The analysis of present condition and the method of medical treatment studies on Scolopendrid Herbal Acupuncture (오공약침(蜈蚣藥鍼)에 대한 연구현황분석(硏究現況分析)과 치료법(治療法)에 관한 연구(硏究))

  • Objectives : We review a result of studies until the present and suggest the method of medical treatment for the clinical treatment of Scolopendrid Herbalacupuncture. Methods : We analysis the paper of the bibliographic studies, the experiment studies and the clinical studies from 2001 developed Scolopendrid Herbalacupuncture and grope for the course of studies. Results : 1. Scolopendrid Herbalacupuncture is proved the clinical safety by the aninmal and human tests. 2. The pharmacological action of Scolopendra subspinipes mutilans L. Koch is anti-convulsive action, analgesic action, lowering blood pressure, anti-inflammatory action, anti-tumor action and microbe inhibition 3. Scolopendrid Herbalacupuncture has been a fine effect to the entrapment neuropathy and inflammatory. 4. Scolopendrid Herbalacupuncture was thought effective on a acute phase and to the excessive symptoms. The Sub-chronic toxicity experiment observing the response after hypodermic medication over 90 days, The Genetic-mutagenic toxity experiment and the clinical effect studies are necessary.

Review of Clinical Researches for Herbal Medicine Treatment in Pediatric Growth - Focusing on recent national studies - (국내 소아성장 한약치료에 대한 고찰)

  • Objectives The purpose of this study is to provide clinical evidences on effectiveness of herbal medicine treatment regarding pediatric growth based on recent clinical studies. Methods Selected 17 clinical studies since January 1998 to June 2018 from the Journal of Pediatrics of Korean Medicine and OASIS Traditional Medicine Knowledge Portal were reviewed. Results Medications that strengthening spleen and stomach (健脾胃藥), digestant (消導藥), supplementing kidneys (補 腎藥), and lungs (補肺藥) were frequently used. Frequently used herbs were Hoelen (茯?), Citri Pericarpium (陳皮), Glycyrrhizae Radix (甘草), Atractylodis Rhizoma Alba (白朮), Dioscoreae Rhizoma (山藥), Angelicae Gigantis Radix (當歸), Amomi Fructus (砂仁), Rehmanniae Radix Preparata (熟地黃). Conclusions This study showed that Korean herb medicine treatment can be effective for pediatric growth.

  • Colorectal cancer (CRC) is the third most common cause of cancer-related death in the world. Although the long-term outcome of patients with metastatic CRC is still poor, target therapy including anti EGFR agents and anti VEGF agents and immunotherapy including anti PD-1 antibody and anti CTLA-4 antibody have shown clinical benefits in the treatment of patient with metastatic CRC. In the future, the personalized treatment strategy based on the clinical characteristics and biologic features of patients with metastatic CRC will be necessary. In this review, we summarized the mechanisms and clinical evidences of target therapy and immunotherapy, and the guideline of clinical practice in patients with metastatic CRC.

Dental students' clinical confidence in denture treatment after clinical experiences (의치진료 경험 후 치과대학 학생들의 임상진료능력에 대한 자기인식도)

  • Purpose: The purpose of this study is to investigate dental students' self-perception of clinical care ability after a denture treatment experience. Materials and methods: Of the 58 fourth-year students at the dental school in 2019, 50 students completed the questionnaire concerning their denture treatment session, including treatment planning and pre-prosthetic treatment (4 questions), clinical and laboratory procedures (20 questions), and students' opinions on clinical denture education (4 questions). Each question was answered on a five-point scale, of which points four and five, "agree" and "strongly agree" respectively, were classified as positive responses while points one and two, "disagree" and "strongly disagree" respectively, were classified as negative. Results: All the items on clinical and laboratory procedures received a positive response rate of over 60%, and the overall self-perception of the students appeared to be positive. However, the questions on the ability to perform denture treatment independently after graduation received only 48% of positive answer. Those on patient management, explanations of denture precautions, customized tray production, boxing, and work model production were answered with the highest positive response rate whereas those on treatment planning, final impression acquisition, and surveying, were the lowest in each session. Conclusion: The dental students with experience in denture treatment generally have a positive opinion of their clinical care ability, but further education may be necessary to improve their ability to treat their patients independently.

  • In clinical trials with repeated measurements, the time-averaged difference (TAD) may provide a more powerful evaluation of treatment efficacy than the rate of changes over time when the treatment effect has rapid onset and repeated measurements continue across an extended period after a maximum effect is achieved (Overall and Doyle, Controlled Clinical Trials, 15, 100-123, 1994). The sample size formula has been investigated by many researchers for the evaluation of TAD in two treatment groups. For the evaluation of TAD in multi-arm trials, Zhang and Ahn (Computational Statistics & Data Analysis, 58, 283-291, 2013) and Lou et al. (Communications in Statistics-Theory and Methods, 46, 11204-11213, 2017b) developed the sample size formulas for continuous outcomes and count outcomes, respectively. In this paper, we derive a sample size formula to evaluate the TAD of the repeated binary outcomes in multi-arm trials using the generalized estimating equation approach. This proposed sample size formula accounts for various correlation structures and missing patterns (including a mixture of independent missing and monotone missing patterns) that are frequently encountered by practitioners in clinical trials. We conduct simulation studies to assess the performance of the proposed sample size formula under a wide range of design parameters. The results show that the empirical powers and the empirical Type I errors are close to nominal levels. We illustrate our proposed method using a clinical trial example.

Opinion on the legalization of the actual duties of clinical dental hygienists (임상치과위생사의 실제적 업무의 법제화에 대한 견해)

  • purpose : The purpose of the study is to investigate opinion on the legalization of the actual work of clinical dental hygienists. Methods : A self-reported questionnaire was completed by 171 dental hygienists in Busan and Gyeongnam from December 1, 2017 to January 31, 2018. Structured questionnaires were uesd for analysis. The collected data was analyzed using IBM SPSS. Statistic 20.0. Results : The first, assist of dental treatment, Second, dental health care education and consulting duties, and third priority preventive duties were found to be the most important duties in the dental office. Currently, it was investigated perform in a lot of workplace. Preventive treatment tasks include scaling, teeth polishing, applying fluoride and assist of dental treatment include preparation for medical care, preparation for implant surgery, preparation for oral surgery, implant surgery assistant and patient care training, and preparation for periodontal surgery. Conclusion : As a result, the dental hygienist performing a lot of dental assist tasks, oral health education counseling and preventive duties. Nonetheless, clinical dental hygienists are outlaws due to the disparity between practical and legal duties. Therefore, it is urgent to legislate actual duties of dental hygienist so that experts can fully exercise the competency of the public in promoting dental health. Relevant governments and related organizations should take reasonable measures to solve this problem.

Review of Domestic Clinical Research Trends of Korean Medicine on Alopecia areata (원형탈모증의 국내 한의학 임상 연구동향)

  • The purpose of this study is to report the research trends of Korean medicine treatment for alopecia areata. We investigated the studies on Korean medicine treatments for alopecia areata via searching 4 web databases of Korea. 13 papers were selected, and we classified them according to publication year, published journals, the types of study, research subject, the methods of treatment, confirmation of recurrence, methods of evaluation, and institutional review board(IRB). We analyzed a total of 13 clinical studies of Korean medicine on alopecia areata based on various criteria and reviewed the research trends. Based on this, we suggested points to be improved and supplemented when conducting clinical research on alopecia areata in the future. Clinical studies on alopecia areata started to be published in 2001, and acupuncture, pharmacopuncture, herbal medicines, etc. were used as treatment methods for alopecia areata. Most of the clinical studies on alopecia areata were case studies, so it is thought that various types of clinical studies are needed. In addition, it is necessary to confirm the recurrence of alopecia areata and to improve the awareness of research ethics.

The clinical success of ART restorations and Hall technique in primary molars: a randomized 18-month follow-up study

  • Objectives: The aim of the study was to evaluate the clinical and radiographic success of the Hall technique (HT) and atraumatic restorative treatment (ART) restorations using high-viscosity glass-ionomer cement for the management of occlusal carious lesions in primary molars. Materials and Methods: This randomized clinical study observed 40 children (aged 5-6 years). For each child, one tooth was treated with HT and one with ART. The primary outcome measures for HT restorations were successful, minor, and major failure rates. Clinical evaluations of ART restorations were performed according to the modified United States Public Health Service criteria during 18-month follow-up. McNemar test was used for statistical analysis. Results: Thirty of 40 (75%) participants returned for 18 months of follow-up. In the clinical evaluations of teeth that were treated with HT, the patients did not have complaints of pain or other symptoms, all crowns remained in the oral cavity, the gums were healthy, and the teeth were functional in all evaluations. At the end of the 18-month follow-up, the surface texture and marginal integrity criteria of ART restorations were recorded as 26.7% and 33.3%, respectively. In the radiographic evaluation of 30 patients treated with ART and HT, all restorations were considered successful. Conclusions: The 18-month clinical and radiographic results after treatments applied to single-surface cavities in anxious children showed that both treatment methods were successful.

A case study of normal pressure hydrocephalus patient with gait disturbance using conservative Korean medical treatment (정상압수두증에 의한 보행장애 환자 치험 1례)

  • A case of a 75-year-old Korean female with gait disturbance due to Normal pressure hydrocephalus (NPH) is presented. She was treated with acupuncture, electroacupuncture and herbal medicine - 柴苓湯(Shirhyung-Tang, Chai-ling-tang, Sairei-to) We used iNPH grading scale, and specified further the grade of gait disturbance category. After Korean medical treatment, there was notable improvement in gait disturbance on our specified scale. Cognitive impairment, tremor and rigidity were improved on each scale alongside. Korean medical treatment may be effective in treating NPH patients.

Impact of Treatment Type on Overall Survival in Elderly Brazilian Women with Breast Cancer

  • Objective: To analyze the survival of elderly patients with breast cancer according to the type of treatment used. Methods: A cohort study of women aged 80 or over with breast cancer registered with the Brazilian National Cancer Institute (Instituto Nacional do $C{\hat{a}}ncer$ - INCA) between 2008 and 2009 was conducted. Prognosis was analyzed according to the cancer treatment performed: surgery, radiotherapy, or hormone therapy. Analysis of the overall 5-year survival rate was performed using the Kaplan - Meier method, and comparisons of curves were undertaken using the log-rank test. For multiple regression analysis, Cox regression was used, adjusting for age and clinical stage, considering values of p < 0.05 as significant. Data were all analyzed using the statistical package SPSS version 20. Results: 70 women with a mean age of $84.0{\pm}3.7years$ at diagnosis participated in the study. The median follow-up time was 37.1 months (range 0.5-75.5), and 31 deaths (44.3%) occurred during this time. The median survival time was 51.2 months (95% CI, 44.9-57.4), higher in those who underwent surgery (p = 0.012) and those who had hormone therapy (p=0.001). Treatment with surgery reduced the risk of death by 61.7% (HR 0.3; 95% CI, 0.1-0.6; p = 0.001) when adjusted for clinical stage and age at diagnosis. However, there was no significant benefit from radiotherapy (HR 1.2; 95% CI, 0.5-2.5; p = 0.694). Conclusion: Treatment with surgery and hormone therapy increased the survival of our Brazilian patients with breast cancer aged 80 or over.
