공군장병의 거부민감성과 우울 및 불안의 관계에서 학교폭력 피해 경험의 조절효과 (The Moderating Effects of School Violence Victimization Experiences in the Relationships between Airmen's Rejection Sensitivity, Depression, and Anxiety)

  • 본 연구의 목적은 공군 장병들의 거부민감성이 우울 및 불안에 영향을 미치는지 알아보고 이러한 관계에서 과거 중 고등학교 시절 학교 폭력 피해 경험이 조절 효과를 보이는지 알아보는 것이다. 또한 이러한 연구 결과를 토대로 군 장병들의 군 생활 적응에 있어 위험 요소들을 찾고 이들의 심리적 적응을 향상시키는 방안을 제시하고자 한다. 이를 위해 S시에 소재하는 공군 장병 126명을 대상으로 거부민감성, 우울, 불안, 학교 폭력 피해 경험을 측정하는 설문지를 실시하였다. 조절효과를 검증하기 위해 거부민감성, 학교 폭력 피해 경험, 이들 간의 상호 작용 항을 투입하여 위계적 회귀분석을 실시하였다. 연구 결과, 거부민감성과 우울의 관계에서 학교폭력 피해 경험의 조절 효과는 유의미하였으며, 거부민감성과 불안의 관계에서 학교 폭력 피해 경험의 조절 효과도 역시 유의미하였다. 이를 통해 학교 폭력 피해 경험은 거부민감성이 우울 및 불안에 미치는 영향을 증가시킴을 알 수 있었다. 이들의 상호작용 패턴에 대해 단순 주효과 기울기 분석을 통해 확인하였다. 마지막으로 연구 결과를 통해 학교폭력피해 경험이 군생활 적응의 고위험 요소이며 이들을 위한 상담적 개입의 중요성 등에 대해 논의하였다.

호도약침액(胡桃藥鍼液)의 DNA chip 을 이용한 유전자 발현 분석 (Gene Expression Analysis Using cDNA Microarray Assay by Juglandis Semen Herbal Acupuncture Solution)

  • Objectives : Juglandis Semen herbal acupuncture solution(JSS) has a broad array of clinical applications in oriental medicine, including treatment of chronic musculoskeletal diseases such as arthritis. This study was performed to investigate the global gene expression profiles using microarray assay in RAW 264.7 cell line treated with JSS and to advance our understanding of the pharmacologic effect of JSS. Methods : Change of the gene expression profile in RAW cell line following treatment with lipopolysaccharide(LPS) alone, or with LPS plus JSS was investigated with a cut-off level of 2 fold change in the expression. Especially, Change of the gene expression by treatment with LPS alone was compared with that by treatment with LPS plus JSS with a cut-off level of 1/2 fold change in the expression. Results: Of the 8170 genes profiled in this study, 51 were upragulated and 21 downregulated following LPS treatment, and 88 were upregulated and 69 downregulated following costimulation of JSS and LPS. Of the 51 genes upregulated following LPS treatment, 10 were downregulated following costimulation of JSS and LPS. Of the 21 genes downregulated following LPS treatment, 3 were upregulated following costimulation of JSS and LPS. Conclusion : JSS treatment induced upregulation of some genes including IL-10 and downregulation of that including MMP13 with its possible implication in an antiinflammatory action of JSS. However, further research on expression profile changes induced by JSS treatment is expected.

  • LDAEP와 세로토닌 기능 간의 직접적인 관계의 초기 증거들은 동물 실험으로부터 도출되었고 이후 세로토닌성 장애에 기반한 질병들의 임상 실험을 통하여 간접적인 뒷받침이 이루어 져왔다. 현재까지 LDAEP와 세로토닌 사이의 연관성을 확인해주는 증거는, 우선 세로토닌 시스템 내에서 유전적 polymorphism을 연구하는 것을 통해, 그리고 직접적으로 약물을 투여 하는 방식으로 확보되었다. LDAEP가 연관된 대부분의 연구는 모노아민 시스템에 초점을 맞춰왔으나 LDAEP에 대한 다른 신경전달 시스템의 효과는 거의 알려진 바가 없다. 향후 세로토닌 외에도 도파민, 노르에피네프린, 아세틸콜린 등 다른 신경전달물질과의 연관성에 대한 연구가 필요하다. In vivo 중추 세로토닌 기능의 유효한 비침습적 표지자로서 LDAEP를 확정짓기 전에, 단기적 조절뿐 아니라 장기적 세로토닌성 조절이 포함된 더 많은 연구가 필요하다. 현재까지의 증거로 보면 LDAEP의 가장 유망한 활용은 초기 항우울 치료반응과 관련된 생물학적 지료서의 활용이다. 최근의 연구들은 LDAEP의 노르아드레날린성 그리고 세로토닌성 항우울제 반응에 대한 민감도를 확인했으며 이러한 긍정적인 기초 결과들을 재현하기 위하여 더 많은 연구들이 필요하다.

두경부 편평세포암종에서 인유두종 바이러스와 p53과 Proliferating Cell Nuclear Antigen 발현의 임상적 의의 (The Clinical Implication of Human Papilloma Virus, p53 and Proliferating Cell Nuclear Antigen Expression in Head and Neck Squamous Cell Carcinoma)

  • The presence of HPV DNA and the expression of p53 protein and proliferating cell nuclear antigen(PCNA) in head and neck squamous cell carcinoma were determined to evaluate the relationship of these factors and their association with their pathologic stages and cervical lymph node metastasis. Among 65 patients the presence of HPV DNA was found in 12 cases(18.5%), p53 was found positive in 32 cases(49.2%) and expression of PCNA was observed in 24 cases(36.9%). The expression of PCNA was more frequent in the HPV positive cancers compared with the HPV negative ones(p =0.0018), and p53 revealed its higher rate of cooccurrence with the expression of PCNA(p =0.008), which might suggest that PCNA expression has a positive relationship with HPV and p53 mutation in head and neck cancer. There might be inverse relationship between HPV and p53 mutation(p =0.063), but 3 cases showed both HPV DNA positivity and p53 expression. HPV was detected at a higher rate in the early pathologic stages than in the advanced stages of cancer, and p53 expression was more frequently found in the advanced stages(p =0.044). These results suggests that HPV and p53 mutation might have different etiologic roles in the development of head and neck cancer, or cases with p53 mutation might have more aggressive behavior. PCNA expression showed no difference between early and late stages of cancer, and between cases with and without cervical lymph node metastasis.

KRAS Mutation as a Biomarker for Survival in Patients with Non-Small Cell Lung Cancer, A Meta-Analysis of 12 Randomized Trials

  • Background: Because there is no clear consensus for the prognostic implication of KRAS mutations in patients with non-small cell lung cancer (NSCLC), we conducted a meta-analysis based on 12 randomized trials to draw a more accurate conclusion. Materials and Methods: A systematic computer search of articles from inception to May 1, 2014 using the PubMed, EMBASE, and Cochrane databases was conducted. The enrollment of articles and extraction of data were independently performed by two authors. Results: Our analysis was based on the endpoints overall survival (OS) and progression-free survival (PFS). Nine records (All for OS, 7 for PFS) comprising 12 randomized trials were identified with 3701 patients who underwent a test for KRAS mutations. In the analysis of the pooled hazard ratios (HRs) for OS (HR: 1.39; 95% confidence interval [CI] 1.23-1.56) and PFS (HR: 1.33; 95% CI 1.17-1.51), we found that KRAS mutations are related to poor survival benefit for NSCLC. According to a subgroup analysis stratified by disease stage and line of therapy, the combined HRs for OS and PFS coincided with the finding that the presence of a KRAS mutation is a dismal prognostic factor. However, the prognostic role of KRAS mutations are not statistically significant in a subgroup analysis of patients treated with chemotherapy in combination with cetuximab based on the endpoints OS (P=0.141) and PFS (P=0.643). Conclusions: Our results indicate that KRAS mutations are associated with inferior survival benefits for NSCLC but not for those treated with chemotherapies integrating cetuximab.

Association of CD4+CD25+FoxP3+ regulatory T cells with natural course of childhood chronic immune thrombocytopenic purpura

  • Purpose: The purpose of this study was to determine the frequency of $CD4^+CD25^+FoxP3^+$ regulatory T cells (Treg) in the peripheral blood of patients with childhood chronic immune thrombocytopenic purpura (ITP) exhibiting thrombocytopenia and spontaneous remission. The findings of this study indicate the possibility of predicting spontaneous recovery and pathogenesis of childhood chronic ITP. Methods: Eleven children with chronic ITP (seven thrombocytopenic and four spontaneous remission cases; mean age, 8.8 years; range, 1.7-14.9 years) were enrolled in this study. Five healthy children and eight healthy adults were included as controls. The frequency of Treg was evaluated by flow cytometry in the peripheral blood. Results: In this study, four patients (36%) achieved spontaneous remission within 2.8 years (mean year; range, 1.0-4.4 years). The frequency of Treg was significantly lower in patients with persisting thrombocytopenia ($0.13%{\pm}0.09%$, P<0.05), than that in the patients with spontaneous remission ($0.30%{\pm}0.02%$), healthy adults controls ($0.55%{\pm}0.44%$), and healthy children controls ($0.46%{\pm}0.26%$). A significantly positive correlation was found between the frequency of Treg and the platelet count in children. Conclusion: These data suggest that a lower frequency of Treg contributes to the breakdown of self-tolerance, and may form the basis for future development of specific immunomodulatory therapies. Furthermore, Treg frequency has prognostic implication toward the natural course and long-term outcomes of childhood chronic ITP.

Detection of Serum IgE Specific to Mite Allergens by Immuno-PCR

  • Background: Although a skin test is the primary option for detecting allergen-specific IgE in clinics, the serum IgE immunoassay is also important because it allows for the diagnosis of allergy without any accompanying adverse effect on the patient. However, the low detection limit of IgE levels by immunoassay may restrict the use of the method in some occasions, and improving its sensitivity would thus have a significant implication in allergy-immunology clinics. Methods: In this study, we attempted to detect specific serum IgE by using immuno-polymerase chain reaction (IPCR) which combines the antigen-antibody specificity of enzyme-linked immunosorbent assays (ELISAs) with the amplification power of PCR. Results: Our results demonstrated that Blo t5-specific serum IgE can be detected by IPCR with a 100-fold higher sensitivity than ELISA, and cross-reactivity of serum IgE to other mite allergens is able to be analyzed by using only $0.3{\mu}l$ of serum sample. Use of real-time IPCR seemed to permit more convenient determination of specific serum IgE as well. Conclusion: We believe that IPCR can serve as a valuable tool in determining specific serum IgE, especially when the amount of serum sample is limited.

Visceral Leishmaniasis in Rural Areas of Alborz Province of Iran and Implication to Health Policy

  • Visceral leishmaniasis (VL) or kala-azar mainly affects children in endemic areas. This study was conducted to determine the seroprevalence of VL using direct agglutination test (DAT) in children living in rural districts of Alborz Province located 30 km from Tehran capital city of Iran. Multi-stage cluster random sampling was applied. Blood samples were randomly collected from 1,007 children under 10 years of age in the clusters. A total of 37 (3.7%) of the studied population showed anti-Leishmania infantum antibodies with titers of ${\geq}1:800$. There was a significant association between positive sera and various parts of the rural areas of Alborz Province (P<0.002). Two children with anti-Leishmania infantum antibodies titers of ${\geq}1:3,200$ indicated kala-azar clinical features and treated with anti-leishmaniasis drugs in pediatric hospital. The findings of this study indicated that Leishmania infection is prevalent in rural areas of Alborz Province. Therefore, it is necessary to increase the awareness and alertness among physicians and public health managers, particularly in high-risk rural areas of the province in Iran.

Electrophysiological and Behavioral Changes by Phosphodiesterase 4 Inhibitor in a Rat Model of Alcoholic Neuropathy

  • Objective : Alcoholic neuropathy is characterized by allodynia (a discomfort evoked by normally innocuous stimuli), hyperalgesia (an exaggerated pain in response to painful stimuli) and spontaneous burning pain. The aim of the present study is to investigate the effect of rolipram, a phosphodiesterase 4 inhibitor, against alcohol-induced neuropathy in rats. Methods : Allodynia was induced by administering 35% v/v ethanol (10 g/kg; oral gavage) to Spraue-Dawley rats for 8 weeks. Rolipram and saline (vehicle) were administered intraperitoneally. Mechanical allodynia was measured by using von Frey filaments. Somatosensory evoked potential (SEP) was proposed as complementary measure to assess the integrity of nerve pathway. Results : The ethanol-induced mechanical allodynia began to manifest from 3 week, and then peaked within 1 week. Beginning from 3 week, latency significantly started to increased in control group. In rolipram treated rats, the shorter latency was sustained until 8 weeks (p<0.05). The mechanical allodynia, which began to manifest on the 3 weeks, intraperitoneal injections of rolipram sustained statistical difference until 8 weeks, the final week of the study (p<0.05). Conclusion : This study suggests that rolipram might alleviate mechanical allodynia induced by alcohol in rats, which clearly has clinical implication.

방사선 생물학을 위한 모델 시스템: 방사선치료의 전임상 연구 (Model Systems in Radiation Biology: Implication for Preclinical Study of Radiotherapy)

  • 방사선 생물학에서 방사선에 대한 반응으로 매개되는 다양한 기작에 대한 분석을 위해 여러 종류의 모델 생물체를 사용해 왔다. 모델 생물체는 생물학적으로 온전한 in vivo 환경을 제공할 수 있기 때문에 방사선에 의해 발생되는 세포 내 현상은 물론 생리적인 현상이나 병리학적인 현상을 규명하는 데 있어서 모델 생물체를 사용하는 것은 효과적인 방법이 될 수 있다. 지금까지 축적된, 모델 생물체를 이용한 방사선 생물학적 연구결과들은 새로운 방사선치료 보조제의 개발, 방사선치료 효율 증진 등에 적용되어 여러 질병에 대한 임상연구의 기초가 되어왔다. 이렇게 유용하게 사용된 여러 모델 생물체에 있어서, 각각의 모델에 대한 개별적인 정보에 대한 연구는 다양한 방면에서 이루어지고 있지만, 통합적인 비교, 분석 및 정리를 한 경우는 부족한 실정이다. 따라서, 본 논문에서는 방사선 생물학에서 지금까지 많이 사용된 모델 생물체 4종(효모, 예쁜꼬마선충, 초파리, 생쥐)에 대해 각 생물체가 갖는 모델로써의 특징과 장단점 그리고 방사선 생물학 연구에 이용된 사례 등을 서술하고자 한다.
