한국인의 흡연과 사망 위험에 관한 코호트 연구 (Cigarette Smoking and Mortality in the Korean Multi-center Cancer Cohort (KMCC) Study)

  • Objectives: The aim of this study was to evaluate the association between cigarette smoking and total mortality, cancer mortality and other disease mortalities in Korean adults. Methods: A total of 14 161 subjects of the Korean Multi-center Cancer Cohort who were over 40 years of age and who were cancer-free at baseline enrollment reported their lifestyle factors, including the smoking status. The median follow-up time was 6.6 years. During the follow-up period from 1993 to 2005, we identified 1159 cases of mortality, including 260 cancer mortality cases with a total of 91 987 person-years, by the national death certificate. Cox proportional hazard regression model was used to estimate the hazard ratio (HR) of cigarette smoking for total mortality, cancer mortality and disease-specific mortality, as adjusted for age, gender, the geographic area and year of enrollment, the alcohol consumption status, the education level and the body mass index (BMI). Results: Cigarette smoking was significantly associated with an increased risk of total mortality, all-cancer mortality and lung cancer mortality (p-trend, < 0.01, <0.01, <0.01, respectively). Compared to non-smoking, current smokers were at a higher risk for mortality [HR (95% CI)=1.3 (1.1 - 1.5) for total mortality; HR (95% CI)=1.6 (1.1 -2.2) for all-cancer mortality; HR (95% CI)=3.9 (1.9-7.7) for lung cancer mortality]. Conclusions: This study's results suggest that cigarette smoking might be associated with total mortality, all-cancer mortality and especially lung cancer mortality among Korean adults.

담배에 부착된 담뱃갑 경고 그림이 금연효과에 미치는 영향 (Effect of Cigarette Warning Sign Attached to Cigarettes on Smoking Cessation Effects)

  • 본 연구는 국내에서 판매되고 있는 담뱃갑 흡연 '담뱃갑경고그림'이 흡연자의 금연효과에 미치는 영향을 조사하여 향후 금연 및 건강관련 보건자료의 기초로 사용할 목적으로 시행된 서술적 조사연구이다. 이를 위해 인터넷 네이버 웹상에서 20세 이상 성인 남녀 500명을 대상으로 네이버 폼 설문지를 이용해 블러그와 카페 등을 이용하여 2018년 10월1부터 10일간 조사하여 응답이 미흡한 설문지를 제외하고 500부를 본 연구에 사용하였다. 분석된 데이터는 SPSS WIN20.0을 이용한 요인 분석, T-검정 및 상관관계를 분석하였다. 주요 연구 결과는 첫째, 인체해 유해한 담뱃갑 경고 그림이 금연효과에 가장 큰 영향을 미친 것으로 나타났다. 둘째, 금연효과는 담뱃갑 경고그림에 대한 설문 전 의식과 가장 높은 상관관계를 보였다. 이는 '담뱃갑 경고그림'이 흡연자에게는 가장 민감한 부분으로 받아들여지고 있음을 나타낸다. 본 논문의 결과를 토대로 담뱃갑 경고 그림의 다양화와 경고그림의 정기적인 교체 등은 금연 본능을 억제시키는 기전으로 작동함에 따라 향후 금연예방의 실천적 방안을 제안 할 수 있는 유용한 기초자료를 제공할 것으로 기대된다. 따라서 흡연경고 그림은 언론이나 매체를 통해 널리 홍보하여 국민들에게 금연을 실행하는데 긍정적인 영향을 미치며, 나아가 국민들의 건강 증진을 위한 보건정책의 기초자료로 활용될 것으로 사료된다.

담배가격 인상에 따른 청소년 흡연율 변화 (Cigarette price rise induced change in youth smoking rate)

  • 본 연구에서는 2015년 담배가격 인상정책 전후 청소년 흡연율 변화, 금연시도율 변화 등에 대해 살펴보고자 하였다. 이에 본 연구는 총 800개교의 중학생 및 고등학생을 대상으로 조사를 실시한 제9차(2013년), 제10차(2014년), 제11차(2015년) 청소년건강행태온라인 조사 원시자료를 이용하여 청소년들의 일반적 특성, 정신건강 및 주관적 학업성적, 음주 및 흡연관련 특성에 따른 담배가격 인상 전후 흡연율 변화 및 금연시도율, 금연시도이유 중 '담배값이 너무 비싸서' 선택률, 흡연량 변화를 분석하였다. 본 연구에서 전체 청소년의 현재 흡연율은 2013년 9.7%에서 2014년 9.2%로 감소하는 모습을 보였고, 담배가격 인상 후인 2015년 흡연율은 7.8%로 전년대비 15.2% 감소율을 보여 담배가격 인상 전보다 큰 폭으로 감소한 것으로 나타났다. 이러한 결과들을 통해 담배가격 인상정책이 청소년 흡연율을 감소시키는데 매우 효과적일 뿐만 아니라 금연동기 및 흡연 예방에도 매우 효과적이라는 것을 확인할 수 있었다. 청소년기 흡연자의 대다수가 성인기 흡연자로 이어진다는 점에서 청소년기의 흡연 예방 및 금연 유도는 매우 중요하다. 향후 흡연실을 허용하지 않는 완전 금연구역의 확대와 담배 광고, 판촉, 후원에 대한 포괄적 금지는 시급히 추진되어야 하며 이미 근거를 바탕으로 작성된 WHO FCTC의 각조약을 충실히 이행할때 금연효과를 극대화 할 수 있을 것이라 사료된다.

  • Background: The smoking prevalence in asthma patients are similar to those in the general population. Asthma and active cigarette smoking can interact to create more severe symptoms, an accelerated decline in lung function and impaired therapeutic responses. Accordingly, asthmatics with a history of smoking were examined to define the clinical characteristics and lung function of smoking asthmatics. Methods: The medical records of 142 asthmatics with a known smoking history were reviewed. The patients were divided into three groups according to their smoking history - current smokers, former smokers and non-smokers. The clinical characteristics, lung function, and annual declines of the forced expiratory volume in one second ($FEV_1$) were compared. Results: Fifty-three of the 142 patients (37%) were current smokers, 24 were former smokers (17%) and 65 were non-smokers (45%). The patients with a hospital admission history during the previous year included 16 current smokers (30%), 4 former smokers (17%) and 7 non-smokers (11%) (p=0.02). The mean $FEV_1$ (% predicted) was 76.8${\pm}$19.8%, 71.6${\pm}$21.1% and 87.9${\pm}$18.7% for current smokers, former smokers and non-smokers, respectively (p< 0.001). The $FEV_1$/forced vital capacity (FVC) (ratio, %) values were 63.6${\pm}$12.6%, 59.3${\pm}$14.9% and 72.1${\pm}$11.8% in current smokers, former smokers and non-smokers, respectively (p<0.001). The corresponding mean values for the individual $FEV_1$ slopes were not significant (p=0.33). Conclusion: Asthmatic smokers demonstrated higher hospital admission rates and lower lung function. These findings suggest that the smoking history is an important predictor of a poor clinical outcome in asthma patients.

The Trend of Cigarette Design and Tobacco Flavor System Development

  • In light of addressing consumer health concern, coping with anti-tobacco movement, and promoting new product, tobacco industry is actively pursuing to make a new generation of cigarettes with low tar and nicotine deliveries, and less harmful substances. Low tar and low nicotine cigarettes increases their market shares dramatically world wide, especially in KT&G, multinational tobacco companies, EU countries, even in China regulated by CNTC to set up yearly target to lower tar and nicotine deliveries. On the other hand, to design a new cigarette with reduced harmful substances begins to gain speed. The "modified Hoffmann list" publishes thirty plus substances in tobacco leaf and main smoke stream, which is the prime suspect causing health problems. Various ways and means are developed to reduce such components including new tobacco breeds, new curing method, tobacco leaf treatment before processing, selected filtration system, innovated casing system to reduce free radicals, as well as some non conventional cigarette products. In TSRC held this year, the main topic is related to reduce tobacco specific nitrosamines in tobacco leaf. The new generation of cigarette is in the horizon. It still needs a lot help to produce commercial products with satisfied taste and aroma characters. The flavor industry is not regulated by many governments demanding which ingredients might or might not be for tobacco use. However, most of the cigarette companies self impose a list of ingredients to guide flavor suppliers to design flavors. Unfortunately, the number of ingredients in those lists is getting shorter every year. It is understandable that the health is not the only reason. Some cigarette companies are playing safe to protect the company from potential lawsuit, while others are just copying from their competitors. Moreover, it is obvious that it needs more assistance from casings and flavors to design new generation of cigarettes with missing certain flavor components in tobacco leaf and main smoke stream. These flavor components are either non-existed or at lower level at new form of cured tobacco leaf or filtered in the main smoke stream along with reduced harmful substances. The use of carbon filters and other selected filtration system poses another tough task for flavor system design. Specific flavor components are missing from the smoke analysis data, which brings a notion of "carbon taste" and "dryness" of mouth feel. It is ever more demanded by cigarette industry to flavor suppliers to produce flavors as body enhancer, tobacco notes, salivating agents, harshness reducer, and various of aromatic notes provided they are safe to use. Another trend is that water based flavor or flavor with reduced ethanol as solvent is gaining popularity. It is preferred by some cigarette companies that the flavor is compounded with all natural ingredients or all ingredients should he GMO free. The new generation of cigarettes demands many ways of new thinking process. It is also vital for tobacco industry. It reflects the real needs for the consumers that the cigarette product should be safe to use as well as bearing the taste and aroma characters smokers always enjoyed. An effective tobacco flavor system is definitely a part of the equation. The global trend of tobacco industry is like trends of any other industries lead by consumer needs, benefited with new technology availability, affected by the global economy, and subjected for various rules and regulations. Anti-tobacco organizations and media exceptionally scrutinize cigarette, as a legal commercial product. Cigarette is probably the most studied commercial product for its composition, structure, deliveries, effects, as well as its new developmental trend. Therefore, any new trend of cigarette development would be within these boundaries. This paper is trying to point out what it would be like for tobacco industry in the next few yews and what concerns the tobacco industry. It focuses mostly on the efforts to produce safer cigarettes. It is such a vital task for the tobacco industry and its affiliate industries such as cigarette papers, filters, flavors, and other materials. The facts and knowledge presented in this paper might be well known for the public. Some of the comments and predictions are very much personal opinion for a further discussion.

녹차 추출물에 의한 니코틴의 코티닌으로 전이 촉진 (Enhanced conversion to cotinine from nicotine by green tea extract)

  • 흡연은 직접 또는 environmental tobacco smoke를 통하여 많은 위해성 물질을 인체에 유입시킨다. 이러한 위해 물질중 니코틴을 인체내의 대사에 따라서 암을 유발시키는 물질로 전이되기도 한다. 니코틴의 대사산물로 대표적인 발암물질인 nitrosamine-4-(methylnitrosamino)-1-(3-pyridyl)-1- butanone (NNK)가 있다. 많은 연구는 녹차의 추출물이 암을 예방하는데 탁월한 효능이 있음을 보고하고 있다. 그러나, 녹차의 암예방 효과에 대한 기작은 거의 알려져 있지 않다. 최근의 보고는 니코틴의 주요대사 산물인 코니틴이 안정한 물질임과 동시에 NNK의 활성 및 형성을 억제하는 것으로 알려져 있다. 본 연구는 세포내의 니코틴의 대사에 있어, 코티닌으로의 전이도에 녹차의 추출물이 미치는 영향에 대하여 연구하였다. 녹차의 추출물은 시험관에서 직접 혼합한 경우와 세포주를 이용한 실험에서, 니코틴이 코티닌으로 전이하는 데 긍정적 영향을 주었다. 이 결과는 녹차의 성분 중에 니코틴의 여러 대사경로중에서 코티닌의 대사를 촉진하는 물질이 함유되어 있음을 의미한다. 이 결과는 아울러, 녹차의 암 예방의 가능성에 대하여 설명할 수 있는 기전을 제시한다.

서울 위생 병원 5일 금연 학교를 방문한 우리나라 흡연자의 특성 및 입원 금연 교육 프로그램의 효과 (The Characteristics of Korean Smoker, Enrolled in '5 day Smoking Cessation School' and The Effect of Inpatient Smoking Cessation Program)

  • 연구배경 : 우리나라의 성인 흡연율은 세계적으로 높고 청소년과 여성의 흡연율도 점차 증가하고 있다. 흡연이 이렇게 사회적으로 문제가 되고 있는 상황에서 효과적인 금연 프로그램이 필요한 상태이다. 이에 본 병원 금연 학교의 입원 금연 교육의 효과와 이에 영향을 미치는 요인을 알아보기 위해 본 연구를 시행하였다. 방 법 : 1998년 1월부터 2001년 1월까지 본원 금연학교를 수료한 640명을 대상으로 설문지를 이용하여 흡연자들의 특성을 조사하였다. 또한, 입원 대상 금연교육 대상자 235명중 응답 가능한 100명을 대상으로 이들의 특성 및 금연 성공 유무 등을 조사하였다. 결 과 : 금연 학교 등록자의 금연 동기로는 건강이 가장 많았고, 흡연 동기로는 스트레스가 가장 많았다. 입원 금연학교의 최소 1년 이상 금연율은 60%를 보였다. 입원 금연 교육 대상자에서 흡연 시작 연령과 직업이 금연 성공 유무와 의미 있는 연관을 보였다. 금연을 실패한 경우 일상 생활의 스트레스가 가장 흔한 원인이 였고, 1주에서 1개월 내에 흡연 재발이 많았다.

청소년의 흡연, 음주와 정신건강과의 관계 (The Relationship Between Smoking, Drinking and the Mental Health in Adolescents)

  • Purpose: This study sought to investigate the relationship between smoking, drinking and the mental health of adolescents. Method: The study subjects included 1,092 randomly sampled third-year middle and high school students from N-City and the L-Kun area in Chonbuk Province. The data were collected using an anonymous questionnaire developed by the Ministry of Health and Welfare(2005). The data were analyzed using the SPSSWIN 15.0 Program. Result: The smoking rate in the adolescents was 10.1%, and the drinking rate was 43.2%. The stress recognition rate in normal daily life was 45.5%, the frequency of depressive symptoms was 33.2% and 40.0% of the subjects reported that they wanted to die at some point within the past year. The rate of stress recognition, depressive symptom experience, suicidal planning and attempt in smokers and drinkers was higher than that in non-smokers and non-drinkers (p<.05). Especially, the frequency of suicidal thoughts was higher in the drinkers than in the non-drinkers (p<.05). Conclusion: The smoking and drinking status of the adolescents was correlated with their mental health a sit relates to stress, depression and suicide. Therefore, an effective program for the prohibition of cigarette smoking and alcohol consumption is needed in order to prevent smoking and alcohol consumption in adolescents and improve their mental health status.

호기 중 일산화탄소 농도에 따른 양도락 상의 변화 연구 (후향적 증례연구) (Study on changes in Ryodoraku test according to carbon monoxide concentration in exhaled breath)

  • Objectives: The aim of this study was to investigate relationships between Ryodoraku and carbon monoxide concentration during expiration of smokers. This study was designed as an exploratory observational study. This study was based on the hypothesis that increased levels of carbon monoxide in the smoker's breath may affect indicators that reflect the respiratory and circulatory systems in the Ryodoraku test. Methods: The participants were 49 people who smoked for over a year. They were examined at least one hour after their last smoking. They were tested in a stable state. When examining carbon monoxide during expiration, they breathed deeply, kept breathing for 20 seconds, and slowly exhaled. The Ryodoraku test was performed on its representative points of twelve meridians. Pearson's correlation analysis was used to investigate correlation between the concentration of carbon monoxide in the breath and the current in the measurement points. Results: The higher the carbon monoxide concentration in the exhalation, the significantly greater the current value of Lt H1·both H2·Lt H3·Rt H5, deviation of Lt H1·Lt H2·Lt H3 current value and the laterality between right and left current of H3. Conclusion: The more cigarette smoked, the more changes in heart, lung, pericardium and triple energizer meridians were observed in the Ryodoraku test. I found that increased carbon monoxide in smokers could affect the results of the Ryodoraku test.

인삼의 섭취가 흡연성인의 인체임파구 SCE 빈도수에 미치는 영향 (The Effect of Ginseng Intake on the Frequency of Sister Chromatid Exchanges of Human Lymphocyte of Adult Smokers)

  • This study was intended to investigate the anticarcinogenic effect of ginseng previously elucidated by other researches in animal studies. The sister chromatid exchange(SCE) method of human lymphocytes was used as a biomarker. Based on the literature search and the results of our laboratory, smoking was used as a parameter elevating the SCE frequency of general human population. To evaluate the smoking and ginseng effect on SCE frequency, 98 male healthy factory workers aged 23 to 58 years were divided into 4 groups : smoker with ginseng (SG), smoker control(SC), non-smoker with ginseng(NSG), and non-smoker control(NSC) groups, according to their smoking habits and ginseng intake. The mean sponteneous SCE per cell for the SG(10.8$\pm$0.3) and SC(10.4$\pm$0.3) groups were significantly higher than the NSG(9.1$\pm$0.2) and NSC(9.3$\pm$0.3) groups(p<0.05). High frequency cells (HFCs, cells with 15 SCEs) in SG and SC groups were also greater than those in NSG and NSC groups. However, the SCE levels of the SG and SC groups were not associated with the personal smoking history and the number of cigaretts smoked per day. Ginseng intake did not show any effect on the increased SCE caused by smoking. There were no correlations of the elevated SCE among smoking and ginseng types, history of ginseng intake, and consumption frequencies of ginseng intake. These results does not support the findings of other researchers that ginseng could be a protective agent to DNA damage.
