PURPOSE: This study aimed to examine the pain, range of motion (ROM), upper extremity task performance, and functional levels of patients after rotator cuff repair according to the timing of a closed chain exercise thereby presenting basic data for an effective rehabilitation program. METHODS: The intervention was applied three times per week, one hour per day, for four weeks to 40 participants, 78 of whom had undergone rotator cuff repair. The participants were divided into four groups and assigned to usual general physical therapy and an open chain exercise. Group I consisted of the open chain exercise only. The closed chain exercise was applied to group II after the 4 times, group III after the 7 times, group IV after the 10 times. Measurement were used ROM, visual analogue scale (VAS), box and block test (BBT), and shoulder pain and disability index (SPADI). A one-way analysis of variance was conducted to test differences. RESULTS: There were significant differences in the internal/external rotation between group I and group II. The VAS significantly differed between group II and group I, group III, and group IV. The BBT results of group II and group I were significantly different compared to those of group IV. The SPADI significantly differed between group II and group I and between group II and group IV. CONCLUSION: The closed chain exercise was effective for patients following rotator cuff repair from the second week after active exercise was prescribed, verifying its applicability in rehabilitation programs.
Closed rupture of the extensor hallucis longus (EHL) tendon is uncommon and rarely reported. We present a Taekwondo player who had sustained a closed rupture of the EHL tendon after striking the other player's shin. He had practiced Taekwondo more than 6 hours a day for more than 6 years, including repetitive striking on the dorsum of the foot, which probably caused degeneration of the EHL tendon. The tendon ends could not be approximated directly, so reconstruction was performed with bisecting the distal tendon and combining the repair of the one end with an intercalary scar tissue and the other with tenodesis to the extensor hallucis brevis. He returned to the preoperative level of activity 6 months postoperatively with a satisfactory range of motion.
Purpose: The purpose of this study was to investigate the effects of forward-and-backward shift trunk exercise using a proprioceptive neuromuscular facilitation (PNF) diagonal pattern in a closed kinematic chain exercise on the upper limb function and activity of daily living (ADL) in a stroke patient. Methods: One subject participated in this study. The study used a reversal A-B-A' design, where A and A' were the baseline period (no intervention), and B was the intervention period. The intervention was a forward-and-backward trunk shift exercise, using a PNF diagonal pattern on both a stand-on-hand position and a quadruped position of closed kinematic chain exercises, for 20 min per day for 2 weeks. The range of motion (ROM) of the shoulder joint was measured and a Fugl-Meyer assessment of upper extremity (FMA-UE) and a functional independence measure (FIM) were performed to measure upper limb function and activity of daily living (ADL). Results: ROMs of shoulder joint (flexion, extension, abduction, and external rotation) increased in the intervention phase. The FMA-UE score increased (from 28 to 36) in the intervention phase. The FIM score increased (from 20 to 25) in the intervention phase. These increases were maintained after intervention (Baseline II). Conclusion: These results suggest that forward-and-backward shift trunk exercises using a PNF diagonal pattern in a closed kinematic chain exercise have a positive effect on stroke patients' upper limb function and ADL ability.
The flame retardancy and foaming properties of NBR foams containing waste ground tire rubber (GTR) were studied. When the composition ratios of NBR/GTR were ranged in $100/0{\sim}80/20$ (w/w) and rubbers/flame retardants were in the range of $1/1.95{\sim}3.70 wt%$, the considerably optimized foam for flame retardancy was obtained with high limiting oxygen indices (LOI, $29.4{\sim}40.0$), low heat release rate(HRR), closed/semi-closed cell of uniformity, and reasonable expandability ($215{\sim}300 %$). The LOI was increased and the heat release rate was decreased with increasing the amount of GTR content.
1. In order to investigate the effect of variation of assembly time on glue bond strength, and to determine the optimum range of assembly time with given glue, this experiment was made at the suggestion of the Wood Technology Laboratory, School of Forestry, Yale University. 2. For this investigation, three-ply-plywoods with 1/22 inch, birch veneer, phenolic resin, and soybean glue were made at the following variation of assembly time, that is, 1, 5, 10, 25, 35, 50, and 70 minutes, under both open and closed assembly manners, and the shear strength test at dry and wet were adoptted. 3. The shear strength and wood failure of each plywood panel constructed at the given assembly time have been illustrated in Tables 1, 2, 3 and 4. It has shown that there is a remarkable tendency, for increasing assembly time to give lower shear strength and wood failure throughout almost all cases. The effective range of assembly time of tested glues in this investigation for both open and closed assembly are summarized in the Table 7. Thus, allowable assembly time for Phenolic resin may be up to 10 minutes under open assembly and up to 50 minutes under closed assembly. For soybean glue, the permissible assembly time may be up to 5 minutes under open assembly and up to 15 minutes under closed assembly. The allowable assembly time for open assembly with the same glue is reduced by approximately one third or more than one third as compared with closed assembly time. This might mean that the closed asembly time for these glues is more practical than the open assembly.

Purpose : The purpose of this study was to determine the effects of upper extremity volume, pain, and range of motion after participation in thera-band exercises according to the hand grip type in patients with breast cancer with upper extremity edema. We also aimed to determine the most efficient type of grip. Methods : The subjects were 10 female patients diagnosed with stage 2 breast cancer who had stage 2 lymphedema. Randomly, 5 patients each were allocated to the experimental and control groups. For six weeks, the patients in both the experimental and control groups exercised daily. In both groups, manual lymph drainage was applied for 1 hour. Afterward, patients in the experimental group placed their hands in the thera-band ring and exercised with their fingers outstretched. Patients in the control group exercised while holding the thera-band ring with a finger. Both the experimental group and the control group underwent measurements of the circumference of the upper extremity, pain, and range of motion of the shoulder joint at weeks 1, 2, 4, and 6 before and after exercise. Results : The upper arm circumference decreased by more in the experimental group in all weeks than before than that in the control group, and there was a statistically significant difference at 6 weeks. Compared with the difference between pre-exercise and 6 weeks post-exercise, the change in pain significantly decreased in the experimental group and showed a statistically significant difference. The shoulder range of motion increased in extension, external rotation, and internal rotation compared with that in the control group, and there was a statistically significant difference. Although the operating range increased in flexion and abduction, there were no statistically significant differences. Conclusion : In this study, we found that thera-band exercises with an open-hand grip are more efficient than thera-band exercises with a closed-hand grip in edema reduction, pain, and range of motion. In addition, it was found that it was more effective to continue the thera-band exercises with open-hand grip extended for at least 6 weeks rather than for a short time.

Purpose : To assess perceptual, acoustic and aerodynamic measure of voice quality in patients with unilateral vocal cord paralysis before and after type I thyroplasty. Methods : The clinical records of patients operated type I thyroplasty in the Departement of otorhinoalryngolgy, Yongdong Severance hospital from November 2001 to November 2003 were reviewed. All patients uderwent a vocal function evaluation including perceptual, acoustic and aerodynamic measures of voice preoperative and on $60^{th}$ postoperative day. The perceptual and acoustic measures were obtained from recording of patients' reading a 'Sanchak' passage. The perceptual evaluation was performed by 2 speech pathologist using a 4-point rating scale. Acoustic parameters(voice range profile low(RAL), voice range profile high(RAH), average fundamental frequency(AFX), closed quotient, harmonic to noise ratio, jitter and shimmer) were investigated by Lx speech studio. Mean flow rate(MFR), subglottic pressure(Psub) and intensity were measured using the Phonatory function analyzer. The maximum phonation time was also measured. The data were statistically analyzed. A paired t-test (p<0.1) was used to compare preoperative and postoperative results. And multiple regression test was used to find which parameter was most correlated to improvement of postoperative voice quality. Results : Among aerodynamic parameters, Psub $(88.11mmH_2O{\rightarrow}58.7mmH_2O)$, MPT(7.87sec${\rightarrow}$12.53sec), MFR (359.8ml/sec${\rightarrow}$161.06ml/sec) were statistically improved. AFx(205.5Hz${\rightarrow}$163.27Hz), AQx(23.9%${\rightarrow}$48.3%), RAL, RAH. Jotter and shimmer were improved. In multiple regression test, AFx and AQx was noted as the two meost correlated parameters to improvement of postoperative breathiness. But general grade of voice quality was more correlated to Psub and shimmer. Conclusion : Vocal fold medialization procedures effectively reduce glottic gap. Increasing of contact area of both vocal folds induced improvement in aerodynamic parameters and leaded stabilizing of vocal fold vibration. That effect results in improvement in acoustic parameters (shimmer, jitter, signal-to-noise ratio, voice range profile) and voice quality.
Maxillectomy is performed to remove the tumor in the palate, maxillary sinus, buccal mucosa or nasal cavity. The resection range depends on the size and the extent of the tumor and it affects speech production or cause nasal regurgitation during feeding. Obturator can occlude an opening such as an oro-nasal fistula and protect the defect area. Successful reconstrucion of the patient's oral cavity who have gone over the maxillectomy is a difficult task. The condition and number of teeth, the remaining support area, and the extent of the defect area have a great influence on manufacturing the obturator. If these factors are disadvantageous, the prognosis of the prosthesis is uncertain. The final obturator must have a sufficient retention in the patient's oral cavity and must not irritate the surrounding tissue and support area where the resection was performed.In this case, a 55 year old female went through the maxillectomy and the only 3 teeth remained. And the retention of the maxillary prosthesis seems to be poor. So that, we fabricated the closed hollow obturator which has reduced weight compared to the conventional obturator. Consequently the closed hollow obturator can give better sealing and the adaptation.
In this study, we classified SNS into open and closed types, and empirically examined in which SNS activity the privacy paradox holds. The idea comes from the argument that privacy paradox may be observed differently in the open SNS, which is more vulnerable to the leakage of personal information due to public profiles, and the closed SNS, which is relatively less vulnerable by limiting the range of acquaintances, The results of the empirical analysis are as follows. First, in case of SNS usage, the privacy paradox holds in the overall SNS activities, but different conclusions are drawn according to open and closed SNS. In particular, it is found that as privacy concerns increase, individuals respond in a reasonable and desirable way to reduce SNS activity in the open SNS, which is more susceptible to infringement. Second, in the case of SNS activity intensity, (i) heavy users are more seriously aware of the probability of privacy infringement than light users, so there is a reasonable response to reducing the intensive margin with increasing privacy concerns, and (ii) this tendency is more clearly observed in open SNS, which is more vulnerable to privacy infringement. Accordingly, insisting that the privacy paradox is empirically established by observing only the overall SNS activities without distinguishing them into open and closed SNS may be interpreted as a "Fallacy of Composition."
Clinical evaluation by Astigmatic Components of autorefractors was performed to examine validity and accuracy compared with subjective refraction. The mean cylindrical powers of Astigmatic Components of Subjective Refraction, Closed-view and Open-view were found to be $-0.689{\pm}0.516D$, $-0.691{\pm}0.530D$, $-0.470{\pm}0.507D$, respectively. The Difference of cylindrical Components between Subjective Refraction and Closed, Open-view were found to be $0.002{\pm}0.191D$, $0.015{\pm}0.137D$, respectively. Approximately 85% of Closed-view and Open-view autorefractor measurements were within ${\pm}20^{\circ}$ range of the difference of cylindrical axis with subjective refraction. The mean $J_0$ Components of Astigmatic Components of subjective refraction, Closed-view and Open-view were found to be $-0.155{\pm}0.332D$, $-0.004{\pm}0.178D$, $-0.001{\pm}0.156D$, respectively. The mean $J_{45}$ Components of Astigmatic Components, Closed-view and Open-view were found to be $-0.023{\pm}0.227D$, $-0.026{\pm}0.172D$, $-0.014{\pm}0.182D$, respectively. Both of the Closed-view and Open-view refractor showed available accuracy in measuring astigmatic components.
