A Study of Factors Related to Job Satisfaction Affecting Service Year: A Dental Hygienist in Seoul (근속연수에 영향을 미치는 직무만족요인에 관한 연구: 서울지역 치과위생사를 중심으로)

  • Despite the high demand for dental care professionals, high turnover of dental hygienists have been reported, especially among workers in the dental clinics. This study aimed to examine job satisfaction factors affecting dental hygienist's service year in Seoul using cross-sectional data. The questionnaire survey was conducted from August 26, 2013 to September 13, 2013. Data were collected from 296 employees of dental clinics and hospitals located in Seoul. Logistic regression analysis was applied for parameter estimates, using PASW Statistics 18.0 and R software version 3.0.3. The Cronbach's ${\alpha}$ for the total job satisfaction factors was 0.922. In descriptive statistics, the group (that less than four years of working and over four years of working) had a statistically significant difference in age, religion, experience of turnover and autonomous factor among job satisfaction factors. In multiple logistic regression model, autonomy in job satisfaction was an important factor to predict the length of service in dental hygienist (odds ratio, 2.65; 95% confidence interval, 1.06~6.60). Autonomous factor was a significant predictor of length of service for dental hygienist. This study encourages future investigations of the role of job satisfaction of service year using better analytical frameworks.

Using the Balanced Scorecard and Organizational Performance (조직의 경영성과 측정과 평가를 위한 균형성과표(BSC) 모형의 도입 및 활용 효과에 관한 연구)

  • The main purpose of this study is to assess the effect of using Balanced Scorecard, and relationship among learning and growth performance, internal business performance, customer performance, financial performance, and business performance. To achieve this study, theoretical and empirical studies related to Balanced Scorecard were carried out simultaneously. A field survey was undertaken through questionnaire sampling a population of construction firms. The established hypotheses related to Balanced Scorecard and organizational performance were verified by the paired-t test analysis using SPSS. The result of this research are as follows : First, BSC firm's learning and growth performance are higher than those of before BSC adopting firms significantly. Second, BSC firm's internal business performance level is higher than that of before BSC firm's significantly. Third, BSC firm's customer performance level is higher than that of before BSC firm's significantly. Fourth, BSC firm's financial performance level is higher than that of before BSC firm's significantly. Fifth, BSC firm's Business performance level is higher than that of before BSC firm's significantly. This study contributes to the BSC research by being the study focus on the BSC at the general indicators and provides evidence that may help understanding the possible relationship between BSC adoption and improvement of organizational performance. There are some limitations, however, of this study. The result are based on a cross sectional sample of construction firms, which may not be reflective of the entire population.

Does Process Quality of Inpatient Care Serve as a Guide to Reduce Potentially Preventable Readmission (PPR)? (의료서비스의 과정적 질과 잠재적으로 예방 가능한 재입원율과의 관계)

  • Objective: The objective of this study is to examine the association between process quality of inpatient care and risk-adjusted, thirty-day potentially preventable hospital readmission (PPR) rates. Data Sources/Study Setting: This was an observational cross-sectional study of nonfederal acute-care hospitals located in two states California and Florida, discharging Medicare patients with a principal discharge diagnosis of heart failure, acute myocardial infarction, or pneumonia January through December 31, 2007. Data were obtained from the Healthcare Cost and Utilization Project State Inpatient Database of the Agency for Healthcare Research and Quality, Centers for Medicare and Medicaid Services Hospital Compare database, and the American Hospital Association Annual Survey of Hospitals. Study Design: The dependent variable of this study is condition-specific, risk-adjusted, thirty-day potentially preventable hospital readmission (PPR). 3M's PPR software was utilized to determine whether a readmission was potentially preventable. The independent variable of this study is hospital performance for process quality of inpatient care, measured by hospital adherence to recommended processes of care. We used multivariate hierarchical logistic models, clustered by hospitals, to examine the relationship between condition-specific, risk-adjusted, thirty-day PPR rates and process quality of inpatient care, after taking clinical and socio-demographic characteristics of patients and structural and operational characteristics of hospitals into account. Findings: Better performance on the process quality metrics was associated with better patient outcome (i.e., low thirty-day PPR rates) in pneumonia, but not generally in two cardiovascular conditions (i.e., heart failure and acute myocardial infarction). Practical Implication: Adherence to the process quality metrics currently in use by CMS is associated with risk-adjusted, thirty-day PPR rates for patients with pneumonia, but not with cardiovascular conditions. More evidence-based process quality metrics closely linked to 30-day PPR rates, particularly for cardiovascular conditions, need to be developed to serve as a guideline to reduce potentially preventable readmissions.

A study on the nursing needs of postpartum women during the postpartum period (산욕기 산모의 간호요구 조사)

  • This study was carried out to understand the nursing needs of postpartum women during the postpartum period. The goal of study was to obtain data needed to develop maternal education programs and to improve the nursing quality for postpartum women. A cross-sectional survey design was employed in this study. The subjects were 101 postpartum women who had delivery at 2 general hospitals in Chung-Buk. Korea. Data were collected from Sep. 29 to Nov. 28. 1997 by means of a structured questionnaire. The instrument used for this study was the one which was consisted of revised Maternal Concerns Questionnaire (MCQ) developed by Bull (1979). The data were analyzed through the SPSS/PC+ program by use of frequency, t-test and ANOVA. The results of the study were as follows; 1) The mean score of concerns about 'self' was 2.98 for primipara and 2.80 for multipara but was not statistically significant difference. Among the items of concerns 'for self', 'Being good mothers' was the highest (primipara 3.93. multipara 3.75). 2) The mean score of concerns about 'baby' was 3.64 for primipara and 3.53 for multipara but was not statistically significant difference. Among the items of concerns 'for baby', 'Normal growth and development' was the highest (primipara 3.91. multipara 3.90). 3) The mean score of concerns about 'husband' was 3.33 for primipara and 3.06 for multipara and was statistically significant difference (t=2.11. p=0.03). Among the items of concerns 'for husband', 'Husband being a good father' was the highest (primipara 3.87, multipara 3.77). 4) The mean score of concerns about 'family' was 2.65 for primipara and 2.80 for multipara but was not statistically significant. Among the items of concerns 'for family', 'Change in the family's lifestyle' was the highest(primipara 3.01, multipara 2.93, t=-1.99, p=0.04). 5) The mean score of concerns about 'community' was 2.48 for primipara and 2.42 for multipara but was not statistically significant. Among the items of concerns 'for community'. 'Getting to health care facilities' was the highest (primipara 2.48, multipara 2.42). 6) On considering the relationship between the postpartum women's nursing needs and their general characteristics, 'regularly antenatal care' was statistically significant (t=2.29. p=0.02). In conclusion, recognition of maternal concerns can be used by nurses to identify nursing diagnoses and to develop care plans that reflect the patient's priorities.

A Study on the Relationship between Income Instability and the Degree of Preparation for Old Life and Satisfaction with Current Life (소득위험과 노후준비정도 및 현재생활의 만족도 간의 관련성)

  • The need for preparation for old life has been further increased due to the increase of the elderly population and changes in economic conditions today. The purpose of this study is to analyze the relationship between income instability and the degree of preparation for old life and satisfaction of current life. The key variables were extracted through prior study review. And the data collected through the survey were statistically analyzed with a structural equation model. The analysis found that the indirect effect of the current income risk on the satisfaction of current life through the preparation for old life had a significant negative effect under the statistically significant level of 0.05. However, it was found that the direct effects of current income risk on the satisfaction of current life were not statistically significant. An additional analysis was conducted by dividing the age, the number of dependents by two groups respectively. To summarize the results, preparation for old life played a significant role as a prerequisite for improving the satisfaction of current life. And the variability(risk) of current income played an important role in preparing for old life. At this time, the degree of relevance between the factors(potential variables) differed somewhat between the two groups. The results of this analysis will be meaningful in providing basic source of data to prepare for a satisfactory life in each individual's current situation. This study, meanwhile, has limitations that have only been done with cross-sectional analysis and would like to analyze time-series changes in the future.

The Relationship among Percieved Social Support, Hope and Quality of Life of Cancer Patients (암환자가 지각한 사회적지지, 희망과 삶의 질과의 관계)

  • The purpose of this study was to investigate the relationship among percieved social support, hope and quality of life of the cancer patients and to gain the baseline data for development of nursing intervention program for promoting quality of life in cancer patients. The design of this study was a cross sectional correlational survey. The subjects were 220 out and in-cancer patients in 5 general hospitals in Pusan. The data were collected from July 2 to August 1, 2001. The instruments were the Percieved social support scale(16 items, 5 point scale) had developed by Tae(1986), Hope scale(12 items, 4point scale) developed by Nowotny(1989) and Quality of life scale(31 items, 10 point scale) developed by Tae et al.(2000). The data was analyzed by the SPSS/PC+ program using frequency & percentage, item mean & standard deviation, t-test, ANOVA & Scheffe test, Pearson's correlation coefficient. The results of this study was as follows: 1) The item mean score of quality of life was $6.05{\pm}1.16$ (range 0-10). The highest score of subarea of the quality of life was the spiritual wellbeing area ($7.09{\pm}1.63$) and the lowest score was social wellbeing area ($5.53{\pm}1.65$). The mean score of perceived social support was $52.65{\pm}10.32$ (최저 1, 최고 80). The mean score of family support was $32.71{\pm}6.66$ (range 1-40) and the mean score of medical team support was $19.93{\pm}5.95$ (range 1-40). The mean score of Hope was $37.02{\pm}5.64$ (range 1-48). 2) There were statistically significant difference in the score of quality of life according to the life effect of religion(F=3.97, p=0.00), treatment method(F=2.94, p=0.01), area of diagnosis(F= 3.48, p=0.01), stage of disease (F=13.74, p=0.00). 3) There was significant correlation between perceived social support(r=0.44, p=0.000 ; family support ; r=0.334, p=0.000, medical support; r=0.395, p=0.000), hope(r=0.563, p=0.000) and quality of life. In conclusion, there was a significant relationship among perceived social support, hope and quality of life. Therefore perceived social support, hope intervention programs should be developed to improve the quality of life in cancer patients.

A Study on Continued Intention of Social Network Services by Applying Privacy Calculus Model: Facebook and KakaoTalk Cases (프라이버시 계산 모형을 적용한 SNS 지속 사용 의도에 대한 연구: 페이스북과 카카오톡 사례 중심으로)

  • Given the proliferation of social network services, it has become important to understand its user's continued use behavior. As users' privacy concerns in SNS have been identified as a critical barrier against forming this continued intention, many studies have focused on inducing continued intention by mitigating privacy concerns. However, this paper suggests to approach users' continued intention not only from the perspective of mitigating privacy concerns but also from the perspective of increasing potential benefits. Under the theoretical framework of privacy calculus model, we conducted cross sectional survey on 150 Facebook and 150 KakaoTalk users. The result suggests that trust mediates the relationships between privacy concerns and continued intention and between network externality and continued intention, and the influence of support for network formation on continued intention. The effect of network externality on continued intention, however, is not significant among Facebook users while it is significant among KakaoTalk users.

Association of Blood Pressure Levels with Carotid Intima-Media Thickness and Plaques (혈압 수준과 경동맥 내중막 두께 및 동맥경화반의 관련성)

  • Objectives : The aim of this study was to investigate the association of blood pressure levels with the common carotid artery intima-media thickness (CCA-IMT) and carotid plaques. Methods : Data were obtained from 2,635 subjects, aged 50 years and over, who participated in the Community Health Survey (a population-based, cross-sectional study) in Dong-gu, Gwangju city between 2007 and 2008. Participants were categorized into three groups according to blood pressure levels; normotensives (<120/80 mmHg), prehypertensives (120-139/80-89 mmHg), and hypertensives ($\geq$140/90 mmHg). Prehypertensives were further categorized as low prehypertensives (120-129/80-84 mmHg) and high prehypertensives (130-139/85-89 mmHg). Carotid intima-media thickness and plaques were evaluated with a high-resolution B-mode ultrasound. Statistical analyses were performed using chi-square test, ANOVA, and multiple logistic regression. Results : Prehypertensives had significantly greater maximal CCA-IMT values than normotensives, with a multivariate adjusted odds ratio of 1.78 (95% CI=1.36-2.32) for abnormal CCA-IMT (maximal CCA-IMT$\geq$1.0 mm), and 1.45 (95% CI=1.19-1.77) for carotid plaques. The multivariate adjusted odds ratio of low prehypertensives was 1.64 (95% CI=1.21-2.21) for abnormal CCA-IMT, and 1.30 (95% CI=1.04-1.63) for carotid plaques compared with normotensives. Subject with hypertension had higher frequency of abnormal CCA-IMT (odds ratio, 2.18; 95% CI=1.49-3.18), and carotid plaques (odds ratio, 1.98; 95% CI=1.46-2.67) compared with normotensives after adjustment for other cardiovascular risk factors. Conclusions : Our results indicate that there is a significant increase in the prevalence of carotid atherosclerosis in subjects with prehypertension (even in low prehypertensives) compared with normotensive subjects. Further studies are required to confirm the benefits and role of carotid ultrasonography in persons with prehypertension.

Health of Apartment Guards and Associated Factors (아파트 경비원의 건강상태와 관련요인)

  • Objectives: The purpose of this investigation was to study the health status of apartment guards and associated factors. Materials and Methods: This investigation was conducted for 1 month starting from August, 1997. Study population in this cross-sectional survey consisted of 182 guards of apartments located throughout the district of Taegu. Each subject completed a questionnaire about his general characteristics, health behaviors, job descriptions, subjective GIT symptoms and somatization and depression using Symptom Checklist-90-Reversion (SCL-90-R) by self administrated questionnaire and personal interview. Results: Study subjects had one or more GIT symptoms(53.3%), somatization symptoms(83.5%) and depression symptoms(79.7%). The mean scores of GIT, somatization and depression symptoms were not different among the age groups and based on marital status, respectively, but, significantly different by education level(p<0.05). The mean scores of GIT, somatization and depression symptoms were not different among the age group and based on marital status, respectively, but, significantly different by regular diet and by the subject's health status(p<0.05). The mean scores of GIT, somatization and depression symptoms were not different by duration of shift work, by job tenure and by the number of managed houses but, those of somatization and depression symptoms were significantly different by level of job satisfaction(p<0.05). Conclusion: The health statuses of guards at apartments were different from other shift workers because of healthy worker effects and characteristics of their jobs.

Distribution of the Population at Risk of Cholangiocarcinoma in Bua Yai District, Nakhon Ratchasima of Thailand Using Google Map

  • Background: Cholangiocarcinoma (CCA), a major problem of health in Thailand, particularly in Northeastern and Northern regions, is generally incurable and rapidly lethal because of presentation in stage 3 or 4. Early diagnosis of stage 1 and 2 could allow better survival. Therefore, this study aimed to provide a distribution map of populations at risk for CCA in BuaYai district of Nakhon Ratchasima province, Northeast Thailand. Materials and Methods: A cross-sectional survey was carried out in 10 sub-districts and 122 villages, during June and November 2015. The populations at risk for CCA were screened using the Korat CCA verbal screening test (KCVST) and then risk areas were displayed by using Google map (GM). Results: A total of 11,435 individuals from a 26,198 population completed the KCVST. The majority had a low score of risk for CCA (1-4 points; 93.3%). High scores with 6, 7 and 8 points accounted for 1.20%, 0.13% and 0.02%. The population at risk was found frequently in sub-district municipalities, followed by sub-district administrative organization and town municipalities, (F=396.220, P-value=0.000). Distribution mapping comprised 11 layers: 1, district; 2, local administrative organization; 3, hospital; 4, KCVST opisthorchiasis; 5, KCVST praziquantel used; 6, KCVST cholelithiasis; 7, KCVST raw fish consumption; 8, KCVST alcohol consumption; 9, KCVST pesticide used; 10, KCVST relative family with CCA; and 11, KCVST naive northeastern people. Geovisual display is now available online. Conclusions: This study indicated that the population at high risk of CCA in Bua Yai district is low, therefore setting a zero model project is possible. Key success factors for disease prevention and control need further study. GM production is suitable for further CCA surveillance and monitoring of the population with a high risk score in this area.
