2000년대의 환경변화에 대응한 건강접근전략 (The Strategy of Health Approach to Cope with the Environmental Change in 2000's)

  • This papaer discusses the recent tendencies of health approach in the developed countries and suggests the strategy of health approach in order to cope with the changing environmental conditions of Korea in 2000's. In 2000's, Korea will have the environmental conditions and health problems similar to those of the developed countries at present. The American and European developed countries have shown the integrative approach trend by the interdisciplinary cooperation based on the holistic health conception, for example, as in the behavioral medicine, with the active drive of the medical self care movement and the national health preactice movement. The basic solution to the 2000's main health problems such as high morbidity of chronic diseases and high medical expenditure is to decrease the health need through the health promotion, disease prevention and early. ditection and early treatment of disease. The above actions need to induce the public to change their health behavior in the desirable direction through the national health practice movement and the health self care movement. The succeed of the movements depends upon how to use effectively the mass media, health and administrative organizations, schools, industrial and medical insurance organizations with the strong government support of health and preventive policies and programs.

국민건강영양조사를 이용한 동반질환 및 다중이환의 패턴분석 (Pattern Analysis of Comorbidity and Multimorbidity in Reference to the 7th KNHANES)

  • This study investigated patterns of co-occuring chronic diseases and disorders in old ages. For this purpose, we utilized data from the Korean National Health and Nutrition Examination Survey for 3,734 old adults aged over 65. Data on 18 conditions were obtained, and analyzed using network analysis, associated rule mining, cluster analysis. The majority of participants has multimorbidity. Association rules analysis reveals unexpected comorbidities with high lift and confidence. Also, some morbidity clusters were present. Diabetes and emotional disorder had the greatest comorbidity and represent complex comorbid conditions. Old age is characterized by a complex pattern of multimorbidity and comorbidity. In conclusion, particular combinations of morbidities were very prevalent and will be needed to policy of health care interventions for old ages.

Comorbidity Analysis on ICU Big Data

  • Comorbidity isthe simultaneous presence of two chronic diseases or conditions in a patient. As part of a larger research study, the aims of this study were to explore comorbid conditions in intensive care unit (ICU) patients and to compare the comorbidity across different demographic groups, and to determine what comorbid health problems coexisted in the patients with hospital-acquired pressure injury (HAPI). The average number of comorbid conditions was 6.4 with range from 0-20 in the ICU patients. African American patients had significantly more comorbid health problems than other race/ethnicity groups. Asian and Hispanic female patients showed higher comorbidity than male patients across age. The patients with HAPIs had significantly more comorbid health problems than the patients without HAPIs -- the average numbers were almost two-fold. We found comorbid health problems that existed with HAPI in ICU patients. 'Other diseases of lung' and 'Disorders of fluid, electrolyte, and acid-base balance' were most frequently coexisting health problems in the ICU patients with HAPI. Exploratory plots are helpful to discover patterns or hypotheses relevant to clinical management in critical care. Inclusion of patients' comorbid health problems to ICU HAPI risk assessment may be helpful. Identification of patients at a high risk for the development of HAPI and the early preventative interventions can help reduce length of stay as well as costly complications.

우리나라 성인에서 복합만성질환이 치주질환에 미치는 영향 (Effects of Multiple Chronic Diseases on Periodontal Disease in Korean Adults)

  • 이 연구는 제5기 국민건강영양조사 원시자료를 활용하여 우리나라 성인의 복합만성질환이 치주질환에 미치는 영향을 파악하고자 실시되었다. 건강설문 및 구강검진 조사를 완료한 만 35세 이상 대상자 중 무응답자를 제외한 4,142명을 최종 분석대상으로 하였다. 종속변수인 치주질환은 CPI 측정값이 3(4mm 이상의 치주낭 형성) 또는 4(6mm 이상의 치주낭 형성)인 경우로 정의하였다. 연구대상자의 성별은 남자 48.5%, 여자 51.5%였으며, 치주질환의 유병률은 30.1%였다. 연령이 증가할수록, 교육수준과 소득수준이 낮을수록, 지역이 농촌일 경우 유병률이 높았다(p<0.01). 만성질환 보유 개수에 따라서 치주질환 유병율은 만성질환이 없는 경우 27.8%, 1개인 경우 31.9%, 2개인 경우 33.1%, 3개 이상인 경우 35.2%로 통계학적으로 유의한 차이가 있었다(p<0.05). 치주질환을 종속변수로 한 로지스틱 회귀분석을 한 결과, 성별, 연령, 교육, 지역, 현재흡연, 구강위생용품 사용여부가 치주질환에 영향을 미치는 요인이었다. 이 연구에서는 신뢰성이 높은 국가자료를 활용하여 우리나라 성인의 치주질환 유병률 30.1%를 파악하였으며, 복합만성질환보다는 인구학적 특성과 구강건강관리행태가 치주질환에 유의한 영향 인자임을 확인하였다. 개인의 인구학적 특성에 맞는 구강건강행태 실천을 유도하여 적극적인 구강건강관리를 통한 치주질환의 예방이 중요하다고 할 수 있다.

A Study on the Regional Gap Analysis and Management of the Elderly Health in an Aging Society

  • Korea is ranked as the world's No. 1 country in its aging rate. While the interest and demand for health is rapidly increasing, the health status of the elderly is in the lowest among OECD members. Increased chronic diseases, the burden of medical costs and digital/untact changes of societies after COVID-19 have caused the direction of healthcare to be changed from treatment oriented to health care and prevention oriented, along with increased income levels and a desire for a healthy life. Amid this paradigm of change, the gap in health standards and health inequality for the elderly according to local structure and social conditions affects not only socio-economic but also the quality of life for individual senior citizen. Utilizing prior data of Aging Research Panel Survey, this study aims to compare and analyze health conditions and regional gaps which are significant influences on the satisfaction of the life of the elderly, and to suggest direction of studies for health care to provide solutions for health inequalities. The findings are intended to be a basic data for researching models of the New Normal Smart Healthcare System that bridge the health gap between the elderly and enhance life satisfaction with health care models suitable for regional characteristics in aging society.

추후관리가 필요한 만성질환 퇴원환자 가정간호 시범사업 운영 연구 (An Operations Study on a Home Health Nursing Demonstration Program for the Patients Discharged with Chronic Residual Health Care Problems)

  • The study was conceived in relation to a concern over the growing gap between the needs of chronic patients and the availability of care from the current health care system in Korea. Patients with agonizing chronic pain, discomfort, despair and disability are left with helplessly unprepared families with little help from the acute care oriented health care system after discharge from hospital. There is a great need for the development of an alternative means of quality care that is economically feasible and culturally adaptible to our society. Thus, the study was designed to demonstrate the effectiveness of home heath care as an alternative to bridge the existing gap between the patients' needs and the current practice of health care. The study specifically purports to test the effects of home care on health expenditure, readmission, job retention, compliance to health care regime, general conditions, complications, and self-care knowledge and practices. The study was guided by the operations research method advocated by the Primary Health Care Operations Research Institute(PRICOR) which constitutes 3 stages of research : namely, problem analysis solution development, and solution validation. The first step in the operations research was field preparation to develop the necessary consensus and cooperation. This was done through the formation of a consulting body at the hospital and a steering committee among the researchers. For the stage of problem analysis, the Annual Report of Seoul National University Hospital and the patients records for last 5 years were reviewed and selective patient interviews were conducted to find out the magnitude of chronic health problems and areas of unmect health care needs to finally decide on the kinds of health problems to study. On the basis of problem analysis, the solution development stage was devoted to home care program development asa solution alternative. Assessment tools, teaching guidelines and care protocols were developed and tested for their validity. The final stage was the stage of experimentation and evaluation. Patients with liver diseases, hemiplegic and diabetic conditions were selected as study samples. Discharge evaluation, follow up home care, measurement and evaluation were carried out according to the protocols of care and measurement plan for each patient for the period of 6 months after discharge. The study was carried out for the period from Jan. 1987 to Dec. 1989. The following are the results of the study presented according to the hypotheses set forth for the study ; 1. Total expenditures for the period of study were not reduced for the experimental group, however, since the cost per hospital visit is about 4 times as great as the cost per home visit, the effect of cost saving by home care will become a reality as home care replaces part of the hospital visits. 2. The effect on the rate of readmission and job retention was found to be statistically nonsignificant though the number of readmission was less among the experimental group receiving home care. 3. The effect on compliance to the health care regime was found to be statistically significant at the 5% level for hepatopathic and diabetic patients. 4. Education on diet, rest and excise, and medication through home care had an effect on improved liver function test scores, prevention of complications and self - care knowledge in hepatopathic patients at a statistically significant level. 5. In hemiplegic patient, home care had an effect on increased grasping power at a significant level. However. there was no significant difference between the experimental and control groups in the level of compliane, prevention of complications or in self-care practices. 6. In diabetic patients, there was no difference between the experimental and control groups in scores of laboratory tests, appearance of complications, and self-care knowledge or self -care practices. The above findings indicate that a home care program instituted for such short term as 6 months period could not totally demonstrate its effectiveness at a statistically significant level by quantitative analysis however, what was shown in part in this analysis, and in the continuous consultation sought by those who had been in the experimental group, is that home health care has a great potential in retarding or preventing pathological progress, facilitating rehabilitative and productive life, and improving quality of life by adding comfort, confidence and strength to patients and their families. For the further studies of this kind with chronic patients it is recommended that a sample of newly diagnosed patients be followed up for a longer period of time with more frequent observations to demonstrate a more dear- cut picture of the effectiveness of home care.

Multiple Stakeholder Perspectives on Cancer Stigma in North India

  • Background: Cancer is a leading cause of death worldwide. A large proportion of cancer deaths are preventable through early detection but there are a range of social, emotional, cultural and financial dimensions that hinder the effectiveness of cancer prevention and treatment efforts. Cancer stigma is one such barrier and is increasingly recognized as an important factor influencing health awareness and promotion, and hence, disease prevention and control. The impact and extent of stigma on the cancer early detection and care continuum is poorly understood in India. Objectives: To evaluate cancer awareness and stigma from multiple stakeholder perspectives in North India, including men and women from the general population, health care professionals and educators, and cancer survivors. Materials and Methods: A qualitative study was conducted with in-depth interviews (IDIs) and focus group discussions (FGDs) among 39 individuals over a period of 3 months in 2014. Three groups of participants were chosen purposively - 1) men and women who attended cancer screening camps held by the Indian Cancer Society, Delhi; 2) health care providers and 3) cancer survivors. Results: Most participants were unaware of what cancers are in general, their causes and ways of prevention. Attitudes of families towards cancer patients were observed to be positive and caring. Nevertheless, stigma and its impact emerged as a cross cutting theme across all groups. Cost of treatment, lack of awarenes and beliefs in alternate medicines were identified as some of the major barriers to seeking care. Conclusions: This study suggests a need for spreading awareness, knowledge about cancers and assessing associated impact among the people. Also Future research is recommended to help eradicate stigma from the society and reduce cancer-related stigma in the Indian context.

Ultrasound-Guided Pain Interventions - A Review of Techniques for Peripheral Nerves

  • Ultrasound has emerged to become a commonly used modality in the performance of chronic pain interventions. It allows direct visualization of tissue structure while allowing real time guidance of needle placement and medication administration. Ultrasound is a relatively affordable imaging tool and does not subject the practitioner or patient to radiation exposure. This review focuses on the anatomy and sonoanatomy of peripheral non-axial structures commonly involved in chronic pain conditions including the stellate ganglion, suprascapular, ilioinguinal, iliohypogastric, genitofemoral and lateral femoral cutaneous nerves. Additionally, the review discusses ultrasound guided intervention techniques applicable to these structures.

  • The labor environment in Korea has changed and problems related to employment types are treated as important issues. Especially, the ratio of irregular workers has not only increased relatively, but the labor conditions in Korea also have worsened in the current years. Studies have reported an association between temporary workers and the prevalence of diseases. However, there is insufficient research on chronic disease and employment types. Methods: The current study examines the prevalence rate of diseases and health behavior by categorizing employment types among Korean adults. Data were obtained from the 2016-2018 Korea National Health and Nutrition Examination Survey. Totally, the data of 2,366 workers (1,239 regular and 1,127 irregular) were analyzed in the study. The types of employment were classified by a questionnaire querying about working conditions. The results showed that irregular workers earned less than regular workers and had a significantly higher prevalence of diabetes mellitus and hypertension. We propose that if discrimination related to working conditions were relieved, irregular workers would be able to invest more time to exercise and doing check-ups regularly. In addition, individual nutrition consultations considering the knowledge and personal environmental factors of each individual are necessary for the improved health of all workers.

The dog as an exercise science animal model: a review of physiological and hematological effects of exercise conditions

  • [Purpose] Exercise is a fundamental way to maintain and improve health and physical fitness. Many human studies have demonstrated the beneficial effects of exercise on various biological parameters. However, studies investigating the effects of exercise in dogs are limited. This review summarized the current data from studies that examined the effects of different exercise conditions (treadmill vs. non-treadmill and acute vs. chronic) on physiological and hematological parameters in dogs. [Methods] Papers addressing the effects of exercise in dogs published from January 2000 to October 2020 were retrieved from the online databases of Scopus, Google Scholar, and PubMed and were selected and reviewed. [Results] The exercise conditions differentially affected physiological and hematological responses and adaptation in dogs. Therefore, the development and comprehensive evaluation of scientific exercise programs for dogs are necessary. [Conclusion] The dog would be a valuable exercise science animal model, and studies aiming at the optimal health, well-being, and quality of life of dogs need to be conducted.
