Cross-sectional Study on Prevalence of Metabolic Syndrome in Stroke Patients (중풍환자의 대사증후군 유병률에 대한 단면조사연구)

  • Objectives : Metabolic syndrome (MetS) is associated with increasing risk of cardiovascular morbidity and mortality but the risk of stroke associated with the MetS is less well established. The objective of this study was to evaluate the prevalence of MetS in stroke patients. Methods : We analyzed the data of 806 patients with acute ischemic stroke from Hanbang Stroke Registry supported by the Ministry of Science and Technology of Korea. Subjects' enrollment was done in the oriental medical hospitals of 4 universities located in the metropolitan region from April 2007 to August 2008. We analyzed all registered data and evaluated the prevalence of MetS and specific components of MetS. Results : The prevalence of MetS in stroke patients according to American Heart Association/National Heart Lung and Blood Institute (AHA/NHLBI) criteria was 59.6%. The prevalence according to the International Diabetes Federation (IDF) criteria was 41.6%. The prevalence of female patients was higher than that of male patients (71.8% vs 51% by AHA/NHLBI criteria; 61.7% vs 27.7% by IDF criteria). The prevalence of central obesity, low HDL cholesterol and hypertension was higher in female patients. There was no significant relationship between MetS and stroke subtype. Conclusions : This work gives us the basic data on the association between MetS and risk of stroke. There should be the prospective cohort studies to evaluate MetS and the risk of stroke.

Does a Blepharospasm mean the Presymptom of Stroke? (안검경련(眼瞼痙攣)을 중풍 전조증상이라 할 수 있는가?)

  • Objectives : The purposes of this study were to investigate that a blepharospasm means the presymptom of stroke based on the traditional hypothesis in the oriental medicine and to compare a blepharospasm with warning signs of stroke in the western medicine. Methods : In the time period Oct. 2005 to Oct. 2006, 409 patients with a first-ever stroke admitted in the department of Internal Medicine of Kyungwon University In-cheon Oriental Medical Hospital, Kyunghee University Oriental Medical Hospital, Dongguk Il-san Oriental Medical Hospital were included. Patients were hospitalized within 14 days after the onset of stroke. Stroke patients were interviewed by residents who studied standard operation procedures in Fundamental Study for Standardization and Objectification of Differentiation and Pattern Identification of Syndrome of Oriental Medicine for Stroke. A questionnaire was completed by a question-and-answer form between patients and residents after explanation details to patients and the agreement of patients. Results : Age, care of patients or grandson, diabetes mellitus, and smoking were higher among cerebral infarction group, while smoking was higher among hemorrhage group. Female, young age, hyperlipidemia, hypertension in the family history and fat body in waist-hip ratio were higher among patients undergoing the blepharospasm before stroke onset. And the incidence of blepharospasm was lower in patients who dislike the fast food. Finally, when we compared a blepharospasm with warning signs of stroke in the western medicine, the incidence of blepharospasm in this study were the most frequent. Conclusion : In this study, the incidence of blepharospasm in patients before stroke onset was more frequent than that of warning signs in the western medicine. But more data from prospective cohort studies should be collected to be accepted that the blepharospasm is the presymptom of stroke as like warning signs in the western medicine.

Literature Review on Health Effect Surveys of Residents in Environmentally Contaminated Areas in South Korea from 1997 to 2021 (한국 환경오염 취약지역 주민 건강영향조사 문헌고찰(1997~2021))

  • Background: The conducting of health effect surveys (HESs) in environmentally contaminated vulnerable areas (ECVAs) by the central and local governments has been increasing apace with the increase in demand for HESs since the Environmental Health Act was enacted in South Korea in 2008. Objectives: This study aimed to review the HESs of residents in ECVAs conducted in South Korea. Methods: An analysis was performed on 125 reports obtained from the Environment Digital Library, PRISM, and local government websites after selecting from 803 projects obtained as ECVAs from the Korea ON-Line E-Procurement System (1997~2021), National Institute Environment Research (2000~2021), and Korea Environmental Industry and Technology Institute (2009~2021). The reports were classified by background (residents' demand, HES, and more), research design (cross-sectional study, cohort, ecological study, and panel), pollution source (abandoned metal mine (AMM), industrial complex (IC), and more), and assessment method of exposure and health effects. The survey area was converted into administrative district codes for geographical mapping. Results: There were 37, 34, 18, and 10 cases associated with AMM, IC, waste incinerators, and coal-fired power plants, respectively. Most of the studies conducted were cross-sectional studies and ecological studies. The proportion of epidemiological investigations by residents' demand showed an increase from 0.0% to 8.9% for the central government while decreasing from 16.7% to 14.3% for local governments after 2008 compared to before 2008. HESs increased at both the central and local government levels since 2014. For the evaluation method, 365 environmental hazards, 319 health outcomes, and 302 biological markers were investigated, with the most commonly investigated items being metals, cancer, and blood metals. Conclusions: HESs of residents in ECVAs in South Korea have been continuously developed both quantitatively and qualitatively. Future improvements are expected, and systematic review and classification of the HESs is warranted.

Prognostic Factors of Neonatal Sepsis Mortality in Developing Country

  • Purpose: Sepsis is the most common cause of neonatal death accounting for 30-50% of mortality annually in developing countries. This study was to determine the prognostic factors of neonatal sepsis mortality. Methods: A retrospective cohort was conducted in Dr. R. Sosodoro Djatikoesoemo Governor Hospital from April 2021 to September 2021 on 121 neonates in the neonatal intensive care unit (NICU) diagnosed with sepsis. The inclusion criteria were neonates aged 0-28 days, admitted to the NICU, and diagnosed with sepsis. The exclusion criteria were incomplete data and the presence of congenital abnormalities. A χ2 test was performed on the sex, gestational age, mode of delivery, birth weight, APGAR score, birthplace, and blood culture. A normality test was performed on leukocytes, lymphocytes, neutrophils, platelets, C-reactive protein (CRP), and length of stay. Then performed a Mann-Whitney test. Results: Birth weight (P=0.038), gestational age (P=0.009), and blood culture (P=0.014) showed a significant relationship with the neonatal sepsis outcome while Mann-Whitney test showed significant differences in the platelets (P=0.018), CRP (P=0.002), and length of stay (P<0.001). Multivariate analysis showed that 3 prognostic factors associated with neonatal sepsis mortality were prematurity (odds ratio [OR], 3.906; 95% confidence interval [CI], 1.344-11.356; P=0.012), low birth weight (LBW, OR, 2.833; 95% CI, 1.030-7.790; P=0.044), and gram-negative bacteria (OR, 4.821; 95% CI, 1.018-22.842; P=0.047). Conclusions: Prematurity, LBW, and gram-negative bacteria were associated with the prognostic factors of neonatal sepsis.

Reliability of Web and Paper-Based Survey Methods for Mibyeong and Cold-Heat Pattern Questionnaire for Korean Medicine Health Assessment: Pilot Study (한의 건강 측정을 위한 미병과 한열설문의 웹과 종이 기반 조사 방법의 신뢰도: 예비연구)

  • This study evaluated the consistency between the web-based and paper-based mibyeong and cold-heat pattern questionnaire, the Korean medicine-based tool for diagnosing and classifying health status. First, a web-based survey was conducted on 72 ordinary people; subsequently, a paper-based survey was conducted after a certain time interval. The equivalence between the web-based and paper-based surveys was evaluated on the basis of the consistency between scores using the Intraclass Correlation Coefficient (ICC) and Bland-Altman methodology. The mibyeong questionnaire showed high reliability for the web-based and paper-based surveys (ICC=0.95, 95% CI 0.92 - 0.97), and the cold-heat pattern questionnaire showed high reliability for both cold syndrome (ICC=0.98, 95% CI 0.96 - 0.99) and heat syndrome (ICC=0.9, 95% CI 0.83 - 0.93). The difference in average scores between the two survey methods was -0.25 for the mibyeong survey, -0.17 for the cold syndrome, and 0.11 for the heat syndrome, showing a similar pattern. Among the respondents, 84% showed positive satisfaction with the web-based survey, and 80% preferred the web-based survey. Overall, this study confirmed the reliability and feasibility of the web-based survey methods for the mibyeong and cold-heat pattern questionnaire. This could be a useful tool for the follow-up of subjects in long-term cohort studies.

Changes in cognitive function and functional disability in older adults - Comparison of groups converted and not converted to dementia among cognitively normal older adults - (노년기 인지기능과 일상생활기능장애 변화에 대한 연구 - 치매발생집단과 정상유지집단 간 차이 -)

  • This study aimed to analyze the trajectories of cognitive function and functional disability, and to identify the impacts of socio-demographic factors on the two variables. Based on the assumption that there are differences according to the progress of cognitive impairment, it focused on examining the differences in trajectories of cognitive function and functional disability between two groups: dementia conversion group and dementia non-conversion group among cognitively normal older adults. This study was conducted based on the data from a 3-wave panel study of a sample of 966 Yeoncheon elderly cohort aged 65 and over between 1997 and 2003. Latent growth curve model and multi-group structural equation modeling were used to examine the hypothesis. Results revealed that dementia conversion group exhibited faster rate of cognitive decline as well as lower initial cognitive level. Difference between two groups was not significant in the initial level of functional disability, yet dementia conversion group showed greater degree of deterioration in the functional disability over time than dementia non-conversion group did. In terms of the influence of socio-demographic factors on cognitive function and functional disability, the cognitive decline was more drastic in the female group, whereas functional decline was more apparent for the male group. The level of education in early life had a strong impact on the cognitive function in later life. Based on these findings, practical implication for high risk groups in cognitive function and functional disability were discussed.

Analysis of the Association between Teacher Relationship, Peer Relationship, and Multicultural Acceptability among Adolescents in Korea: Using Latent Growth Modeling (교우관계와 교사관계가 청소년의 다문화수용성에 미치는 영향에 대한 분석: 다변량 잠재성장모형의 적용)

  • In this study, we examined the longitudinal change of multicultural acceptability, peer relationship, and teacher relationship using latent growth curve modeling. This study used data from the second, third, and fourth waves of the middle school student cohort (N=2,178) of the Korean Children-Youth Panel Survey (KCYPS). The dependent variable was multicultural acceptability. The independent variables were peer relationship and teacher relationship. The major longitudinal findings of this study are as follows: First, peer relationships, teacher relationships and multicultural acceptability increased with time. Second, peer relationship showed a significant effect on the multicultural acceptability over time. Finally, the teacher relationship showed a significant effect on the multicultural acceptability over time. These results show differentiation with previous cross-sectional studies of multicultural acceptability. Furthermore, it is expected that this study will provide educational implications for the cultivation of multicultural acceptability.

Effect of Lifestyle Risk Factors on Daily Life and Cognitive Function of the Older Adults in the Community (지역사회 노인의 라이프스타일 위험요인이 일상생활 활동과 인지기능에 미치는 영향)

  • Objective : To identify the effect of lifestyle risk factors on the daily activities and cognition of the older adults in the community using the National Health Insurance Corporation 2015 geriatric cohort database. Methods : Lifestyle risk factors were defined as body mass index (BMI), smoking, drinking, vigorous exercise, moderate exercise, and walking, and basic and instrumental activities of daily living (ADL) and cognitive function variables were included in the analysis. ADL and cognitive function according to sex and age were analyzed using a t-test and one-way ANOVA. The correlation between lifestyle risk factors, ADL, and cognitive function was analyzed using Pearson's correlation analysis, and multiple regression analysis was performed to analyze their influence. Results : The factors affecting basic ADL (BADL) were sex and walking exercises, with an explanatory power of 1.7%. Instrumental ADL (IADL) included age, drinking, and walking exercises, with an explanatory power of 2.6%. Cognitive function included sex, age, BMI, vigorous exercise, and walking, with an explanatory power of 5.3%. Conclusion : Lifestyle risk factors partially affected BADLs/IADLs and cognitive function in community-dwelling older adults. This suggests the need to systematically manage lifestyle risk factors to improve and maintain the healthy lives of older adults facing biological aging.

Deciphering the DNA methylation landscape of colorectal cancer in a Korean cohort

  • Aberrant DNA methylation plays a pivotal role in the onset and progression of colorectal cancer (CRC), a disease with high incidence and mortality rates in Korea. Several CRC-associated diagnostic and prognostic methylation markers have been identified; however, due to a lack of comprehensive clinical and methylome data, these markers have not been validated in the Korean population. Therefore, in this study, we aimed to obtain the CRC methylation profile using 172 tumors and 128 adjacent normal colon tissues of Korean patients with CRC. Based on the comparative methylome analysis, we found that hypermethylated positions in the tumor were predominantly concentrated in CpG islands and promoter regions, whereas hypomethylated positions were largely found in the open-sea region, notably distant from the CpG islands. In addition, we stratified patients by applying the CpG island methylator phenotype (CIMP) to the tumor methylome data. This stratification validated previous clinicopathological implications, as tumors with high CIMP signatures were significantly correlated with the proximal colon, higher prevalence of microsatellite instability status, and MLH1 promoter methylation. In conclusion, our extensive methylome analysis and the accompanying dataset offers valuable insights into the utilization of CRC-associated methylation markers in Korean patients, potentially improving CRC diagnosis and prognosis. Furthermore, this study serves as a solid foundation for further investigations into personalized and ethnicity-specific CRC treatments.

Neutrophil to Lymphocyte Ratio and Serum Biomarkers : A Potential Tool for Prediction of Clinically Relevant Cerebral Vasospasm after Aneurysmal Subarachnoid Hemorrhage

  • Objective : Subarachnoid hemorrhage (SAH) is a condition characterized by bleeding in the subarachnoid space, often resulting from the rupture of a cerebral aneurysm. Delayed cerebral ischemia caused by vasospasm is a significant cause of mortality and morbidity in SAH patients, and inflammatory markers such as systemic inflammatory response index (SIRI), systemic inflammatory index (SII), neutrophil-to-lymphocyte ratio (NLR), and derived NLR (dNLR) have shown potential in predicting clinical vasospasm and outcomes in SAH patients. This article aims to investigate the relationship between inflammatory markers and cerebral vasospasm after aneurysmatic SAH (aSAH) and evaluate the predictive value of various indices, including SIRI, SII, NLR, and dNLR, in predicting clinical vasospasm. Methods : A retrospective analysis was performed on a cohort of 96 patients who met the inclusion criteria out of a total of 139 patients admitted Trakya University Hospital with a confirmed diagnosis of aSAH between January 2013 and December 2021. Diagnostic procedures, neurological examinations, and laboratory tests were performed to assess the patients' condition. The Student's t-test compared age variables, while the chi-square test compared categorical variables between the non-vasospasm (NVS) and vasospasm (VS) groups. Receiver operating characteristic (ROC) curve analyses were used to evaluate the diagnostic accuracy of laboratory parameters, calculating the area under the ROC curve, cut-off values, sensitivity, and specificity. A significance level of p<0.05 was considered statistically significant. Results : The study included 96 patients divided into two groups : NVS and VS. Various laboratory parameters, such as NLR, SII, and dNLR, were measured daily for 15 days, and statistically significant differences were found in NLR on 7 days, with specific cut-off values identified for each day. SII showed a significant difference on day 9, while dNLR had significant differences on days 2, 4, and 9. Graphs depicting the values of these markers for each day are provided. Conclusion : Neuroinflammatory biomarkers, when used alongside radiology and scoring scales, can aid in predicting prognosis, determining severity and treatment decisions for aSAH, and further studies with larger patient groups are needed to gain more insights.
