least concentration of stress. Only the force from horizontal direction showed large shear stress on internal portion of the root, root apex and alveolar bone. 4. PPM crown with composite core rarely showed the concentration of stress on root and periodontal tissue. 5. As for alveolar bone, remarkable shear stress was concentrated on labial and palatal side by horizontal load.
이 연구의 목적은 파절강도와 반투명도 측정을 통해 Computer Aided Design/Computer Aided Manufacturing (CAD/CAM)으로 제작한 유전치 지르코니아 전장관의 임상적 적용 가능성을 확인하는 것이다. 기성 유전치 모형 중 #61을 스캔하여 얻은 3차원 형상 데이터로 CAD 디자인 프로그램을 이용하여 전장관을 디자인하고, 지르코니아 블록으로 CAM 밀링 기계를 이용하여 전장관을 제작하였다. 지르코니아 전장관은 두께에 따라 0.3, 0.5, 그리고 0.7 mm의 3군으로 나누어 1.0 mm 두께의 레진 전장관과 파절강도를 비교하였다. 파절강도는 만능시험기를 이용하여 30° 각도에서 1 mm/min 속도로 절단면에 하중을 가하여 측정하였다. 반투명도는 가로와 세로가 7.0 mm, 그리고 두께가 각각 0.3, 0.5, 0.7 mm 인 사각형 지르코니아 시편을 제작하여 분광측색장치로 측정하였다. 파절강도 측정 결과 지르코니아 전장관은 두께가 증가할수록 파절강도가 높아졌으며 모든 두께의 지르코니아 군에서 대조군인 레진 전장관보다 파절강도가 높았다(p < 0.05). 반투명도는 0.3 mm 군에서 가장 높았으며 0.5, 0.7 mm로 두께가 증가할수록 감소하였다(p < 0.05). CAD/CAM으로 제작한 지르코니아 전장관은 개별 치아에 맞추어 얇게 디자인 및 제작 가능하며, 얇은 전장관을 이용한 수복은 임상적으로 치아 삭제량과 치수노출 위험성 감소 뿐만 아니라, 더 심미적인 수복을 가능하게 해줄 것이다.
