Study about Etiologic Classification and Commonly Used Meridians in Acupuncture Therapy on Headache by Considering through the Oriental Literature (두통(頭痛)의 병인(病因) 분류(分類)와 침구치료(鍼灸治療)에 대한 문헌적(文獻的) 고찰(考察))

  • Object : The purpose of this study is assistant to medical treatment for patient, who suffers from headache, by classifing etiologies of headache and investigating using meridian and acupuncture point.Method : By considering through the oriental literature, we investigated etiologies and frequency of using meridian and acupuncture point on headache.Result:1. The Oriental etiologies of headache is classified in 'wind(風)', 'hot and feverish(熱)', 'humidity(濕)', 'cold(寒)', 'defidiency of qi(氣處)', 'deficiency of blood(血虛)', 'extravasated blood(瘀血)', 'asthenia of kidney(賢處)', 'anger by depression(鬱怒)', 'Damhwa(痰火)'2. The frequently used meridians on headache are followings : the 1st is Choksoyang-Tam-Kyong(足少陽膽經), the 2nd Choktaeyang-Pabggwabg-Kyong(足太陽膀胱經), the 3rd Tok-maek(督脈), and the 4th Chokyangmyong Wi-Kyong(足陽明胃經).3. The frequently used acupuncture points on headache are followings : the 1st is paek'oe(百會), the 2nd Hapkok(合谷) and the 3rd P'ungji(風池).Conclusion:1. The books about treatment of headache by using acupuncture are The Yellow Emperor's Classic on internal Medicine(黃帝內經) and Gab-UI-Kyoung(甲乙經) and so on.2. In The Yellow Emperor's Classic on Internal Medicine(黃帝內經), they mainly used treatment by following the stream of meridian on headache.3. After Gab-U1-Kyoung(甲乙經), they suggested specialized acupunctre point.4. Three Yang meridians(三陽經) that has many acupuncture point located on head area, are related to medical treatment on headache.

A Philological Research on the Way of Medical Study of ShangHanLun (『상한론(傷寒論)』의 의학연구방식에 대한 문헌학적 고찰)

  • Objectives : ShangHanLun is a clinical guideline, and its core is in the part of Six-meridian Diseases. In recent philological sutdies, independent textual analysis of Shanghanlun is essential to define the scope of research. Methods : By the textual study, I defined the Shanghanlun clinic model. And I researched about medical range of Shanghanlun, and relations between Shanghanlun and JinGuiYaoLue, and between Shanghanlun and Huangdineijing. Results : In six topic sentences, the word '-之爲' means 'a process'. In provisions following topic sentences, the word '者' means 'a person'. So Shanghanlun is describing processing factors of Six-meridian Diseases and related human changes with clinical therapy. In the philological studies, meaning of '傷寒' was possibly just the 3rd provision of the greater yang disease part. Practically Shanghanlun's study range is over the concept of Cold damage today. Additionally Jinguiyaolue deals diseases, but Shanghanlun deals human. Gangpyeong-Shanghanlun's phrases can be separated into five part, and oldest part is independent of Huangdineijing. Conclusions : So we need to set a new and independent clinic model of Shanghanlun to verify it. The Shanghanlun's clinic model has a 3 steps. 1st step is to find 6 type defined symptoms, which act as processing factors of Six-meridian Diseases. 2nd step is to confirm a human changes after a Disease appeared. 3rd step is to treat a patient as a provision suggests. Philologically Shanghanlun's clinic range is not limited by the word '傷寒'. And many concepts in Shanghanlun should be independent of Huangdineijing.

Enhancement of Antinociception by Co-administrations of Nefopam, Morphine, and Nimesulide in a Rat Model of Neuropathic Pain

  • Background: Neuropathic pain is a chronic pain due to disorder in the peripheral or central nervous system with different pathophysiological mechanisms. Current treatments are not effective. Analgesic drugs combined can reduce pain intensity and side effects. Here, we studied the analgesic effect of nimesulide, nefopam, and morphine with different mechanisms of action alone and in combination with other drugs in chronic constriction injury (CCI) model of neuropathic pain. Methods: Male Wistar rats (n = 8) weighing 150-200 g were divided into 3 different groups: 1- Saline-treated CCI group, 2- Saline-treated sham group, and 3- Drug-treated CCI groups. Nimesulide (1.25, 2.5, and 5 mg/kg), nefopam (10, 20, and 30 mg/kg), and morphine (1, 3, and 5 mg/kg) were injected 30 minutes before surgery and continued daily to day 14 post-ligation. In the combination strategy, a nonanalgesic dose of drugs was used in combination such as nefopam + morphine, nefopam + nimesulide, and nimesulide + morphine. Von Frey filaments for mechanical allodynia and acetone test for cold allodynia were, respectively, used as pain behavioral tests. Experiments were performed on day 0 (before surgery) and days 1, 3, 5, 7,10, and 14 post injury. Results: Nefopam (30 mg/kg) and nimesulide (5 mg/kg) blocked mechanical and thermal allodynia; the analgesic effects of morphine (5 mg/kg) lasted for 7 days. Allodynia was completely inhibited in combination with nonanalgesic doses of nefopam (10 mg/kg), nimesulide (1.25 mg/kg), and morphine (3 mg/kg). Conclusions: It seems that analgesic drugs used in combination, could effectively reduce pain behavior with reduced adverse effects.

  • Acute transverse myelitis(ATM) is defined as an acute intramedullary dysfunction of the spinal cord, ascendng or static involving both halves of the cord and appearing without any history of previous neurological diseases due to traumatic accident, tumor of all kind, encephalitis and of course excluding all possible viral, bacterial and fungal infection. It is mainly characterized by acute motor disorder of both limbs in respect to which spinal segments are affected as well as sensory disorder and dysuria & dyschezia. The exact cause is unknown, however it is recently suggested that immunological factors are highly involved. It has been reported by several reliable sources that it is often accompanied by immunological diseases such as systemic lupus erythematosus(SLE). As treatments non steroid anti-inflammatory drugs(NSAIDS) are primarily recommended as to steroids, limited doses are injected only with the proper prescription from the physician. Operative methods are not options as traumatic accidents and tumors are excluded as factors. To enhance muscle strength and prevent articular contracture physical therapy and passive exercise is imperative. The following patient whose chief complaints were mainly about hypoesthesia of Rt. lower limb and stiffness of phalanges of both fingers as well as to weakness of lower extermity. Therefore it has been diagnosed as arthalgia syndrome. In oriental medicine factors such as wind evil heat-evil, dampness-heat evil, cold evil cause the arthalgia syndrome. In this case the patient was diagnosed as dampness-heat evil and herbal medicine Chunglijagam-Tang and Dong-Qi acupuncture was applied to treat bladder disorder.

A Study on the Application of Non-glutinous rice in Shanghanlun (『상한론(傷寒論)』의 갱미(粳米) 입약법(入藥法)에 대한 고찰)

  • Objectives : To examine various ways non-glutinous rice has been applied in different formulas of "Shanghanlun" and the relationship between non-glutinous rice and the formulas' effects. Methods : First, texts that included non-glutinous rice were selected from the "Shanghanlun". Then, they were categorized according to the application of non-glutinous rice, followed by analysis of its relationship with each formula. Results : Most formulas that used non-glutinous rice in raw form when decocting with other ingredients included Shigao. Non-glutinous rice alleviates coldness of Shigao as in the case with Taohuatang where it stops diarrhea. Non-glutinous rice used in Wumeiwan warms the center and boosts Qi and helps the medicinal ingredients to bind when made into pellets. In Guizhitang and Lizhongwan, non-glutinous rice assists Stomach Qi. In Shizaotang, porridge is taken after diarrhea to help quick recovery. To prevent damage of Zheng Qi, Baisan can be mixed with rice water for administration. The level of cathartic effect can be managed through giving cold or hot porridge to the patient according to his/her response to the medicine. Mazirenwan, Mulizexiesan, Banxiasan, Sinisan, Wulingsan are mixed in rice water for administration. It makes it easier to swallow than when mixed with plain water. The application of rice flour is either internal or external. In Zhufutang, where rice flour is parched, it harmonizes the center and stops diarrhea. In Daqinglongtang when there is too much perspiration, warm powder is sprinkled on the skin to stop sweating. Clear porridge in Jiuziyisifang and rice flour in Shechuangzisan were exclusive to "Jinguiyaolue", while Baisan and rice water mixture could only be found in "Shanghanlun". Conclusions : The excavation and research on various ways non-glutinous rice was applied in treatment holds certain meaning in contemporary practice of Korean Medicine where treatment leans heavily on medicinal treatment rather than food, and all ingredients are boiled together in large quantities for most decoctions.

Features and Interpretation of Olfactory and Gustatory Disorders in the Corona Virus Disease-19 (코로나바이러스감염증-19에서 나타나는 후미각손상의 특성과 한의학적 분석)

  • Besides respiratory infection, COVID-19 has many neurological symptoms not only loss of smell and taste but also fatigue and brain fog. But it is a challenge to treat the neurological symptoms especially of anosmia and ageusia. In order to search for the therapeutic methods, the geographical diversity and pathological mechanisms of the COVID-19 and two symptoms were investigated from the latest clinical studies. Because the environmental conditions of the monsoon climate zone of East Asia and the Mediterranean and Oceanic climate zone of Italy, Britain, United States and tropical Brazil are different, each of diverse etiology and internal milieu should be considered differently in the treatment. SARS-CoV-2 exhibits the dampness-like characteristics and the olfactory and gustatory disorders are particularly more common than other flu or cold. and it tends to show features of damaging the lung qi of olfaction and heart-spleen qi of gustation. The mechanisms of olfactory and gustatory loss are various according to precursory, inflammatory, non-inflammatory and sequelar forms, so the therapeutic method should be designed for each period and pathology. If the process of inflammation arises from nasal and respiratory, olfactory epithelium to the central nervous structure by way of blood brain barrier, the treatment should be corresponded with the stage and depth of pathogen place. And if the olfactory loss is asymptomatic or in the initial stage, it can be applied intranasal topical scent therapy to relieve temporary locking of qi movement, but maybe also used in parallel together with herbs of relieving dampness toxin latent in the lung parenchyma.

A clinical study on a patient with hypothyroidism (생간건비탕가미방(生肝健脾湯加味方)을 이용한 갑상선기능저하증 치험1예(例))

  • Objectives : Hypothyroidism is a common endocrine disorder in which the thyroid gland does not produce enough thyroid hormone. It can cause a number of symptoms, such as tiredness, poor ability to tolerate cold, and weight gain. The purpose of this study was to report the clinical effects of herbal medicine on hypothyroidism. Methods : We employed oriental medical treatments; herbal-medication (Saenggangeonbi - tang gamibang), acupuncture and moxibustion. At the same time, the patient started to exercise. We treated the patient two or three times a week with oriental therapy method. She took medicine three times a day after a meal. During taking medicine, we let the patient avoid fatty food, flour based food, and alcohol. The symptoms and normalization of the thyroxine and TSH levels are important points of evaluating the patient's condition. So the patient measured the body weight and took a blood test a time per two months and compared the results with previous results. Results : After taking treatment - acupuncture and moxibustion during 6 months - and taking herbal-medicine, the level of TSH, fT4, T4 and T3 became normalized. The body weight was decreased about 18 pounds. In advance, the symptom of tiredness, edema was much improved. Conclusion : Herbal medicine (Saenggangeonbi-tang gamibang) was effective in the treatment of hypothyroidism and it helped to normalize the level of TSH, fT4, T4 AND T3.

Study for Enhanced Skin Penetration of Cosmetics by Plasma-Sono Stimuli (플라즈마-초음파 자극에 의한 화장품의 피부 침투 증진 연구)

  • The demand for skincare has increased due to the end of the COVID-19 pandemic, leading to a focus on skincare devices and technologies designed to improve the delivery of cosmetics. Among these technologies, skincare medical devices that utilize plasma therapy (Plasma) and sonophoresis (Sono) are commonly used in dermatology clinics. However, there is still a lack of quantitative analysis for transdermal absorption effects of Plasma and Sono skincare medical devices. In this study, we quantified enhanced transdermal absorption effects of Plasma and Sono devices through in-silico and ex-vivo studies. The Sono treatment demonstrated an increased transdermal absorption effect, showing a 10~13% difference in penetration compared to the control group in the in-silico experiment, and 159% and 184% increase in the ex-vivo experiment. The Plasma treatment revealed increased transdermal absorption effects, with a 1.0~2.5% penetration difference in the in-silico experiment, and a 124% increase in the ex-vivo experiment compared to the control group. We also observed a synergistic effect from the combined treatment of Plasma and Sono, as indicated by the highest increases of 197% and 242% in penetration. Furthermore, we have determined the optimal device settings and treatment conditions for Plasma-Sono skincare medical devices. Notably, higher on/off durations (Intensity levels) and longer Sono treatments resulted in greater transdermal absorption effects.

Field-in-Field Technique to Improve Dose Distribution in the Junction of the Field with Head & Neck Cancer (Field-in-Field Technique을 이용한 두경부암의 접합부위 선량개선에 관한 고찰)

  • Purpose: In treating head and neck cancer, it is very important to irradiate uniform dose on the junction of the bilateral irradiation field of the upper head and neck and the anterior irradiation field of the lower neck. In order to improve dose distribution on the junction, this study attempted to correct non uniform dose resulting from under dose and over dose using the field-in-field technique in treating the anterior irradiation field of the lower neck and to apply the technique to the treatment of head and neck cancer through comparison with conventional treatment. Materials and Methods: In order to examine dose difference between the entry point and the exit point where beam diffusion happens in bilateral irradiation on the upper head and neck, we used an anthropomorphic phantom. Computer Tomography was applied to the anthropomorphic phantom, the dose of interest points was compared in radiation treatment planning, and it was corrected by calculating the dose ratio at the junction of the lower neck. Dose distribution on the junction of the irradiated field was determined by placing low-sensitivity film on the junction of the lower neck and measuring dose distribution on the conventional bilateral irradiation of the upper head and neck and on the anterior irradiation of the lower neck. In addition, using the field-in-field technique, which takes into account beam diffusion resulting from the bilateral irradiation of the upper head and neck, we measured difference in dose distribution on the junction in the anterior irradiation of the lower neck. In order to examine the dose at interest points on the junction, we compared and analyzed the change of dose at the interest points on the anthropomorphic phantom using a thermoluminescence dosimeter. Results: In case of dose sum with the bilateral irradiation of the upper head and neck when the field-in-field technique is applied to the junction of the lower neck in radiation treatment planning, The dose of under dose areas increased by 4.7~8.65%. The dose of over dose areas also decreased by 2.75~10.45%. Moreover, in the measurement using low-sensitivity film, the dose of under dose areas increased by 11.3%, and that of over dose areas decreased by 5.3%. In the measurement of interest point dose using a thermoluminescence dosimeter, the application of the field-in-field technique corrected under dose by minimum 7.5% and maximum 17.6%. Thus, with the technique, we could improve non.uniform dose distribution. Conclusion: By applying the field-in-field technique, which takes into account beam divergence in radiation treatment planning, we could reduce cold spots and hot spots through the correction of dose on the junction and, in particular, we could correct under dose at the entry point resulting from beam divergence. This study suggests that the clinical application of the field-in-field technique may reduce the risk of lymph node metastasis caused by under dose on the cervical lymph node.

Research of 6 MeV Electron Dose Distribution (6 MeV Electron Therapy에서의 Electron Dose Distribution에 관한 연구)

  • Purpose : Electron is used for the treatment of skin cancer, breast cancer, and head and neck cancer in clinic. Our study is performed to check the isodose distribution in source surface distance(SSD)and source bolus distance(SBD)setup, nipple influence to isodose distribution of electron, junctional area isodose variation of photon and electron field. Materials and Methods : The electron dose distribution measures the diameter for 20 cm hemisphere paraffin phantom 2 made. It inserted the film between 2 paraffin phantom and it investigated it got radiation and dose distribution curve. Results : The 8% of isodose difference is with the surface distance(SSD)and source bolus distance(SBD)setup. The electon when the nipple exists inside the field, as nipple size it cuts the bolus and when it puts out and there is a possibility of getting the dose distribution which is homogeneous. When in the junction of electron and photon it uses the bolus it uses in the electron field whole, there is a possibility of getting the dose distribution which is homogeneous. Conclusion : The dose distribution decrease from the SBD setup. To reduce the influence of nipple, corresponding volume of bolus should be removed. And bolus covering all the electron field reduced hot and cold spot of junctional area of photon. In the future becomes the research which sees an effective electron therapy.
