교정치료 전 환자의 측두하악관절에 관한 방사선학적 연구 (Tomographic study of temporomandibular joints before orthodontic treatment)

  • Purpose : To determine whether there is a relationship between abnormal temporomandibular joint radiographic findings and age or gender in a sample of preorthodontic patients. Materials and Methods : Individualized corrected sagittal tomographs of 188 temporomandibular joints in 94 preorthodontic patients, aged 11 to 29 years, were taken and viewed by an observer blinded to clinical records. The study population was divided into 2 aged groups (11-15 years and 16-29 years) for comparative purposes. Temporomandibular joint radiographic findings were classified as normal or abnormal. Results: There was no difference in ratio of abnormal to normal findings between the both aged groups. The frequency of osseous abnormalities was similar with that of abnormalities of condylar position. Abnormalities in CP and JS were most frequent in all aged groups. Conclusion : There is no significant difference between ages or genders for temporomandibular joint radiographic abnormalities.

Cone Beam CT를 이용한 하악 과두의 골 이상에 대한 방사선적 분석 (Radiographic examination of the Osseous Abnormalities of the Mandibular Condyle Using Cone Beam Computed Tomography)

  • 본 연구의 목적은 연령과 성별 및 CT 촬영 목적에 따른 하악 과두의 골 이상에 대한 양상과 발병률 그리고 호발 부위를 살펴보고, 촬영 목적에 있어서 TMD 진단을 위한 그룹과 그 외의 그룹간의 발병 양상을 비교해 보기 위함이다. 원광대학교 산본 치과 병원으로부터 $i-CAT^{TM}$ Cone Beam Computed Tomography를 이용하여 촬영된 199명의 3차원 CT 영상을 수집하고, 촬영된 MPR 이미지는 하악 과두의 연속된 시상 이미지와 관상 이미지를 얻기 위하여 TMJ 모드로 전환되었다. 3명의 관찰자는 이를 토대로 골 이상의 양상 및 발병률, 호발 부위, 그리고 하악 과두의 전반적인 외형에 대해 각기 독립적으로 판독한 결과 type F가 type N보다도 훨씬 흔하게 관찰 되었다. 시상 이미지에서의 호발 부위는 type D-C를 제외하고는 하악 과두의 전상방 및 상방 부위였으며, type D-C에서는 후상방 부위가 가장 흔하였다. 관상 이미지에서는 type E를 제외하고는 외상방 및 상방 부위에 호발하였으나, type E에서는 내상방 및 상방 부위에서 빈발 하였다. 하악 과두의 골 이상 소견은 type D-C를 제외하고는 TMD 진단을 위한 그룹에서 보다 높게 나타났으며, 그 외의 그룹에서는 type N 및 type D-C가 보다 높게 나타났다. 본 연구에서는, 하악 과두의 골 이상 소견을 6가지 분류해 보았으며, 이러한 골 이상 소견은 각기 빈발하는 부위가 있는 것으로 나타났다. 그리고 TMD 진단 그룹이 type D-C를 제외하고는 골 이상 소견이 더 높게 나타나는 경향을 보였다.

한국인 부정교합자의 측두하악장애(TMD) 유병율과 그 기여요인에 관한 연구 (The Prevalence Study of TMD and the Associated Factors in Korean Malocclusion Patients)

  • 서울대학교병원 치과 교정과에 내원한 6세 1개월에서 45세 8개월사이(평균 19세 6개월)의 부정교합자 205명(남 67명, 여 138명)을 대상으로 TMD 증상과 기여요인에 관한 설문조사와 Orthopantomogram, 경두개 방사선 사진(transcranial view)의 판독, 교정모형 검사 등을 시행하여 다음과 같은 결론을 얻었다. 1. Helkimo Anamnestic index는 Ai 0 $46.8\%$, Ai I $22.0\%$, Ai II $31.2\%$로 TND의 주관적 증상이 연령에 따라 증가하는 추세(p<0.001)를 보였으며 여성에서 높았다(p<0.05). 2. Orthopantomogram상 하악두의 이상소견은 flattening($4.4\%$)이 가장 많았으며 전체적으로 한가지 이상의 이상소견을 보인 경우는 $8.3\%$였다. 3. 목과 어깨의 동통(p<0.001) 이악물기, 입술을 깨무는 습관(이상 p<0.01). 두통(p<0.05)이 있는 경우 Ai가 높았다. 4. Angle II급에서 Orthopantomogram상 하악두 이상소견의 빈도가 높았고, 상악 대구치의 교모가 있을때 Ai가 높은 경향이 있었다(이상 p<0.05). 그외의 교합 요소는 TMD의 증상과 무관하였다. 5. 하악두 길이(Co'-Inc')와 하악지 길이(Co'-Go')가 짧고 하악두의 비율(Co'-Inc'/Co'-Go')이 낮을때, 그리고 좌우 하악지 길이 (Co'-Go'), 하악두 비율(Co'-Inc'/Co'-Go')이 다를때 Orthopantomogram상 하악두 이상소견이 많았다.

초기 편마비 보행 환자에서 양측 무릎의 초음파 검사 소견 (Ultrasonographic Findings of Both Knee in Hemiplegic Ambulators with Recent Stroke)

  • To evaluate of the ultrasonographic changes in hemiplegic and unaffected knee joints of hemiplegic ambulators with recent onset stroke. Twenty patients (40 knees) with prevalence duration of 14-39 weeks (mean 24.70 weeks) after a stroke insult were included. All participants were walking independently without leg dragging, had no previous knee injury. There were significant differences in intercondylar cartilage thickness and pes anserinous tendinopathy (PAT) between hemiplegic and unaffected knees (p<0.05). There were no differences in cartilage thickness of medial condyle and lateral condyle, patellar tendinitis/bursitis, suprapatellar effusion, synovitis, joint space narrowing, and Baker's cyst of ultrasonographic findings and x-ray findings in hemiplegic knee compared to the control except PAT (p<0.05). In hemiplegic knee, medial or lateral condylar cartilage thickness was significantly correlated with body weight, intercondylar cartilage thickness, and PAT (p<0.05). PAT was significantly correlated with Brunnnstrom stage and condylar cartilage thickness (p<0.05). Knee pain was significantly correlated with lateral condylar cartilage thickness (p<0.1). The hemiplegic knee had more ultrasonographic abnormalities including PAT and more cartilage thickness preservation in patient with recent onset stroke. Intraarticular sonographic findings including cartilage thickness was significantly correlated with extraarticular findings including PAT and a symptom of knee pain.

측두하악관절내장증에서 하악과두운동과 측두하악과절조영상의 상호관계 (CORRELATION OF CONDYLAR MOBILITY AND ARTHROTOMOGRAPHY IN PATIENTS WITH INTERNAL DERANGEMENTS OF THE TEMPOROMANDIBULAR JOINT)

  • Arthrography of the temporomandibular joint is a useful method of demonstrating the soft tissue abnormalities related to disc dysfunction. In this study, 19 asymptomatic joints, 31 joints with anterior disc displacement with reduction, 31 joints with anterior disc displacement without reduction which were classified by arthrotomography under fluoroscopy were evaluated to determine the linear measurement of anterior recess of inferior joint space and the relationship between the condylar anterior translation and the severity of the internal derangements. Their fluoroscopic images were also evaluated to describe the characteristics of condylar paths in internal derangements of the temporomandibular joints. The results were as follows; 1. The mean lengths of the anterior recess in asymptomaic group. reduction group. and non-reduction group were 8.7±1.6㎜. 11.2±1.7㎜, 12.8±1.7㎜ respectively. The length of the anterior recess was increased according to the severity of the internal derangements(P<0.05). 2. Linear measurements of anterior movement of condyle on maximum mouth opening were 13.1 ±4.2㎜, 15.9±4.1㎜, 5.0±3.7㎜ in asymptomatic group. reduction group, and non-reduction group respectively. Compared with asymptomatic group, reduction group showed hypermobolity of the condyle and non-reduction group showed hypomobility. 3. Condyles moved beyond the crest of articular eminence in 80% of reduction group and did not reach it in 70% of non-reduction group. 4. The condyle moved mainly superiorly in reduction group(66%) and horizontally in asymptomatic group(47%). There were no cases to move superiorly in non-reduction group.

구강검진의 효과 증진을 위한 파노라마방사선사진의 필요성에 관한 연구 (Usefullness of panoramic radiograph for the improvement of periodic oral examination)

  • Purpose : This study was designed to evaluate the efficacy and utility of panoramic radiograph for the improvement of the periodic oral examinations. Materials and Methods : Clinical examinations and panoramic examinations were done for the 242 subjects of oral examinations. The results of panoramic radiograph interpretation were compared with the clinical findings. Two questionnaires were created. One was carried out before the panoramic examination and the other done afterwards, to find out the subjects' cognition and satisfaction for the clinical and panoramic examinations. Results : 1. Panoramic findings showed a higher detection rate of 31.9% for periodontal diseases, and 23.1% for dental caries than clinical findings. 2. The additional abnormalities detected through panoramic examinations were impacted tooth in 81 subjects (33.6%), maxillary sinus abnormalities in 28 subjects (11.6%), condylar abnormalities in 5 subjects (2.1%), congenital and acquired dental anormalies in 59 subjects (24.5%), and other miscellaneous abnormalities in 34 subjects (14.1%). 3. 164 subjects (67.8%) were satisfied with the current periodic oral examination, and 75 subjects (31.1%) hoped for better accuracy. 4. In the first and second questionnaire, 154 subjects (67.0%) and 163 subjects (70.6%) responded respectively that panoramic examination was necessary, and 193 subjects (83.2%) responded that it actually helped. Conclusion : The panoramic examination was revealed to improve the effectiveness of the periodic oral examination and to increase the satisfaction of the subjects of examination.

Osseous changes in the temporomandibular joint in rheumatoid arthritis: A cone-beam computed tomography study

  • Purpose: To evaluate osseous changes of temporomandibular joint (TMJ) in patients with rheumatoid arthritis (RA) using cone-beam computed tomography (CBCT) and to correlate the imaging findings with the severity of TMJ dysfunction, clinical findings, and laboratory findings. Materials and Methods: This study consisted of 28 subjects, including 14 RA patients and 14 controls, who were scheduled to undergo CBCT imaging for the diagnosis of a complaint not related to or affecting the TMJ. The Fonseca's questionnaire was used to assess the severity of TMJ dysfunction. Rheumatoid factor (RF) and the erythrocyte sedimentation rate (ESR) were assessed in the RA patients. CBCT was then performed in all subjects and osseous TMJ abnormalities were assessed. Results: According to the Fonseca's questionnaire, 14.3% of the patients had no TMJ dysfunction, while 50%, 21.4%, and 14.3% had mild, moderate, and severe dysfunction, respectively. RF was positive in 64.3% of patients, and the ESR level was high in 100%. Imaging findings revealed a statistically significantly higher prevalence of erosion (85.7%), flattening (89.3%), osteophyte formation (32.1%), subchondral cyst (32.1%), sclerosis (64.3%), and condylar irregularities (28.6%) in the RA patients than in the controls. No correlations were found between CBCT findings and the clinical findings, the severity of TMJ dysfunction, disease duration, or laboratory results. Conclusion: RA patients might show extensive osseous abnormalities with no/mild clinical signs or symptoms of TMJ dysfunction that necessitate TMJ imaging for these patients. CBCT is a valuable and efficient modality that can assess osseous TMJ changes in RA patients.

심한 안모 비대칭 환자 치험 2례 (TREATMENT OF SEVERE FACIAL ASYMMETRY:REPORT OF 2 CASES)

  • Classification of facial asymmetry has not been yet well-organized because of their variety on etiologic factors, involved sites and clinical expressions. And surgical treatments are also variable and depend upon their causes and clinical abnormalities. This is a case report on surgical experiences of two patients who had severe facial asymmetry and could not treated pre-surgical orthodontics before surgery. One patient was belong to hemifacial microsomia and another was a very unusual complex type related to unilateral condylar hyperplasia, unilateral macrognathia and unilateral mandibular hypoplasia. The authors used a simultaneous two-jaw surgery, bone shaving and onlay-type bone graft in former case, and a simultaneous two-jaw surgery, condylectomy, bone shaving and only-type bone graft in latter case. In two cases, immediate post-operative results in function and esthetics were excellent, however, progressive resorption of onlay-type bone grafts have been noticed.

악관절내장증을 동반한 Eagle's Syndrome (A CASE REPORT OF EAGLE'S SYNDROME WITH TMJ INTERNAL DERANGEMENT)

  • A thirty two year-old male was referred with a chief complaint of mouth opening limitation, and maxillofacial pain including left TMJ area. The patient had been treated in a private clinic with medications and conservative treatments, without any improvements in symptoms. MRI findings showed a limitations in condylar head movements, and signs of disc adhesion without the Positional change. Panoramic views showed elongation of stylohyoid process. Brain C-T was taken due to the patient's complaint of headache, facial pain, and paresthesia of tongue, and the result showed no abnormalities. The first surgery included meniscoplasty and the removal of disc adhesion of left TMJ under the preliminary diagnosis of internal derangement with adhesion, and the patient displayed marked improvements in opening movements despite of persistent hemiparesthesia of left facial areas and tongue, and mild dysphasia. Therefore, the second surgery was carried out with the preliminary diagnosis of Eagle's Syndrome, and a resolution of neurologic symptoms was obtained. The final diagnosis of this case was Eagle's Syndrome with internal derangement, and the difficulties in diagnosis and treatment might be due to the existence of two separate diseases with similar symptoms.

치료의치와 CAD-CAM 기술을 이용한 불안정한 하악위를 가진 완전 무치악 환자의 치료 증례 (Full mouth rehabilitation in edentulous patient with unstable mandibular position using flat table treatment dentures and CAD-CAM technology)

  • 과두 골절 및 형태 이상으로 인해 불안정한 하악 운동 및 비정상적인 안모를 갖는 무치악 환자의 경우 환자의 안정된 하악위를 찾는 것이 중요하다. 본 증례에서는 flat table 치료의치를 이용하여 환자의 안모개선, 하악 운동의 안정, 의치의 안정성이 향상시켰다. 또한 flat table 치료의치의 수직고경 및 구순 지지도를 유지하고자 CAD-CAM 기술을 이용하여 제작한 총의치 수복을 통해 환자의 불편감을 개선하는 양호한 결과를 얻었기에 이를 보고하는 바이다.
