국내 지역사회 거주 노인의 허약과 건강결과 간의 관계에 대한 통합적 고찰 (Association of Health Outcomes with Frailty in Community-Dwelling Korean Older Adults: An Integrative Review)

  • Purpose: Frailty is associated with an increased risk of adverse health outcomes. We aimed to review the relationships between frailty and health outcomes in community-dwelling Korean elderly individuals. Methods: Whittemore and Knafls' framework for conducting integrative reviews was used. PubMed, Cumulative Index to Nursing and Allied Health Literature, and six Korean databases were searched. For analysis, the study included articles written in English and Korean published between January 1960 and June 2018. Of the total 1,488 studies found in the databases, we analyzed 15 studies that met the quality of the evaluation criteria. Results: The prevalence of frailty in Korean elderly individuals ranged from 6.5% to 11.7% when divided into three levels of frailty. The health outcomes assessed in relation to frailty were divided into five domains: quality of life, physical health, psychosocial health, health behavior, and health care quality. Frailty was negatively associated with all five domains. Conclusions: Our study suggested that nurses should be aware of the limitations in the physical and cognitive functions of frail elderly individuals and provide tailored interventions for Korean elderly individuals. Furthermore, a large-scale study is needed to develop the Korean model of the frailty assessment tool and to verify the conceptual model of this study.

우리나라 지역사회 재가노인의 체질량지수 관련 요인: 전기 및 후기 노인 비교 (Body Mass Index (BMI)-Related Factors of Community-Dwelling Elders: Comparison between Early and Late Elderly People)

  • Purpose: The purpose of the study is to identify differences in BMI between early and late elderly people, and factors having influence of them. Methods: This study is an analysis of secondary data that used the raw materials from the KNHANES from 2008 to 2010. The subjects involved in the final analysis were 4,772 elders aged 65 or higher. Descriptive statistics, $x^2$-test and F-test, and CSGLM from the complex sample design were used for the data analysis with SPSS/WIN 19.0. Results: Significant differences were observed in the socio-demographic characteristics, health behaviors and diet habits between early and late elderly people. Adjusted for gender, location of residence, and living alone, the factors that affected BMI of the early elderly people included current smoking status, number of disease, difficulty in chewing, and number of meals per day while those that affected BMI of the late elderly people were current smoking status, number of disease, self-rated health, and difficulty in chewing. Conclusion: The study concludes that we should consider age-specific traits for monitoring the weight status of the elderly and providing appropriate weight management programs for the elderly.

  • Objectives : To analyze factors associated with elderly falls in a community dwelling, a comparatively important, but somewhat neglected, health mailer. Methods : Data came from personal interview surveys using a questionnaire of 552 people aged 65 or older living in a community. Socioeconomic, and health related characteristics were investigated as independent variables and experiences of falling in the previous year as the dependent variable. Results : 118(21.4%) of the elderly subjects experienced a fall in the previous year. 24 subjects experienced more than two falls, accounting for 20.3% of the elderly fall victims studied. Factors affecting the falls were families living together, level of daily activity, heart disease, and aconuresis;, therefore an elderly person who lives alone, lives independently, has heart disease, or suffers urinary incontinence had a demonstrated increased chance of failing. Conclusion : Groups at risk for luting included elderly persons living alone, living independently, suffering heart disease, or experiencing urinary incontinence. When an elderly fall prevention program is developed and implemented, these results should be considered.

노인의 낙상에 영향을 주는 요인을 평가하기 위한 ABC-BBS의 적용: 사전연구 (Combining Two Scales to Assess Risk Factors of Falling in Community-Dwelling Elderly Persons: A Preliminary Study)

  • The purpose of this preliminary study was to develop a measurement for assessing risk factors for falling in community-dwelling elderly persons. Rasch analysis and principal component analysis were performed to examine whether items on the Activities-Specific Balance Confidence (ABC), assessing self-efficacy, and items on the Berg Balance Scale (BBS), assessing balance function, contribute jointly to a unidimensional construct in the elderly. A total of 35 elderly persons (4 men, 31 women) participated. In this study, each item of ABC (16 items) and BBS (14 items) was scored on a 5-point ordinal rating scale from 0 to 4. The initial Rasch and principal component analysis indicated that 3 of the ABC items and 2 of the BBS items were misfit for this study. These 5 items were excluded from further study. After combining ABC and BBS, Rasch and principal component analyses were examined and finally 23 items selected; 12 items from ABC, 11 items from BBS. The 23 combined ABC-BBC items were arranged in order of difficulty. The hardest item was 'walk outside on icy sidewalks' and the easiest item was 'pivot transfer'. Although structural calibration of each 5 rating scale categories was not ordered, the other three essential criteria of Linacre's optimal rating scale were satisfied. Overall, the ABC-BBS showed sound item psychometric properties. Each of the 5 rating scale categories appeared to distinctly identify subjects at different ability levels. The findings of this study support that the new ABC-BBS scale measure balance function and self-efficacy. It will be a clinically useful assessment of risk factors for falling in the elderly. However, the number of subjects was too small to generalize our results. Further study is needed to develop a new assessment considering more risk factors of falling in elderly.

노인주거를 위한 건물의 재사용과 거주자의 만족도 (Adaptive Reuse for Elderly Housing and Residents' Satisfaction)

  • The adaptive reuse of older buildings to housing for the elderly persons is a relatively new but growing phenomenon in the Western Societies. With regard to many tangible and intangible benefits, adaptive reuse can be a way to help older persons who choose "aging-in-place" in the "long-term" community. The exploratory case study indentifies changes in the stisfaction level of residents after relocation to an adaptive reuse elderly apartment in a rural community in the U.S. Data were collected using personal interviews with 17 pre-occupancy and 16 post-occupancy respondents. Findings described characteristics of respondents and dwelling, neighborhood, and overall satisfaction level changes. This exploratory study, one of few studies about adaptive reuse for housing for the elderly, suggests implications for futher studies.

농촌지역 노인의 저작능력과 영양섭취상태와의 관련성 (The Relationship between Chewing Ability and Nutritional Intake Status in the Elderly of Rural Community)

  • This study aims to examine the relationship between chewing ability and nutritional intake status in the rural elderly. The subjects were 150 rural-dwelling elderly persons(68 males and 82 females) aged 65 years and over in Sungju-Gun, Kyunfsangpookdo. The respondents were interviewed using the questionnaires and measurments taken from a 24-hour recall method from February to April in 1998. Fifty-nine subjects(39.3%) were classified with normal chewing ability and sixty-four subjects(42.7%) could carry out all of the 10 ADL items by themselves. In addition, the group who were able to chew had more teeth and significantly higher ADL scores than the other group(p<0.01), whereas DMF value was significantly lower(p<0.001). The subjects with normal chewing ability also had higher of intakes of energy, protein, fat, carbohydrates, dietary fiber, salt, potassium, niacin, thiamin and riboflavin than in the unable group(p<0.05). As far as daily food intakes were concerned, considerable differences were revealed in the levels of grain and products and vegetables consumed depending on chewing ability(p<0.001). The results of the stepwise and vegetables consumed depending on chewing ability daily living activities and health self-assessment(p<0.05) were the most significant factors for energy intake status.

일 지역 알츠하이머병 노인에서 Apolipoprotein E ${\varepsilon}4$와 인지변화의 연관에 대한 전향적 연구 (A Prospective Study on an Association between Apolipoprotein E ${\varepsilon}4$ and Cognitive Change in Community-Dwelling Elders with Alzheimer's Disease)

  • Objectives : The aim of this study was to examine the prospective impact of the apolipoprotein E (APOE) ${\varepsilon}4$ on cognitive performance in the community-dwelling elderly individuals with Alzheimer's disease (AD). Methods : The total number of subjects was 30 (12 men and 18 women) who were diagnosed with AD from a Korean project of "Early Detection of Dementia". People aged 65-85 years were included in the analysis. The eight neuropsychological domains from the Korean version of Consortium to Establish a Registry of Alzheimer's Disease (CERAD-K) were conducted to test subjects. They have been followed at 24-month intervals with the same assessments at each interval. Their cognitive performance at 2 year intervals was compared by the occurrence of the APOE ${\varepsilon}4$. Results : The impact of ${\varepsilon}4$ allele was significant in the Word List Memory Test (WLMT, F = 4.345, df = 1, p = 0.021) and Word List Recall Test (WLRT, F = 5.569, df = 1, p = 0.033). Conclusions : The APOE ${\varepsilon}4$ allele was significantly correlated especially with verbal episodic memory domain in community-dwelling elders diagnosed with AD.

지역사회 거주 노인 대상의 그룹 프로그램에 대한 체계적 고찰 (A Systematic Review of Group Programs for Community-dwelling Elderly)

  • 목적 : 본 연구는 지역사회에 거주하는 노인 대상의 그룹 프로그램 연구를 체계적으로 고찰하여 분석하고자 한다. 연구방법 : 2009년부터 1월부터 2019년 12월까지 NDSL, DBPia, Riss, PubMed의 데이터베이스를 이용하여 지역사회 노인 대상의 그룹 프로그램에 대한 논문을 검색하였으며, 1차 검색된 147편의 논문 중 선정기준 및 배제기준에 따라 16편의 논문을 최종 선택하여 연구 대상자 특성, 그룹 프로그램의 세부내용, 중재기간 및 회기, 결과 측정방법, 그룹 프로그램의 효과를 분석하였다. 결과 : 총 16편의 선정된 연구 중 근거수준 III에 해당하는 단일 그룹 사전-사후 연구가 8편(50.0%)으로 가장 많았고, 지역사회 거주 일반 노인 대상의 연구가 5편(25.0%)이었다. 총 16편의 연구에서 적용한 그룹 프로그램의 유형은 10개로 구분되었고, 운동 그룹 프로그램이 4편(25.0%)이었다. 그룹 프로그램 중재 기간은 12주 동안의 중재 기간이 6편(37.5%)이었고, 회기는 8회기와 12회기가 4편(25.0%)이었으며, 그룹 프로그램의 적용 시간은 60분이 7편(43.8%)이었다. 결론 : 본 고찰 연구는 지역사회에서 다양한 인지수준의 노인을 위한 작업치료 그룹 프로그램 개발의 기초자료로 활용될 수 있다.

노인 건강증진센터 모형개발을 위한 연구: 일 지역보건소에 적용한 사회적지지 프로그램의 효과 (Model Development of an Elderly Health Promotion Center: The Effect of a Social Support Program at a Community Health Center)

  • Purpose: The purpose of this study was to develop a model of an elderly health promotion center after witnessing the effect of a social support program on dependent variables in older adults at a local community health center. Method: The subjects were 60 female adults over 65 years dwelling in a rural area, and they were divided into experimental and control groups each with 30 people. A social support program was implemented 6 hours a day, 3 times a week, for 4 months in the experimental group. Included was health assessment, health education, counseling, consultation, exercise, physical & occupational therapy, primary care, recreation, lunch & transfer service. Data was collected from May 1stto September 14th, 2002 by questionnaires, and analyzed by $x^2$-test, t-test, Pearson's correlation coefficient and stepwise multiple regression using SAS. Result: The social support program in the elderly was very effective on all dependent variables of physical health (t=4.68, p=.001), health knowledge (t=3.60, p=.001), life satisfaction (t=8.65, p=.001), and health promoting behaviors (t=5.23, p=.001). Conclusion: The Social Support Program at a Community Health Center was effective on health promoting behaviors in the elderly.

The Physical & Psychosocial Functioning of Korean Immigrant Elderly in Northwest America

  • The growing population of the elderly who have diverse health needs is receiving more attention from the health promotion field. This paper describes some of the findings of quantitative study into the health function of Korean immigrant elderly living in Washington State of Northwest America. One hundred-two community-dwelling Korean immigrant elderly, aged from 65 to 93 with a mean 77.97 years, were interviewed with a structured questionnaire to report their health function defined in three domains; perceived health status, physical functioning, and psychosocial functioning. The main results of the study were identified: (a) Overall, Korean immigrant elderly's perceived health status and physical functioning were good, but psychosocial functioning was poor; (b) Elderly with higher health functioning scores perceived better health status. Data indicates that health functioning measures are good indicators of the degree of perceived need for immigrant elderly. The paper argues that the understanding of physical and psychosocial functioning of elderly is vitally important in the provision of health care.
