Facial fractures and associated injuries in high- versus low-energy trauma: all are not created equal

  • Introduction: Facial fractures (FFs) occur after high- and low-energy trauma; differences in associated injuries and outcomes have not been well articulated. Objective: To compare the epidemiology, management, and outcomes of patients suffering FFs from high-energy and low-energy mechanisms. Methods: We conducted a 6-year retrospective local trauma registry analysis of adults aged 18-55 years old that suffered a FF treated at the Santa Barbara Cottage Hospital. Fracture patterns, concomitant injuries, procedures, and outcomes were compared between patients that suffered a high-energy mechanism (HEM: motor vehicle crash, bicycle crash, auto versus pedestrian, falls from height > 20 feet) and those that suffered a low-energy mechanism (LEM: assault, ground-level falls) of injury. Results: FFs occurred in 123 patients, 25 from an HEM and 98 from an LEM. Rates of Le Fort (HEM 12% vs. LEM 3%, P = 0.10), mandible (HEM 20% vs. LEM 38%, P = 0.11), midface (HEM 84% vs. LEM 67%, P = 0.14), and upper face (HEM 24% vs. LEM 13%, P = 0.217) fractures did not significantly differ between the HEM and LEM groups, nor did facial operative rates (HEM 28% vs. LEM 40%, P = 0.36). FFs after an HEM event were associated with increased Injury Severity Scores (HEM 16.8 vs. LEM 7.5, P <0.001), ICU admittance (HEM 60% vs. LEM 13.3%, P <0.001), intracranial hemorrhage (ICH) (HEM 52% vs. LEM 15%, P <0.001), cervical spine fractures (HEM 12% vs. LEM 0%, P = 0.008), truncal/lower extremity injuries (HEM 60% vs. LEM 6%, P <0.001), neurosurgical procedures for the management of ICH (HEM 54% vs. LEM 36%, P = 0.003), and decreased Glasgow Coma Score on arrival (HEM 11.7 vs. LEM 14.2, P <0.001). Conclusion: FFs after HEM events were associated with severe and multifocal injuries. FFs after LEM events were associated with ICH, concussions, and cervical spine fractures. Mechanism-based screening strategies will allow for the appropriate detection and management of injuries that occur concomitant to FFs. Type of study: Retrospective cohort study. Effects of Temperature and Supplementation with Skim Milk Powder on Microbial and Proteolytic Properties During Storage of Cottage Cheese

  • The aim of this study was to determine the effects of temperature and supplementation with skim milk powder (SMP) on the microbial and proteolytic properties during the storage of cottage cheese. Cottage cheese was manufactured using skim milk with 2% SMP and without SMP as the control, and then stored at $5^{\circ}C$ or $12^{\circ}C$ during 28 days. The chemical composition of the cottage cheese and the survival of the cheese microbiota containing starter lactic acid bacteria (SLAB) and non-starter culture lactic acid bacteria (NSLAB) were evaluated. In addition, changes in the concentration of lactose and lactic acid were analyzed, and proteolysis was evaluated through the measurement of acid soluble nitrogen (ASN) and non-protein nitrogen (NPN), as well as electrophoresis profile analysis. The counts of SLAB and NSLAB increased through the addition of SMP and with a higher storage temperature ($12^{\circ}C$), which coincided with the results of the lactose decrease and lactic acid production. Collaborating with these microbial changes, of the end of storage for 28 days, the level of ASN in samples at $12^{\circ}C$ was higher than those at $5^{\circ}C$. The NPN content was also progressively increased in all samples stored at $12^{\circ}C$. Taken together, the rate of SLAB and NSLAB proliferation during storage at $12^{\circ}C$ was higher than at $5^{\circ}C$, and consequently it led to increased proteolysis in the cottage cheese during storage. However, it was relatively less affected by SMP fortification. These findings indicated that the storage temperature is the important factor for the quality of commercial cottage cheese.

Englishness represented in a Cottage Garden (코티지 가든에 표상된 영국성)

  • Gardening activities, plant raising, and general flowerbeds the public makes today can be found in the original form of cottage gardens in the United Kingdom. A cottage garden is a popular garden style of modern Britain, implying unique Englishness including ethnic sense and vernacular. In addition, the purpose of this study is to consider the modern movement in the United Kingdom in the past 200 years and read Englishness of cottage gardens through style differentiation and background of occurrence of cottage gardens appearing in this process. Therefore, this study is summarized as follows. First, a view of nature of the Englishman loving freedom and landscape acts as a key part of patriotism and is connected to the preservation of idyllic England. For this ideal of the Englishman of the country, idyllic British characteristics are found in various literatures and artistic fruits; cottage gardens, that is a form of new garden, were made with invigoration of supply and collection of plants. Second, an early form of cottage gardens was the domestic garden, in which there is a vegetable garden by middle-class move to a suburb according to urbanization, but evolved into a form of garden having both artistry and regionality, vernacular, and ecological characteristics with various situations of modern society(handicraft promotion movement, preservation of remains, and ancient building restoration movement). Wild gardens occurring in this process are a type of garden realizing wild fields and forests in the United Kingdom;they have made a big impact on many garden designers up to now. Cottage gardens, reflecting a variety of Englishness, is a subject of city planning and flower shows and is a culture symbolizing the United Kingdom.

Effects of Carboxymethyl Chitosan on Yield and Whey Protein Loss in Cottage Cheese

  • A standard $1\%$ w/v solution of CM-chitosan made from squid pen was added to milk at levels of $0.5\sim3\%$ (v/v) to improve the yield and rheological properties of cottage cheese by whey protein retention. Cheese curd did not form at levels higher than $3\%$ (v/v) CM-chitosan standard solution. Yield and total protein of cottage cheese increased up to $2\%\;by\;11\;to\;42\%\;and\;17\;to\;38\%$ respectively, compared to control cheese. Whey protein losses were decreased by 11 to $42\%$ and thus accounted for all of the increase in yield. Anomalous results were obtained at the $0.8\%$ level, which neither improved yield or whey protein retention nor stabilized rheological parameters, and at the $0.5\%$ level, which improved yield and total protein without increasing whey protein retention. Elasticity and cohesiveness of CM-chitosan-containing cheese were generally improved and stabilized during storage. Monitoring of cheese chromaticity values for four weeks revealed a delay in the onset of yellowing in cheeses with CM-chitosan compared to the controls, while the concentration of added CM-chitosan had little influence on cheese chromaticity. The addition of CM-chitosan solution could be applied directly to industrial scale cottage cheese-making without the need for any modification of the production process.

A Study on the Improvement in Spatial Planning of Orphanage Facilities (아동양육시설의 공간계획 개선에 관한 연구)

  • This study aims to propose the improvement direction of spatial organization of orphanage facilities by reflecting the international trend of child welfare facilities including 'enhancement of habitability', 'opening to the local community' and 'multi-functionality' on the basis of ideas of 'right of housing' and 'normalization'. Orphanage facilities are evolving from 'facilities' to accommodate unfortunate children to the concept of the 'community-care', and the residential space is also rapidly shifting to 'cottage' type resembling a residential type of ordinary family so as to enhance the self-esteem and relationship. To suggest the future-oriented changeability of current orphanage facilities, the present study conducted a nationwide survey of child welfare facilities and four Visiting researches of cottage type orphanage with different locations to investigate the appropriateness of housing type, organization of common use space, mode of management and facilities criteria. The results of this study are following: 1) For enhancement of habitability it is suggested that cottage type with various plans in the form of ordinary housing is appropriate, that the number of children per cottage is six or so, and that the number of less than two or three children per room is recommended. At the same time the adjustment of facilities criteria, simplified or complex, is suggested to support a similar residence pattern to ordinary home. 2) Specialized programs must be introduced to establish a base of welfare-network for community children according to features of location and a complex management must be sought in the connection with neighboring public facilities. 3) To secure the residential environment and quality of life for children, the concept of a simple playground space by the current facilities criteria must be broken away to reinforce the network of various outdoor spaces closely connected with living space.

Decreasing Return to Scale in Cottage Industries: Empirical Evidence from the Coconut Sugar Industry in Banyumas, Indonesia

  • This study aims to analyze the economies of scale of the cottage industry for coconut sugar production in Banyumas, Indonesia. This study applies a survey method to coconut sugar craftsmen. Data analysis was performed by regression analysis based on the Cobb-Douglas production function approach. The findings indicated that decreasing returns to scale was a problem for the domestic production of coconut sugar. These findings show that the production of coconut sugar is not very productive. Labor and financial capital inputs have a significant positive share but the resulting increase in output is less proportional than the increase in the two inputs. Social capital, experience, and education do not affect industrial performance. To increase input productivity and production efficiency, it is necessary to apply more effective production techniques and technologies to produce quality products so that the selling price can be higher. Additionally, it is required to enhance the sap's quality through its extraction methods and the regrowth of high-yielding coconut tree varieties. From the institutional aspect, the development of this industry requires stronger partnerships with related parties such as local governments, exporting companies, cooperatives, and universities through research and development.

A Study on the Relationship between the Studying Activities of Children and Spaces In the Orphanage (아동양육시설의 주거형태에 따른 공부행위에 관한 연구)

  • Children of three-to-eighteen-years-old have been brought up in the institution (Orphanage) for several reasons such as family dissolution, divorce and poverty since the Korean Was of 1950. The Cottage, where these children are housed and brought up has dining room, library and bedrooms, plays as a significant social interaction place as well as study. Since the most facilities were built during the 1950-1960s period, most of the cottages are now in the condition of being rebuilt or remodelled. Because living space and studying space are mixed, it will be necessary to provide another studying space separated from living space. And then it will be also necessary to provide a space for play or rest between the two spaces. This study was conducted to obtain some basic data necessary for design of spaces in the orphanages by examining the relations between the studying activities of children and residence types.

  • IHWA 는 분리개발 된 객체 컴포넌트를 통합함으로써 새로운 응용 시스템을 작성할 수 있는 CBSE(Component Based Software Engineering)기법을 바탕으로 한, 웹 기반 정보 저장/검색의 전형적 모델을 제공하는 소프트웨어 시스템이다. 현재 본 연구팀은 IHWA 시스템의 적용 사례로서 e-business 를 위한 쇼핑몰을 구축하고자 검색, 지불, 운용분야의 모듈들을 S/W 컴포넌트로 개발 중에 있다. 본 논문에서는 그 중, IHWA 의 검색분야에서 상품정보를 보다 효율적으로 수집하고 사용자에게 양질의 정보를 제공하기 위해 필수적으로 요구되는 정보 수집 서비스의 구조에 대하여 설명하고, 특히 비 정형화 된 XML 웹 소스에서의 정보수집 방법에 있어 현재 진행중인 연구에 대하여 설명 하고자 한다.
