Development of a Reward Scale for Hospital Nurses (병원간호사의 보상 측정도구 개발)

  • Purpose: This study aimed to develop and test a reward scale for hospital nurses. Methods: The initial items were identified through a literature review and focus group interviews with ten hospital nurses. The content validity of the items was evaluated by ten experts. Fifty-one items were derived from the pilot survey. Four hundred eighty-eight nurses participated in the study: 248 for exploratory factor analysis (EFA) and 240 confirmatory factor analysis (CFA). Data were analyzed using item analysis, EFA, CFA, convergent validity, known-group validity, and internal consistency using IBM SPSS Statistics 29.0 and IBM SPSS AMOS 29.0. Results: The final scale consisted of 31 items and eight factors (decent wage, opportunity to grow and develop, support for special situations, various benefits, flexibility of work, job-related achievement, reflecting career and performance, and recognition), which explained 67.3% of the total variance. The eight-subscale model was validated by CFA. Convergent validity was evaluated by analyzing correlation with intention to leave (r = - .63, p < .001) and job satisfaction (r = .54, p < .001). The known-group validity was evaluated by comparing the reward scales according to age, clinical career, income level and hospital type. The scale was found to be reliable with a Cronbach's α of .89. Conclusion: Both the validity and reliability of the reward scale for hospital nurses are verified, which can enhance the understanding of the range of rewards and may assist nurse managers in establishing an effective reward system.

Development of the Hospital Nurses' Silence Behavior Scale (병원 간호사의 침묵 행위 측정도구 개발)

  • Purpose: This study aimed to develop a scale to measure hospital nurses' silence behavior and examine its validity and reliability. Methods: A total of 52 preliminary items on hospital nurses' silence behavior were selected using a content validity test by seven experts on 53 candidate items derived from a literature review and in-depth interviews with 14 nurses. A total of 405 hospital nurses participated in a psychometric testing. Data analysis comprised item analysis, exploratory and confirmatory factor analyses, and convergent and discriminant validity tests. Pearson's correlation coefficient was used for assessing concurrent validity, and Cronbach's alpha was used for the reliability test. Results: The final scale consisted of nine factors with 31 items, exhibiting acceptable model fit indices, convergent validity, and discriminant validity. The score of the entire scale was positively correlated with the 'Organizational Silence Scale (OSS)-the issues on which nurses remain silent' (r = .60, p < .001) and 'OSS-the reasons why nurses remain silent' (r = .68, p < .001). Cronbach's α of the scale was .92, and α of each subscale ranged from .71 to .90. Conclusion: The Hospital Nurses' Silence Behavior Scale is a useful tool for assessing multifaceted silence behavior among nurses. It can provide basic data for developing better communication strategies among nurses and other hospital staff.

Scale Development of Job Stress for Home Care Nurses (가정전문간호사의 직무스트레스 측정 도구 개발)

  • Purpose: The purpose of this study was to develop a job stress scale for hospital-based home care nurses in Korea. The process was construction of the conceptual framework, development of the preliminary items, verification of the content validity, item analysis and test of the reliability. The preliminary items were based on literature review and in-depth interviews with home care nurses. As a result, eight categories and sixty items were selected. These were reviewed by seven specialists for content validity and finally fifty one items were chosen. Data was collected from 180 home care nurses who were engaged in 87 hospitals from August to September 2003. The result of item analysis one was excepted, The final item count was 50. Categories were as follows: overload work(8 items), lack of specialized knowledge and technique(5 items), ethical dilemma(4 items), role conflict(5 items), interpersonal relationships(6 items), visiting home environment(9 items), driving conditions(4 items) and lack of administrative support(9 items), The reliability of the scale by Cronbach's alpha was .948 and the domain's reliability ranged from .649 to .841. The result of this study could be used to measure the job stress of home care nurses. However, for further validity and reliability, repeated studies will be necessary.

Development and Psychometric Testing of the Clinical Nursing Competency Scale for Clinical Preceptor Use (CNCS-CP) (임상간호실습 현장지도자용 임상간호역량 평가도구 개발)

  • Purpose: The purpose of this study was to develop and establish the psychometric properties of a clinical nursing competency evaluation tool to be utilized by clinical preceptors. Methods: The initial items were identified through in-depth literature review and field interviews based on a hybrid model. Content validation of the items was evaluated through three rounds of content validity testing. Participants were 34 clinical preceptors and 443 nursing students participating in clinical practice. Data were analyzed using exploratory and confirmatory factor analysis, convergence and discriminant validity, internal consistency and inter-rater reliability. Results: The final scale consisted of 23 items and four factors, fundamental nursing skills performance, critical thinking skills based on the nursing process, basic nursing knowledge, and professional attitude; these factor explained 69.7% of the total variance. The analysis with multi-trait/multi-item matrix correlation coefficients yielded 100.0% and 95.7 % convergence and discriminant validity, respectively. Cronbach's alpha for the total items was .95. The four subscale model tested by confirmatory factor analysis was satisfactory. Inter-rater reliability ranged from .912 to .967. Conclusion: This scale was found to be a reliable and valid instrument that clinical preceptors can apply for evaluating the clinical nursing competency of nursing students in clinical settings.

Development of Sexual Satisfaction Measurement Tool (여성의 성만족 측정을 위한 도구개발)

  • This study was conducted to develop a sexual satisfaction tool for married women in Korea. The data was collected from July 19 to Aug 18, 1997 by means of questionnaires developed by researchers. The subject were 417 married women living in Seoul and Pusan. The process of this study was as follows : 1) The concept of sexual satisfaction was defined. 2) A conceptual framework was identified based on the extensive review of relevant literature and interviews with married women. 3) The preliminary question items containing the attributes and elements of the concept of sexual satisfaction were listed. 4) The preliminary items were revised after a pilot study. 5) The Index of Content Validity(CVI) was calculated from the content specialists' rating. 6) The reliability and validity of the sexual satisfaction measurement tool were tested. As a result of the item and factor analysis, 17 out of 30 items were found to be valid, consequently could be used to measure sexual satisfaction for married women. These final 17 items were divided into two factors. These factors were labeled as "situation factor" (10 items) and "response factor"(7 items) according to the attributes of the clustered items. The reliability of the final 17 items was .9118. Further research is needed to confirm the reliability and validity of the tool by applying it to a group of healthy married women and to a group of married women having health-related problem.

Development of Evidence-based Dysphagia Nursing Care Protocol for Nursing Home Residents (노인요양시설 거주 노인을 위한 한국형 근거기반 연하장애 간호 프로토콜 개발)

  • Purpose: The purpose of this study was to develop an evidence-based dysphagia nursing care protocol for nursing home (NH) residents in Korea. Methods: The protocol was developed based on international guidelines and literatures. After testing content validity by experts, the protocol was applied to the intervention group (n=35) for 4 weeks at one NH in December, 2011, whereas the control group (n=34) received routine care. Results: The protocol was composed of one-page algorithm and detailed guidelines. Algorithm pathway was organized in 3 parts, including screening dysphagia risk, grouping by dysphagia risk level, and assigning nursing care into each group. The nursing care included positioning, oral care, meal time care, diet modification, providing exercise and maneuver, and checking dysphagia signs and symptoms. The experts verified the content validity. Protocol was revised to fit NH practice after the participant observation. Clinical validity was established upon evaluating usefulness, appropriateness, and convenience of the protocol by NH nurses. Dysphagia risk of the intervention group was significantly decreased. Conclusion: Developed protocol will improve the quality of dysphagia care in nursing homes as it can serve as a consistent and integrated standard for nursing care of residents with dysphagia.

Study on Family Caregiving Burden Scale of Dementia-Korea(FCBSD-K) (치매환자 가족부담감의 한국형 도구개발)

  • The purpose of this study was to develop and validate the scale to measure dementia patient's caregiver burden of Korea. In the first phase of the study, 15 caregivers of dementia patients were interviewed to provide narrative data from which items were developed. Initially 65 items were generated from the interview data of 15 caregivers. Content validity was judged by two separate panels of experts with 27 professionals and 30 family caregivers. These items were analyzed through the Index of Content Validity and 33 items were selected which met .80 or more of the CVI. This preliminary FCBSD-K was tested with 207 adult caregivers for reliability and construct validity including item analysis and orthogonal(Varimax) factor analysis. Eight items were deleted because of high or low item-item correlation. The result of the second factor analysis produced six factors that coincided with the conceptual framework posed for the scale developed. The six factors were labeled as 'physio social factor' 'emotional factor' 'family cultural factor' 'role obligation' 'guilt feeling' and 'financial & supportive system factor'. The alpha coefficient relating to internal consistency was .9264 for reliability. In conclusion, cultural factor is related to dementia patient's caregiver burden and FCBSD-K was useful in assessing the dementia patient's caregiver burden in Korea.

Development of the Program Evaluation Measurement of Continuing Nursing Education Programs (간호사 보수교육 프로그램 평가도구 개발)

  • Purpose: This study was done to develop a measurement tool for evaluation of continuing nursing education programs and to verify its validity for effective management and quality of education programs. Methods: The draft of the evaluation measurement was developed from consultation with professionals, focus group interviews targeting groups of nurses, and individual interviews with education program planners. After 6 professionals examined content validity, 46 items were retained. A pilotsurvey was conducted to confirm the time required to complete the questionnaire and the level of understanding of general content and each item in the questionnaire. Construct validity was verified through exploratory factor analysis of data from a survey with 44 items completed by 452 nurses and 59 education program planners. Results: The final evaluation measurement for continuing nursing education programs consisted of 6 evaluation factors and 36 evaluation items. The 6 evaluation factors included identifying program goals and target groups, program planning, performance, operation and management, program outcomes, and program effectiveness. Conclusion: The evaluation measurement for continuing nursing education programs developed in this study is considered suitable to utilize as an evaluation measurement of the quality of continuing education programs for nurses.

The Development of Instrument for Evaluating Web Site about Elementary Science Education (초등학교 과학 교육을 위한 웹사이트의 평가 도구 개발)

  • The purpose of this study was to develop an instrument for evaluating quality of the web site about elementary science education. Eight experienced specialists (three elementary science education specialists and five elementary school teachers engaging) in elementary science education verified content validity twice. The evaluation criteria for elementary school science were considering aspects of the characteristic and the quality of web site. The evaluation criteria can be categorized as being in five areas, fifty-seven items. The five areas were a purpose, a credibility, an efficiency, a data-type, an interactivity. Four web sites were analyzed for the subject of this study. Evaluators were nine elementary school teachers, who were educated in the use of instrument. The developed evaluation instrument in this study is considered valid and reliable. The content validity is 91.1%, and the reliability has a Cronbach's ${\alpha}$ of .82. The developed evaluation instrument in this study will verify the quality of web site, provide teachers and students with efficient information about elementary science education. In addition it suggests guidelines about design and devise web site to instructional developer or subject-matter expert.

Development of a Tool to Measure Maternal Behavior for Health Promotion for Preschool Children with Disabilities (학령전기 장애아동의 건강증진을 위한 어머니의 행위 측정도구 개발)

  • Purpose: The purpose of this study was to develope a tool to assess mothers' behavior related to healt promotion if their preschool children had disabillties. Method: The research design was a methodological study. This tool was developed in 4 stages: first, a content matrix as a conceptual framework was developed using Gordon's 11 functional health promotion patterns and 5 categories (Lee, Jung & An, 1996); second,40 preliminary items were selected through references and interviews with 9 mothers of children with disabilities; third, 2 items were discarded after content validity was done by 3 experts; fourth, reliability and validity were tested with a sample of 243 mothers of children with disabilities who were of preschool age. Results: The final tool consisted of 19 items and Cronbach's alpha coefficient for internal consistency was .82. Five factors were extracted by factor analysis and they explained 55.58% of the total variance. Conclusions: This tool was identified to have a high degree of reliability and validity. So it can be utilized to assess mothers' behavior in providing health promotion for their children who have disabilities.
