방사선 투시를 이용한 개폐구 운동에 따른 수직 고경 계측점의 안정성에 관한 연구 (A FLUOROSCOPIC STUDY ON THE STABILITY OF SKIN REFERENCE POINTS DURING JAW OPENING AND CLOSING MOVEMENT)

  • The current clinical technique for occlusal vertical dimension recording is based on marking the skin reference points on the patient's face and measuring between these points using caliper-like device. And it is difficult to achieve reliable measurements by this technique because of movable soft tissue. The purpose of this study is to reveal the stability of skin reference points by comparing the relative movement between extra-oral skin reference points and intra-oral reference points using X-ray fluoroscope. 10 test subjects were divided into 2 groups : Group I (natural dentition) and Group II (denture-wearer whose vertical dimension was lost) and Group III consists of identical test subjects to Group II with their upper denture removed and record base inserted. Attaching the 3 mm diameter steel ball to nose tip, lower lip, chin and to existing denture (or record base), fluoroscopic examination and recording were taken during 2 jaw opening and closing movements. After subsequent digitization using personal computer, 1219 still pictures with 0.1 second interval were made. Using the 2 dimensional graphic software, measurements between reference points were executed. Dividing the entire jaw movement into 3 ranges (total, 1st half opening, 2nd half opening), rate of movement and relative movement between extra-oral and intra-oral reference points were calculated and statistically analyzed. The results of this study are as follows. 1 Within the same experimental group, no statistical difference was found in the stability of skin reference between lower lip point and chin point during total range of jaw opening and closing movement (p>.05) 2. In the first half range of jaw opening, statistical difference was found between Group I (natural dentition) and Group II (denture wearer) (p<.05) Group I has greater skin reference stability than Group II. 3. In the first half range of jaw opening, statistical difference was found between Group I and Group III (record base wearer) (p<.05). Group I has greater skin reference stability than Group III. 4. In the first half range of jaw opening, no statistical difference was found in the stability of skin reference between Group II and Group III (p>.05). 5. In the second half range of jaw opening, no statistical difference was found in the stability of skin reference between any experimental groups (p>.05). 6. In patients with their occlusal vertical dimension lost, employing other measuring references rather than skin is recommended because of low stability.

생쥐의 자궁내(子宮內) 장치(裝置)가 배아발생(胚兒發生) 및 이동속도(移動速度)에 미치는 영향(影響)에 관한 연구(硏究) (Studies on Effect of the Intrauterine Device on the Embryonal Development and Transport in the Mouse)

  • 생쥐(C3H/JMS)의 한쪽 자궁(子宮)에 봉합계(縫合系)를 삽입한 후 초기배아(初期胚兒)의 발생과정(發生過程), 이동속도(移動速度) 그리고 착상(着床)의 상태를 관찰하였다. 임신한지 2일째 되는 날에 자궁(子宮)에 장치를 받지 않은 쪽에서는 초기배(初期胚)의 일부가 이미 수난정(輸卵精)의 뒷부분에 도달하고 있었으나 장치를 받은 쪽에서는 아직도 수난정(輸卵精) 중부(中部)에 머물러 있었다. 3일째가 되면 대조(對照)가 되는 바른쪽 자궁(子宮)에서는 $8{\sim}16$세포기에 드는 배아(胚兒)가 79%에 이르며 24%는 이미 자궁(子宮)에까지 도달하고 있으나 장치를 한 쪽에서는 $8{\sim}16$세포기의 배아(胚兒)가 69%가 되며 겨우 8.2%만이 자궁(子宮)에 이르고 있다. 4일째가 되면 대조자궁(對照子宮)에서는 전체 배아(胚兒)의 75%가 그리고 장치 자궁(子宮)에서는 61%가 상실배(桑實胚)를 이루며 전자(前者)에서 60%, 후자(後者)에서는 40%가 자궁(子宮)내에 진입하게 된다. 정상적(正常的)인 생쥐에서는 임신 4일이 되면 거의가 배낭기(胚囊期)에 이르는 것으로 알려져 있으나 한쪽 자궁(子宮)에 실을 장치한 생쥐에서는 대조자궁(對照子宮)이거나 장치 자궁(子宮)이거나 모두 배낭(胚囊)의 형성(形成)이 늦어지고 있다. 한 자궁(子宮)당 발견되는 정상배(正常胚)의 평균치(平均値)는 대조구(對照區)에서 6.3정도로 임신 2일에서 4일 사이에 큰 차이가 없으나 처리구(處理區)에서는 2일에서 4일 사이에 6.5에서 3.8로 감소한다. 이상배(異常胚)는 대조구(對照區)에서 2일째에 2.2였던 것이 0.4로 크게 줄지만 처리구(處理區)에서는 대조구(對照區)에서 처럼 급격한 감소를 볼 수 없다. 임신한지 17일 되는 날 대조구(對照區)에서 15개의 자궁(子宮)가운데 한지 2개의 자궁(子宮)만이 착상(着床)흔적을 보였지만 처리구(處理區)에서는 전혀 착상(着床)되었던 흔적을 볼 수 없었다. 결국 생쥐에서는 한 쪽 자궁(子宮)에 이물질(異物質)을 삽입했을 때 장치를 받은 자궁(子宮)뿐 아니라 다른 한 쪽 자궁(子宮)에서도 착상(着床)이 일어나지 않을 뿐 아니라 이상난자(異常卵子)의 발생(發生)을 증가시키며 배아(胚兒)의 난할속도(卵割速度)가 늦어지고 또 배아(胚兒)의 이동(移動)이 늦어진다는 것을 알게 되었다.

입원환자 낙상 발생 실태와 원인에 관한 분석 연구 (An Analysis of Fall Incidence Rate and Its Related Factors of Fall in Inpatients)

  • Background: The purpose of this research was to examine the fall incidence rate and its related factors of fall in inpatients. Methods: The data were collected from the 138 fall incident reports in one tertiary hospital in Seoul from April 1st 1999 to September 30th 2001. The Fall Incident Report Form was originally developed based on that of Massachusetts General Hospital revised in 1995. And this was modified for this survey by the collaborating work of QI team including researcher and department of nursing service of this particular hospital. The contents of Fall Incident Form were general characteristics of patient. factors related to fall. types and places of fall. circumstances, nursing interventions. and outcome. Results: 1) The incidence rate of fall was 0.08% of total discharged patients and 0.081 per 1000 patient-day. This incidence rate is much lower than that of several hospitals in USA. This finding might result from the different incidence report system of each hospital. 2) The characteristics of fall-prone patient were found as follows. They were mostly over 60 years old, in alert mental status, ambulatory with some assistance, and dependent on ambulatory device. The types of diseases related high incidence rate were cerebrovascular disease(3.2), hypertension(1.6), cardiovascular disease(1.4), diabetes(1.3) and liver disease(0.6). 3) The majority of fall events usually occurred m bed. bedside(walking or standing) and bathroom in patient room. Usually they were up on their own when they fell. And there were more falls of elderly occurred during night time than day or evening. 4) 63.8% of fall events resulted in physical injuries such as fracture and usually the patients had diagnostic procedures and some treatment(ex. suture) which caused additional cost to the patients and their families. 5) The found risk factors of fall were drugs(antihypertensive drug, diuretics) and environmental factors like too high bed height, long distance of bedside table and lamp switch, and slippery tile of bathroom floor. Conclusion: Considering these results, every medical and nursing staff should be aware of the risk factors of patients in hospital, and should intervene more actively the preventive managements, specially for the elderly patients during night. Therefore, it is recommended that the development of Fall Prevention Programs based on these results.

의치상의 종류에 따른 상악 의치상의 유지력에 관한 임상적 연구 (A CLINICAL STUDY ON THE RETENTION OF MAXILLARY COMPLETE DENTURE WITH DIFFERENT DENTURE BASE MATERIALS)

  • For the successful treatment of complete denture, obtaining a good retention is essential. There are lots of factors affecting denture retention. Denture material, one of those factors affecting denture retention, was the subject of this study, and internal surface treatment also considered for the method of enhancing denture retention. Two resin(Lucitone $199^{(R)}$(heat cured resin) Vertex $CP^{(R)}$(self cured resin)) and two metal($Biosil^{(R)}$(Co-Cr alloy), $Vitallium^{(R)}$(Co-Cr alloy)) denture base materials were used for making test denture base. Newly developed device was used for measuring denture retention. After the retention was measured. We treated internal surface of test denture base with $50{\mu}m\;Al_2O_3$ powder, under 90psi, for 1 minute. Then the retention was measured again. The result was analyzed with K-S test, one-way ANOVA test and independent t-test to deter mine the significant differences as the 95% level of confidence. The results are as follows : In cases of without internal surface treatment, the retention was increased in order of $Vitallium^{(R)},\;Biosil^{(R)},\;Vertex CP^{(R)}$ and Lucitone $199^{(R)}$. Except between Vertex $CP^{(R)}$ and $Biosil^{(R)}$, retention of the other materials was significantly different (p<0.05). After the treatment of internal surface, the retention was increased in order of $Vitallium^{(R)},\;Biosil^{(R)},\;Lucno\;199^{(R)},\;Vertex\;CP^{(R)}$. Except between Lucitone $199^{(R)}$ and Vertex $CP^{(R)}$, $Vitallium^{(R)}$ and $Biosil^{(R)}$ the retention of remaining groups was significantly different each other (p<0.05). In the matter of each material, after the internal surface treatment the retention was increased with Vertex $CP^{(R)},\;Biosil^{(R)}\;and\;Vitallium^{(R)}$ and the value of differences were statistically significant. When we compare the retention of resin and metal denture base, the retention of both denture bases increased significantly with internal surface treatment, and resin denture base showed better retention. As the results show, selecting denture base material could be an important choice of complete denture treatment. To increase denture retention, internal surface treatment can be considered as a possible method.

간호교육에 적용한 가상현실 융합시뮬레이션 연구에 대한 통합적 고찰 (Integrative Review on Nursing education Adopting Virtual Reality Convergence Simulation)

  • 가상현실 융합시뮬레이션을 이용한 교육은 간호에 대한 지식뿐만 아니라 임상간호 실무능력 향상을 위한 새로운 교수법으로 주목받고 있다. 본 연구의 목적은 양적, 질적 연구에 대한 통합적 분석방법을 통해 가상현실 융합시뮬레이션을 적용한 간호교육의 효과에 대해 분석하고자 함이다. 총 382편의 연구 중, 논문 질 평가방법을 통해 최종적으로 17편(양적연구 12편, 질적연구 5편)이 선정되었다. 효과적인 가상현실 시뮬레이션 교육을 위한 조건과 지식, 태도, 실천적 측면에서의 가상현실 시뮬레이션 교육의 효과 등 총 네 가지 측면으로 내용을 합성하였다. 효과적인 교육조건 측면에서 가상현실기계 사용에 대한 준비성과 플랫폼에 대한 마스터링, 흥미 있는 시나리오가 필요하였다. 지식, 태도 및 실천적 측면에서의 가상현실 간호교육 융합시뮬레이션의 효과는 간호대학(원)생의 지식 증가 및 유지 연장, 반복학습을 통한 과정 및 순서 암기력 향상, 공감능력 및 친밀감 형성 등의 효과가 나타났다. 따라서 간호교육의 융합시뮬레이션에 가상현실을 적용한다면 교육의 효과는 극대화 될 것이다.

국내 평가 가이드 라인 제시를 위한 전동식 부항기의 특성 조사에 관한 연구 (Study of Mechanical Characteristics of Electric Cupping Apparatus in Korea for Suggestion of its Assessment Guideline)

  • Objectives : The mechanical properties of Korean electric cupping systems are studied via experimental measurements. The study aimed at establishing the fundamentals of industrialization and systemization of oriental medicine device industry, as well as improving the quality of life for many Koreans. Methods : We reviewed the studies on traditional cupping as well as modern one to fine necessary factors for electric cupping systems. To characterize the mechanical properties of Korean electric cupping systems, we measured the pressure characteristics of commercially available electric cupping system by using an automatic pressure acquisition system and a standard cup. The pumping capability was checked at 40 seconds, and the stability of the suction cup was checked at 600 seconds. We also acquired the noise level of each system in clinical setting. To check the portability of each system, we also measured its physical dimensions. We scrutinized system manuals provided by the system manufacturers. Results : It took less than 5 second to reach the pressure if the connection between the air hose and the vacuum valve of the cupping system was secure. Pressure diminished to no more than 10% for 600s for all systems. Noise levels were 55~70 dB. Increase in pressure was too fast to control for a designated vacuum level except for one product. Conclusions : The Pumping ability of the systems is impressive and reliable. Pressure retention ability of each cup is quite reliable and reproducible. Therefore, their mechanical performances were worthy of recommendation. Some of them had noise level higher than 60 dB and they were bothersome. It was also suggested that the control for low to middle pressure needed to be accomplished by the cupping system.

하악골 신장술에서 압축자극을 통한 골 재생방식에 대한 생체 역학적 평가 (BIOMECHANICAL EVALUATION ON BONE REGENERATION IN MANDIBULAR DISTRACTION OSTEOGENESIS COMBINED WITH COMPRESSION STIMULATION)

  • The purpose of this study was to investigate the clinical, biomechanical, and histologic changes in new distraction osteogenesis(DO) technique combined with a compression stimulation in accordance to different compression-distraction force ratio. 23 adult male rabbits underwent open-osteotomy at the mandibular body area and a external distraction device was applied. In the control group of 8 rabbits, only a 8 mm of distraction was performed by conventional DO technique. In an experimental group of 15 rabbits, a distraction followed by a compression force was performed according to the ratio of compression-distraction suggested by authors. The rate of experimental group I was set up as a 2 mm compression versus 10 mm distraction and the rate of experimental group II was set up as a 3 mm compression versus 11 mm distraction. All the rabbits were sacrificed for a gross finding, biomechanical, histomorphometric and histologic findings at the time of 55 days from the operation day. The results were as follows: 1. On the gross findings, because all rabbits had a sufficient healing time, every distracted new bone had good bone quality and we could not find any difference among all three groups. 2. In the histologic findings, rapid bone maturation(wide lamellar bone formation in the cancellous and cortical bone areas) was observed in two experimental groups compared to the control group. 3. On the bone density tests, the experimental group II showed higher bone density than the other experimental group and control group(control group-$0,2906g/cm^2$, experimental group I-$0.2961g/cm^2$, experimental group II-$0.3328g/cm^2$). 4. On the biomechanical tests, the experimental group II had significantly higher bone microhardness than the other experimental group and control group(control group-252.7 MPa, experimental group I-263.5 MPa, experimental group II-426.0 MPa). 5. On the microhardness tests, when we compared the hardness ratio of distracted bone versus normal bone, we could find experimental group II had significantly higher hardness ratio than the other experimental group and control group(control group-0.47, experimental group I-0.575, experimental group II-0.80). From this study, we could deduce that the modified distraction osteogenesis method with a compression stimulation might improve the quality of bone regeneration and shorten the consolidation period in comparison with conventional distraction osteogenesis techniques.

성인 상악 전치 형태에 따른 치은의 임상적 소견 (Clinical features of the gingiva according to maxillary anterior teeth form in adult)

  • It has been suggested that morphologic characteristics of the periodontium are partly related to the shape and form of the teeth. Furthermore, the severity of symptoms of periodontal disease have been proposed to differ among these various morphologic entities or "biotypes". The purpose of the present study was to examine the relationship between the form of the crowns in the maxillary anterior tooth segment and (1) a group of morphological characteristics and (2) the thickness of the gingiva. The thickness of gingiva was measured by ultrasonic device(SDM). 100 subjects devoid of symptoms of destructive periodontal disease were examined regarding, e.g., probing depth, gingival recession, width of keratinized gingiva, thickness of the keratinized gingiva. From maxillary study cast, the width(at the apical third-CW) and the length(CL) of the crowns of the 6 anterior teeth were determined. A CW/CL-ratio was calculated for each tooth and averaged for each tooth region. The individual mean CW/CL-ratio values for the central incisors were ranked. The 10 subjects ranked highest and the 10 ranked lowest were selected as having either a long-narrow(group N) or a short-wide(group W) form of the crown of the tooth. The data for each of the examined parameters were averaged for each tooth region in each subject and mean values for subjects in groups W and N were compared using the Student t-test. Stepwise multiple regression analysis, including data from the whole sample, was performed for each tooth region with the thickness of the free gingiva as the dependent variable. The results from the analyses demonstrated that individuals with a long-narrow form of the central incisors displayed, compared to individuals with a short-wide crown, form (l) a narrow zone of keratinized gingiva, (2) a pronounced "scalloped" contour of the gingival margin. There was no significant difference between groups N and W with respect to the thickness of the keratinized gingiva. The CW/CL-ratio data revealed that a certain form of the crowns in the central incisors was accompanied by a similar form in the lateral incisors and canine tooth region. The regression analyses demonstrated that the thickness of the keratinized gingiva in central, lateral incisors and canines was significantly related to the width of the keratinized gingiva.

초등학교 어린이의 평균 구순폐쇄력과 이에 영향을 미치는 요소 (Lip Closing Force and the Related Factors in Elementary School Children)

  • 성장기 어린이에게서 나타나는 구순폐쇄 부전은 구강기능 및 구강 주위 조직의 구조적 발달 이상을 초래하게 되며, 이는 구강안면 근기능 이상으로 발전된다. 이처럼 입술을 다무는 것은 필수적이고 중요한 행동이므로, 입술을 다무는 힘을 측정하는 것은 구강 감각 및 운동 이상이 있는 환자를 평가함에 있어서 매우 유용할 수 있다. 이 연구의 목적은 학령기 어린이의 연령, 성별에 따른 평균 구순폐쇄력을 측정하고, 이에 영향을 주는 요인들과의 상관성을 분석하는 것이다. 전주시에 소재한 초등학교 5곳의 7 - 12 세 어린이 765명을 대상으로, 교합상태, 구순폐쇄 여부에 대한 임상 검사와 구순폐쇄력 측정을 시행하였다. 구순폐쇄력은 3회 측정하였고, 측정값의 평균을 대상자의 구순폐쇄력으로 활용하였다. Angle 분류법 및 안정시 구순폐쇄 여부는 구순폐쇄력 고저군 분포와 통계적으로 유의한 상관관계를 나타냈다(p = 0.016, 0.004). 완전 구순폐쇄 군에서는 높은 구순폐쇄력 군의 비율이 높았던 반면, 불완전 구순폐쇄 군에서는 낮은 구순폐쇄력 군의 비율이 더 높게 나타났다.

6 MV 광자선의 선량 상승 영역에 대한 자기장 영향 (Effect of Transverse Magnetic Field on Build-up Region of 6 MV Photon Beam)

  • 저 강도 자기장을 이용하여 기존의 6 MV 광자선에 대한 선량 상승보다 향상된 선량 변조 방법을 제안하고 이를 실험적으로 선량 효과를 확인하고자 하였다. 0.5 T (Tesla) 강도를 지니는 두 개의 네오디뮴 영구 자석을 광자선에 수직 방향으로 자기장을 인가하였다. 자석과 자석 간의 거리(MMD)와 자석과 물 표면 간의 거리(MSD)에 따라 자기장을 인가한 경우와 인가하지 않은 경우에서의 선량 상승 영역의 선량 변화를 측정하였다. 자석과 자석 간의 거리가 6 cm이고 자석과 물 표면 간의 거리가 2.5 cm 조건에서 기존 6 MV 광자선의 선량 상승 곡선과 비교하여 $D_{0mm}$, $D_{2mm}$, $D_{5mm}$, $D_{10mm}$ 가 각각 6.8 %, 14.6 %, 6.9 %, 2.1 %의 향상된 선량 효과를 보였다. 본 연구를 통해 피부와 매우 인접한 곳에 위치해 있는 표적 체적을 방사선 치료할 경우, 인체 외부에 바로 자기장을 인가하면서 기존 광자선보다 향상된 선량 상승을 기대할 수 있어 임상 적용 가능성이 높을 것으로 기대된다.
