Opisthorchis viverrini (Ov) infection is the major etiological factor for cholangiocarcinoma (CCA), especially in northeast Thailand. We have previously reported significant involvement of PI3K/AKT/PTEN and $Wnt/{\beta}$-catenin in human CCA tissues. The present study, therefore, examined the expression and activation of PI3K/AKT/PTEN and $Wnt/{\beta}$-catenin signaling components during Ov-induced cholangiocarcinogenesis in a hamster animal model. Hamsters were divided into two groups; non-treated and Ov plus NDMA treated. The results of immunohistochemical staining showed an upregulation of PI3K/AKT signaling as determined by elevated expression of the $p85{\alpha}$-regulatory and $p110{\alpha}$-catalytic subunits of PI3K as well as increased expression and activation of AKT during cholangiocarcinogenesis. Interestingly, the staining intensity of activated AKT (p-AKT) increased in the apical regions of the bile ducts and strong staining was detected where the liver fluke resides. Moreover, PTEN, a negative regulator of PI3K/AKT, was suppressed by decreased expression and increased phosphorylation during cholangiocarcinogenesis. We also detected upregulation of $Wnt/{\beta}$-catenin signaling as determined by increased positive staining of Wnt3, Wnt3a, Wnt5a, Wnt7b and ${\beta}$-catenin, corresponded with the period of cholangiocarcinogenesis. Furthermore, nuclear staining of ${\beta}$-catenin was observed in CCA tissues. Our results suggest the liver fluke infection causes chronic inflammatory conditions which lead to upregulation of the PI3K/AKT and $Wnt/{\beta}$-catenin signaling pathways which may drive CCA carcinogenesis. These results provide useful information for drug development, prevention and treatment of CCA.
This study was carried out to evaluate the effects of all-trans retinoic acid(RA) on the development of cholangiocarcinoma in hamsters. Eighty six female Syrian golden hamsters were divided into four groups. Group I was for the induction of the cholangiocarcinoma, which was infected orally with C sinensis and given dimethylnitrosamine(DMN, 15ppm) in drinking water for 4 weeks. Group II was for evaluating the effect of all-trans RA treatment on the cholangiocarcinogenesis, which was treated the same as group I and orally given RA(1mg/kg, 5 times per week) for 15 weeks. Group III was given only RA hr 15 weeks. Control group IV was given only soybean oil which was solvent for RA treatment. More than 5 heads of hamsters in each group were sacrificed at 4, 7, 11 and 15 weeks after the begining of the experiment. The livers were examined grossly, histopathologically, and immunohistochemically. The results obtained were as follows : 1. Death of animals started from the 11 weeks after the begining of the experiment. One of the total 22 animals(5%) and 7 of the total 24 animals(29%) died in group I and group II, respectively. 2. Proliferation of oval cell was peaked at 11 weeks in group I and at 7 weeks in group II, and decreased gradually after those periods of the time. 3. Cholangiocarcinomas were found in 1 of 6 animals(17%) at 11 weeks and in 4 of 6 animals(67%) at 15 weeks in group I, respectively. But in group II, the cholangiocarcinomas occured in 1 of 5 animals(20%) at 7 weeks, in 7 of 12 animals(58%) at 11 weeks and in 2 of the rest animals(100%) at 15 weeks, respectively. 4. Expression of $\alpha$-fetoprotein(AFP) of the oval cells in the group II showed the same degree of positive reaction at that of group I at 4 weeks. But AFP postive oval cells decreased gradually and AFP negative oval cells(ductlike oval cells) increased gradually. 5. Expression of cytokeratin of the oval cells in group II was shown slightly at 4 weeks and the degree of expression increased moderately from the 7 weeks. But the expression of the oval cells in group I was shown slightly after the 7 weeks. These results suggested that all-trans RA promoted the occurrence and the rate of cholangiocarcinoma by inducing differentiation of small cells and oval cells in the liver of hamsters infected with C sinensis and treated with DMN.
Oval cell is considered as facultative precursor cells for both hepatocytes and biliary cells, as well as origin of hepatocellar and cholangiocellular carcinoma (CCC) during carcinogenesis or toxic liver injury. To clarify the cellular origin or differentiation of cholagiocarcinogensis, the fate of carcinogen-induced oval cells was pathologically and phenotypically chased in Syrian golden hamster liver after Clonorchis sinensis (CS) infection which would give rise to a promoting effect. Two week treatment of hamsters with 0.005% diethylnitrosamine (DEN) followed by 2 week treatment of 1% 2-acetylaminofluorene (AAF) under choline deficient diet resulted in massive proliferation of BrdU labeleed and PCNA positive oval cells showing various distinct morphology, histochemical and immunohistochemical phenotypes for GGT, cytokeratin 19 and OV-6. Oval cells also frequently form ductular-like structures or phenotypically show hepatocyte-like characteristics. After CS infection, the oval cells showed sequential morphological changes to atypicl proliferating bile ductules and all hamsters thereafter developed well differentiated and anaplastic CCC at 16 week after CS infection. In electron microscopy, some bile ductules were constructed by intermediate oval cells and bile ductular cells surrounded by basement membrane. The results of this study strongly suggest that CCC developed in the present study were originated from hepatic stem-like oval cells, supporting the theory of stem cell origin of cancers. In addition, this hamster model would be valuable for the molecular mechanistic study during chemical-triggered cholangiocarcinogenesis.
This study was carried out to examine the role of small cells and oval cells in cholangiocarcinogenesis in the hamsters infected with Clonorchis(C) sinensis. Forty two female Syrian golden hamsters were divided into two groups. Group I was for the induction of the cholangiocarcinoma, which was infected orally with C sinensis and given dimethylnitrosamine(15ppm) in drinking water for 4 weeks. Group II was served as control. More than 5 heads of hamsters in each group were sacrificed at 4, 7, 11 and 15 weeks after the beginning of the experiment. The livers were examined histopathologically, electron microscopically and immunohistochemically. The results obtained were as follows; 1. Cholangiocarcinomas were occurred in 1 of 6 animals at 11 weeks and in 4 of 6 animals at 15weeks after the beginning of the experiment. 2. Small cells and oval cells were proliferated around the portal triads from 4 weeks and peaked at 11 weeks, and slightly decreased after then. 3. The strong positive reaction to the $\alpha$-fetoprotein was shown in many of small cells and oval cells. But ductlike oval cells, which were arranged rosette form, showed week positive reaction to the $\alpha$-fetoprotein. 4. Most of small cells and oval cells showed negative reaction to the cytokeratin. But weak positive reaction in ductlike oval cells, and moderate positive reaction in cholangiocarcinoma cells were observed. These results suggested that cholangiocarcinoma induced by infection of C sinensis was believed to originate from the proliferated small cells around the portal triads which would be able to differentiate to the oval cells, ductlike oval cells, and cholangiocarcinoma cells gradually.
Clonorchiasis, caused by direct contact with Clonorchis sinensis worms and their excretory-secretory products (ESPs), is associated with chronic inflammation, malignant changes in bile ducts, and even cholangiocarcinogenesis. Our previous report revealed that intracellular free radicals enzymatically generated by C. sinensis ESPs cause NF-${\kappa}B$-mediated inflammation in human cholangiocarcinoma cells (HuCCT1). Therefore, the present study was conducted to examine the role of upstream Toll-like receptors (TLRs) on the initial host innate immune responses to infection. We found that treatment of HuCCT1 cells with native ESPs induced changes in TLR mRNA levels in a time-dependent manner, concomitant with the generation of free radicals. ESP-mediated free radical generation was markedly attenuated by preincubation of the cells with TLR1-4-neutralizing antibodies, indicating that at least TLR1 through 4 participate in stimulation of the host innate immune responses. These findings indicate that free radicals triggered by ESPs are critically involved in TLR signal transduction. Continuous signaling by this pathway may function in initiating C. sinensis infection-associated inflammation cascades, a detrimental event leading to progression to more severe hepatobiliary diseases.
Cholangiocarcinoma (CCA) is a rare but highly fatal cancer for which the molecular mechanisms and diagnostic markers are obscure. We therefore investigated the kinetic expression of isocitrate dehydrogenase-1 (IDH1), isocitrate dehydrogenase-2 (IDH2) and homogentisate 1,2-dioxygenase (HGD) during the tumorigenesis of O. viverrini infection-associated CCA in an animal model, and confirmed down-regulation of expression in human cases of opisthorchiasis-associated CCA through real time PCR. Kinetic expression of HGD, IDH1 and IDH2 in the animal model of O. viverrini infection-induced CCA was correlated with human CCA cases. In the animal model, expression of HGD was decreased at all time points (p<0.01) and expression of both IDH1 and IDH2 was decreased in the CCA group. In human cases, expression of HGD, IDH1 and IDH2 was decreased more than 2 fold in 55 cases (70.5%), 25 cases (32.1%) and 24 cases (30.8%) respectively. The present study suggests that reduction of HGD, IDH1 and IDH2 may be involve in cholangiocarcinoma genesis and may be useful for molecular diagnosis.
The cholangiocarcinoma (CCA) is a relatively rare cancer worldwide but it is highly prevalent in Thailand where the liver fluke, Opisthorchis viverrini is endemic. There are reports that interleukin 6 (IL-6) may play an important role in the pathogenesis of opisthorchiasis associated CCA. Functionally, IL-6 can act on target cells through its receptor, IL-6R, and IL-6R polymorphisms may affect the functional activity of IL-6 leading to susceptibility to cholangiocarcinogenesis. Therefore, we assessed the association of the 48892 A/C (Asp358Ala) polymorphism in exon 9 of the IL-6R gene in 79 CCA cases compared to 80 healthy controls using the PCR-RFLP technique. The results showed significant differences between CCA cases and controls in overall genotype (p=0.001) and allele frequencies (p=0.0002). Chi-square for trend test revealed a significant association between genotype and CCA susceptibility (p=0.0002). The odds ratios (ORs) for genotype were 0.283 (95% CI=0.131-0.605, AC vs. AA; p=0.0003) and 0.206 (95% CI=0.196-1.245, CC vs. AA; p=0.0416), the OR for alleles was 0.347 (95% CI=0.187-0.633, allele C vs. allele A; p=0.0002) and that for the carrier C variant was 0.272 (95% CI=0.130-0.564; p=0.0001). This study demonstrated a close association between an IL-6R polymorphism, specifically higher A allele, and cholangiocarcinoma.
