Clinical Research Trends of Gut Microbiome for Respiratory Diseases (호흡기질환에 대한 장내 미생물의 임상 연구 동향)

  • Objectives: This study aimed to review the clinical research of the gut microbiome for respiratory diseases to assist the design of trials for respiratory diseases by regulating the gut microbiome with herbal medicine later. Methods: We searched three international databases (PubMed, CENTRAL and EMBASE) to investigate randomized controlled trials (RCTs) of the gut microbiome for respiratory diseases. The selected trials were analyzed by study design, subject diseases, inclusion/exclusion criteria, sample size, study period, intervention group, control group, outcome measures, and study results. Results: A total of 25 studies were included and published from 1994 to 2021 mostly in Europe and Asia. Subject diseases were many in the order of respiratory tract infection, cystic fibrosis, allergy, and so on. As outcome measures, the gut microbiome in a fecal sample was analyzed by 16S rRNA sequencing analysis method, and symptom assessment tools related each disease were used. Major intervention drugs were probiotics and the results were mostly improved in the composition and diversity of the gut microbiome. Conclusion: Clinical studies of the gut microbiome for respiratory diseases have confirmed various effects and this review provides basic data for a well-designed clinical study for respiratory diseases by regulating the gut microbiome with herbal medicine.

A Study on the Critical Thinking Disposition and Clinical Competency of Nursing Students (간호대학생의 비판적 사고성향과 임상수행능력에 관한 연구)

  • Purpose: The purpose of this study was to identify the relationship between critical thinking disposition and clinical competency of nursing students. Method : The sample consisted of 151 nursing students, who have ever had clinical practice. Data were collected by self reporting questionnaire for 13 days from April 18-30, 2005. The data were analyzed by descriptive statistics, t-test, ANOVA, Duncan test, Pearson Correlation Coefficient with SPSS Programs. Result : The result of this study were as follows : 1. The total mean score of critical thinking dispositions in the nursing students was moderately(3.50). There was a statistically significant difference in critical thinking disposition according to satisfaction with the nursing major(F=5.563, p=.005). 2. The total mean score of clinical competency in the nursing students was slightly high(3.37). There was a statistically significant difference in clinical competency according to adaptation with the nursing major(F=5.202, p=.007), satisfaction with clinical practice(F=3.172, p=.045). 3. A significant positive correlation between critical thinking disposition and clinical competency was founded(r= .421, p<.000). Conclusion : In conclusion, this study revealed that critical thinking disposition influences clinical competence. Therefore, the finding of this study may provide significant basic data for nursing education and nursing practice.

A Clinical Usefulness of Korean Version of Strengths and Difficulties Questionnaire (강점.난점설문지의(SDQ-Kr)의 임상 활용도)

  • Objectives : This study aimed to evaluate the clinical usefulness of Korean version of Strengths and Difficulties Questionnaire(SDQ-Kr) by comparing with Korean version of Childhood Behavior Checklist (K-CBCL). Methods : The parent version of SDQ-Kr and K-CBCL were administered to parents of 313 children in psychiatric clinic, 91 children referred for psychiatric consultation from pediatric clinic and 93 control children in pediatric clinic. All children aged 4-11 years old. Children in psychiatric clinic were diagnosed with one of followings;ADHD, emotional disorders and oppositional/conduct disorder. Mean scores were compared among three groups by gender. Coefficients were calculated for corresponding problem scales and total scores of both questionnaires. ROC analysis was performed for discriminant validity to distinguish psychiatry and pediatrics samples, and also to distinguish diagnostic groups of psychiatry sample. Results : The psychiatry sample showed highest means of problem scales and total scores, followed by consultation and control sample of pediatrics. SDQ-Kr and K-CBCL were significantly correlated, and equally able to distinguish those samples with SDQ-Kr showing better results. Within the psychiatry sample, SDQ-Kr had higher predictive power for all diagnostic groups. Conclusion : This study supports clinical usefulness of SDQ-Kr. We suggest that SDQ-Kr could be used not only as a diagnostic tool for children in psychiatric clinic but also as an effective and efficient screening instrument for children in need for psychiatric evaluation in pediatric clinic.

A Systematic Review on Prevention and Treatment of Nipple Pain and Fissure: Are They Curable?

  • Averagely 80% to 90% of breastfeeding women experience the nipple pain and fissures. The important factor for successful breastfeeding is to treat this problem. This study has done as a review with the aim of analysis of the clinical trials in the field of the prevention and treatment of the nipple fissures and pain due to the importance of breastfeeding. For this purpose, the key words of sore, nipples, fissure, trauma, wound, prevention, treatment, therapeutics, therapy, clinical trial, breastfeeding and their Persian synonyms and all of their possible combinations were searched in the national databases: SID and Iran Medex and Magiran, and in the international databases: PubMed, Scopus, Medline, Science direct by May 2017. The Jadad criterion was used to assess the quality of the articles and the articles with a score of 3 or more were included in this study. Finally, 48 clinical trials were reviewed that 17 of them (sample size 1801) scored 3 or more based on the Jadad criterion. Seven articles were also in the non- drug treatment group (sample size 491) and 2 articles in the drug treatment group (sample size 337) and 8 articles in the herbal treatment group (sample size 973).The results show that menthol and warm water compress as well as teaching the correct breastfeeding methods are effective treatments to prevent and treat the nipple pain and fissures. Moreover, applying the herbal medicine for prevention and treatment of the issues raised from breastfeeding may have beneficial such as Aloe vera, Portulaca olearacea. However, more studies with a great methodology are necessary to obtain more accurate evidence.

  • The increasing utilization of radioactive isotopes in industry, medicine and research has raised the question, 'How should hospitals deal with radiation injuries when they occur?' A system for initial management of radiation injuries has been developed by Radiation Management Corporation. Radiation injuries are classified and a treatment plan outlined for each at the emergency and short term medical care phase. This system includes clinical prognosis as well as a detailed plan for quick set up or a Radiation Emergency Area in any hospital. Procedures for patient admission, preparation of the facility, general decontamination, sample taking, and wound decontamination are included.

A Nonparametric Test for Clinical Trial with Low Infection Rate

  • This paper evaluates a new clinical trial designs for low infection rate disease. This type of sparse disease reaction makes the traditional two sample t-test or Wilcoxon rank-sum test inefficient compared to a new test suggested. The new test, which is based solely on the larger changes, is shown to be more effective than existing method by simulation for small samples. However, this test can be shown to be connected to the locally most powerful rank test under certain practical conditions. This design is motivated in testing the treatment effects in periodontal disease research.

The Relationship between Depression, Perceived Stress, Fatigue and Anger in Clinical Nurses (임상간호사의 우울과 지각된 스트레스, 피로 및 분노간의 관계)

  • Purpose: The purpose of this study was to identify the relationship between depression, perceived stress, fatigue and anger in clinical nurses. Method: A descriptive survey was conducted using a convenient sample. Data was collected by questionnaires from four hundred clinical nurses who worked at a university hospital. Radloff's CES-D for depression, Cohen, Kamarck & Mermelstein's Perceived Stress Scale, VAS for Fatigue, and Spielberger's STAXI for anger were used. The data was analyzed using the pearson correlation coefficient, students' t-test, ANOVA, and stepwise multiple regression with SPSS/WIN 12.0. Result: The depression of clinical nurses showed a significantly positive correlation to perceived stress(r=.360, p=.000), mental fatigue(r=.471, p=.000), physical fatigue(r=.350, p=.000), trait anger(r=.370, p=.000), anger-in expression(r=.231, p=.000), and anger-control expression(r=.120, p=.016). There was a negative correlation between depression and age(r=-.146, p=.003). The mean score of depression of nurses, 26, was a very high score and 40.8% of clinical nurses were included in a depression group. The main significant predictors influencing depression of clinical nurses were mental fatigue, trail anger, perceived stress, anger-in expression, and state anger, which explained about 32.7%. Conclusion: These results indicate that clinical nurses with a high degree of perceived stress, mental fatigue and anger-in expression are likely to be depressed.

Stress, Depression, Coping Styles and Satisfaction of Clinical Practice in Nursing Students (일 간호대학생의 스트레스, 우울, 대처방법과 임상실습 만족도)

  • Purpose: The purpose of this study was to investigate the relationship among stress, depression, coping styles and satisfaction of clinical practice in nursing students. Method: Two hundred fifty-eight nursing college students in 1 city were selected by convenient sample. A questionnaire measured the level of stress, depression, coping styles and satisfaction of clinical practice. The data were analyzed using descriptive statistics, t-test, ANOVA, and Pearson's correlation coefficient. Result: The mean score of stress was 2.82 (${\pm}.39$), depression was 19.23 (${\pm}7.70$), coping styles was 2.42 (${\pm}.42$), and satisfaction of clinical practice was 2.81 (${\pm}.41$). There were significant differences on four variables according to the satisfaction of a nursing major. Stress had a significant positive correlation with depression, and a negative correlation with problem focused coping and satisfaction of clinical practice. Depression had a significant negative correlation with satisfaction of clinical practice. Emotional focused coping had a significant positive correlation with depression, and problem focused coping had a significant negative correlation with depression. Conclusion: From the studies reviewed, one can expect a positive effect on decreasing stress and depression among nursing students when the nursing educators use counseling and instruction for clinical practice.

Telephone Survey for Grasping Clinical Actual State of Bloodletting Therapeutics in Korea (국내 자락(사혈)요법 임상 실태 파악을 위한 전화조사)

  • Objectives : Bloodletting therapeutics is one of the most popular oriental treatments in Korea. In this study, we operate the Telephone Survey for grasping clinical actual state bloodletting therapeutics in Korea. Methods : The list of the Korean medical doctors with experiences more than 10 years is provided by the Association of the Korean Oriental Medicine. A stratified random sample of Korean medical doctors is drawn for the telephone interviews. We choose a bound on the error of estimation equal to 3.2 percentage, and the sample size is 321 for the national sample. We develop a questionnaire for telephone interviews. We carefully design the questionnaire to minimize non-sampling errors by using the focus group activities and the pretest. We give the details of the results based on the sample design. Also, we present some socialdemograhpic characteristics for the samples. Telephone interviews with them were conducted by the well-trained interviewers of College of Korean Medicine student from 11th May 2006 to 17th May 2006. Results : 1. Two hundred eighty eight(89.4%) out of 322 Korean oriental medical doctors used bloodletting therapeutics. 2. The most common main bloodletting treatment is reported by patients was musculo-skeletal disorder(60.2%). 3. Two hundred ninety one(90.3%) out of 322 Korean oriental medical doctors opposed that commercial using bloodletting without medical diagnosis. Conclusion : Bloodletting therapeutics is one of the most popular oriental treatments in Korea. The majority(89.4%) of Korean medical doctors used bloodletting therapeutics in this survey.

Detection of the Carcinogenic Liver Fluke Opisthorchis viverrini Using a Mini Parasep SF Faecal Parasite Concentrator

  • The Mini Parasep SF fecal parasite concentrator (MPSFC) is a new modification of the closed concentration system, which can easily be adopted in any routine clinical pathology laboratory. Here we describe our experience with the system in diagnosing Opisthorchis viverrini. A total of 199 fecal samples was submitted for routine examination in the clinical pathology laboratory of Suranaree University of Technology hospital, Nakhon Ratchasima province, Thailand, during August to October 2015. Out of all samples examined, 10 (5.03%) were positive with intestinal parasites including O. viverrini (2.01%), followed by Strongyloided stercoralis (1.51%), Hookworm (0.5%), Taenia spp. (0.5%), and Entamoeba coli (0.5%). Regarding the distribution of intestinal parasites in relation to the methods used, and found that 4 samples (2.01%) were positive using the direct wet smear method while 10 (5.03%) were positive with the Mini Parasep SF method; the difference was statistically significant ($X^2$-test = 116.506, p-value =0.001). Mean time for processing using the Parasep system was 6.03 min/sample, the conventional direct wet smear method at 0.3 min/sample. Cost per test, conventional direct wet smear method costing less than the Parasep method at USD 0.74/sample versus USD 1.47/sample. This first report of O. viverrini detection using MPSFC indicates that Parasep concentration test is useful in the routine laboratory, increasing the yield of parasites as compared to direct microscopy, but with greater processing time and cost. Further comparisons between the Parasep concentration test and common methods for O. viverrini detection are required, particularly concerning use in epidemiological surveys.
