Quantitative Analysis of Lung Contusion (폐좌상의 정량분석)

  • Lung contusion due to blunt chest trauma is the most common lung injury and correlated with the clinical course and prognosis. Its diagnosis by CT[Computerized Tomogram] gives a more clear and understandable three dimensional view, by which we are able to measure the volume of the contused and entire lung. Other variables are arterial blood gas, number of rib fracture, presence of hemopneumothorax, sternal fracture and clavicle fracture, number of associated non-thoracic injuries, ventilator time and presence of pulmonary complication. Percentage[%] of lung contusion are expressed as mean $\pm$ standard deviation and data analysis was performed by means of multivariate repeated measures analysis of variance to detect significant differences in variables between positive thoracic injury group and negative group. The paired t-test was used. Differences of percentage of lung contusion between groups were assessed by one-way analysis of variance. Simple linear regression was used to perform correlation analysis in the number of rib fracture and ventilator time. A p value less than 0.05 was considered statistically significant. Pneumothorax and the number of associated other injuries affect the amount of lung contusion and pulmonary complication group has more contused lung volume. Arterial blood gas study shows no correlation with the amount of lung contusion statistically. The number of rib fracture correlated with the amount of lung contusion, which also correlated with ventilator time[r=0.56, p<0.05]. In conclusion, quantitative anlysis of lung contusion by CT predicts the clinical course and treatment such as ventilator care.

Improvement of Pain according to Magnetic Resonance Imaging Classification in Bone Contusion around Foot and Ankle (족부 족관절 골좌상에서 자기공명영상 분류에 따른 통증의 호전)

  • Purpose: Bone contusion is usually treated with conservative therapy for 3 months. Bone contusion around knee and hip joints has been extensively reported on, but there are scant reports on this condition in foot and ankle joints. This study evaluated the nature, characteristics and location of bone contusion around foot and ankle joints to enlighten clinicians on how to better treat this disease entity. Materials and Methods: We classified bone contusion of the 76 patients into three types (102 sites; 47 ankle sprains, 18 traffic accidents, 11 falls) according to the Costa-Paz system with employing magnetic resonance imaging (MRI), and the study then analyzed the common sites and areas of occurrence according to the mechanism of injury and duration of pain after first conducting conservative therapy. Results: Of the 76 patients (102 sites) on the MRI, 43 case (42.2%) for talus, 19 cases for distal tibia, and 12 cases for calcaneus were involved. The classification, according to the Costa-Paz system, was Type I, 51 cases; Type II, 32 cases; and Type III, 19 cases. The duration of pain after conservative treatment was 12.15±2.17 weeks for Type I, 14.5±2.15 weeks for Type II, and 21.0±3.8 weeks for Type III. Conclusion: The most common location of post-traumatic bone contusion around both the foot and ankle is the talus, distal tibia, and calcaneus. The most common type of injury noted on MRI is a diffuse signal with change of the medullary component (Type I), In cases of bone contusion extending to a subjacent articular surface or disruption or depression of the normal contour of the cortical surface (Types II, III), the patients' pain appears to last longer. Thus, it is necessary to consider a longer period of conservative treatment in cases of Types II and III bone contusion because the patients' pain may last longer than 3 months.

  • Spinal cord contusion injury is one of the most serious nervous system disorders, characterized by high morbidity and disability. To mimic spinal cord contusion in humans, various animal models of spinal contusion injury have been developed. These models have been developed in rats, mice, and monkeys. However, most of these models are developed using rats. Two types of animal models, i.e. bilateral contusion injury and unilateral contusion injury models, are developed using either a weight drop method or impactor method. In the weight drop method, a specific weight or a rod, having a specific weight and diameter, is dropped from a specific height on to the exposed spinal cord. Low intensity injury is produced by dropping a 5 g weight from a height of 8 cm, moderate injury by dropping 10 g weight from a height of 12.5-25 mm, and high intensity injury by dropping a 25 g weight from a height of 50 mm. In the impactor method, injury is produced through an impactor by delivering a specific force to the exposed spinal cord area. Mild injury is produced by delivering $100{\pm}5kdyn$ of force, moderate injury by delivering $200{\pm}10kdyn$ of force, and severe injury by delivering $300{\pm}10kdyn$ of force. The contusion injury produces a significant development of locomotor dysfunction, which is generally evident from the $0-14^{th}$ day of surgery and is at its peak after the $28-56^{th}$ day. The present review discusses different animal models of spinal contusion injury.

Clinical Characteristics and Prognostic Factors of Pulmonary Contusion with Traumatic Lung Cyst (흉부 외상에서 폐좌상(Pulmonary contusion)과 외상성 폐낭종 (Traumatic lung cyst)을 동반한 환자의 임상경과 및 예후 예측 인자)

  • Purpose: A traumatic lung cyst (TLC) is a rare complication and is usually detected with a pulmonary contusion. This study attempted to identify the prognostic factors and the clinical characteristics for pulmonary contusion with TLCs. Methods: We retrospectively reviewed the medical records and chest CT findings of 71 TLC patients who visited our hospital from January 2006 to December 2007. Patients were assessed for any clinical characteristics. We evaluated significant differences between the survival and the death groups for patients with a traumatic lung cyst. Results: The male-to-female ratio of patients with TLCs was 54:17, and the mean age of the patients was $37.70{\pm}19.78years$ with 36.6% of the patients being under 30 years fo age. The cause of blunt thoracic trauma was mainly pedestrian traffic accidents (26.8%) and falls (25.4%). Associated conditions included pulmonary contusion in 68 patients (95.7%), hemopneumothorax in 63 patients (88.7%), and rib fracture in 52 patitents (73.2%). There was no consistent relationship between the number of TLCs and the pulmonary contusion score. The overall mortality rate of TLC patients was 26.8%. Death correlated with a need for ventilatory assistance, mean arterial pressure, worst mean arterial pressure in 24 hours, initial pH and base excess, worst pH and base excess in 24 hours, refractory shock, initial GCS score, and pulmonary contusion score. Conclusion: The presence of the aforementioned predictors indicate serious injury, which is the main determinant of the outcome for thoracic injuries with TLCs.

Correlation Between Pulmonary Contusion and Myocardial Contusion in Patients with Multiple Injuries (다발성 외상환자에서 폐좌상과 심근좌상의 상관관계)

  • Purpose: This study was conducted to evaluate the correlations among pulmonary contusion severity, trauma score and cardiac troponin I (cTnI) level. Methods: We prospectively evaluated patients with multiple injuries who had been admitted to the emergency department (ED) from July 2007 to July 2008. We first measured the total creatinine kinase (CK), the MB fraction of CK (CK-MB), TnI, and myoglobin within 2 hours after the injury. We then checked the electrocardiogram, x-ray, and computed tomography (CT) results. Finally, we assessed the injuries as variables and then compared the results for patients with elevated TnI levels (group A) and patients whose TnI levels fell within the normal range (group B). Results: Eighty-six of the 92 patients admitted to the ED were enrolled. The pulmonary contusion score (PCS) was well correlated with $PaO_2/FiO_2$. TnI levels were correlated with PCS. When TnI levels were above 0.86 ng/ml, the mortality was estimated with 100% sensitivity and 86.1% specificity. Conclusion: Pulmonary contusion severity is correlated with TnI level. When the PCS is high and the cTnI level is elevated in multiple-injury patients, we recommend continuous cardiac monitoring and further evaluation

  • Muscle contusion usually results from a direct blunt impact and is frequently associated with contact sports. Muscle contusion results from microscopic muscle fiber and capillary disruption causing a microhemorrhage dissecting torn fibers and remaining viable muscle fibers. Recent studies concluded that some physical methods, including pulsed ultrasound (PU) and pulsed electromagnetic field (PEMF) treatment, accelerate and facilitate wound healing, improve scar quality and have beneficial effects on muscle and tendon healing. However, there are few studies on the effects of the early use of physical methods, such as PU and PEMF, on the expression of neurotrophic factors. The objective of this study was to investigate the effects of the early application of PU and PEMF, measured through the expression of BDNF in the muscles (gastrocnemius) and spinal cords of rats after skeletal muscle contusion. In the spinal cords and muscles, there was a significant increase of BDNF expression in the PEMF and PU groups, a greater increase was found in the PEMF group than in the PU group. In conclusion, PEMF is a useful therapeutic method that improves muscle healing after muscle contusion.

Long-term Follow-up of Cutaneous Hypersensitivity in Rats with a Spinal Cord Contusion

  • Sometimes, spinal cord injury (SCI) results in various chronic neuropathic pain syndromes that occur diffusely below the level of the injury. It has been reported that behavioral signs of neuropathic pain are expressed in the animal models of contusive SCI. However, the observation period is relatively short considering the natural course of pain in human SCI patients. Therefore, this study was undertaken to examine the time course of mechanical and cold allodynia in the hindpaw after a spinal cord contusion in rats for a long period of time (30 weeks). The hindpaw withdrawal threshold to mechanical stimulation was applied to the plantar surface of the hindpaw, and the withdrawal frequency to the application of acetone was measured before and after a spinal contusion. The spinal cord contusion was produced by dropping a 10 g weight from a 6.25 and 12.5 mm height using a NYU impactor. After the injury, rats showed a decreased withdrawal threshold to von Frey stimulation, indicating the development of mechanical allodynia which persisted for 30 weeks. The withdrawal threshold between the two experimental groups was similar. The response frequencies to acetone increased after the SCI, but they were developed slowly. Cold allodynia persisted for 30 weeks in 12.5 mm group. The sham animals did not show any significant behavioral changes. These results provide behavioral evidence to indicate that the below-level pain was well developed and maintained in the contusion model for a long time, suggesting a model suitable for pain research, especially in the late stage of SCI or for long term effects of analgesic intervention.

A Study of the influence of both of Shùnqìdǎotántāng and Huàyūtāng on thrombosis, contusion-hyperemia, and hyperlipidemia (혈전증(血栓症)과 타박성(打撲性) 충혈(充血) 및 고지혈증(高脂血症)에 순기도담탕(順氣導痰湯) 및 화어탕(化瘀湯)이 미치는 영향(影響))

  • To see both $Sh{\grave{u}}nq{\grave{i}}daot{\acute{a}}nt{\bar{a}}ng$(dissipate phlegm and promote vital energy circulation) and $Hu{\grave{a}}y{\bar{u}}t{\bar{a}}ng$(blood circulation and disperse blood stasis) influencing on thrombosis, contusion-hyperemia, and hyperlipidemia, at first we measured the density of FDP, the quantity of fibrinogen, prothrombin time, and the number of platelet of rat taken thrombosis by endotoxin. Secondly we measured the increase-rate of "paw swelling", the number of platelet, the quantity of fibrinogen, and prothrombin time of rat taken contusion-hyperemia. And then we measured the quantity of total cholesterol in serum and of H.D.L-cholesterol and of triglyceride and of phospholipid and of ${\beta}-lipoprotein$, its weight, and the variation of the quantity of electrolyte of rat taken hyperlipidemia by the oral-injection of choleserol. As a result, we can conclude as follows : 1. Out of the test of thrombosis, we can recognize not only the noticeable increae of the number of platelet and the quantity of fibrinogen, but also the noticeable decrease of prothrombin time and the density of FDP in case of $Sh{\grave{u}}nq{\grave{i}}daot{\acute{a}}nt{\bar{a}}ng$-injected rat and $Hu{\grave{a}}y{\bar{u}}t{\bar{a}}ng$-injected rat. 2. Out of the test of contusion-hyperemia, we can recognize not only the noticeable increase of the number of platelet and the quantity of fibrinogen, but also the noticeable decrease of prothrombin time and "increase-rate of paw swelling" in case of $Sh{\grave{u}}nq{\grave{i}}daot{\acute{a}}nt{\bar{a}}ng$-injected rat and $Hu{\grave{a}}y{\bar{u}}t{\bar{a}}ng$-injected rat. 3. Out of the test of hyperlipidemia, at first we can recognize that test rat's weight increased as close as that of normal rat. And we can recognize the noticeable decrease of the triglyceride and phospholipid and ${\beta}-lipoprotein$." Also, in case of the variation of electrolyte we can recognize the decrease of calcium and potassium in $Sh{\grave{u}}nq{\grave{i}}daot{\acute{a}}nt{\bar{a}}ng$-injected rat, and of sodium and magnesium in $Hu{\grave{a}}y{\bar{u}}t{\bar{a}}ng$-injected rat. Thus, as the above-mentioned, in covering thrombosis, contusion-hypermia, and hyperlipidemia, the effect of $Sh{\grave{u}}nq{\grave{i}}daot{\acute{a}}nt{\bar{a}}ng$ and $Hu{\grave{a}}y{\bar{u}}t{\bar{a}}ng$ can be recognized. Granting that $Hu{\grave{a}}y{\bar{u}}t{\bar{a}}ng$ reveals its effectiveness in thrombosis and contusion-hyperemia, and $Sh{\grave{u}}nq{\grave{i}}daot{\acute{a}}nt{\bar{a}}ng$ in hyperlipidemia, it can be inferred that contusion-hyperemia is like "model of blood stasis form" as thrombosis and hyperlipidemia "phlegm-retention diseases form", and both phlegm-retention and blood stasis have correlation each other.

  • The following case study suggests strongly the efficacy of Danggwisusan and acupuncture therapy in the treatment of patients with severe contusion and tissue injury of the liver and kidney. The chief symptoms of this case were severe pain in the right side, purpura hemorrhagica, insomnia and third degree bums at the shoulder and face. Analysis of blood chemistry indicated high AST, high ALT, high BUN, high creatine, high creatine kinase and high LDH. Fallowing medication of the patient with Danggwisusan, the symptoms and indexes for tissue injury decreased rapidily within the first week. This results support a potential role for Danggwisusan in the treatment of severe contusion with tissue injury of the liver and kidney.

Pulmonary Contusion Similar to COVID-19 Pneumonia

  • The Coronavirus disease 2019 (COVID-19) has rapidly spread across the world and caused a pandemic. It can be transmitted by an infected person or an asymptomatic carrier and is a highly contagious disease. Prevention and early identification of COVID-19 are important to minimize the transmission of COVID-19. Chest computed tomography (CT) has a high sensitivity for detecting COVID-19, but relatively low specificity. Therefore, chest CT may be difficult to distinguish COVID-19 findings from those of other infectious (notably viral types of pneumonia) or noninfectious disease. Pulmonary contusion has also a lot of similarities on chest CT with COVID-19 pneumonia. We present trauma patients with pulmonary contusion whose CT scans showed findings similar to those of COVID-19, and we report our experience in the management of trauma patients during the COVID-19 pandemic.
