The Importance of Complete Pericardiectomy and the Role of the Apical Suction Device in Chronic Constrictive Pericarditis

  • Background: The aim of this study was to analyze the preoperative attributes and clinical impacts of complete pericardiectomy in chronic constrictive pericarditis. Methods: A total of 26 patients were treated from January 2001 to December 2013. The pericardium was resected as widely as possible. When excessive bleeding or hemodynamic instability occurred intraoperatively, a cardiopulmonary bypass (CPB; n=3, 11.5%) or an apical suction device (n=8, 30.8%) was used. Patients were divided into 2 groups: those who underwent ${\geq}80%$ resection of the pericardium (group A, n=18) and those who underwent <80% resection of the pericardium (group B, n=8). Results: The frequency of CPB use was not significantly different between groups A and B (n=2, 11.1% vs. n=1, 12.5%; p=1.000). However, the apical suction device was more frequently applied in group A than group B (n=8, 30.8% vs. n=0, 0.0%; p=0.031). The postoperative New York Heart Association functional classification improved more in group A (p=0.030). Long-term follow-up echocardiography also showed a lower frequency of unresolved constriction in group A than in group B (n=1, 5.60% vs. n=5, 62.5%; p=0.008). Conclusion: Patients with chronic constrictive pericarditis demonstrated symptomatic improvement through complete pericardiectomy. Aggressive resection of the pericardium may correct constrictive physiology and an apical suction device can facilitate the approach to the posterolateral aspect of the left ventricle and atrioventricular groove area without the aid of CPB.

Toxicity Assessment of Titanium (IV) Oxide Nanoparticles Using Daphnia magna (Water Flea)

  • Objectives: Titanium dioxide ($TiO_2$), a common nanoparticle widely used in industrial production, is one of nano-sized materials. The purpose of this study was to determine the acute and chronic toxicity of $TiO_2$ using different size and various concentrations on Daphnia magna. Methods: In the acute toxicity test, four concentrations (0, 0.5, 4, and 8 mM) for $TiO_2$ with 250 or 500 nm and five concentrations (0, 0.25, 0.5, 0.75, and 1 mM) for $TiO_2$ with 21 nm were selected to analyze the toxic effect to three groups of ten daphnia neonates over 96 hours. In addition, to better understand their toxicity, chronic toxicity was examined over 21 days using 0, 1, and 10 mM for each type of $TiO_2$. Results: Our results showed that all organisms died before the reproduction time at a concentration of 10 mM of $TiO_2$. In addition, the exposure of anatase (21 nm) particles were more toxic to D. magna, comparing with that of anatase (250 nm) and rutile (500 nm) particles. Conclusions: This study indicated that $TiO_2$ had adverse impacts on the survival, growth and reproduction of D. magna after the 21days exposure. In addition, the number of test organisms that were able to reproduce neonates gradually were reduced as the size of $TiO_2$ tested was decreased.

Effects of an Agro-Healing Activity Program on the Physiological Condition of Adults with Chronic Metabolic Diseases

  • This study was conducted to find out the effects of agro-healing activities and strolling on the physiological conditions of patients with chronic metabolic diseases. A total of 11 subjects participated in agro-healing activities, and their average age was 49.6±11.3. Changes in their salivary cortisol were compared and analyzed before and after participating in the agro-healing activity program, and it was found that the stress hormone decreased after participating in the program. Differences in brain wave between before and after participating in agro-healing activities and strolling were measured, and the ratio of alpha to high beta (RAHB) of subjects increased in most of the measured points after participating in agro-healing activities. In addition, the increase in the value in the temporal lobe (T3) that controls mortor skills and occipital lobe (O2) that controls visual functions indicates that the activities were effective in improving relaxation and stability. The relative low beta (RLB) power spectrum of subjects also statistically significantly decreased in the right occipital lobe (O2) that controls visual functions, the relative mid beta (RMB) and relative high beta (RHB) power spectrum decreased in the left temporal lobe (T3) and the right occipital lobe (O2). These results indicate that the activities were effective in improving relaxation under no stress and tension. The systolic and diastolic blood pressure of subjects after participating in agro-healing activities decreased from the prehypertension level to the normal level. These results indicate that horticultural activities and visual elements in healing farms, such as agro-healing activities and strolling, have positive impacts on patients with chronic metabolic diseases accompanied with diabetes or high blood pressure by increasing the brain activity and psycho-physiological conditions of participants.

우울, 신체화, 턱기능장애가 측두하악장애 환자의 만성통증척도에 미치는 영향 (Impacts of Depression, Somatization, and Jaw Disability on Graded Chronic Pain in TMD Patients)

  • 측두하악장애 연구진단기준 (Research Diagnostic Criteria for Temporomandibular Disorders; RDC/TMD)을 이용하여 측두하악장애 환자들에서 심리학적 상태 및 턱기능장애의 정도가 통증의 정도 및 지속기간, 통증과 관련된 일상생활 장애의 정도 및 기간, 만성통증척도에 미치는 영향 등을 알아보기 위하여 RDC/TMD axis I criteria에 따라 측두하악장애로 진단받은 평균연령 30.1$\pm$12.5 세인 93 명의 환자들(남성 17명, 여성 76명)을 대상으로 RDC/TMD axis II 병력 설문지를 작성하게 한 뒤, 우울증점수, 신체화장애점수, 턱기능장애점수와 만성통증척도의 통증 강도, 통증 지속기간, 통증과 연관된 장애 점수, 통증과 연관된 장애의 기간 및 만성통증척도 사이의 연관성을 분석하여 다음과 같은 결론을 얻었다. 1. 93명의 측두하악장애 환자들에서 만성통증척도의 경도의 장애를 보이는 군 (low disability group, grade I and II)은 42.0% 이었으며, 심도의 장애를 보이는 군 (high disability group, grade III and IV)은 51.5% 이었다. 2. 우울증점수는 통증 강도, 통증 지속기간, 통증과 연관된 장애 점수, 통증과 연관된 장애의 기간, 만성통증척도 모두에 통계적으로 유의한 영향을 미치지 않았다. 3. 신체화장애점수는 통증 강도 (p<0.01), 통증과 연관된 장애의 기간 (p<0.01), 만성통증척도 (p<0.01)에 유의한 영향을 미쳤으나, 통증 지속기간이나 통증과 연관된 장애 점수에는 통계적으로 유의한 영향을 미치지 않았다. 4. 턱기능장애점수는 통증 강도 (p<0.001), 통증과 연관된 장애의 기간 (p<0.01), 만성통증척도 (p<0.001)에 유의한 영향을 미쳤으나, 통증 지속기간이나 통증과 연관된 장애 점수에는 통계적으로 유의한 영향을 미치지 않았다. 측두하악장애환자에서 신체화장애와 턱기능장애는 환자가 느끼는 통증의 강도 및 장애의 정도와 밀접하게 관련되어 나타날 수 있으며, 환자가 호소하는 정서적 스트레스에 대한 포괄적인 이해와 기능장애에 대한 적절한 관리가 측두하악장애 환자의 성공적인 치료에 매우 중요하리라 생각된다.

건강보험 보장성 확대가 의료이용 및 건강수준에 미치는 영향 (Impacts of Health Insurance Coverage Expansion on Health Care Utilization and Health Status)

  • 본 연구는 최근에 이루어진 '건강보험 보장성 확대' 정책의 효과를 개인의 의료이용 및 건강수준을 중심으로 평가하였다. 2005년부터 시행된 건강보험 보장성 확대 정책은 암 등의 중증질환자에 대한 본인부담 경감, 입원환자 식대의 급여전환, 비급여항목의 급여전환, 본인부담상한제 시행 등을 주요 내용으로 한다. 이러한 정책 변화에 대해 만성질환을 보유하고 있어 의료욕구가 일반적으로 높은 제도의 주요 수혜집단에 비해 급성질환 등으로 의료서비스를 이용한 집단의 입원이용이 상대적으로 증가한 것과 제도의 직접적인 수혜집단인 중증질환자 집단의 입원이용이 상당 정도 증가한 것을 확인하였다. 또한 이러한 건강보험제도의 변화가 입원이용에서 혜택을 누린 중증질환자의 단기간의 건강평가지표인 2주간 이환경험 면에서 긍정적인 개선을 이끌어내고 있음을 확인하였다. 본 연구는 건강보험제도가 의료이용 및 건강수준에 인과적인 효과를 나타내고 있음을 확인하였고, 이후 건강보험 보장성 확대의 정책설계에 대한 정책적 함의를 제공한다.

Roles of Inflammatory Biomarkers in Exhaled Breath Condensates in Respiratory Clinical Fields

  • Background: Exhaled condensates contain inflammatory biomarkers; however, their roles in the clinical field have been under-investigated. Methods: We prospectively enrolled subjects admitted to pulmonology clinics. We collected exhaled breath condensates (EBC) and analysed the levels of six and 12 biomarkers using conventional and multiplex enzyme-linked immunosorbent assay, respectively. Results: Among the 123 subjects, healthy controls constituted the largest group (81 participants; 65.9%), followed by the preserved ratio impaired spirometry group (21 patients; 17.1%) and the chronic obstructive pulmonary disease (COPD) group (21 patients; 17.1%). In COPD patients, platelet derived growth factor-AA exhibited strong positive correlations with COPD assessment test (ρ=0.5926, p=0.0423) and COPD-specific version of St. George's Respiratory Questionnaire (SGRQ-C) score (total, ρ=0.6725, p=0.0166; activity, ρ=0.7176, p=0.0086; and impacts, ρ=0.6151, p=0.0333). Granzyme B showed strong positive correlations with SGRQ-C score (symptoms, ρ=0.6078, p=0.0360; and impacts, ρ=0.6007, p=0.0389). Interleukin 6 exhibited a strong positive correlation with SGRQ-C score (activity, ρ=0.4671, p=0.0378). The absolute serum eosinophil and basophil counts showed positive correlations with pro-collagen I alpha 1 (ρ=0.6735, p=0.0164 and ρ=0.6295, p=0.0283, respectively). In healthy subjects, forced expiratory volume in 1 second (FEV1)/forced vital capacity demonstrated significant correlation with CC chemokine ligand 3 (CCL3)/macrophage inflammatory protein 1 alpha (ρ=0.3897 and p=0.0068). FEV1 exhibited significant correlation with CCL11/eotaxin (ρ=0.4445 and p=0.0017). Conclusion: Inflammatory biomarkers in EBC might be useful to predict quality of life concerning respiratory symptoms and serologic markers. Further studies are needed.

인구집단의 비교를 통한 폐금속광산 지역 주민의 카드뮴 노출수준 및 건강영향평가 (Health Impact Assessment for Cadmium Exposure: Comparison of Residents around Abandoned Mines with the General Population)

  • Objective: We compared the level of blood cadmium exposure with health impacts by using data from a survey of residents near an abandoned mine and a national health survey. Methods: For this study, we used data from 7,046 individuals who participated in the Health Effects Survey of Abandoned Metal Mines (AMS, 2008-2011) and 6,871 individuals who participated in the Korea National Health and Nutrition Examination Survey IV-VI (KNHANES, 2008-2013). To evaluate the health impacts, the quartiles (S1 to S4) were classified according to blood cadmium concentration, and then the odds ratios of S2 to S4 over S1 for exceeding the reference values of renal function, blood pressure, and bone density were compared. Similarly, the odds ratio of AMS over KNHANES was confirmed. Results: In the AMS, adjusted for general characteristics, the geometric mean of blood cadmium concentration was 1.34 ㎍/L, which was statistically significantly higher than that of the KNHANES 1.22 ㎍/L (p<0.001). In the integrated data of AMS and KNHANES, the estimated odds ratio of S4 over S1 for exceeding the reference value was found to be 1.70 for serum creatinine, 1.71 for hypertension, and a T-score 2.02 for the tarsal bone. They were all statistically significant. Conclusion: Residents around abandoned metal mines had a higher blood cadmium concentration than the general population, and the odds for exceeding the reference values were higher for some health indicators. Continuous biomonitoring should be conducted for vulnerable areas such as around abandoned metal mines, and measures to reduce cadmium exposure and management of chronic diseases are needed.

성인의 유산소 신체활동과 구강건강 및 구강건강행태와의 관련요인 : 제8기 국민건강영양조사 이용 (Factors Related to Aerobic Physical Activity, Oral Health and Oral Health Behavior in Adult : Use of the 8th national health and nutrition survey)

  • Purpose : Many studies have confirmed the relationship between physical exercise, chronic diseases, and quality of life, but few of those studies were limited to aerobic exercise. Above all, no research has confirmed the relationship between aerobic exercise and the oral health. This study is significant because it is the first study to confirm the relationship between aerobic exercise, which is practiced more frequently than other exercises, and oral health in adults. Through this study, we hoped to confirm the complex impacts of aerobic exercise on health-related quality of life, oral health-related behavior, and oral health status in adults and to use these impacts as basic data on the importance of aerobic exercise. Methods : In this study, the following analysis was conducted based on a complex sample design that applied stratification variables, cluster variables, and weights using SPSS version 21.0. Complex sample cross-analysis was conducted to identify general characteristics according to aerobic physical activity practice, and oral health-related characteristics according to the aerobic physical activity practice rate. Then, complex sample logistic regression analysis was conducted to determine the effect of aerobic physical activity practice on oral health-related characteristics. During the statistical analysis, missing values were treated as valid values, and the statistical significance level was set at .05. Results : Aerobic physical activity practice was 1.39 times higher among the respondents who brushed their teeth after lunch (p<.001), 1.43 times higher among those who used dental floss (p<.001), 1.24 times higher among those who used mouthwash (p=.040), and 1.37 times higher was among those who had not experienced dental treatment (p=.040), which were statistically significant differences. Conclusion : This study found that when an individual's health status is maintained, positive oral health behavior can be achieved by paying attention to oral health, and this appears to contribute to improving oral health.

Pentachlorophenol impact assessment of haematological parameters in olive flounder, Paralichthys olivaceus

  • The toxic effects of pentachlorophenol (PCP) on haematological parameters in olive flounder (Paralichthys olivaceus) after chronic exposure to dietary PCP (0.1, 0.5 and 1.0 mg/kg diet) for 2, 4 and 6 weeks were studied. A significant decrease in total RBC count, haemoglobin concentration and hematocrit value was noted in fish exposed to PCP compared to the non-exposed fish. The PCP treatment group showed significantly lower concentration of serum total protein and albumin, and significantly higher serum chloride, magnesium and total bilirubin levels compared with those in the control group. However, PCP had no major effects on serum glucose, total cholesterol, phosphate and calcium ions in flounder. These results demonstrated that PCP have induced adverse haematological impacts in the olive flounder, Paralichthys olivaceus. Because we found damages in blood-forming function and disruption in blood homeostasis due to chronic exposure to PCP, it is needed to develop further experimental studies for the risk assessment of this environmental pollutant.

치과위생사의 감정노동, 근무환경 분위기, 직무몰입이 이직의도에 미치는 영향 (Effects of emotional labor, organizational climate, and job involvement on turnover intention in Korean dental hygienists)

  • Objectives: The purpose of this study was to investigate the effects of emotional labor, organizational climate, and job involvement on turnover intention in Korean dental hygienists. Methods: A total of 806 dental hygienists were enrolled in this study. A self-administered questionnaire was used to evaluate the demographic and occupational characteristics, emotional labor, organizational climate, job involvement, and turnover intention of the subjects. A hierarchical multiple regression analysis was performed. All statistical analyses were performed using the Statistical Package for the Social Sciences for Windows (version 23.0) and p-values < 0.05 were considered significant. Results: Highly chronic exposure to emotional labor were more likely to increase the risk of turnover intention among dental hygienists, especially in the sub-scales of "over-load and conflict in customer service" (OR: 1.64; CI: 1.01-2.64), "organizational surveillance and monitoring" (OR: 1.76; 95% CI: 1.21-2.57), and "lack of a supportive and protective system in the organization" (OR: 1.76; 95% CI: 1.36-2.46). Job involvement (OR: 0.91; 95% CI: 0.86-0.96) and organizational climate (OR: 0.82; 95% CI: 0.75-0.90) were negatively associated with turnover intention among dental hygienists. Conclusions: Exposure to chronic and excessive emotional labor might be a contributing factor for turnover intention, and job involvement and a positive organizational climate contribute to protecting the negative impacts of emotional labor on turnover intention in dental hygienists.
