Study for Revision of the Korean Patient Classification System (한국형 환자분류체계의 개정연구)

  • Purpose: The purpose of this study was to revise the KPCS-1 and to standardize the three patient classification systems for general ward, ICU and NICU. The actual utilization of the KPCS-1 score and each nursing activity was evaluated and the relationships between KPCS-1 score and nursing related variables were reviewed. Methods: The 47,711 KPCS-1 scores of 6,931 patients who discharged from $1^{st}$ to $30^{th}$ April 2017 were analyzed and the statistical significance between KPCS-1 score and nursing related variables was reviewed by Generalized Estimating Equation. The revision of the KPCS-1 was carried out by Partial Least Square model. The 3 patient classification systems (KPCS-1,KPCSC and KPCSN) were standardized by professional reviews. Results: KPCS-1 was a valid instrument to express nursing condition adequately and was revised as a new version which has 34 nursing activity items. The names and terminologies of pre-existing 3 patient classification systems developed by KHNA were standardized as KPCS-GW, KPCS-ICU, KPCS-NICU. Conclusion: KPCS-1 was a valid instrument to represent diverse nursing conditions precisely and was revised as a 34-item KPCS-GW. The terminologies of the other patient classification systems by KHNA were standardized as KPCS-ICU and KPCS-NICU.

A Comparative Study of Item Difficulty Hierarchy of Self-Reported Activity Measure Versus Metabolic Equivalent of Tasks

  • The purposes of this study were: 1) to show the item difficulty hierarchy of walking/moving construct of the International Classification of Functioning, Disability and Health-Activity Measure (ICF-AM), 2) to evaluate the item-level psychometrics for model fit, 3) to describe the relevant physical activity defined by level of activity intensity expressed as Metabolic Equivalent of Tasks (MET), and 4) to explore what extent the empirical activity hierarchy of the ICF-AM is linked to the conceptual model based on the level of energy expenditure described as MET. One hundred and eight participants with lower extremity impairments were examined for the present study. A newly created activity measure, the ICF-AM using an item response theory (IRT) model and computer adaptive testing (CAT) method, has a construct on walking/moving construct. Based on the ICF category of walking and moving, the instrument comprised items corresponding to: walking short distances, walking long distances, walking on different surfaces, walking around objects, climbing, and running. The item difficulty hierarchy was created using Winstep software for 20 items. The Rasch analyses (1-parameter IRT model) were performed on participants with lower extremity injuries who completed the paper and pencil version of walking/moving construct of the ICF-AM. The classification of physical activity can also be performed by the use of METs that is often preferred to determine the level of physical activity. The empirical item hierarchy of walking, climbing, running activities of the ICF-AM instrument was similar to the conceptual activity hierarchy based on the METs. The empirically derived item difficulty hierarchy of the ICF-AM may be useful in developing MET-based activity measure questionnaires. In addition to convenience of applying items to questionnaires, implications of the finding could lead to the use of CAT method without sacrificing the objectivity of physiologic measures.

Adaptive Classification of Subimages by the Fuzzy System for Image Data Compression (퍼지시스템에 의한 부영상의 적응분류와 영상데이타 압축에의 적용)

  • This paper presents a fuzzy system that adaptively classifies subimages to four classes according to image activity distribution. In adaptive transform image coding, subimage classification improves the compression performance by assigning different bit maps to different classes. A conventional classification method sorts subimages by their AC energy and divides them to classes with equal number of subimages. The fuzzy system provides more flexible classification to natural images with various distribution of image details than does the conventional method. Clustering of training data in the input-output product space generated the fuzzy rules for subimage classification. The fuzzy system of small number of fuzzy rules successfully classified subimages to improve the compression performance of the transform image coding without sorting of AC energies.

  • In this article, we investigated plane figures introduced in elementary school mathematics in the perspective of traditional classification, and also analyzed plane figures focused on the invariance of plane figures out of traditional classification. In the view of traditional classification, how to treat trapezoids was a key argument. In the current mathematics curriculum of the elementary school mathematics, the concept of sliding, flipping, and turning are introduced as part of development activities of spatial sense, but it is rare to apply them directly to figures. For example, how are squares and rectangles different in terms of symmetry? One of the main purposes of geometry learning is the classification of figures. Thus, the activity of classifying plane figures from a symmetrical point of view has sufficiently educational significance from Klein's point of view.

Korean Jouranl of Sport Biomechanics, Vol. 18, No. 2, pp. 59-64, 2008. This study was conducted to study the real-time sports activity classification and monitoring using single waist mounted tri-axial accelerometer. This monitoring system detects events of sports activities such as walking, running, cycling, transitions between movements, resting and emergency event of falls. Accelerometer module was developed small and easily attachable on waist using wireless communication system which does not constrain sports activities. The sensor signal was transferred to PC and each movement pattern was classified using the developed algorithm in real-time environment. To evaluate proposed algorithm, experiment was performed with several sports activities such as walking, running, cycling movement for 100sec each and falls, transition movements(sit to stand, lie to stand, stand to sit, lie to sit, stand to lie and sit to lie) for 20 times each with 5 healthy subjects. The results showed that successful detection rate of the system for all activities was 95.4%. In this study, through sports activity monitoring. it was possible to classify accurate sports activities and to notify emergency event such as falls. For further study, the accurate energy consumption algorithm for each sports activity is under development.

Study of General Characteristics of the Korean Elderly and Their Leisure Time Spending : Analyzing the Data of Hours of Living 2014 (한국노인의 일반적 특성과 여가생활시간에 관한 연구 : 2014년 생활시간조사자료 분석 중심으로)

  • Objective : This study aimed to identify types of leisure time spending of the Korean elderly according to the data of Hours of Living 2014 from National Statistical Office, and compare and analyze features of leisure time spending based on general characteristics of the elderly. Method : This study has only adopted the data of 9228 elder people whose ages were above 65, and their general characteristics and time spending features form the data of Hours of Living 2014. The final comparison and analysis has been made through behavior classification (1 major classification, 7 medium classification, 39 minor classification), which reflects how the elderly spend their leisure time according to their household and personal characteristics. Result : Firstly, the Korean elderly were found to spend most of their leisure time with media. Secondly, analyzing the features of leisure time spending based on household characteristics of the elderly, their gender and household income showed significant differences in all types of leisure time activities except for cultural and sightseeing activity. And their marital status showed significant differences in most of the leisure time activities excluding leisure time activity with media and cultural and sightseeing activity. Thirdly, analyzing the features of leisure time spending based on personal characteristics of the elderly, their education level has made significant differences in all types of leisure time activities. As for their economic activity status and average monthly income, they showed meaningful differences in most of the leisure time activities excluding cultural and sightseeing activity and ritual activity. Conclusion : We should help to find meaningful occupation within personal contexts and arrange it properly with balanced leisure time spending. Moreover, it is highly important to have appropriate occupational approach to prevent social isolation of the Korean elderly, and ensure their fruitful and successful later life.

Transfer Learning Backbone Network Model Analysis for Human Activity Classification Using Imagery (영상기반 인체행위분류를 위한 전이학습 중추네트워크모델 분석)

  • Recently, research to classify human activity using imagery has been actively conducted for the purpose of crime prevention and facility safety in public places and facilities. In order to improve the performance of human activity classification, most studies have applied deep learning based-transfer learning. However, despite the increase in the number of backbone network models that are the basis of deep learning as well as the diversification of architectures, research on finding a backbone network model suitable for the purpose of operation is insufficient due to the atmosphere of using a certain model. Thus, this study applies the transfer learning into recently developed deep learning backborn network models to build an intelligent system that classifies human activity using imagery. For this, 12 types of active and high-contact human activities based on sports, not basic human behaviors, were determined and 7,200 images were collected. After 20 epochs of transfer learning were equally applied to five backbone network models, we quantitatively analyzed them to find the best backbone network model for human activity classification in terms of learning process and resultant performance. As a result, XceptionNet model demonstrated 0.99 and 0.91 in training and validation accuracy, 0.96 and 0.91 in Top 2 accuracy and average precision, 1,566 sec in train process time and 260.4MB in model memory size. It was confirmed that the performance of XceptionNet was higher than that of other models.

The cognition of denture fabrication activities and the costs in National Health Insurance for elderly denture (노인틀니 국민건강보험의 틀니제작행위 및 비용에 관한 인식)

  • Purpose: The purpose of this study was to investigate the cognition of denture fabrication activities and its cost in National Health Insurance for elderly denture. Methods: A self-administered questionnaire was completed by 41 dental laboratories' owner who was research subjects of HIRA(Health Insurance Review & Assessment Service)'s policy research for elderly denture in 2011. The questionnaire consisted of general characteristics of the subjects, dental technicians' knowledge of national health insurance coverage of elderly denture, job-related changes after national health insurance coverage of elderly denture including validity of denture fabrication activity classification and the cost for each service. Each question was measured by Likert 5 point scale or frequency. The collected data were analyzed by SPSS 16.0. Results: Most of the research subjects had been fabricating national health insurance coverage denture(92.7%), also had ample knowledge of national health insurance coverage denture for elderly. Job-related changes after national health insurance coverage of elderly denture revealed marginal differences in the quality. The validity of fabrication activity classification of resin based complete denture was $3.71{\pm}1.023$ by Likert 5 point scale. Conclusion: The goal of national health insurance for elderly denture is to promote elderly's health and well-being. To fabricate denture is a very important part of the denture treatment. For this reason, denture fabrication activity classification and the cost analysis plan should be duly reflected in the policy of national health insurance for elderly denture.

Analysis of the Nursing Practice in a Medical ICU Based on an Electronic Nursing Record (간호기록을 이용한 중환자실 간호업무 조사연구)

  • Purpose: The purpose of this study was to identify the entity of critical care nursing practices through analyzing nursing statements described by electronic nursing records in a MICU. Methods: 176,459 nursing statements of 188 patients during a 6 month-stay were analyzed statement by statement according to the nursing process(nursing phenomena, nursing diagnosis, & nursing activity) and 21 nursing components of Saba's Clinical Care Classification. Results: Among 176,459 single statements, the statements of nursing activity ranked first in number. The contents of the statements were analyzed and categorized by main themes. Among 489 categorized themes, the number of themes of nursing phenomena statements was the highest. When analyzed by Saba's clinical Care Classification, the nursing statements mainly included a physiological component. Among 21 components, the respiratory component ranked in the first position in nursing phenomena, nursing diagnosis and nursing activity. The extra statements not included in the 21 components were 9,294(15.1%) in nursing phenomena and 21,949(22.7%) in nursing activity. Most are statements related to tests and the doctor. Conclusion: The entity of MICU nursing practice expressed by electronic nursing records was mainly focused on physiological components and more precisely on respiratory components.
