제주지역 치위생과 학생의 COVID-19와 감염관리 임상실습 수행 실태 (Clinical Practice of COVID-19 and Infection Control by Dental Hygiene Students in Jeju Province)

  • 본 연구의 목적은 제주 치위생과 학생을 대상으로 COVID-19 상황에 임상 실습 수행 중 감염관리에 영향을 미치는 요인을 분석하는 데 있다. 112명의 온라인 자기 기입식 설문 응답을 SPSS 20.0을 이용하여 빈도분석, T-test, one-way ANOVA와 더미 회귀분석을 하였다. 감염관리 인식도와 실천도는 2학년이 3학년보다 '청소 및 표면 소독', '의료폐기물 처리', 'COVID-19 감염관리' 항목에서 통계적으로 유의하게 높았다. 임상실습기관별 결과는 '청소 및 표면 소독', '기구 소독 및 멸균', '개인 및 환자 방호', 'COVID-19 감염관리' 항목에서 대형병원이 통계적으로 유의하게 높았다. 임상실습 지역은 의료폐기물 처리' 항목에서만 수도권이 제주지역보다 높았다. COVID-19 감염관리 요인 분석 결과, 3학년보다 2학년, 실습 기관 중 대형병원, 실습 지역 중 수도권 지역, 보호장구 애로사항이 없던 학생이 통계적으로 높은 영향을 미치는 것으로 나타났다. 본 연구는 제주지역 치위생과 학생들의 임상 실습시의 감염관리와 COVID-19의 현장을 대상으로 한 첫 번째 실태 조사 연구로 의의를 두며, 임상 실습 지침 개선을 위한 연구가 지속되어야 할 것이다.

신규간호사의 이직 경험 (Experience of Turnover in New Nurses)

  • Purpose: The purpose of this study was to try to understand the essence of the experience early turnover of the new nurse by applying the phenomenological method and to provide basic data for a community-based management program. Method: Phenomenological approach was used to identify subjects experience. Subjects were five new nurses, with less than one year of clinical experience in clinical practice. This study used in-depth interview. Results: there were 104 meaningful sentences or phrases, with 41 generally comprehensive thema. Finally, thema were classified into 12 thema clusters. Conclusion: Finally, based on the results, some suggestions regarding management of early turnover of new nurses are needed. First, we proposed a new characterized of hospital selection method for each hospital. Second, practical training in clinical practice in the school and the community is necessary in order to reduce the real impact of new nurses. New nurses require various support elements in order to mitigate the real shock the first time they encounter clinical practice. Third, addition of work-related training and promotion of a self-esteem program will be needed. Fourth, interview opportunities with seniors who adapted successfully in clinical should be provided for new nurses. In addition, continuous communication should be provided for new nurses.

A Systematic Review of Evidence for Education and Training Interventions in Microsurgery

  • Over the past decade, driven by advances in educational theory and pressures for efficiency in the clinical environment, there has been a shift in surgical education and training towards enhanced simulation training. Microsurgery is a technical skill with a steep competency learning curve on which the clinical outcome greatly depends. This paper investigates the evidence for educational and training interventions of traditional microsurgical skills courses in order to establish the best evidence practice in education and training and curriculum design. A systematic review of MEDLINE, EMBASE, and PubMed databases was performed to identify randomized control trials looking at educational and training interventions that objectively improved microsurgical skill acquisition, and these were critically appraised using the BestBETs group methodology. The databases search yielded 1,148, 1,460, and 2,277 citations respectively. These were then further limited to randomized controlled trials from which abstract reviews reduced the number to 5 relevant randomised controlled clinical trials. The best evidence supported a laboratory based low fidelity model microsurgical skills curriculum. There was strong evidence that technical skills acquired on low fidelity models transfers to improved performance on higher fidelity human cadaver models and that self directed practice leads to improved technical performance. Although there is significant paucity in the literature to support current microsurgical education and training practices, simulated training on low fidelity models in microsurgery is an effective intervention that leads to acquisition of transferable skills and improved technical performance. Further research to identify educational interventions associated with accelerated skill acquisition is required.

여성건강간호학의 시뮬레이션 실습교육에서 간호대학생의 환자역할경험의 교육적 효과 (The Educational Effects of the Experience of Nursing Students' Patients Role in the Simulation Practice Education for the Women's Health Nursing)

  • Purpose: The purpose of this study is to identify the educational effect among nursing students who performed the patient role in women's health nursing simulations. Methods: In this exploratory qualitative study, a sample of 31 third- and fourth-grade nursing students who participated in scenario-based patient roles in clinical performance examination of the women's health nursing simulation practice training. Data were collected through focus group interviews. Qualitative data were analyzed using qualitative content analysis. Results: Three themes emerged from the data analysis. Participants experienced an enhancement of patient-centered nursing competence, deep learning immersion and display of self-regulated learning. The difficulty of performing the patient role contributed additional effects such as the difficulty to perform the patient role in the psychological training environment. Conclusion: It is recommended to utilize nursing students as patients in simulation practice training. On the other hand, the psychological training environment can cause difficulties in performing patient roles, a burden on the role of the patient, and involves the interruption of the role.

  • Mobile computing devices (MCDs), such as smartphones and tablets, are revolutionizing medical practice. These devices are almost universally available and offer a multitude of capabilities, including online features, streaming capabilities, high-quality cameras, and numerous applications. Within the surgical field, MCDs are increasingly being used for simulations. Microsurgery is an expanding field of surgery that presents unique challenges to both trainees and trainers. Simulation-based training and assessment in microsurgery currently play an integral role in the preparation of trainee surgeons in a safe and informative environment. MCDs address these challenges in a novel way by providing valuable adjuncts to microsurgical training, assessment, and clinical practice through low-cost, effective, and widely accessible solutions. Herein, we present a review of the capabilities, accessibility, and relevance of MCDs for technical skills acquisition, training, and clinical microsurgery practice, and consider the possibility of their wider use in the future of microsurgical training and education.

임상실습을 경험한 간호대학생의 감염관리 활동의 관찰과 수행을 통한 감염관리에 대한 인식 및 심리적 갈등 (Perception and Inner Struggle Experienced by Nursing Students in Relation with Infection Management through Observation and Performance of Infection Control Activities)

  • Purpose: To study the internal psychological conflicts among nursing students during an infection control protocol carried out in the hospital by measuring their observation skills and performance during clinical training. Methods: Investigation of both pre- and post- infection control was conducted using questionnaires for clinical infection practices. We identified and evaluated the students' observation skills, clinical performance, clinical perception, and internal conflict regarding clinical infection control. We also interviewed the students as part of our study. Results: Among parameters such as clinical performance, observation skills, clinical perception, and internal conflict, the average observation skills (t=5.49, p<.001) were significantly lower, while internal conflict among students (t=-7.23, p<.001) was significantly higher than expected prior to clinical training. Generally, there was a negative correlation between observation skills and internal conflict in every aspect of infection control practice (r=-.281, p=.031). Internal conflict was significantly higher than expected in the context of hand hygiene (t=-2.135, p=.037), personal hygiene (t=-3.48, p=.002), and ventilator management (t=-3.69, p<.001). Clinical performance of students in the context of hand hygiene (t=4.69, p<.001), personal hygiene (t=2.06, p=.044), and ventilator management (t=2.68, p<.001) was significantly lower than expected prior to clinical training. Conclusion: Our findings showed that internal psychological conflict is higher when infection control practices are observed or performed to a lesser degree. Therefore, reinforcing education regarding infection control among students, such as developing a systematic program, or consecutive training and monitoring, is suggested.

치기공과 학생의 임상실습 만족도가 진로결정수준에 미치는 영향 (The effect of Clinical Practice Satisfaction on Career Decision Levels among Dental Technology Students)

  • Purpose: This study aims to assess clinical practice satisfaction among dental technology students and to identify the factors that may influence their career decision levels. In doing so, the present study provides preliminary data as to ways to improve clinical practice satisfaction levels and career decision levels that would be optimal for one's competence. Methods: he survey was conducted on dental technology students. The collected data was analyzed by the statistical program SPSS 21.0. The results were analyzed by reliability, frequency, multiple-way ANOVA, correlation, multiple regression. To test for significance on each item, p<0.05 has been decided as a standard. Results: Analysis results indicate that the factors influencing clinical practice satisfaction of dental technology students include school year, satisfaction of major, area of clinical practice, and assigned department for clinical practice, while the factors that influence career decision levels include satisfaction of major and clinical practice satisfaction. Conclusion: Results of present study suggest that in order to improve clinical practice satisfaction among dental technology students and thereby career decision levels, one needs to focus on satisfaction of major and quality of clinical practice. Such efforts will enable a more active and effective approach toward clinical practice for the dental technology students, and with appropriate understanding and assessment of the experience, they will be better equipped to make a career decision where they can live up to their competence.

비의료인에게 실시한 심폐소생술 교육의 효과 (The Effect of a CPR Training for Non-Healthcare Providers)

  • Purpose: The purpose of this study was to find out the effectiveness of the CPR training for non-healthcare providers by checking the performing ability of the subjects before and after the teaching class. Method: This study was a single primary experimental study designed with pre and post test. Study subjects were 37 office staffs who participated in all test from March, 10th, 2005 to April, 18th, 2005. The tools of this study were CPR performing evaluation sheet and CPR teaching computer program. Data analyzed by descriptive statistics, t-test, repeated ANOVA and Bonferroni with SPSS 12.0 program. Result: The CPR practice performing ability statistically significant increased by the step of test. The CPR practice performing ability of learning experience group had significantly high score than non experience group. The self-confidence about CPR practice when faced emergency situation was increased after practice training. Conclusion: One to one CPR practice teaching method using simulation by CPR teaching specialists was turned out to be very effective in improving the ability of CPR practice of non-healthcare providers in a tertiary hospital than self-study.

간호대학생의 전공만족도와 진로준비 행동과의 관계에서 임상실습만족도의 매개효과 (Mediating Effect of Satisfaction with Clinical Practice on the Relation between Satisfaction with Major and Career Preparation Behavior of Nursing Students)

  • Objectives : This study identified the mediating effect of satisfaction with clinical practice on the relation between nursing students' satisfaction with major and career preparation behavior. Methods : Study participants were 454 nursing students with a clinical practice experiences who were enrolled as members of two university in Korea. Data were collected with self-report questionnaire and analyzed with SPSS 19.0 program. Results : Satisfaction with major was positively correlated with career preparation behavior, and satisfaction with clinical practice was also positively associated with career preparation. A mediating effect of satisfaction with clinical practice was found in the relation between satisfaction with major and career preparation behavior. Conclusions : Universities and departments should provide much effort and support for the development of clinical practice experience curriculum and hands-on training that are appropriate for the students' level and interests so that their outcomes can be connected to the career preparation behavior.

간호대학생의 임상실습에서의 자기주도학습에 영향을 미치는 요인 (Factors Influencing on Self-directed Learning in Clinical Practice of Nursing Students)

  • Purpose: The purpose of this study was to identify the effects of nursing students' belongingness and self-esteem on self-directed learning in clinical practice. Methods: The participants were 163 senior nursing students. Using a structured-questionnaire, data were collected from September to October 2016. Data were analyzed using independent t-test, ANOVA, $Scheff\acute{e}$, test Pearson's correlation coefficients, and multiple linear regression. Results: Self-directed learning in clinical practice had statistically significant correlation with belongingness (r=.47, p<.001) and self-esteem (r=.52, p<.001). Factors affecting self-directed learning were self-esteem (${\beta}=.38$, p<.001), belongingness (${\beta}=.30$, p<.001), and more than 5 members of clinical practice (${\beta}=.15$, p=.017). These three variables explained 35% of self-directed learning. Conclusion: In order to enhance self-directed learning in clinical practice of nursing students, nursing schools should provide training to increase self-esteem in nursing students. In addition, an appropriate number of students should be considered, so that they can have a well-developed sense of belonging to clinical practice.
