Using Quality of Life Scales with Nutritional Relevance after Gastrectomy: a Challenge for Providing Personalized Treatment

  • Purpose: This study evaluated the changes in nutritional status based on quality of life (QoL) item-level analysis to determine whether individual QoL responses might facilitate personal clinical impact. Materials and Methods: This study retrospectively evaluated QoL data obtained by the European Organisation for Research and Treatment of Cancer (EORTC) Quality of Life Questionnaire-Core 30 (QLQ-C30) and Quality of Life Questionnaire-Stomach (QLQ-STO22) as well as metabolic-nutritional data obtained by bioelectrical impedance analysis and blood tests. Patients were assessed preoperatively and at the 5-year follow-up. QoL was analyzed at the level of the constituent items. The patients were categorized into vulnerable and non-vulnerable QoL groups for each scale based on their responses to the QoL items and changes in the metabolic-nutritional indices were compared. Results: Multiple shortcomings in the metabolic-nutritional indices were observed in the vulnerable groups for nausea/vomiting (waist-hip ratio, degree of obesity), dyspnea (hemoglobin, iron), constipation (body fat mass, percent body fat), dysphagia (body fat mass, percent body fat), reflux (body weight, hemoglobin), dry mouth (percent body fat, waist-hip ratio), and taste (body weight, total body water, soft lean mass, body fat mass). The shortcomings in a single index were observed in the vulnerable groups for emotional functioning and pain (EORTC QLQ-C30) and for eating restrictions (EORTC QLQ-STO22). Conclusions: Long-term postoperative QoL deterioration in emotional functioning, nausea/vomiting, pain, dyspnea, constipation, dysphagia, reflux, eating restrictions, dry mouth, and taste were associated with nutritional shortcomings. QoL item-level analysis, instead of scale-level analysis, may help to facilitate personalized treatment for individual QoL respondents.

Saenggangeonbi-tang's Effect on Alcoholic Liver Cirrhosis - 1 Case (생간건비탕가감을 투여한 알콜성 간경변증 환자 치험 1례)

  • We treated a 53 year-old male patient. He had alcoholic liver cirrhosis and diabetes. The main symptoms were fatigue, ascites, edema, anorexia, indigestion, constipation, and abdominal pain. We diagnosed him as 'the retention of pathogenic damp-heat in the liver and gall bladder’ and adminstered Saenggangeonbi-tang three times a day. We observed clinical symptoms, liver function test(AST, ALT, ${\gamma}-GT$, ALP, total bilirubin), nutritional state(total protein, albumin) and capillary blood glucose. After treatment, all the main symptoms disappeared and the patient's nutritional state has improved. Child's grade using the improvement index in liver cirrhosis has changed from B to A. Although the very mechanism of Saenggangeonbi-tang was not proven, we could suggest that Saenggangeonbi-tang is not toxic to the liver and has a good effect on alcoholic liver cirrhosis with diabetes mellitus.

A Survey of College Students' Eating Behavior and Perception related with Acne (대학생의 여드름과 관련된 식생활 태도 및 인식도 조사)

  • The purpose of this study was to know the perception on acne for the college students to provide basic data for the suitable care of acne. Self-administrated questionnaires were completed by 641 college students. Dietary attitude, food habit, general perceptions on acne, knowledge level on acne, and relationship perception between special food and acne were analyzed. The results were as follows. 1. Self reported health status, smoking, obesity index were significantly different by experience of acne. 2. Dietary attitude and food habits were not significantly different by experience of acne. 3. Acne experienced subjects had acne on their face mostly and responded positively to specialized treatment. Proportions of getting the knowledge on acne were 40.6% from friends, 35.4% from magazine and 21.7% from TV or radio. 4. Proportions of correctly answered for the questions about pregnancy, hair cosmetics, oily food, male hormone, family history, constipation related with acne were less than 50% for acne experienced subjects. 92.5% of acne experienced subjects were answered eating chocolate, nut, and fat were related with breaking out and aggravating of acne. Perceptions on breaking out factors and aggravating factors of acne were not significantly different by gender. 5. More than 70% acne experienced subjects stated that instant foods, oily foods, meats, cookies, nuts were related with developing and aggravating acne. Soybean, Seaweed, fermented fish were recognized as less related with developing and aggravating acne.

4 Case Reports of Eumyangyeokchahunobok-byung treated by Herbal medicine based on Shanghanlun provisions (『상한론(傷寒論)』 변병진단체계(辨病診斷體系)에 근거하여 치료한 음양이차후노복병(隂陽易差後勞復病) 증례 4례)

  • Objective : This study is to report 4 Eumyangyeokchahunobok-byung cases diagnosed and treated according to newly hypothesized Eumyangyeokchahunobok-byung definition based on Shanghanlun disease pattern identification diagnostic system. Methods : We analyzed 4 clinical cases diagnosed with Eumyangyeokchahunobok-byung and treated by herbal medicine, which included in Eumyangyeokchahunobok-byung chapter of Shanghanlun according to newly hypothesized definition. Results : Dermatology Life Quality Index decreased from 25 to 10 on first patient, dizziness handicap inventory decreased from 64 to 2 on second patient, dizziness handicap inventory also decreased from 56 to 2 on third patient, and fatigue severity scale decreased from 7 to 2 on fourth patient. Conclusions : Not only all the patients' chief complaints relieved but their other clinical problems such as allergic rhinitis, body pain, constipation, reflux esophagitis also improved. These results highly insist that there is quite a possibility that Eumyangyeokchahunobokbyung means various diseases caused by reversed sleep cycle.

Frequency of Irritable Bowel Syndrome, Perceived Stress, and Mental Health Among Women (서울·경인지역사회 성인여성의 과민성 장 증후군 빈도와 스트레스, 정신 건강에 관한 연구)

  • Purpose: This study was performed to examine the frequency of irritable bowel syndrome(IBS), perceived stress, and mental health among community-dwelling women. Methods: This study conducted a survey to collect data on 869 women in Seoul and Gyeonggi-do. The measurements included the Rome III criteria(to diagnose IBS), the Global Assessment Recent Stress(GARS scale to evaluate perceived stress), and symptom check list 90R(SCL-90R to evaluate psychological distress). Data were analyzed using SPSS 15.0 program by frequency, $x^2$-tests, t-test, and analysis of variance(ANOVA). Results: The frequency of IBS in women was 15.1%. Eighty-six women(65.6%) had mixed constipation and diarrhea subtype of IBS. Compared to the women without IBS, those with IBS reported significantly higher scores on GARS, all subscales of GARS, SCL-90R, all subscales of SCL-90R, and the global severity index(GSI). No significant differences were observed among stress, mental health, and the IBS subtypes. Conclusion: Nursing interventions targeting women with IBS should be developed in order to reduce the associated stress and psychological distress.

A Clinical Study on the Factors that Influence Functional Evaluation of Stroke (중풍의 기능평가에 영향을 미치는 요인에 관한 임상적 고찰)

  • Objectives: This study investigated significant factors that influence functional evaluation of stroke so as to be a fundamental data for estimating prognosis of stroke patients. Methods: 204 patients were studied within 7 days of admission, after the diagnosis of stroke through brain CT scan, brain MRI scan and clinical observations. They were hospitalized in the oriental medical hospital of Dongeui University from February to July in 2001. They were examined at the early stage of onset, after 2 weeks, 4 weeks and 6 weeks, and measured for average mark and the degree of improvement by using the Activity Index. Results: Ischemic stroke, past history of stroke, hypertension, diabetes mellitus, risk factor of obesity, non-professional emergency treatment and hospitalizing time after 1 day from onset, high blood pressure, tachycardia pulse and high blood sugar in abnormal vital sign in acute stage, conscious, cognitive or communication disorder, motor aphasia, dysphagia, constipation for more than 3 days, urinary incontinence, visual field defect, insomnia, and chest discomfort in early stage of onset had a negative influence on functional evaluation. Conclusions: Type of stroke, past history, risk factors, emergency treatment and hospitalizing time after onset, abnormal vital sign and intercurrent symptoms in Acute stage were relevant factors in predicting functional evaluation of stroke.

Clinical practice recommendations for Bangpungtongseong-san (Bofutsusho-san) and Bangkihwangki-tang(Boiogito) in obesity (비만치료 및 체중 감량에서 방풍통성산과 방기황기탕 사용에 대한 임상 권고안)

  • Objectives These prescribing recommendations have been written to guide clinicians on the appropriate use of Bangpungtongseong-san(BT) and Bangkihwangki-tang(BH) in the treatment of obesity. These recommendation are aimed at providing evidence based information concerning diagnosis and management of obesity. Methods We collected all relevant references about treatment effect of BT and BH on obesity in the forms of meta-analysis, systematic review, randomized controlled trial, case-control study, observational study and practice guideline from international and domestic databases and paper journals. We examined treatment effect, side effects, recommendations for dose, indication and contraindication of BT and BH. Results The treatment effect of BT and BH on obesity has been proved through clinical trial. BT is indicated for obese patients (Body mass index, $BMI{\geq}25$) with strong abdomen and a tendency to constipation, BH is indicated for obese patients ($BMI{\geq}25$) with a fair skinned, soft muscled, edematous and sweat easily. Conclusion We wish the information contained in theses recommendations will help clinicians reach a reasonable and beneficial decision with evidence-based results. Further studies are strongly needed to develop better treatment strategies for herbal medicines on obesity.

Effect of Middle-Aged Food Habit and Preference on Health Status (중년기의 식습관 및 기호가 건강상태에 미치는 영향)

  • This research was attempted to investigate the correlation between food habits as well as preferences and health conditions of middle-aged people. 380 people living in Dae-gu, kimch$\breve{o}$n and k$\breve{u}$mr$\breve{u}$ng gun in the 40's or 50's were selected for this research as subjects from July 1 to August 15, 1990. The data analysis was made by way of frequency, percentage, ${\chi}^2-test$ and Pearson correlation using SAS package. The summarized results are as follows. 1. Among the total subjects of this research, 200 people (52.6%) were male and 180 people (47.4%) were female. The regional distribution was like this; 115 people (30.3%) lived in large city, 154 people (40.5%) in small and medium city and 111 people (29.2%) in rural region. 2. The survey on food life attitude on the subjects showed that they had a relatively good attitude, and there were little significant differences (p<.05) between men and women. But there were significant differences (p<.001) between rural region and city. 3. On the preferences for taste, food and cooked food the subjects showed the lowest preferences for processed milk-fat food and the highest for soybean and processed soybean food. 4. The research on health condition 1) 30.8% of the subjects were obesity. And this research showed that the percentage of obesity was higher among men (33.5%) than women (27.8%), and higher in cities (40.4, 34.4%) than rural region (16.2%), (p<.05, p<.01). 2) 90.8% of the subjects showed negative according to Diabetes inspection. 3) 2.0% of the subjects were hypertensive, and the percentage was higher among women than men, and higher in rural region than cities. 4) 12.6% of the subjects were anemia, and the percentage was higher among women (17.8%) than men (8.0%), and higher in rural region (23.4%) than cities (7.0, 9.1%). 5) DMFT index and DMF rate of dental caries was higher among women (DMFT index; 10.6 DMF rate; 88.9) than men (DMFT index; 7.3 DMF rate; 81.5), and higher in rural region (DMFT index; 11.8 DMF rate; 90.1) than cities (DMFT index; 7.4, 7.9 DMF rate; 79.1, 85.7). 6) According to the survey on self-diagnosed health status of subjects, the percentages of articular·neuralgia (48.9%) was the highest. And that of stomach digestion troubles (31.1%), headache (22.4%), anxiety excitement (12.9%), spastic constipation (12.4%), insomnia (9.7%), melancholia (7.9%) and etc (1.6%) followed. 7) People had allergied food which contained animal protein such as pork, chicken, mackerel, siakworm pupa, clam and so on. 8) In female cases, 46.7% of women became already menopausal and 13.3% of them was under menopause. 5. This research showed that there were significant correlation between food life attitude and health condition in obesity, anemia and dental caries but not in blood pressure. 6. And this research also showed that there were significant correlations between food preference and health condition in obesity, anemia and dental caries but not in blood pressure.

A Study on the Changes of Early Puerperium Symptoms and Blood Stasis Index of Women Taking Gungguijohyeol-eum-gamibang (궁귀조혈음가미방(芎歸調血飮加味方)을 복용한 여성의 산욕 초기 증상과 어혈 변화에 대한 연구)

  • Objectives: The purpose of this study was to investigate the changes of early puerperium symptoms and Blood Stasis Index in women during the first two weeks after childbirth. Methods: 38 women staying at Korean postpartum management facilities were studied. Questionnaires were filled out on the date of arrival and departure. Changes in overall physical conditions including neuropsychiatric, circulatory, digestive, musculoskeletal, and urogenital and breast symptoms as well as Blood Stasis Pattern were studied. Results: 1. Neuropsychiatric symptoms were shown in order of Depressed Feeling, Insomnia, Dizziness, Headache, and Easy to Anger and Nervous on arrival. All symptoms showed improvement after two weeks, without significance. 2. Circulatory symptoms were shown in order of Excessive Sweating, Edema, Fatigue, Hot Flash, Anemic Feeling, Alternating Chills and Fever, Cold Hypersensitivity, Chills, Oppression in the Chest, and Palpitation on arrival. Hot Flash, Excessive Sweating, and Edema showed significant improvement after two weeks. 3. Digestive symptoms were shown in order of Dyspepsia, Dry Mouth, Constipation, and Loss of Appetite. All symptoms showed improvement after two weeks, without significance. 4. Pain of Muscles & Joints showed improvement after two weeks, without significance. Pain of Teeth & Gingiva showed slight worsening after two weeks, without significance. 5. Urogenital and Breast symptoms were shown in order of Wound Pain, Fever and Distending Pain in the Breast, and Lower Abdominal Pain on arrival. All symptoms showed significant improvement after two weeks. 6. Blood Stasis Index showed significant improvement after two weeks. Conclusion: Most symptoms were improved in women staying at Korean postpartum management facilities for two weeks. Hot Flash, Excessive Sweating, Edema, Wound Pain, Lower Abdominal Pain, Fever and Distending Pain in the Breast and Blood Stasis Index showed significant improvement.

Correlation among Dietary Habits Score, Life Stress Score and Health-Related Quality of Life (HRQL) Score for Female College Students with Functional Constipation (기능성 변비를 가진 여대생의 식습관, 생활 스트레스 및 건강관련 삶의 질의 관계)

  • The study purpose was to investigate correlations among dietary habits score, life stress score and health-related quality of life (HRQL) score for female college students with functional constipation (FC). The data were collected in 2008 from 265 female college students: 86 with FC and 179 without FC. The diagnosis of FC was based on the Rome II criteria, and life stress and HRQL were assessed using self-administered life stress questionnaire and SF-36, respectively. The correlations among dietary habit score, life stress score, and HRQL score were evaluated. The averages of age, height, weight, body fat percentage and body mass index (BMI) of female college students were 20.8 years, 160.9 cm, 53.7 kg, 27.8%, and $20.7\;kg/m^2$, respectively. The average dietary habit score of "having meals with diverse foods" was significantly lower in the FC group than in the normal group (p < 0.05). The average faculty problem (p < 0.05), grade problem (p < 0.05), economy problem (p < 0.05), value problem (p < 0.05), and total stress (p < 0.01) scores of the FC group were significantly higher than those of the normal group. The average HRQL score was 70.4 in the FC group and 74.3 in the normal group (p < 0.05). There were significantly negative correlations between total stress score and total food habits score (r = -0.221; p < 0.01) and HRQL score (r = -0.539; p < 0.01) in the FC. These results suggest that female students with FC are highly perceptive to stress and have low HRQL. In addition, female students with FC may require nutritional education about eating diversity.
