Evaluation of Negative Results of BacT/Alert 3D Automated Blood Culture System

  • Although automated continuous-monitoring blood culture systems are both rapid and sensitive, false-positive and false-negative results still occur. The objective of this study, then, was to evaluate negative results occurring with BacT/Alert 3D blood culture systems. A total of 1032 samples were cultured with the BacT/Alert 3D automated blood culture system, using both aerobic (BPA) and anaerobic (BPN) media, and 128 of these samples yielded positive results. A total of 904 negative blood samples were then subcultured in $5\%$ sheep blood agar, eosin methylene blue, chocolate agar, and sabouraud-dextrose agar. Organisms growing on these subcultures were subsequently identified using both Vitek32 (bioMerieux, Durham, NC) and conventional methods. Twenty four $(2.6\%)$ of the 904 subcultures grew on the subculture media. The majority $(83.3\%)$ of these were determined to be gram-positive microorganisms. Fourteen $(58.3\%)$ were coagulase-negative staphylococci, two $(8.3\%)$ were Bacillus spp., one $(4.2\%)$ was Staphylococcus aureus, and one $(4.2\%)$ was identified as Enterococcus faecium. Streptococcus pneumoniae and Neisseria spp. were isolated together in two $(8.3\%)$ vials. Gram-negative microorganisms comprised $12.5\%$ of the subcultures, of which two $(8.3\%)$ were found to be Pseudomonas aeruginosa, and one $(4.2\%)$ was Pseudomonas fluorescens. The other isolate $(4.2\%)$ was identified as Candida albicans. We conclude that the subculture of negative results is valuable in the BacT/Alert 3D system, especially in situations in which only one set of blood cultures is taken.

수의학강좌 II: 연쇄상구균과 장구균으로 인한 환경성 유방염의 감염위험요인과 통제방법

  • 최근 몇년간 국립수의과학검역원에서 조사한 바에 따르면, 국내 젖소에서 유방염을 일으키는 주된 원인균은 황색포도상구균을 제외하고는 거의 대부분 젖소의 환경으로부터 유래하는 환경성 세균에 의한 것으로 확인된 바 있다. 국내 젖소의 유방염 원인균으로 작용하고 있는 이러한 환경성 세균 중 coagulase-음성 포도상구균 (Coagulase-negative Staphylococci, CNS)과 E. coli 등의 그람 음성균이 가장 많은 비율을 차지하였지만 그 외에 Streptococcus uberis를 비롯한 환경성 연쇄상 구균과 장구균도 상당한 비율을 차지하는 것으로 조사되었으며 이에 대해서는 지난 9월호에서도 상세히 기술한 바 있다. 환경성 연쇄상구균과 장구균은 젖소에서 유선감염 및 임상형 유방염의 중요한 원인이며 환경성 연쇄상구균 중 가장 흔히 발견되는 유방염 원인체는 Streptococcus uberis 로서, 이는 국내에서뿐만 아니라 미국, 유럽, 뉴질랜드, 호주 등에서 수행된 많은 연구결과에서도 마찬가지로 나타났다. 장구균 중 가장 흔히 분리되는 균종은 E. faecium과 E. faecalis로서 검역원에서 지난 몇 년간 조사된 결과에서도 이 두 가지 균 종이 유방염우유에서 분리된 장구균의 90% 이상을 차지하였다. 이러한 환경성 연쇄상구균과 장구균은 사람과 동물의 장관, 분변, 감염된 유방 및 전체적인 목장 환경에서 분리되고 있다. 장구균은 일반적으로 환경성 연쇄상구균/장구균 유방염 감염증 중의 일부일뿐이고 장구균에 의한 유방염이 목장의 주된 문제가 되는 일은 비교적 드물다. 현재, 장구균에 의해 발생하는 유방염은 S. uberis를 통제하기 위해 사용되는 방법과 동일한 방법으로 통제할 수 있다고 여겨진다. 원인체에 상관없이 유방염을 통제하기 위한 기본적인 원칙은 유방염을 일으킬 수 있는 병원체에 유두끝이 노출되는 기회를 줄이거나 감염에 대한 젖소의 저항성을 증가시키는 두 가지 방법이다. 역으로 얘기하면, 병원체에 대한 유두끝 노출을 증가시키거나 감염에 대한 젖소의 저항성을 감소시키는 요인들은 우군 내에서 더 많은 유방염을 발생시킬 가능성이 높기 때문에 위험요인으로 간주된다. 본 고에서는 환경성 연쇄상구균에 의한 유방염과 관련이 있는 것으로 알려진 이러한 여러 가지 위험요인들에 대해 알아보고 그러한 위험요인을 줄일 수 있는 방법에 대해서도 고찰하고자 한다.

Therapeutic Dose, Duration, and Efficacy of Bee Venom for Treating Clinical Mastitis in Dairy Cow

  • This study was conducted to explore the efficacy of bee venom as a treatment for mastitis and to determine the optimal dosage and treatment period. When 6 mg or 12 mg of bee venom was administered to each experimental quarter of mastitis in dairy cow, the clinical symptoms in the 12 mg quarter were noticeably improved compared to those in the 6 mg quarter. There was no significant difference in the somatic cell count (SCC) in the milk between normal and mastitis quarters between the 6 and 12 mg doses, but there was a steady decrease in the 12 mg-treated quarter (p = 0.34). To determine the treatment period, bee venom was administered for 2, 4, and 7 days. After administering 12 mg of bee venom for 7 days, the SCC in the milk was compared before 6 days and after 7 days, and the SCC was significantly decreased to less than 100,000 SC/mL after 7 days (p = 0.01). In addition, to investigate the efficacy of bee venom, the minimum inhibitory concentration for S. aureus, E. coli, and coagulase negative staphylococci was measured, and the results showed that Gram-positive bacteria were more sensitive to bee venom than Gram-negative bacteria, and treatment for Gram-negative bacteria was limited. As a result of this study, it was confirmed that a dose of 12 mg of bee venom and a treatment period of more than 7 days were required to treat mastitis, and that treatment with bee venom alone against Gram-negative bacteria was negative.

Treatment and Prognosis according to Causative Organisms in Neonatal Bacterial Meningitis (신생아 세균성 뇌막염의 원인균에 따른 치료와 예후)

  • Purpose : Neonatal bacterial meningitis is the disease which clinical manifestations are nonspecific and several neurologic complications may occur. We studied neonatal bacterial meningitis, particularly in treatment and prognosis according to causative organisms -gram positive and gram negative bacteria- to assist in treatment of neonatal bacterial meningitis. Methods : We analysed twenty-four cases retrospectively who had been admitted in NICU or pediatric ward in Chung-ang Gil hospital from Jan. 1991 to Jun. 1996, and who had proven causative organisms in culture or latex agglutination[n test in CSF. Results : 1) The ratio of male to female was 2.4: 1. The mean birth weight and gestational age in cases with gram positive bacterial meningitis were $2.91{\pm}0.79kg$ and $38.4{\pm}2.74$ weeks and those in cases with yam negative bacterial meningitis were $3.30{\pm}0.90kg$ and $37.7{\pm}3.33$weeks respectively. There was no significant difference between the two groups. 2) The perinatal predisposing factors were pematurity, mecoinium staining amnionic fluid, matemal diabetes and pregnancy-induced hypertension, etc. The clinical manifestations Were fever, seizure, poor oral intake and fontanel bulging, etc. There were eleven cases with early onset bacterial meningitis(four cases by gram positive bacteria, seven cases by gram negative bacteria), and thirteen cases with late onset bacterial meningitis(seven cases by gram positive bacteria, six cases by gram negative bacteria). There was no significant difference between the two groups in terms of onset. 3) There were eleven cases with yam positive bacterial meningitis and they were coagulase-negative staphylococci(three cases), group B streptococci(three cases), Staphylococcus aureus(two cases), Streptococcus viridans(two cases), and enterococci(one case). And there were thirteen cases with gram negative bacterial menir gitis and they were Escherichia coli(seven cases), Klevsiella pneumoniae(three cases), Pseudomonas aeruginosa(one case), Acinetobactor(one case) and Enterobacter(one case). 4) The initial CSF WBC counts in cases with yam negative bacterial meningitis were significantly higher than those in cases with gram positive bacterial meningitis but the CSF protein and glucose levels were no significant difference in the two groups statistically. 5) The number of cases with abnormal findings in brain ultrasonography was seven in gram positive bacterial meningitis and ten in gram negative bacterial meningitis. 6) There were relatively high sensitivity to penicillin derivatives, the first generation cephalosporin and vancomycin in gram positive bacteria and to the third generation cephalosporin and amikacin in gram negative bacteria. 7) The mortality rate was 20.8%(5 cases were expired or discharged hopelessly). There was no significant difference between the two groups in prognosis. Conclusions : We recommend active treatment in noenatal bacterial meningitis to improve prognosis because the prognosis is poor.

The Study of MLS-Resistant Gram Positive Cocci Isolated In a Korean Hospital (임상 분리 그람 양성 구균에 대한 MLS계 항생물질의 내성)

  • A total of 398 strains of Staphylococcus aureus (99), coagulase-negative staphylococci (CNS) (99), enterococci (100), Streptococcus pyogenes (18) and Streptococcus agalatiae (82) were in Korean tertiary hospital from Dec. 2004 to Mar, 2005. When minimal inhibitory concentrations (MIC), phenotypes and genotypes were determined, 70% of S. aureus, 65.7% of CNS, 78% of enterococci and 37.8% of S. agalatiae were resistant to erythromycin and 95.7% of erythromycin-resistant (EMR) S. aureus, 92.3% of EMR CNS and all of EMR enterococci and S. agalatiae had erm of the methylase gene or msr(A) of the efflux gene.

Prospective Multicenter Surveillance Study of Surgical Site Infection after Spinal Surgery in Korea : A Preliminary Study

  • Objective : This study aimed to investigate the rates, types, and risk factors of surgical site infection (SSI) following spinal surgery using data from a Korean SSI surveillance system that included diagnoses made by surgeons. Methods : This was a prospective observational study of patients who underwent spinal surgeries at 42 hospitals in South Korea from January 2017 to December 2017. The procedures included spinal fusion, laminectomy, discectomy, and corpectomy. Univariate and multivariate logistic regression analyses were performed. Results : Of the 3080 cases included, 30 showed infection, and the overall SSI rate was 1.0% (an incidence of 1.2% in spinal fusion and 0.6% in laminectomy). Deep incisional infections were the most common type of SSIs (46.7%). Gram-positive bacteria caused 80% of the infections, and coagulase-negative staphylococci, including Staphylococcus epidermidis, accounted for 58% of the gram-positive bacteria. A longer preoperative hospital stay was significantly associated with the incidence of SSI after both spinal fusion and laminectomy (p=0.013, p<0.001). A combined operation also was associated with SSI after laminectomy (p=0.032). Conclusion : An SSI surveillance system is important for the accurate analysis of SSI. The incidence of SSI after spinal surgery assessed by a national surveillance system was 1.0%. Additional data collection will be needed in future studies to analyze SSI in spinal surgery.

Effects of teat dipping disinfectant containing biodegradable iodophor microspheres on preventing dry period intramammary infection in dairy cows (생분해성 iodophor microsphere 함유 유두침지 소독제의 건유기 젖소에서의 유방내 신규감염 예방 효과)

  • 1% iodophor loaded microspheres of PLGA (Poly[DL-Lactide-co-Glycolide]) were prepared by solvent evaporation method and were applied to the cows on dry period for evaluating it's preventive effects on intramammary infections. The morphology of the microspheres were evaluated using scanning electron microscopy and their releasing patterns were investigated. On investigating idophor releasing patterns of the microsphere, burst releasing pattern was detected until 2 days after in vitro incubation and sustained releasing was observed until 4 weeks. In field trial of teat dipping solution containing idophor loaded microspheres in dry cows showed significant preventive effects of intramammary infection caused by S. aureus, S. agalactiae, coagulase negative Staphylococci and coliform bacteria (p<0.05).

Bacterial profile of suction drains and the relationship thereof to surgical-site infections in prosthetic breast reconstruction

  • Background Despite the increasing popularity of prosthetic breast reconstruction, scant data exist on the microbiological profile of drainage fluid from closed-suction drains and the relationship thereof to surgical-site infections (SSIs) in breast reconstruction surgery. This study aimed to determine whether bacteria isolated from drainage fluid were associated with the development of SSIs, and whether the bacterial profile of drainage fluid could be a clinically useful predictor of SSIs. Methods We performed a retrospective chart review of 61 women who underwent tissue expander/implant or direct-to-implant reconstructions. Patient demographics and culture studies of drainage fluid from suction drains collected on postoperative day 7 were evaluated. Results Sixteen patients (26.23%) were culture-positive, and 45 patients (73.77%) were culture-negative. The most frequently isolated bacteria were coagulase-negative staphylococci, followed by Staphylococcus aureus. SSIs were diagnosed in seven patients and were mostly resolved by systemic antibiotics; however, the tissue expander or implant was explanted in two patients. Positive culture of drainage fluid from closed-suction drains was significantly associated with the development of SSIs (P<0.05). The positive predictive value was 37.50%, and the negative predictive value was 97.78%. Conclusions To our knowledge, this study is the first to demonstrate a significant association between the microbiological profile of drainage fluid from closed-suction drains and the development of SSIs in patients with prosthetic breast reconstructions. The high negative predictive value suggests that microbial testing of drainage fluid from closed-suction drains may have clinical utility. Further prospective studies with larger sample sizes are required to confirm our findings.

Etiological Agents in Bacteremia of Children with Hemato-oncologic Diseases (2006-2010): A Single Center Study (최근 5년(2006-2010)간 소아 혈액 종양 환자에서 발생한 균혈증의 원인균 및 임상 양상: 단일기관 연구)

  • Purpose : This study was performed to identify the etiologic agents and antimicrobial susceptibility patterns of organisms responsible for bloodstream infections in pediatric cancer patients for guidance in empiric antimicrobial therapy. Methods : A 5-year retrospective study of pediatric hemato-oncologic patients with bacteremia in Seoul National University Children's Hospital, from 2006 to 2010 was conducted. Results : A total of 246 pathogens were isolated, of which 63.4% (n=156) were gram-negative, bacteria 34.6% (n=85) were gram-positive bacteria, and 2.0% (n=5) were fungi. The most common pathogens were Klebsiella spp. (n=61, 24.8%) followed by Escherichia coli (n=31, 12.6%), coagulase-negative staphylococci (n=23, 9.3%), and Staphylococcus aureus (n=22, 8.9 %). Resistance rates of gram-positive bacteria to penicillin, oxacillin, and vancomycin were 85.7%, 65.9%, and 9.5%, respectively. Resistance rates of gram-negative bacteria to cefotaxime, piperacillin/tazobactam, imipenem, gentamicin, and amikacin were 37.2%, 17.1%, 6.2%, 32.2%, and 13.7%, respectively. Overall fatality rate was 12.7%. Gram-negative bacteremia was more often associated with shock (48.4% vs. 11.9%, P<0.01) and had higher fatality rate than gram-positive bacteremia (12.1% vs. 3.0%, P=0.03). Neutropenic patients were more often associated with shock than non-neutropenic patients (39.6 % vs. 22.0%, P=0.04). Conclusion : This study revealed that gram-negative bacteria were still dominant organisms of bloodstream infections in children with hemato-oncologic diseases, and patients with gram-negative bacteremia showed fatal course more frequently than those with gram-positive bacteremia.

Study of bacterial contamination from the Cockroaches in hospital (병원내 바퀴의 세균오염에 관한 연구)

  • Hospital has a limited environment in which all different patients are accommodated and therefore it should always be maintained as clean as possible in all its aspects. However the habitation of cockroaches which may be frequently observed in hospital gives us a very unfavuoraqble impression. It may not b e difficult to presume that cocrkoaches carry various microganisms from ward to ward. This study was carried out from July to the end of August. 1979 with a total of 259 cockroaches captured from the three hospitals and general residences in Seoul. Investigation was made to see what kind o f organisms they were imbued with and the results obtained are summarized as follows. Totaly 1) The nine species of the microganisms were isolated from the captured cockroaches and the isolated rate was showed 96.6 %. This organisms consisted of Aerobacter aerogenes(29.7% ). staphylocacci (22.4% ). gram negative bacilli(16.6%). E. coli(10.4%), streptococci(9.7%). fugus(7.3), and yeast (0.4 %). respectively. 2) The ratio of organism isolated from the cockroaches captured in each hospital appeared 98.6% in K hospital. 91.4% in P hospital and 100% in S hospital. respectively. 3) The total ratio of microganisms isolated from cockroaches captured in each ward of the hospitals was 96.9% and that by each ward was 96.0% in medical ward. 98.0% in surgical ward. 96.4% ill operating room. and 100% in infant care room. 4) The ratio of the microganisms isolated from the cockroaches captured in the kitchen of each hospital was 94.1% and that in the K and S hospitals was 100% and the P hospital. 83.3%. 5) The germ-carrying ratio of cockroaches captured from the jiving rooms and kitchens of general residences was 100%. 6) The staphylococci was isolated as 22.4% of 259 the corchrnahes and all of them belonged to negative coagulase. 7) Finaly. E. coli (10.4%) and Aevbarter aerogenes(29.7%) were isolted from the 259 cock-roaches. This was sugested that a great number of habitable cockroaches contacts directly with faces.
