훈증요법을 병행한 기침환자 28명에 대한 임상적 평가 (Clinical Assessment of 28 Patients with Cough Treated with Herbal Steam Therapy)

  • The purpose of this study was to investigate the clinical effects of herbal steam therapy on cough. Twenty-eight patients with cough who had treated with herbal steam therapy(chungsang, chungpae) between 3rd of January and 30th of April, 2013 were analyzed. The effects were evaluated the severity and frequency of cough as the modified questionnaire, severity of discomfort using visual analogue scale(VAS), and the patients' subjective satisfaction. Eighteen patients who had been treated herbal steam therapy over 3 times were analyzed improvements before and after therapy. Chungsang and Chungpae steam therapies significantly decreased the severity and frequency of cough. VAS scores also decreased from $7.78{\pm}0.878$ to $4.39{\pm}2.033$. Patients were satisfied with herbal steam therapy. Chungsang and Chungpae steam therapies seem to be effective in the treatment of cough. Further studies with a large number of cases will be needed in the future.

호흡근 강화 훈련이 뇌졸중 환자의 호흡기능, 호흡근력과 기침능력에 미치는 영향 (The Effects of Respiratory Muscle Training on Respiratory Function, Respiratory Muscle Strength, and Cough Capacity in Stroke Patients)

  • PURPOSE: The purpose of this study was to examine the effects of respiratory muscle training on respiratory function, respiratory muscle strength, and cough capacity in stroke patients. METHODS: This study used a nonequivalent control group pre-post test design. We recruited thirty-four stroke patients(16male, 18female), who were assigned to intervention (n=17), or control (n=17) groups. Both groups participated in a conventional stroke rehabilitation program, with the intervention groups also receiving respiratory muscle training 20 minutes a day, three times a week, for 4 weeks. Respiratory function (forced vital capacity) and respiratory muscle strength (maximal inspiratory pressure, maximal expiratory pressure) were assessed by spirometry. Cough capacity (peak expiratory flow) was assessed using a peak flow meter. The collected data were analyzed by independent and paired t-tests. RESULTS: The intervention group showed a significant increase in the forced vital capacity (FVC), maximal inspiratory pressure (MIP), maximal expiratory pressure (MEP) and peak expiratory flow (PEF) at the end of the program, while the control group showed no significant changes. CONCLUSION: This study showed that respiratory muscle training increased respiratory function, respiratory muscle strength, and cough capacity in stroke patients and prevented a decrease in cough capacity. These findings suggest that respiratory muscle training effect on respiratory function, respiratory muscle strength and cough capacity for rehabilitation in patients with stroke.

오적산합생맥산(五積散合生脈散)으로 호전된 위식도 역류에 의한 만성기침 환자 3례 (Case Study of Three Gastro-esophageal Reflux-induced Chronic Cough Patients Treated with Ojeok-san plus Saengmaek-san)

  • Objectives: Gastro-esophageal reflux-induced chronic cough is not easy to diagnose and treat. In this study, three patients diagnosed with this disease were treated with Ojeok-san plus Saengmaek-san. Methods: Three patients with chronic cough, suspected to be gastro-esophageal and reflux-based, were diagnosed according to symptoms and abdominal examinations. They were treated with the herbal medicine, Ojeok-san plus Saengmaek-san. To evaluate the effectiveness of the treatment, a visual analogue scale (VAS) for measuring the severity of a cough and the Korean version of the Leicester Cough Questionnaire (LCQ-K) were used to determine the patients' quality of life. Results: After administration of Ojeok-san plus Saengmaek-san, the severity of the patients' coughing decreased based on VAS scores and quality of life improved based on LCQ-K scores for all patients. Conclusions: Ojeok-san plus Saengmaek-san is a potent therapeutic agent for gastro-esophageal reflux-induced chronic cough.

만성기침 변증도구 개발을 위한 기초연구 (Preliminary Study for Development of Pattern Identification Tool of Chronic Cough)

  • Objectives : The aim of this study was to develop a standard tool of pattern identification for chronic cough, which will be applied to clinical research. Methods : The items and structures of the instrument were based on reviews of textbooks and published literature. The advisor committee on this study consisted of 11 Korean respiratory internal medicine professors, one clinical fellow, and five clinicians with 10 years of experience. The questionnaire, which includes the signs and symptoms of chronic cough, was studied by the delphi method. Delphi examination was carried out via email, through evaluating the importance of symptoms included in each pattern. Results : We divided the pattern identification of chronic cough into five patterns: Wind-Cold, Phlegm-Turbidity, Liver-Fire, Lung Deficiency, and Kidney Yang Deficiency. By the Delphi method and a score evaluation, 38 items were chosen for pattern identification of chronic cough. Conclusions : Through this study, we created a Korean instrument for the pattern identification tool for chronic cough. We expect to apply this tool to subsequent research as its validity and reliability are further confirmed.

만성기침에 대한 보조제제로써 청인(淸咽)트로키 복합투여의 임상적 유효성 평가 (Clinical Effects of Chungin-troche As an Adjuctive Medicine with Chronic Cough Patients)

  • Objectives : We designed this study to evaluate the effectiveness of Chungin-troche(CIT) preparation as an adjunctive medicine to a herbal extract of Chungsangboha-tanggagambang(CSBH). Methods : A paralled, comparative study of CSBH+CIT and CSBH was conducted on 42 patients of chronic cough who attended Division of Allergy, Immune & Respiratory System, Department of Internal Medicine, College of Oriental Medicine, Kyung Hee University between December, 2007 and June, 2008. The treatment in each group lasted for two weeks. Patients were asked to fill in the Leicester Cough Questionnaire (LCQ) before and after administration of each treatment legimen. Results : 42 patients completed the trial and no adverse effects were reported. Both CSBH +CIT group and CSBH group showed statistical significant improvement in total LCQ score after 2 weeks from baseline. Social domain score and total score in patients taking CSBH+CIT showed a statistical significant increase compared to those scores in CSBH patients. Conclusion : CSBH+CIT compositive preparation and CSBH separate preparation appeared to have some benefits in the treatment of chronic cough. However, additional CIT preparation to CSBH can improve social domain in LCQ of chronic cough patients.

해수(咳嗽)의 병인분류(病因分類)와 침구치료혈(鍼灸治療穴)에 대(對)한 문헌적(文獻的) 고찰(考察) (Bibilographic Study on the Classification Methods of the Cause of Disease and the Acupuncture Points on the Cough(咳嗽, hae-soo))

  • Objectives : The objectives of this study is to find out the classification methods of the cause of disease and the Acupuncture points on the cough(咳嗽, hae-soo) from the oriental medical literature. The results obtained as follows. Methods : We surveyed the oriental medical books from Hung-Ti-Nei-Ching $\ll$黃帝內經$\gg$ to recent books concerning the Acupuncture therapy for the cough(咳嗽, hae-soo). Results : 1. There are the classification methods of the cause of the cough(咳嗽, hae-soo) by affection by exopathogen and internal injury, by five zang-organs and six fu-organs, by time-belt, and by cold and heat. 2. The acupuncture points at P'yesu(肺兪, BL13), T'aeyon(太淵, LU9), Ch'okt'aek(尺澤, LU5), P'ungmun(風門, BL12), Yolgyol(列缺, LU7), Ch'ondol(天突, CV22), Taech'u(大椎, GV14), Hapkok(合谷, LI4), Kohwang(BL43), T'aegye(太谿, KI3), Chok-samni(足三里, ST36) are most frequently used on the acupuncture therapy for the cough(咳嗽, hae-soo). Conclusions : Among the classification methods of the cause of the cough(咳嗽, hae-soo), the classification methods by affection by exopathogen and internal injury may be most effective and the acupuncture points at Lung meridian(手太陰肺經, LU), Bladder mendian(足太陽膀胱經, BL) are most frequently used on the acupuncture therapy for the cough(咳嗽, hae-soo).

폐전이로 유발된 기침 환자에 대한 한약 치료 증례보고 (A Case Report of Cough in a Patient with Metastasis to Lung Improved by Korean Medicine)

  • Objectives: This case report aimed to present the effect of Korean medicine therapies, including Sikyungbanga-tang, on cough in a breast cancer patient with metastasis to the lung. Methods: The patient was treated for 48 days with Sikyungbanga-tang and Saengmaek-san to control cough and other symptoms. To assess the changes in the symptoms and the patient's quality of life, the numeric rating scale (NRS), the Leicester Cough Questionnaire (LCQ), and the EORTC quality of life questionnaire Core 30 (EORTC QLQ-C30) were used. Results: Following treatment with herbal medicine, the clinical symptoms and quality of life improved. Moreover, the patient's cough did not recur even one month later. Conclusions: This case report demonstrates that Korean herbal medicine, including Sikyungbanga-tang, may be effective in treating cough in breast cancer patient with metastasis to the lung. However, further research is needed to confirm these findings.

복부 기능적전기자극이 목척수손상환자의 최대기침유량과 노력성폐활량에 미치는 효과 (Effects of Abdominal Functional Electrical Stimulation on Peak Cough Flow and Forced Vital Capacity in Patients with Cervical Spinal Cord Injury)

  • Purpose : The purpose of this study was to investigate the effects of abdominal functional electrical stimulation on peak cough flow and forced vital capacity in patients with cervical spinal cord injury. Methods : The study examined 20 patients with cervical spinal cord injury. The subjects were randomly divided into two groups. All subjects performed conservative physical therapy for 30 minutes. The experimental group also underwent abdominal functional electrical stimulation for at least 20 minutes per day. Abdominal functional electrical stimulation was applied to the rectus abdominis muscle twice each day, three times a week, for four weeks. In all subjects, the peak cough flow was measured using a peak flow meter and forced vital capacity was assessed using a spirometer. Results : The experimental group showed a significant increase in peak cough flow and forced vital capacity in pre-post measurements (p<.05), while the control group showed a significant increase only in peak cough flow. Conclusion : These findings suggest that conservative physical therapy in combination with the abdominal functional electrical stimulation can improve peak cough flow and forced vital capacity in patients with cervical spinal cord injury.

기침에 대한 반하사심탕의 효과 : 체계적인 문헌고찰과 메타분석 (The Effect of Banhasasim-tang in Cough: A Systematic Review and Meta-analysis)

  • Objective: This study confirms the safety and efficacy of Banhasasim-tang (BST) against cough through systematic review and meta-analysis. Methods: PubMed, Cochrane, ScienceDirect, Wiley online library, OASIS, KISS, NDSL, RISS, Science and Technology Society Village, DBpia, CNKI, J-STAGE, and CiNii were used as search engines. The search period was from the start of search engine support to June 30. 2020. And search language was not limited. Cochrane's risk of bias (RoB) tool was used to evaluate the quality of the studies. A meta-analysis of the total effective rate (TER), cough symptom score, and spirometry results was performed using the Review Manager program. Results: 6 studies were finally selected. The group using BST showed significant symptom relief, as the odds ratio (OR) value of TER was 5.56 times higher (95% CI: 3.50, 8.82, P<0.0001) compared to the control groups. The reported adverse effects did not occur in the intervention group. In addition, the cough symptom score and spirometry results showed that BST was more effective than Western medicine alone. Conclusion: BST can be expected to relieve cough, and it can be applied to chronic cough that is not treated with existing treatments. However, due to the low quality of the selected articles, additional high-quality RCTs will be required in the future.

소아 해수에 대한 추나요법의 효과: 체계적 문헌 고찰과 메타분석 (Chuna Manual Therapy for Pediatric Cough: A Systematic Review and Meta-analysis)

  • Objectives This review was conducted to evaluate the effectiveness and clinical significance of Chuna manual therapy (CMT) in the treatment of pediatric cough. Methods Ten electronic databases (PubMed, EMBASE, Cochrane, CNKI [CAJ], KMBASE, KISS, KISTI, NDSL, RISS, and Oasis) were searched. Only randomized controlled trials (RCTs) using CMT for pediatric cough treatment were included. The methodological quality of each RCT was evaluated using the Cochrane risk-of-bias tool (v1.0). Results Nine RCTs met the inclusion criteria. The meta-analysis revealed positive effects of CMT in the treatment of pediatric cough. Both CMT alone and CMT combined with Chinese medication or Chinese medicine acupoint application (CMAA) were effective in treating pediatric cough. Conclusions This systematic review shows the reliability, effectiveness, and clinical significance of CMT in the treatment of pediatric cough. However, the evidence is limited due to the defective design of the included RCTs. More well-designed RCTs are required to provide clearer evidence for this claim.
