A COMPARISON STUDY OF BENZODIAZEPINES TO A CHLORAL HYDRATE AND HYDROXYZINE COMBINATION IN THE SEDATION OF PEDIATRIC DENTAL PATIENTS (Chloral hydrate와 Hydroxyzine 복합투여와 Benzodiazepines의 진정효과에 관한 비교연구)

  • The purpose of this study was to compare the clinical sedation effect of chloral hydrate and hydroxyzine combination, midazolam, triazolam when young children were sedated for dental treatment. The uncooperative 22 children aged, 25 to 52 months of age(ASA class I) and weighting between 11 and 17kg, participated in the study. Each patient was assigned randomly to receive chloral hydrate(50mg/kg) and hydroxyzine HCl(25mg), midazolam(0.5mg/kg), and triazolam(0.25mg) oral administration : alternative regimens were administered at next appointment. According to rating scale, sleep, crying, movement, and overall behavior response were checked for evaluation of the clinical sedation effect. Pulse rate and oxygen saturation were also measured for monitoring the patients during treatment period by pulse oximeter. The results were as follows: 1. In the evaluation of sedation effect, 90.9% in chloral hydrate and hydroxyzine combination and midazolam, and 77.3% in triazolam were rated "good" or "very good". 2. Sleep was demonstrated to be statistically significant increase in chloral hydrate and hydroxyzine combination group. Despite the fact that chloral hydrate and hydroxyzine group was in a deeper state of sleep, all the children were easily aroused. 3. There were no statistically significance among the three regimens with regard to cry ing, movement, overall behavior. 4. The adverse side effect was vomiting in one patient of chloral hydrate and hydroxyzine combination. There were no clinical sign of respiratory depression.

Effect of Smoking and Alcohol Consumption on Seminal Quality in Young Mice

  • The purpose of this study is to evaluate the effects of alcohol or cigarette smoking on seminal parameters in a large group of mice model. Nine groups (n=20/group) of mice were treated intensive noxious materials that abdominal injection of 21% (v/v) of ethanol, cigarette smoke (10, 20, 30 minutes/day), and combination of ethanol and 30 minutes of smoking. In addition, vitamin C and selenium were also treated to mice exposed to combination of alcohol and smoking to identify the recovering effect. Sperm viability and motility were significantly decreased in either alcohol consumption or smoking exposed group, and combination of both materials have additive detrimental effects on seminal parameters. Mice groups that exposed to alcohol and smoking showed statistically significant decrease in motility and increase of static spermatozoa. Moreover, combination of both treatments showed cumulative effect in increase of static spermatozoa. Treatments of either vitamin C or selenium dramatically recovered detrimental effects of alcohol and smoking on seminal quality, although combination of both antioxidant molecules did not show any additive effect. In conclusion, detrimental effects of alcohol and cigarette consumption on sperm quality and motility were identified in mice model, and these detrimental effects can be compensated to uptake of anti-oxidant molecules.

Antitumor Activity of Combination Therapy with Metformin and Trametinib in Non-Small Cell Lung Cancer Cells

  • Metformin has been widely used as an antidiabetic drug, and reported to inhibit cell proliferation in many cancers including non-small cell lung cancer (NSCLC). In NSCLC cells, metformin suppresses PI3K/AKT/mTOR signaling pathway, but effect of metformin on RAS/RAF/MEK/ERK signaling pathway is controversial; several studies showed the inhibition of ERK activity, while others demonstrated the activation of ERK in response to metformin exposure. Metformin-induced activation of ERK is therapeutically important, since metformin could enhance cell proliferation through RAS/RAF/MEK/ERK pathway and lead to impairment of its anticancer activity suppressing PI3K/AKT/mTOR pathway, requiring blockade of both signaling pathways for more efficient antitumor effect. The present study tested the combination therapy of metformin and trametinib by monitoring the alterations of regulatory effector proteins of cell signaling pathways and the effect of the combination on cell viability in NCI-H2087 NSCLC cells with NRAS and BRAF mutations. We show that metformin alone blocks PI3K/AKT/mTOR signaling pathway but induces the activation and phosphorylation of ERK. The combination therapy synergistically decreased cell viability in treatment with low doses of two drugs, while it gave antagonistic effect with high doses. These findings suggest that the efficacy of metformin and trametinib combination therapy may depend on the alteration of ERK activity induced by metformin and specific cellular context of cancer cells.

Immunostimulation Activity of the Crude Polysaccharides Fractionated from Eleutherococcus senticosus, and its Application to Prevent of Tumors by Combination Therapy with Cisplatin (오가피로부터 분리된 조다당 분획물의 면역자극활성 및 Cisplatin과의 병용에 의한 항암 상승작용의 유도)

  • In order to study the clinical usefulness of crude polysaccharides fractionated from Eleutherococcus senticosus, EN-3, in eliminating tumors, we have investigated the effect of combination therapy on the murine tumor metastasis and growth models. In experimental metastasis of colon26-M3.1 cells, prophylactic intravenous (i.v.) administration of EN-3 (0.5, 5, and 50 $\mu\textrm{g}$/mouse) inhibited tumor metastasis compared with tumor control group in 33.6, 66.8, and 81.8% respectively. The administration of EN-3 (50 $\mu\textrm{g}$/mouse) also exhibited a 66.1% therapeutic effect on lung tumor metastasis. Although EN-3 induced no toxic effect on both tumor cell and normal splenocyte in the concentration below 100 $\mu\textrm{g}$/mι in in vitro, it induced significant proliferating activity on normal splenocyte in the concentration-dependent manner. In an analysis of NK-cell activity, i.v. administration of EN-3 (4∼100 $\mu\textrm{g}$/mouse) significantly augmented NK cytotoxicity to YAC-1 tumor cells. The combination treatments of cisplatin (10 $\mu\textrm{g}$) and EN-3 (5 $\mu\textrm{g}$) induced synergistic effect on the inhibition of tumor metastasis in experimental tumor metastasis model produced by colon26-M3.1 cells. In addition, the combination treatments also exhibited prolongation of lifespan in S∼180 tumor bearing mouse for over the 60 days. Even though cisplatin (2.5 $\mu\textrm{g}$/mι) exhibited cytotoxicity to tumor cells and inhibited tumor growth over 95% in in vitro, combination treatment with EN-3 (20 $\mu\textrm{g}$/mι) was induced splenocyte proliferation and produced cytokines, such as TNF-$\alpha$, IL-1 and IL-12, from the macrophages. These results suggested that EN-3 stimulate immune system non-specifically and apply to the biological response modifiers (BRM) in chemo-immunotherapy for tumor prevention.

Synergistic Effects of Exemestane and Aspirin on MCF-7 Human Breast Cancer Cells

  • Objective: The purpose of this study is to investigate the combined effects of exemestane and aspirin on MCF-7 human breast cancer cells. Methods: Antiproliferative effects of exemestane and aspirin, alone and in combination, on growth of MCF-7 human breast cancer cells were assessed using the MTT assay. Synergistic interaction between the two drugs was evaluated in vitro using the combination index (CI) method. The cell cycle distribution was analyzed by flow cytometry and Western blotting was used to investigate the expression of cyclooxygenase-1, cyclooxygenase-2 and Bcl-2. Results: MTT assays indicated that combination treatment obviously decreased the viability of MCF-7 human breast cancer cells compared to individual drug treatment (CI<1). In addition, the combination of exemestane and aspirin exhibited a synergistic inhibition of cell proliferation, significantly arrested the cell cycle in the $G_0/G_1$ phase and produced a stronger inhibitory effect on COX-1 and Bcl-2 expression than control or individual drug treatment. Conclusion: These results indicate that the combination of exemestane and aspirin might become a useful method to the treatment of hormone-dependent breast cancer. The combination of the two inhibitors significantly increased the response as compared to single agent treatment, suggesting that combination treatment could become a highly effective approach for breast cancer.

ABT-737 ameliorates docetaxel resistance in triple negative breast cancer cell line

  • Purpose: This study aimed to validate the synergistic effect of ABT-737 on docetaxel using MDA-MB-231, a triple negative breast cancer (TNBC) cell line overexpressing B-cell lymphoma-2 (Bcl-2). Methods: Western blot analysis was performed to assess expression levels of Bcl-2 family proteins and caspase-related molecules. Cell viability was assessed by 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide (MTT) assay. Cell cycle distribution was determined by flow cytometry analysis. Benzyloxycarbonyl-Val-Ala-Asp(OMe)-fluoromethylketone (z-VAD-fmk) was used for pretreatment to assess the role of caspases. Results: Cell viability of MDA-MB-231 after combination treatment with ABT-737 and docetaxel was significantly lower than that after docetaxel or ABT-737 monotherapy based on MTT assay (both P < 0.001), with a combination index of 0.41. The proportion of sub-G1 population after combination treatment was significantly higher than that after docetaxel or ABT-737 monotherapy (P = 0.001, P = 0.003, respectively). Pretreatment with z-VAD-fmk completely restored cell viability of MDA-MB-231 from apoptotic cell death induced by combination therapy (P = 0.001). Although pro-caspase-8 or Bid did not show significant change in expression level, pro-casepase-9 showed significantly decreased expression after combination treatment. Cleaved caspase-3 showed increased expression while poly (ADP-ribose) polymerase cleavage was induced after combination treatment. However, hypoxia-inducible factor 1-alpha and aldehyde dehydrogenase 1 totally lost their expression after combination treatment. Conclusion: Combination of ABT-737 with docetaxel elicits synergistic therapeutic effect on MDA-MB-231, a TNBC cell line overexpressing Bcl-2, mainly by activating the intrinsic pathway of apoptosis. Therefore, adjunct of ABT-737 to docetaxel might be a new therapeutic option to overcome docetaxel resistance of TNBCs overexpressing Bcl-2.

Development of GOCHUJANG for Controlling V. parahaemolyticus with Green Tea and Natural Products (녹차 및 천연산물로부터 장염 비브리오균(Vibrio. parahaemolyticus)의 제어를 위한 고추장의 개발)

  • Vibriocidal GOCHUJANG was manufactured using extract of natural products and green tea. Antioxidative capacity of natural products and combination of natural products was measured with Oxidation-Reduction Potential (ORP) system value. Inhibitory ability of vibriocidal GOCHUJANG was compared with Inhibition Zone Diameter (IZD) values. Phellodendri cortex, Schizandrae fructus, Theae folium, Scutellaria baicalensis george and Acanthodanacis cortex and combination of these natural products were showed higher antioxidant capacity and inhibitory effect against V. para-haemolyticus. Two kinds of combinations, A combination was composed of Phellodendri cortex, Schizandrae fructus, Theae folium, Scutellaria baicalensis george, and B combination was consisted of Acanthodanacis cortex and A combination, were presented higher inhibition ratio with IZD values as $1.57\pm0.0051$ and $1.56\pm0.0071$, respectively. Vibriocidal GOCHUJANG was prepared based on A and B combination with their higher IZD Value as $1.76\pm0.0103$ and $1.79\pm0.0103$, respectively. When vinegar was added to A and B based GOCHUJANG, their IZD value was more higher as $2.15\pm0.0071$ and $2.44\pm0.0086$, respectively B combination based GOCHUJANG was exhibited higher inhibitory effect than A based GOCHUJANG. Combination of natural products was more important process for increasing vibriocidal effect. But addition of soybean malt or chitosan or a garlic into A or B combined GOCHUJANG, no different effect was exhibited.

Enhanced Antitumor Effect of Curcumin Liposomes with Local Hyperthermia in the LL/2 Model

  • Curcumin previously was proven to inhibit angiogenesis and display potent antitumor activity in vivo and in vitro. In the present study, we investigated whether a combination curcumin with hyperthermia would have a synergistic antitumor effect in the LL/2 model. The results indicated that combination therapy significantly inhibited cell proliferation of MS-1 and LL/2 in vitro. LL/2 experiment model also demonstrated that the combination therapy inhibited tumor growth and prolonged the life span in vivo. Furthermore, combination therapy reduced angiogenesis and increased tumor apoptosis. Our findings suggest that the combination therapy exerted synergistic antitumor effects, providing a new perspective fpr clinical tumor therapy.

Clinical Study of Combination Treatment of Oriental Medicine on Thalamic Pain Syndrome (시상증후군(Thalamic Pain Syndrome) 환자의 한방 병합치료에 관한 임상적 고찰)

  • Objectives : To evaluate the pain control effect of combination treatment of Oriental Medicine on patients who suffered from thalamic pain syndrome caused by thalamic stroke. Methods : We reviewed the medical records and brain imaging data of all patients with thalamic stroke from September 1998 to August 2000 who visited to Department of Oriental Internal Medicine, National Medical Center. We evaluated clinical features of thalamic pain syndrome, including incidence, onset interval from stroke, nature, pain distribution, and assessed the pain control effect of combination treatment by Visual Analog Scale(VAS). Results : 64 cases were selected under the inclusion criteria, and 17 patients(26.5%) with thalamic pain syndrome were identified from 64 thalamic strokes. VAS proved combination treatment effective to control pain of thalamic pain syndrome. In 12 cases(70.5%), pain onset was within the first week poststroke. The patients with allodynia were 6(35.3%). In 12 cases(70.5%), the lesion was mainly located in the posterolateral areas of thalamus. Conclusion : We conclude that combination treatment of the Oriental Medicine modalities have pain control effectiveness on thalamic pain syndrome.(t=-5.47, p=0.0001)

Evaluation of the Effects of a Combination of Silicate Minerals in Duck Diets on Growth Performance and Litter Quality

  • An experiment was conducted to evaluate the efficacy of a mixture of bentonite and illite as feed additives on the growth performance and litter quality of 90 Pekin ducks. The ducks were individually weighed and randomly divided into two treatments (control and 1% combination of silicate minerals), with three replicate pens per treatment, and 15 ducks per pen. Growth performance was not significantly affected (p>0.05) by the combination of bentonite and illite, but a trend of increased growth performance was observed in the control groups. Total nitrogen content and pH in the litter decreased following supplementation with the combination of bentonite and illite (p<0.05) when compared with the control group. This data indicates that the dietary supplementation with the combination of bentonite and illite (1% level) has no positive effect on the growth performance and litter quality of Pekin ducks.
