The Role of Surgery in the Treatment of Spinal Myeloma

  • Objective: Spinal myeloma has been treated with radiation therapy and chemotherapy. However, the role of surgery was not fully evaluated. This study is performed to evaluate the efficacy of surgery in the treatment of spinal myeloma. Methods: 22 patients who were treated with surgery for spinal myeloma from August 1999 to April 2003 were analyzed. Radiological finding, surgical methods and result were reviewed in retrospective study. For compression fracture due to myeloma infiltration, percutaneous vertebroplasy(PVP) was done. Decompression surgery with or without fixation was performed for patients with neurologic deficit. The modalities of surgery consist of PVP (14 cases), corpectomy and fixation (7 cases), and laminectomy and epidural mass removal (3 cases). To evaluate clinical outcome, visual analogue pain score and Frankel neurological scale were used. Results: In 14 cases of PVP, total 57 vertebral segments were treated including 21 thoracic vertebral bodies and 36 lumbar vertebral bodies. Pain relief was achieved in all cases. The pain score changed from 7.7 (preoperatively) to 2.5 (postoperatively). And pain relief effect was maintained over than one year. Frankel grade improved in decompression cases. Conclusion: Surgical treatment can alleviate pain and improve neurologic deficit immediately in spinal myeloma patients.

  • 최근 주 5일제 근무 실행과 사회복지의 확산, 여가 선용에 대한 욕구와 더불어 건강증진에 대한 싸이클이 주목을 받고 있음. 싸이클웨어(Cycle Wear)에 요구되는 개발요소는 경기력 향상을 위한 기능적 요소와 패션성을 부여하는 심미적 요소로 크게 구분 가능하며 해외에서는 기능성과 패션성이 적절하게 조화를 이룬 제품을 계속 출시되고 있음. 본 연구에서는 운동 시 발생하는 열을 흡한속건 기능으로 효과적으로 발산하고 동절기에는 보온 기능을 갖는 세섬도 하이멀티 OY형 이형단면사 및 잠재권축사를 이용한 고신축 환편 및 경편물 개발하고, 극한환경에서도 고견뢰도를 유지할 수 있는 섬유의 염색법 및 기능성 발현 가공법의 적용, 내마모성과 필링이 우수한 아라미드+나일론 복합가공사 신축직물 제직 및 염색가공 공정 개발을 통해 기능성을 발현할 수 있는 싸이클 웨어 원단을 개발하였음. 또한 종래 Compression Wear에만 적용하던 테이핑 요법을 응용한 근력강화형 싸이클웨어 패턴과 디자인 개발을 통해 다양한 형태의 근력강화형 싸이클웨어를 개발하였으며, 무산소파워, 유산소파워, 젖산분석, EMG 분석 및 에너지 대사분석 등의 운동능력 성능평가를 통해 테이핑 요법이 적용된 싸이클웨어의 근력강화 효과를 확인하였음. 이와 같이 개발된 싸이클웨어는 무산소파워, 유산소파워, EMG 분석에서 각각 근력강화 효과를 보였으며, 피로물질인 젖산의 경우는 발생의 정도가 낮게 나타났음. 또한 여성에 비해 남성의 근력강화 효과가 크게 나타나는 경향을 보였음.

Therapeutic Efficacy of Pulsed Radiofrequency Treatment in Lumbar Radicular Pain (요추부 신경근통 환자에서의 박동성 고주파술의 치료 효과)

  • Background: Lumbar radicular pain is a frequent and often debilitating event. Although many treatment methods have been described in several studies, the available evidences regarding efficacy is not sufficient enough to draw definitive conclusions on an optimal therapy regime. Pulsed radiofrequency (RF) treatment was found to exert a beneficial effect on intractable radicular pain in individuals. The purpose of this study was to assess the efficacy of pulsed RF of the dorsal root ganglion for chronic lumbar radicular pain. Methods: Twenty five patients with chronic lumbar radicular pain that was refractory to selective nerve root blockage met the inclusion criteria of our study and received pulsed RF treatment. The average numeric rating scale (NRS) for leg pain during usual activities and the Oswestry disability index (ODI) were measured at 1 and 3 months after the procedure. Results: Of the 25 patients accepted for pulsed RF treatment, one dropped out due to a vertebral compression fracture during this study. ODI and NRS showed a positive trend in favor of the pulsed RF treatment. No significant complications were observed during the study period. Conclusions: It appears that pulsed RF treatment of the lumbar spinal dorsal root ganglion may be an effective treatment method for patients suffering from lumbar radicular pain, and who were not responsive to selective nerve root blockage.

Clinical Analysis of Decompressive Craniectomy for Acute Massive Cerebral Infarction (악성 뇌경색증에 대한 감압 두개골절제술의 임상분석)

  • Objective : Massive cerebral infarction could be accompanied by severe brain swelling and death secondary to transtentorial herniation. Approximately 10% to 15% of middle cerebral artery infarctions are associated with this phenomenon. However, the effectiveness and timing of decompressive surgery are still controversial. In this study, we present our results on the effect of decompressive craniectomy in life threatening cerebral infarction. Method : We retrospectively analyzed 15 patients who underwent decompressive craniectomy for massive cerebral infarction from January 1997 to April 1999. Surgical indication was based on the clinical signs such as neurological deterioration, pupillary reflex, and radiological findings. Clinical outcome was assessed by Glasgow Outcome Scale (GOS). Results : All 15 patients(five men, ten women ; mean age, 52.3 years ; right 11, left 4) were treated with wide craniectomy and duroplasty. The average time interval between onset of symptom and surgical decompression was 2.9 days. Clinical signs of uncal herniation(anisocoria, or fixed and dilated pupils) were presented in 13 of 15 patients. Mean Glasgow coma scale(GCS) was 12.4 points on admission, 8.1 points on preoperative state and 11.8 points postoperatively. Overall outcomes were favorable in 5 cases(Glasgow outcome scale : GOS I, II), unfavorable in 6 cases(Glasgow outcome scale : GOS III, IV) and dead in 4 cases. Conclusion : Early decompressive craniectomy before brain stem compression is considered as an effective lifesaving procedure for massive cerebral infarction unresponsive to aggressive medical therapy.

The Comparison of Effectiveness between Near Acupuncture Point Needling and Remote Acupuncture Point Needling on Treating Ankle Sprain of Acute Stage (급성기 족관절 염좌의 근위취혈(近位取穴)과 원위취혈(遠位取穴)의 치료 효과 비교 연구)

  • Objective : The purpose of this study is to compare the effects of near acupuncture point needling and remote acupuncture point needling on treating ankle sprain of acute stage. Methods : From March 1st, 2007 to May 30th, 2007, the 50 patients who had visited Iksan oriental medical hospital, Wonkwang university with acute ankle sprain were divided into 2 groups ; one group took near acupunture point needling, and the other group took remote acupunture point needling. Both group had been treated with the same additional rest, ice, compression and elevation(RICE) therapy. To evaluating the efficiency of each treatment, Visual Analog Scale(VAS) and Ankle-Hindfoot Scale(AHS) were applied before 1st treatment and after 3rd treatment. Results : As a result of evaluation by using AHS, the score change comparison between the two groups had no significance after the treatment. As a result of evaluation by using VAS, treatment score of remote acupucture point needling was marked lower than score of near acupucture point needling. Conclusion : Remote acupucture point needling is more effective than near acupucture point needling in controlling the pain of acute ankle sprain.

  • An inflammatory myofibroblastic tumor (IMT) is a rare disease entity reported to arise in various organs. It is thought to be a neoplastic or reactive inflammatory condition, controversially. The treatment of choice for myofibroblastic tumor is surgery, and recurrence is known to be rare. The optimal treatment method is not well-known for patients ineligible for surgery. We report a 47-year-old patient with aggressive recurrent IMT of the lungs. The patient had been admitted for an evaluation of back-pain two years after a complete resection of pulmonary IMT. Radiation therapy was performed for multiple bone recurrences, and the symptoms were improved. However the patient presented again with aggravated back-pain six months later. High-dose steroid and non-steroidal anti-inflammatory drugs were administered, but the disease progressed aggressively, resulting in spinal cord compression and metastasis to intra-abdominal organs. This is a very rare case of aggressively recurrent pulmonary IMT with multi-organ metastasis.

Deep Vein Thrombosis Due to Hematoma as a Rare Complication after Femoral Arterial Catheterization

  • Hematoma is quite a common complication of femoral arterial catheterization. However, to the best of our knowledge, there have been no previous studies regarding deep vein thrombosis (DVT) caused by compression of a vein due to a hematoma. We report a case of a hematoma developing after femoral arterial catheterization and causing extensive symptomatic DVT. A 59-year-old male was seen in our Emergency Department with right lower leg swelling 15 days after coronary stent implantation performed using right femoral artery access. Computed tomographic (CT) scanning revealed a large hematoma (45 mm in its longest diameter) compressing the common femoral vein and with DVT from the right external iliac vein to the popliteal vein. Due to the extensive DVT involvement, we decided to release the compressed common femoral vein by surgical evacuation of the large hematoma. However, even following evacuation of the hematoma, as the DVT did not resolve soon, further mechanical thrombectomy and catheter-directed thrombolysis were performed. Angiography then showed nearly resolved DVT, and the leg swelling was improved. The patient was discharged with the anticoagulation medication, warfarin.

  • Objective : Ossification of ligaments within spinal canal, i.e., OPLL and OLF, is uncommon clinical entity as a cause of the progressive compression myelopathy or radiculopathy. More and more cases being reported in the field of occidental medicine, but very few cases have been reported in the field of oriental medicine. The purpose of this study is to report on oriental medical approaches to OPLL and OLF. Methods : Subjects of this study are 3 cases of OPLL and 3 cases of OLF who visited Won-kwang oriental medical hospital(Dept. of acupuncture and Moxibustion) from May, 2002 to October, 2003. These patients undergo oriental medical treatment such as acupuncture, cupping, Bee-Venom therapy and herbal medication and so on. We made a comparison JOA scores between before treatment and after treatment and we evaluated results of treatment. Results : The results of treatment in these six cases are as follows ; One case was evaluated 'Excellent', one case was evaluated 'Good', two cases were evaluated 'Fair' and two cases were evaluated 'Failure'. One of these cases had a surgical operation after discharge from this hospital. Conclusions : After oriental medical care for these cases, there are some improvements such as decrease of pains, relief of myelopathy etc. But, it had little effect on some cases, therefore we considered that more special study to find various and effective methods of oriental medical treatment for these diseases should be made.

  • Our experiment studied that grafted stem cells reduced behavioral deficiency in rodent animal models of clip compressive surgery inducing spinal cord infarction. Our research proved the effect of embryonic stem cells to the spinal cord infarction caused by compressing T9-10 with an aneurysm clip, focusing the application of grafted stem cells for reduction of infarction and regeneration of spinal cord nervous injury. Therefore, our research suggests manifest results that implantation of mouse embryonic stem cell could show behavioral improvement after severe spinal cord damage. Therefore, mouse embryonic stem cell (mESC) could be useful application for the method in neurological injury. Conclusively, stem cell implant therapy may enhance the effectiveness of stem cell implant for central nervous system injury.

  • Meralgia paresthetica is a rare femoral disease and various symptoms appear such as pain, numbness, and paresthesia in the anterolateral thigh due to entrapment of the lateral femoral cutaneous nerve. We treated the meralgia paresthetica patients with Korean medicine treatment including herbal medicine, acupuncture, Chuna manual therapy and pharmacopuncture during 12 days. Numerical rating scale (NRS), Euroqol five dimension (EQ-5D) index, and the changes of symptoms were measured for assessment. After 12 days inpatient treatment, NRS decreased from 7 to 4, EQ-5D index and the symptoms of the patient also were improved. In conclusion, this case shows Korean medicine treatment might be an effective treatment for Meralgia paresthetica.
