Treatment of Anterior Glenoid Rim Fracture with Comminuted Fragment Using Arthroscopic Reduction and AO Headless Compression Screw Fixation - A Case Report - (관절경하 AO 무두 압박 나사를 이용한 견갑골 전방 관절와 분쇄 골절의 치료 - 증례 보고 -)

  • Purpose: We present a case of anterior glenoid rim comminuted fracture that was treated with arthroscopic reduction and an AO headless compression screw (HCS) fixation. Materials and Methods: A 31-year old man complained of left shoulder pain after falling down on stairs. The anterior glenoid comminuted fragments were arthroscopically reduced. Fixation with an AO HCS was done after placement of 1.1 mm Kirschner wire as a guide pin through a standard cannulated anterosuperior portal. Results: Twelve months after the operation, union of the fracture was achieved and the range of motion was fully recovered. He did not complain of any discomfort during his activities of daily living. Conclusion: An AO HCS had various screw sizes and this was good for fixation of a small glenoid fracture and a long drill bit and screw driver were useful for fixation of deep seated glenoid fracture. A short guide wire could be replaced by a 1.1 mm K-wire. An AO HCS was useful for fixation of an anterior glenoid rim comminuted fracture.

  • The Discrete Element Method, DEM, is increasingly used in fracture studies of non-homogeneous continuous media, such as rock and concrete. A 2D circular rigid DEM formulation, developed to model concrete, has been adopted. A procedure developed to generate aggregate particles with a given aspect ratio and shape is presented. The aggregate particles are modelled with macroparticles formed by a group of circular particles that behave as a rigid body. Uniaxial tensile and compression tests performed with circular and non-circular aggregates, with a given aspect ratio, have shown similar values of fracture toughness when adopting uniform strength and elastic properties for all the contacts. Non-circular aggregate assemblies are shown to have higher fracture toughness when different strength and elastic properties are set for the matrix and for the aggregate/matrix contacts.

  • In metalforming, ductile fracture criteria have been used for the purpose of predicting fracture occurrence in the stage of process design prior to manufacturing. In the present investigation, some of popular criteria are reviewed to find the most suitable one among them. As a result, it is found that the modified Cockroft and Latham criterion is better than others. The reasons are: it agrees with Roy's and McClintock's void growth models, it is more general than Oyane's and Kuhn's criteria, and it predicts fractures in compression as well as in tension well. However, it is also found that the criterion is incapable of predicting fractures in torsion.

  • Osteoporosis is a worldwide problem and it mainly affects postmenopausal women. Osteoporosis associated with pregnancy or lactation is a rare condition. The incidence and mechanism of this phenomenon has not been clarified, but it can cause one or more vertebral compression fractures with severe, prolonged back pain in the affected women. We experienced this uncommon case, treated it with percutaneous vertebroplasty. A 35-old-woman visited our hospital with complaints of severe back pain and flank pain 2 months after normal vaginal delivery. She was diagnosed with osteoporotic vertebral compression fractures on the T5, 8, 9 and 11 vertebral bodies and we performed percutaneous vertebroplasty on the T8, 9 and 11 vertebrae with a good result. We present here an unusual case of pregnancy-associated compression fractures treated by percutaneous vertebroplasty.

  • The effects of a -phase morphology on the static and dynamic deformation behavior of a Ti-6Al-4V alloy was investigated in this study. Static tension test, static and dynamic tension test and hot compression test were conducted on three microstructures of Ti-6Al-4V alloy, i.e., equiaxed, widmanstatten and bimodal microstructures. Fracture surfaces of all three microstructures represented ductile fracture appearance, though the formation of adiabatic shear bands was noticed at dynamic torsion test. The susceptibility of forming adiabatic shear bands was greatest in the equiaxed microsoucture and lowest in the bimodal microstructure, which was evidenced by hot compression test.

  • Although the incidence of epidural abscess is rare, once it occurs, its high morbidity rate and high mortality rate create a great deal of serious sequalae for these patient, if this condition is not diagnosed in time. We experienced a case of epidural abscess after performing percutaneous vertebroplasty in a patient who had a lumbar spinal compression fracture. This case will remind the pain clinician of the possibility of epidural abscess after such a procedure.

  • Psoas compartment block has been used to provide anesthesia and analgesia of hip joint. This block is advocated for relief of pain of various origins in the thigh, leg and lumbar area. A-40-year-old women complained of pain in the left thigh and lumbar area after operation of the second lumbar vertebral compression fracture. To relieve pain, caudal block was performed. This block reduced in lumbar pain but left thigh pain persisted. So, we were performed psoas compartment block using mepivacaine and dexamethasone, which relieved the pain in the left thigh and lumbar area. We recommend psoas compartment block as useful and simple method for patients with thigh and lumbar area pain, especially when the epidural block is not feasible.

  • The effect of loading speed on soil failure was studied by using a high speed triaxial compression test. Tests were conducted at 0.35-6.2m/s loading speed to compress soil specimens of sandy loam at different moisture contents. The axial stress at fracture increased with increase in loading speed up to certain critical speeds, however they decreased as the speed up to certain critical speeds, however they decreased as the speed increased further. Experiments were also conducted in the field of sandy loam soil with the vibrating tillage tool. Tests were done at 0.33-0.85m/s tractor speed oscillating frequency 13.7hz and oscillating amplitude 59mm. The maximum oscillating velocity of tillage tool was 2.5m/s. It was observed that for the oscillating operation, initially draft slightly increased with increase in forward speed and then it decreased .For the non-oscillating operation, draft increased continuously with increase in forward speed. Approach of studying soil failure in the laboratory test can be related to the field experiments.

  • The effect of cyclic compressive stress on fracture responses of Al2O3 was investigated under uniaxial stress cycling. Experimental data were obtained for Al2O3 tension specimens under uniaxial tension-unloading and tension-compression cyclic loading conditions. To investigate the effect of compressive stress on the crack growth, theoretical results from the crack growth rate were compared with measured stress vs. failure relations. At low stress level in tension-compression cycling, residual tensile strains were also observed about failure time.

  • This paper is to estimate the load carrying capacities of concrete-filled steel tubular columns and the important parameters are selected the size, length and concrete strength. he concrete-filled tube structures has many excellent structural properties, that is, high load capacity, good plastic deformation and high resistance local buckling. Under these background, this study Investigated to the structural compression behaviors, the maximum strength, the confinement effects, the fracture mechanism, local buckling failure and concrete strength effects.
