초음파 생체 현미경을 이용한 증상이 없는 홍채모양체 종양의 진단 1 증례 (Diagnosis of an Asymptomatic Iridociliary Tumor using Ultrasound Biomicroscopy in a Dog)

  • A 10-year-9-month-old spayed female Shih Tzu was presented with ocular discharge and corneal opacity to Veterinary Medical Teaching Hospital of Seoul National University. Complete ophthalmic examination revealed bilateral keratoconjunctivitis sicca accompanied by the severe corneal neovascularization and edema in the right eye (OD). Ultrasound biomicroscopy incidentally showed an iridociliary mass located at 6~7 o'clock in the OD. No evidence of metastasis was observed on thoracic and abdominal radiography. Enucleation was selected and performed on the OD, considering the risk of metastasis or recurrence. Non-invasive and pigmented iridociliary adenoma was confirmed on histopathological evaluation.

남성 갑상선 유두암 환자의 임상 특징과 예후에 미치는 인자에 관한 연구 (Clinical Features and Factors Affecting Prognosis of Papillary Thyroid Carcinoma in Male Patients)

  • Background and Objectives: National cancer center institute reports that male patients of papillary thyroid carcinoma (PTC) are annually increasing. This study aimed to analyze the features of the male patients with PTC. Materials and Method: We retrospectively reviewed and analyzed clinical records of 170 patients who were treated for PTC in male patients between 2000 and 2010. Clinical features, size, pathologic type, extrathyroidal extension, recurrence, multiplicity, extent of surgery, and lymph node metastasis were retrospectively evaluated.Univariate and multivariate analyses of various clinical factors were performed. Results: Total 4145 patients received surgery for papillary thyroid carcinoma. The number of male patients was 170 (4.1%) among them. Of170 male patients, only 16(9.4%) patients underwent the recurrence of PTC. The size of tumor, central neck node metastasis, lateral neck node metastasis, extrathyroidal extension and RAI ablation therapy were associated with recurrence(p< 0.05) in univariate analysis. However, only the extrathyroidal extension [p=0.03, Odds ratio=3.58(95% CI. 1.09~14.24)] was related to the recurrence in multivariate analysis. Conclusion: Re-estimation of clinical features in male PTC patients should be concerned. The recurrence of PTC in male patients was 16(9.4%) and nearly same as the other studies. The extrathyroidal extension was revealed as an associated factor for the recurrence. Evaluation of regional or distant metastasis should be considered in patients with the extrathyroidal extension in male PTC patients during long-term follow-up.

Prediction of Residual Axillary Nodal Metastasis Following Neoadjuvant Chemotherapy for Breast Cancer: Radiomics Analysis Based on Chest Computed Tomography

  • Objective: To evaluate the diagnostic performance of chest computed tomography (CT)-based qualitative and radiomics models for predicting residual axillary nodal metastasis after neoadjuvant chemotherapy (NAC) for patients with clinically node-positive breast cancer. Materials and Methods: This retrospective study included 226 women (mean age, 51.4 years) with clinically node-positive breast cancer treated with NAC followed by surgery between January 2015 and July 2021. Patients were randomly divided into the training and test sets (4:1 ratio). The following predictive models were built: a qualitative CT feature model using logistic regression based on qualitative imaging features of axillary nodes from the pooled data obtained using the visual interpretations of three radiologists; three radiomics models using radiomics features from three (intranodal, perinodal, and combined) different regions of interest (ROIs) delineated on pre-NAC CT and post-NAC CT using a gradient-boosting classifier; and fusion models integrating clinicopathologic factors with the qualitative CT feature model (referred to as clinical-qualitative CT feature models) or with the combined ROI radiomics model (referred to as clinical-radiomics models). The area under the curve (AUC) was used to assess and compare the model performance. Results: Clinical N stage, biological subtype, and primary tumor response indicated by imaging were associated with residual nodal metastasis during the multivariable analysis (all P < 0.05). The AUCs of the qualitative CT feature model and radiomics models (intranodal, perinodal, and combined ROI models) according to post-NAC CT were 0.642, 0.812, 0.762, and 0.832, respectively. The AUCs of the clinical-qualitative CT feature model and clinical-radiomics model according to post-NAC CT were 0.740 and 0.866, respectively. Conclusion: CT-based predictive models showed good diagnostic performance for predicting residual nodal metastasis after NAC. Quantitative radiomics analysis may provide a higher level of performance than qualitative CT features models. Larger multicenter studies should be conducted to confirm their performance.

Pulmonary Carcinosarcoma within Bronchogenic Cyst: 1례 보고 (Pulmonary Carcinosarcoma within Bronchogenic Cyst)

  • Carcinosarcoma is an unusual and rarest neoplasm in man, consisting of carcinomatous parenchyme and sarcomatous stroma. Two clinical types of carcinosarcoma were present. One type of tumor was centrally located [endobronchial type], infrequent metastasis, and better prognosis than parenchymal type. The other type was peripherally located [parenchymal type], frequent metastasis, and poor prognosis. The histogenesis of carcinosarcoma is many hypothesis, but controversial; 1] sarcomatous degeneration of stroma, 2] intermingling of simultaneously arising carcinoma & sarcoma, 3] multiple primary tumor, 4] blastomatous changes in hamartoma, 5] stromal reaction to squamous cell carcinoma, 6] true & collision carcinosarcoma. In this case, 52 year-old male patient was hospitalized due to intermittent hemoptysis & known pulmonary lesions. Since 1968, chest PA showed round haziness within cyst & multiple cyst on RUL & RLL. Radical pneumonectomy was performed and histopathology showed carcinosarcoma, surrounded by bronchial epithelium. The patient maintain general well-being without clinical evidence of recurrence till now.

폐에 발생한 선편평상피세포암종;1례 보고 (Adenosquamous Carcinoma of the Lung - One Case Report -)

  • Adenosquamous carcinoma of the lung is a rare disease entity and its clinical characteristics, treatment, and prognosis are poorly described. Stage I disease are amenable to operation as in bronchogenic carcinoma of non-small cell type, but stage III adenosquamous carcinoma shows highly aggressive behavior with rapid progression. We have experienced a case of stage III adenosquamous carcinoma of the lung in a 38-year-old male with a large mass in the right upper lobe with multiple metastasis in the mediastinal lymph nodes. Interestingly, all of the metastatic lymph nodes proved to be adenous component histologically. After right pneumonectomy and extensive lymph node dissection, the patient had received one cycle of chemotherapy. However, clinical signs of distant metastasis to bone and brain were discovered within 5 months postoperatively.

신장의 투명세포 육종 1예 (Clear Cell Sarcoma of the Kidney - a Case Report -)

  • Clear Cell Sarcoma of the Kidney(CCSK) is a rare malignant childhood tumor with frequent metastasis to the bone. We report a case of right sided in a 5 month-old girl. A radical nephrectomy was performed. It was clinical stage III with renal capsular invasion and lymph node metastasis by the classification of NWTS-5. Histologic examination revealed the classic pattern of CCSK. Postoperative adjuvant chemotherapy with doxorubicin and radiotheraphy were applied.

소아의 수부골에 전이된 신장의 투명세포육종 -1례 보고- (Metastatic Clear Cell Sarcoma of the Kidney in a Child's Hand Bone - A Case Report -)

  • Clear cell sarcoma of the kidney (CCSK) is a rare malignant tumor of the renal origin in childhood, distinguished from Wilms tumor by its pathologic and clinical features. Bone metastasis is one of the characteristic clinical features. The common site of metastasis of the clear cell Sarcoma of the kidney is axial skeleton including skull, spines, ribs and femur. A cases of clear cell Sarcoma of the kidney presented to us, which solely metastasized to the hand bones without metastasizing to any other tissues including axial skeleton. We report this case with review of literatures.

Triaditis in a Cat with Suspected Malignant Hepatobiliary Tumor

  • A nine-year-old spayed female Turkish angora cat presented for evaluation of anorexia, lethargy, vomiting, ptyalism and jaundice. Based on clinical examinations including laboratory examinations, concurrent inflammatory condition of the biliary system, pancreas and intestines (triaditis) was suspected. The cat was under antibiotic and immune-suppressive therapy, but there was no response. Further examination revealed the possibility of malignant hepatobiliary tumor with pulmonary metastasis. The condition of the cat continued to deteriorate and the cat died 3 weeks after the diagnosis. This case demonstrates the clinical findings of triaditis combined with suspected malignant hepatobiliary tumor.

Creatine Kinase (CK)-MB-to-Total-CK Ratio: a Laboratory Indicator for Primary Cancer Screening

  • Background: For the determination of creatine kinase (CK)-MB, the immunoinhibition method is utilized most commonly. However, the estimated CK-MB activity may be influenced by the presence of CK isoenzymes in some conditions like cancer. Thus, a CK-MB-to-total-CK ratio more than 1.0 could be found in such a situation. The study aimed to explore the relationship of cancer to high CK-MB-to-total-CK ratio. Materials and Methods: From January 2011 to December 2014, laboratory data on all CK-MB and total CK test requests were extracted at Far Eastern Memorial Hospital (88,415 requests). Patients with a CK-MB-to-total-CK ratio more than 1.0 were registered in this study. Clinical data including tumor location, tumor TNM stage and metastatic status were also collected. Results: A total of 846 patients were identified with a CK-MB-to-total-CK ratio more than 1.0. Of these, 339 (40.1%) were diagnosed with malignancies. The mean CK-MB-to-total-CK ratio was significantly higher in malignancy than in non-malignancy ($1.35{\pm}0.28$ vs $1.25{\pm}0.23$, p<0.001) groups. The most frequent malignancy with a CK-MB-to-total-CK ratio more than 1.0 was colorectal cancer ($1.42{\pm}0.28$, 16.5%, n=56), followed by lung cancer ($1.38{\pm}0.24$, 15.9%, n=54) and hepatocellular carcinoma (14.5%, n=49). Higher CK-MB-to-total-CK ratios in hematological malignancies ($1.44{\pm}0.41$)were also noted. Additionally, the CK-MB-to-total-CK ratio was markedly higher in advanced stage malignancy than in early stage ($1.37{\pm}0.26$ vs. $1.29{\pm}0.31$, p=0.014) and significantly higher in liver metastasis than in non-liver metastasis ($1.48{\pm}0.30$ vs. $1.30{\pm}0.21$, p<0.001). Conclusions: The CK-MB-to-total-CK ratio is an easily available indicator and could be clinically utilized as a primary screening tool for cancer. Higher ratio of CK-MB-to-total-CK was specifically associated with certain malignancies, like colorectal cancer, lung cancer and hepatocellular carcinoma, as well as some cancer-associated status factors such as advanced stage and liver metastasis.

두개골 또는 뇌로 전이된 갑상선암 - 7례 분석 - (Skull or Brain Metastasis from Thyroid Carcinoma - Analysis of 7 Cases -)

  • Skull or brain metastasis from thyroid carcinoma is rare. Between Jan. 1993. and Jan. 2000. the authors experienced 7 cases of skull(4 cases) or brain(3 cases) metastasis from thyroid carcinoma. Clinical characteristics, radiologic findings and pathologic features were analyzed retrospectively. Among 4 cases of skull metastasis, 3 cases were follicular carcinoma and one was papillary carcinoma with follicular variant. All cases of brain metastasis were papillary carcinoma. We treated the patients by combination with surgical resection, radioactive iodine therapy and radiation therapy. Characteristics of skull or brain metastasis from thyroid carcinoma is reviewed with a pertinent literature.
